【摘要】 目的  评估腹腔镜与开腹手术患者术前抑郁、焦虑的心理状况。 方法  2007年10月-2008年9月将收治的60例行胆囊术患者随机分为腹腔镜组与开腹组,各30例,术前1 d使用抑郁自评量表和焦虑自评量表对患者的焦虑、抑郁水平进行评估。 结果  两组患者抑郁、焦虑水平均高于全国平均水平(常模);开腹组术前抑郁、焦虑水平高于腹腔镜组但无统计学意义(P gt;0.05)。开腹组急诊手术患者抑郁水平高于择期手术(P lt;0.05),焦虑水平无明显差异。开腹组女性抑郁和焦虑水平均高于男性(P lt;0.05);腹腔镜组女性抑郁水平高于男性(P lt;0.05),焦虑水平男女之间差异无统计学意义。开腹组40.0%的患者处于抑郁状态,高于腹腔镜组的16.7%。 结论  医师术前应注意与患者积极沟通,消除患者对手术的顾虑,尤其是女性患者及急诊手术患者。
【Abstract】 Objective  To evaluate the preoperative depression and anxiety level of the patients undergoing laparoscopic cholecystectomy or laparocholecystectomy. Methods  Thirty patients undergoing laparoscopic cholecystectomy and thirty patients undergoing laparocholecystectomy were assessed by the self-rating depression scale and self-rating anxiety scale from October 2007 to September 2008. Results  Patients undergoing laparoscopic cholecystectomy or laparocholecystectomy showed the significantly greater depression and anxiety than the norm. The preoperative depression levels were higher in patients undergoing laparocholecystectomy than those undergoing laparoscopic cholecystectomy but without significance (P gt;0.05), and in emergency laparocholecystectomy the patients had the higher depression levels than elective operation (P lt;0.05). The preoperative depression and anxiety score were high in female than in male in laparocholecystectomy, while in laparoscopic cholecystectomy only the depression level of female was higher than that of male (P lt;0.05). The patients undergoing laparocholecystectomy in the depression state constituted nearly 40.0% versus 16.7% in laparoscopic cholecystectomy patients, especially for some females. Conclusion  Physicians should pay attention to preoperative active communication with patients, so as to eliminate the concerns of patients about surgery, especially the female during preoperative evaluation.
