  • Clinical analysis of vitreous hemorrhage after vitrectomy in diabetic retinopathy

    Objective To analyze the reasons, methods of treatment, and effects on prognosis of vitreous hemorrhage after vitrectomy in patients with diabetic retinopathy. Methods The clinical data of 98 patients (122 eyes) with diabetic retinopathy (VI stage) who had undergone vitrectomy were retrospectively analyzed. Results Post-vitrectomy vitreous hemorrhage (gt;grade 2) was found in 25 eyes with the occurrence of 20.5%, in which the hemorrhage occurred 1 week after the surgery in 8 eyes, 1 week to 1 month in 6 eyes, and more than 1 month in 11 eyes. In the 25 eyes, C3F8 tamponade eyes occupied 31.1%, silicone oil tamponade eyes occupied 6.1%, air tamponade eyes occupied 33.3%, and infusion solution tamponade eyes occupied 26.3%. Peripheral fibrovascular proliferation was found in 9 eyes. In the 3 eyes with silicone oil tamponade, the hemorrhage was absorbed in 2, and epiretinal membrane was found in 1 which was moved when the silicon oil was taken out. In the 22 eyes without silicone oil tamponade, the hemorrhage was absorbed in 6 and aggravated in 2 without any timely treatment, neovascular glaucoma occurred in 1, and wide vitreo-retinal proliferation and retinal detachment was observed in 1 with the visual acuity of no light perception. Operations such as fluid-air exchange, vitrectomy were performed on 14 eyes 2 weeks after the hemorrhage absorption stopped. Recurrent vitreous hemorrhage was not found in 12 eyes after single operation. At the end of the follow up period, the visual acuity was no light perception in 3 eyes, hand moving in 2 eyes, counting finger-0.1 in 10 eyes, under 0.3 in 4 eyes, and over 0.3 in 6 eyes. Conclusion Most of the patients with vitreous hemorrhage after vitrectomy due to DR had peripheral fibrovascular proliferation. The visual prognosis after re-operation is good.   • Surgical management for Coats′ disease

    ObjectiveTo evaluate the surgical effects for Coats′ disease.MethodsA total of 16 patients (17 eyes) with Coats′ disease accompanied by exudative retinal detachment underwent cryocoagulation therapy and vitrectomy. The post-operative follow-up lasted 4.25-62.25 months (mean 13.10 months). ResultsAfter the operation, retina reattached completely in 8 eyes (without silicon oil tamponade) with the reattachment rate of 47%, reattached with silicone tamponade in 1 eye, and didn′t completely reattached in 8 eyes. The post-operative vision improved in 5 eyes, remained still in 2 eyes,and decreased in 7 eyes. The operative complications included transitory exacerbation of exudative retinal detachment, focal preretinal proliferation, cataract, secondary glaucoma and vitreous hemorrhage.ConclusionMost of the patients with Coats′ disease accompanied by exudative retinal detachment may have reattached retina after the surgery, and some patients have improved visual acuity.(Chin J Ocul Fundus Dis, 2005,21:145-147)

  • Phacoemulsification with intravitreal triamcinolone injection to treat diabetic macular edema with severe cataract

    ObjectiveTo observe the efficacy of phacoemulsification with intravitreal triamcinolone injection (IVTA) in diabetic patients with severe cataract and diabetic macular edema (DME). MethodsTwenty-one patients (25 eyes) with severe cataract and DME were enrolled in this retrospective study. Fifteen eyes underwent standard phacoemulsification and intraocular lens implantation with 4 mg IVTA at completion of surgery. Ten eyes underwent standard phacoemulsification and intraocular lens implantation. All the patients were followed up for best corrected visual acuity (BCVA), optical coherence tomography(OCT) and ophthalmological examination.Changes in logarithm of he minimal angel of resolution (logMAR) BCVA and central macular thickness (CMT) were evaluated preoperatively and 1 month, 3 months and 6 months postoperatively by repeated measures ANOVA. Correlations between logMAR BCVA and CMT preoperatively and postoperatively were analyzed by Pearson correlation analysis. Postoperative 6 months logMAR BCVA and affecting factors were evaluated by multivariate linear regression analysis. ResultsBoth groups showed significant improvements of logMAR BCVA after surgery (F=4.855, 6.235; P=0.037, 0.020). There were no statistical differences of logMAR BCVA improvement at different time points postoperatively (F=0.007, 0.006, 0.023; P=0.973, 0.938, 0.882). The CMT reductions in IVTA group at month 1 and month 3 postoperatively were statistically significant than the reductions in group without IVTA(F=10.449, 7.374; P=0.012, 0.026), and there was no statistical difference of CMT reduction at month 6 postoperatively between two groups(F=2.173; P=0.114). Correlation Coefficients between CMT and BCVA were not statistically significant preoperatively and 6 months postoperatively(r=0.279, 0.172; P=0.295, 0.574). Analysis of multiple linear regression showed that external limiting membrane status and duration of diabetes are factors affecting the visual recovery (β=0.577, -0.411; P=0.025, 0.030). ConclusionsPhacoemulsification with concurrent IVTA for treatment of patients with severe cataract and DME is effective in reducing edema. But IVTA does not further improve logMAR BCVA postoperatively.

  • Serious complications of Eales disease treated by vitrectomy

    Objective To observe the clinical efficacy of vitrectomy on the serious complications of Eales disease. Methods The clinical data of 30 eyes of 27 patients (20 males and 7 females) with Eales disease who underwent vitrectomy were retrospectively analyzed. The age was ranged from 15 to 54 years old, with a mean of 30.7 years. The best corrected visual acuity (BCVA) was ranged from hand movement to 0.5. The patients were divided into the vitreous hemorrhage group (3 eyes), proliferative vitreoretinopathy group (14 eyes), local retinal detachment group (7 eyes), and wide retinal detachment group (6 eyes) according to the results of examinations. The standard pars plana vitrectomy (PPV) were performed and scleral buckling, lensectomy, endolaser, transscleral cryotherapy/cautery, membrane removal, retinotomy, fluidair exchange, intravitreal injection of triamcinolone, gas/oil tamponade can be combined if necessary. Those eyes underwent 1 to 8 times (with a mean of 2.4 times) of surgery. The followup was ranged from 6 months to 10 years. The BCVA, retinal reattachment, complications before and after surgery was comparatively analyzed. Results At the end of the follow-up, the BCVA was ranged from light perception to 1.5. The BCVA was ge;0.1 in 24 eyes (80.0%),0.03 in 1 eye (3.3%), counting finger in 1 eye (3.3%), hand moving in 3 eyes (10.0%) and light perception in 1 eye (3.3%). The BCVA improved in 22 eyes (73.3%), stable in 2 eyes (6.7%) and decreased in 6 eyes (20.0%). The differences are statistically significant between pre- and postoperative BCVA (t=5.132,P<0.01). The BCVA of wide retinal detachment group was less than other 3 groups (F=4.570, P<0.05); while the BCVA of vitreous hemorrhage group, proliferative vitreoretinopathy group and local retinal detachment group was the same (P>0.05). Complete retinal reattachment was achieved in all eyes at the next day after PPV. At the end of the follow-up, retina reattached in 24 eyes without silicone oil tamponade, local retinal detachment occurred in 1 eye (the patient refused further surgery) and silicone oil tamponadedependant retinal reattachment 5 eyes. During the follow-up, 16 eyes (53.3 %) had developed some complications. Conclusions Vitrectomy is an effective way to cure serious complications of Eales disease. The BACV prognosis of patients with wide retinal detachment is poor.
