Objective To review and evaluate the global clinical research literatures about the surgical management for prostatic abscess in the past decade, so as to provide useful information for clinical practice. Methods Based on the principles of evidence-based medicine, and the methods of bibliometrics, the PubMed database was searched from January 1st 2000 to April 10th 2011. The literatures about prostatic abscess were screened according to the predefined inclusion and exclusion criteria, the quality was assessed, the valid data were extracted and then systematical evaluation was performed after the establishment of the database with OpenOffice.org 3.8 Calc. Results a) A total of 205 articles were found initially and 81 were finally included with the total 388 cases involved (on average, 4.79 cases per paper). The patients’ ages ranged from 10 days to 83 years old. Two studies were diagnostic tests, and all the other 79 were surgical intervention studies; b) There was no controlled clinical trail. A total of 16 studies involving more than 5 cases for each and 311 cases in all which were retrospective case analyses, all the other 65 were individual case reports; c) American scholars published 13 papers (16.5%), ranked as the first. American and European scientists published 36 papers (44.44%). Chinese scholars published quite fewer studies, of which only 2 were in English; d) The incidence increased in younger patients gradually. The pathogens included fungi and bacteria. Most bacteria were gram-positive cocci (such as Staphylococci), followed by gram-negative bacillus (such as Klebsiella). There were also some minority pathogens. The pathogens were complex and quite diverse in different districts. The predisposing factors included both regional and systematic conditions, with diabetes mellitus, hepatocirrhosis, manipulations of lower urinary tract and urinary tract infections as the common; e) The diagnostic procedures included evaluation on symptoms and signs, physical examination, identification of pathogens, and medical imaging examinations (TRUS, CT, MRI). The therapeutic options included routine managements, conservative antibiotic therapies and surgical drainages. The surgical routes were transcutaneous, transperineal, transrectal, and transurethral. The fine needle aspiration, indwelling catheter drainage, or incision and drainage were performed under the guidance of TRUS, EUS or CT through transperineal or transrectal routes. The TUR, TURP or TUIP drainages were performed through transurethral route. The failed cases of fine needle aspiration were then treated by transurethral or transperineal incision and drainages, and some patients were also treated by urinary diversion; f) The best surgical method could not be concluded for lack of controlled data; and g) Few cases died from severe complications, and the outcomes of the majority cases were good. Conclusion a) The literatures about prostatic abscess are abundant but of low quality, with all retrospective studies or individual case reports, and most are published by American and European scholars; b) The prostatic abscess affects all ages but tends to increase in the young. ObjectiveTo explore the clinical features of atypical bronchial asthma with chief complaint of palpitations. MethodsTwelve cases with unexplained palpitations diagnosed by asthma were analyzed retrospectively. ResultsThe patients were diagnosed with asthma through bronchodilation test,skin prick tests,fractional exhaled nitric oxide (FeNO) and diagnostic treatment.The ratio of male to female cases was 1:5. The ventilation lung function of all patients was in normal range. The median time of misdiagnosis was 13.5(3-120) months. 41.6% of cases had dust mite positive skin prick test. The median value of FeNO was 32.75(18-54) ppb. The symptoms of all patients were relieved completely after standard treatment. ConclusionPalpatation may be a specific presentation of asthma characterized with eosinophilic airway inflammation which may be misdiagnosed as other diseases for a long time,especially in elder women. To improve the knowledge of this kind of atypical asthma,detailed medical history, experimental examinations and diagnostic treatment response are important to confirm the diagnosis in time.
ObjectiveTo improve the diagnosis and treatment of multiple systemic sarcoidosis (MSS) and avoid misdiagnosis. MethodsTo analyze the diagnosis and treatment of a MSS patient misdiagnosed as lymphoma. Related literatures were also reviewed. ResultsThe patients' clinical manifestations were not specific including cough and stethocatharsis. Lung and thoracic lymph nodes were most commonly involved in MSS. MSS was characterized by symmetrical lymph nodes enlargement in the bilateral lung hilus and/or mediastinum. The enlarged lymph nodes had a clear boundary and showed homogeneous enhancement. Symmetrical fluorodeoxyglucose (FDG) uptake in the hilar and/or mediastinal node was a typical finding of sarcoidosis on FDG PET/CT. Mucosal inflammation and mucosal nodules could be seen in the bronchoscope. Sarcoidosis was characterized by the presence of noncaseating groanulomas histologically. Hormonal therapy was effective for MSS. ConclusionSarcoidosis is a kind of disease involving multiple systems and organs with unknown etiology. The clinical manifestation of sarcoidosis is nonspecific,so it's likely to be misdiagnosed. Imaging examination and laboratory examination are helpful to the diagnosis of MSS. The definitive diagnosis depends on the pathologic biopsy.
Objective To study the incidence,clinicopathological characteristics,diagnosis,treatment, and prognosis of synchronous or metachronous primary cancers in patients with gastric cancer. Methods The clinical data of 103 gastric cancer patients with synchronous or metachronous primary cancers from January 1983 to December 2010 were collected and reviewed.Results The incidence of synchronous or metachronous primary cancer in addition to gastric cancer was 2.26%(103/4 552).The age of these patients was (63.98±11.93) years (30~84 years) when gastric cancer was diagnosed.There were 29 cases of synchronous primary cancer and 74 cases of metachronous primary cancer,in which there were 46 pre-metachronous cancer patients and 28 post-metachronous cancer patients.All the patients had 113 primary tumors of other organs.Colorectal cancer was the most common type of primary cancer in other organs (27.43%, 31/113),followed by lung cancer (15.04%, 17/113).The time interval between gastric cancer and metachronous primary cancer was (87.95±92.98) months (7~506 months),and 65.49% (74/113) of other primary tumors were diagnosed within 5 years before or after gastric cancer. The overall 5-year survival rate was 48.43%. The 5-year survival rate of post-metachronous group (69.52%) was significantly better than synchronous cancer group (36.40%, P=0.009) or pre-metachronous group (42.31%, P=0.023).In 33 patients who had definite cause of death, 20 patients died of gastric cancer.Conclusions Primary cancer of other organs should be considered in treatment of gastric cancer.Gastric cancer may be the most important factor impacting the prognosis of these patients.
Objective To investigate the pathological features, diagnosis, treatment, and prognosis of multiple primary colorectal cancer (MPCC). Methods Clinical data of 41 patients with MPCC treated in The Fourth Affiliated Hospital and The First Affiliated Hospital of China Medical University from Aug. 1993 to Mar. 2009 were retrospectively analyzed. Results Forty one patients with MPCC, including 29 patients with synchronous colorectal cancer (SCC) and 12 patients with metachronous colorectal cancer (MCC), accounted for 1.8% (41/2 340) of colorectal cancer during the same period of time, and with adenomatous polyps in 19 cases and polyps canceration in 10 cases. Among 29 patients with SCC, 15 cases (51.7%) were diagnosed by preoperative fiberoptic colonoscopy, 9 cases (31.0%) were diagnosed by preoperative fiberoptic colonoscopy, abdomen CT, and barium enema, 5 cases (17.2%) were diagnosed by intraoperative exploration and intraoperative fiberoptic colonoscopy, respectively. All of the 12 patients with MCC were diagnosed by preoperative fiberoptic colonoscopy and abdomen CT. For 29 patients with SCC, tumor locations were from proximal appendix to distal rectum, but 12 patients with MCC were adverse. Sixty-five (77.4%) tumors were tubular or papillary adenocarcinoma, and 56 (66.7%) tumors were well and moderately differentiated adenocarcinoma. The TNM stage of most tumors (72) was stageⅡ or Ⅲ phase, account for 85.7%. Radical surgeries were performed in 37 patients and palliative surgeries in 4 patients, and there were no complications after operation. During the follow-up for 3-5 years (mear 3.6 years), the overall survival rate of 3- and 5-year were 48.8% (20/41) and 34.1% (14/41), respectively. In detail, 3-year survival rate of SCC group and MCC group were 48.3% (14/29) and 50.0% (6/12), respectively;5-year survival rate were 31.0% (9/29) and 41.7% (5/12), respectively. Conclusions Cause of MPCC has not been clear, but it has possible relationship with adenomatous polyps. Preoperative fiberoptic colonoscopy, abdomen CT, and barium enema are very important for patients with SCC, and intraoperative fiberoptic colonoscopy is also necessary. Patients with MCC should enhance postoperative follow-up with fiberoptic colonoscopy. Further more, radical resection should be performed as early as possible.
Objective To summarize and analyze the MRI imaging findings of advanced gastric cancer in order to improve the level of image diagnosis. Methods The plain and dynamic enhanced MRI findings in 8 volunteers and 30 patients with pathologically proven advanced gastric cancer were retrospectively analyzed. Results The stomach wall of advanced gastric cancer was inhomogeneous thickening with iso intensity or little hypo intensity signal on T1WI, and iso intensity or little hyper intensity signal on T2WI. Cases with serosal infiltration, the serous appearred indistinctly and rough. In some cases, the low signal zones between gastic wall and fat space were interrupted on T1WI out-of-phase image. Cases with adjacent organs invaded, the fat space was rough or disappeared. The lesions showed obvious irregular or hierarchy enhancement on MRI dynamic contrast scanning. Conclusions Inhomogeneous thickening of gastic wall with abnormal signal intensity, indistinctness or disappearance of fat space, irregular or hierarchy enhancement are very valuable as diagnostic signs in patients suspective of advanced gastric cancer.
Objective To investigate the early diagnosis and treatment methods of primary duodenal papilla carcinoma. Methods The medical records of 54 patients with primary duodenal papilla adenocarcinoma underwent operation between January 2002 and December 2008 were reviewed. Results Thirty seven cases received fiberduodenoscopy and 35 cases received ERCP, and the accuracy of them were both 100%. Forty four patients received duodenopancreatectomy and 10 patients received jaundice-reducing operation. The 1-, 3-, and 5-year cumulation survival rate was 68%, 50%, and 29%, respectively. Conclusions Fiberduodenoscopy and ERCP are the effective diagnostic methods for duodenal papilla carcinoma. Early diagnosis and early rational radical operation are essential for successful treatment of duodenal papilla carcinoma.
ObjectiveTo investigate the diagnostic value of endoscopic retrograde cholangiopancreatography (ERCP) in obstructive jaundice of elderly patients. MethodsTotally 338 patients with obstructive jaundice underwent ERCP were divided into elderly group (age ≥60 years old) and nonelderly group (age lt;60 years old) based on age. The levels of serum amylase (AMY), ALT, and TBIL in 6, 24, and 48 h after ERCP were detected. The success rate of cholangiopancreatography, accuracy rate of diagnosis, and incidence of complications after ERCP in two groups were analyzed. ResultsThe difference of serum AMY, ALT, and TBIL levels of patients in 6, 24, and 48 h after ERCP were not significant between two groups (Pgt;0.05). The success rate of cholangiopancreatography in nonelderly group was 96.3% (130/135) and in elderly group was 96.1% (195/203), and no difference was found (Pgt;0.05). However, the accuracy rate of diagnosis of ERCP in nonelderly group (84.6%, 110/130) was significantly lower than that in elderly group (98.5%, 192/195), Plt;0.05. The difference of the incidence of complications was not significant between two groups 〔14.8% (20/135) vs. 17.2% (35/203)〕, Pgt;0.05. There was no mortality in two groups patients. ConclusionERCP is a safe, effective, and accurate method, which is of importance to the diagnosis of obstructive jaundice in elderly patients.
Objective To detect the serum protein fingerprint in gastric cancer patients by using the surface-enhanced laser desorption-ionization time-of-flight mass spectrometry (SELDI-TOF-MS) and protein chip array technology, screen biomarker candites, build diagnostic models and evaluate its clinical significance. Methods The serum proteomic patterns were detected in 40 patients with gastric cancer, 20 patients with gastric ulcer and 20 healthy blood donors. The diagnostic models were developed and valited by discriminant analysis. Results The peak intensity of differential expression proteins was not found in healthy blood donors, and 1 case was found in patient with gastric ulcer (m/z: 5 910,4 095). The peak intensity of 5 329, 4 095, 5 910, 8 691 and 3 300 (m/z) proteins were significantly higher in 40 gastric cancer patients than those in 20 gastric ulcer patients and 20 healthy blood donors ( P <0.05). Three differential expression proteins were set up a diagnostic model together to diagnose gastric cancer. The diagnostic model made up of the differential expression proteins of 4 095, 5 910 and 8 691 had a sensitivity of 92.5% and a specificity of 97.5% . Conclusion Using SELDI-TOF-MS shows great potential to detect, and screen novel and better biomarkers for gastric cancer.
Objective To analyze the effect of meglumine diatrizoate on diagnosing and treating adhesiveness small intestinal obstruction. Methods The clinic data of 484 cases of adhesiveness small intestinal obstruction were analyzed retrospectively. Those patients were treated with radiography with 76% of meglumine diatrizoate by orally or injected. Results After taking meglumine diatrizoate, 362 patients were cured, and the other 122 cases were diagnosed clearly and treated with surgery. Conclusions Meglumine diatrizoate can be used to diagnose adhesiveness small intestinal obstruction and confirm where the obstruction is. It can be the routine treatment for adhesiveness small intestinal obstruction and can be used repeatedly. It also can provide evidence to surgical treatment and guide to make the surgical project.
