类风湿关节炎（RA）是以慢性关节炎为特征的自身免疫性疾病，常累及肺脏引起RA相关间质性肺疾病（ILD）。RA相关性ILD多发生于RA之后，发生在RA之前者较少见，且易误诊。本文报道1例发生在RA之前的ILD，误诊为特发性间质性肺炎长达5年。
