  • Catheter Directed Thrombolysis versus Mechanical Thrombectomy Combined with Catheter Directed Thrombolysis for Lower Extremity Deep Vein Thrombosis

    目的研究置管溶栓联合球囊扩张导管碎栓治疗下肢深静脉血栓形成(DVT)的疗效及安全性。 方法回顾性分析2011年9月至2015年1月本院收治的82例急性下肢DVT患者的临床资料。将患者分成两组:A组行单纯经导管直接溶栓(CDT)治疗,共32例,其中男10例、女22例,平均年龄(56±15)岁;B组行CDT联合机械碎栓(球囊扩张导管碎栓)治疗,共50例,其中男18例、女32例,平均年龄(57±17)岁。比较2组治疗前后静脉通畅率、静脉通畅度评分、健侧和患侧下肢周径差,比较2组尿激酶用量及溶栓导管留置时间,以出血并发症、肺栓塞的发生率评价治疗的安全性。 结果2组共82例患者完成溶栓及碎栓疗程,B组较A组溶栓时间更短,A组平均9(7,12)d;B组平均5(4,7)d,2组差异有统计学意义(P<0.01);B组较A组尿激酶用量更少,A组平均7.250(6.355,8.255)×106 U,B组平均4.925(3.715,5.810)×106 U,2组差异有统计学意义(P<0.01);B组治疗后的静脉通畅度评分显著优于A组,A组65.0%(40.5%,86.5%),B组为100%(90%,100%),差异有统计学意义(P<0.01)。治疗后2组出血并发症发生率差异无统计学意义(P=0.0976)。2组患者随访时间3~18个月,A组随访率50.0%,B组随访率58.0%,均未发生肺动脉栓塞。 结论在下腔静脉滤器保护下,CDT联合球囊扩张导管碎栓治疗急性下肢DVT较单纯CDT治疗,溶栓效果好、尿激酶用量少、不增加肺栓塞及出血发生率。

  • 综合预防老年下肢手术深静脉血栓的临床观察

    目的 探讨采用综合措施预防老年下肢骨科手术后深静脉血栓的临床疗效。 方法 对2009年1月-2010年12月间收治的116例行下肢骨科手术患者,其中男41例,女75例;年龄65~94岁,平均77.8岁。术后采用下肢关节收缩运动、间歇性空气压力波治疗仪、低分子肝素钙等综合措施预防术后深静脉血栓。 结果 术后1周经二维彩色超声检查,发生股静脉上段周围型血栓2例,腘静脉血栓1例,发生率2.6%。随访12个月以上患者血栓未脱落、长大,下肢也无明显肿胀。 结论 采用综合措施预防老年患者下肢骨科手术后深静脉血栓,方法简便、疗效可靠。

  • Interventional Treatment for the Patients with Type 2 Diabetes Combined with Vascular Lesions of Lower Extremities

    【摘要】 目的 探讨2型糖尿病合并下肢血管病变血管内介入治疗的临床意义。 方法 2009年1-5月对4例2型糖尿病合并下肢血管病变患者,根据血管狭窄情况选择不同介入治疗方式,行下肢动脉造影及动脉球囊扩张或支架成形术。 结果 4例患者均有表现静息痛及间歇性跛行,下肢血管超声显示糖尿病下肢动脉有不同程度的斑块、狭窄与血栓形成,病变累及下肢股动脉、髂动脉及胫前、足背动脉。介入治疗后患者下肢血管灌注得到明显改善,静息痛及间歇性跛行明显改善,皮温改善,需要截肢患者截肢平面显著降低。 结论 通过下肢血管DSA造影检查,准确了解糖尿病患者下肢血管的阻塞部位及程度,在保守治疗基础上选择不同方式的介入治疗,有助于下肢血管病变的明显改善。【Abstract】 Objective To investigate the clinical significance of intervention therapy for patients with type 2 diabetes combined with vascular lesions of lower extremities. Methods From January to May, 2009, four diabetic patients with vascular lesions of lower extremities were examined by Doppler ultrasonography and digital subtration angiography (DSA). All patients were treated by percutaneous transluminal angioplasty (PTA) or stenting therapy. Results Stenoses and obstruction of lower extremity blood vessels were observed in all patients. After intervention therapy, vascular perfusion of lower extremities was improved and signs of rest pain and intermittent claudication were relieved; the skin temperature was improved, and the amputation level was apparently decreased. Conclusion It suggests that DSA is effective in judging extend and location of blood vessel stenosis,and the interventional treatment could lead to a satisfying prognosis.

  • Preparation and Evaluation of Chronic Hindlimb Ischemia of Lewis Rat

    Objective To establish chronic hindlimb ischemia model with suture-occluded method in rats, and then compare the effects of chronic hindlimb ischemia model with acute ischemia model. Methods Models of chronic hindlimb ischemia were established by using suture-occluded femoral artery method. The laser Doppler blood flow analysis and angiography were performed on day 7, 14, 28, 42, and 49 after operation, and then the rats were sacrificed after angiography, respectively, the quadriceps and gastrocnemius of contralateral and ipsilateral (surgical side) were gotten, which were tested by HE staining and α-actin immunohistochemistry staining, and then calculate arteriolar density. Results There were no lameness and limb necrosis after operation in chronic hindlimb ischemia models. Laser Doppler analysis found that chronic hindlimb ischemia models were still maintained in ischemia state on day 49 after operation compared with acute ischemic models. The resluts of HE staining showed no acute necrosis and muscle fibrosis in chronic hindlimb ischemia model group. Chronic hindlimb ischemia models after operation did not appear obvious lameness and limb necrosis. The arteriolar density of quadriceps femoris on day 7 after operation in chronic hindlimb ischemia models were less than that in acute hindlimb ischemia models (0.015 2 vs. 0.036 4). Conclusions Compared with the commonly used acute ischemic models, the duration of arterial limb ischemia in chronic hindlimb ischemia rats, which were established by suture-occluded method, is longer and less likely to be affected by the compensatory mechanisms. So suture-occluded method can provide a new animal hindlimb ischemia model for further study of ischemia angiogenesis mechanism and treatment of severe lower extremity ischemia.

  • Application of Potaried Technique with Trivex System in Treatment for Venous Skin Ulcer with Deep Venous Thrombosis Sequelae of Lower Limbs

    ObjectiveTo explore the value of potaried technique with Trivex system in treatment for venous skin ulcer with deep venous thrombosis sequelae (DVTS) of lower limbs. MethodsTotal 166 patients with venous skin ulcer with DVTS of lower limbs were included in this study. The patients of operation group (94 patients involving 94 legs) were treated by using potaried technique with Trivex system. The patients of nonoperation group (72 patients involving 72 legs) were treated by using nonoperative method. The clinical indexes of skin infection rate, skin necrosis rate, shrinkage rate of wound area, skin depigmentation rate, ulcer healing rate and ulcer recurrence rate were used to assess the clinical curative effect between two groups on 5, 20, 120 and 360 d after operation or treatment, respectively. ResultsThere were no skin infection and skin necrosis in two groups on 5 d after operation or treatment. The rate of shrinkage of wound area and skin depigmentation of patients in operation group were significantly higher than those in nonoperation group on 20 d after operation or treatment 〔(95.8±2.138)% vs. (68.7±3.125)%,P=0.048; (87.6±1.263)% vs. (12.3±1.324)%, P=0.018〕. The rate of the ulcer healing of patients in operation group was significantly higher than that in nonoperation group on 120 d after operation or treatment (97.9%vs. 8.3%, P=0.014). The rate of the ulcer recurrence of patients in operation group was significantly lower than that in nonoperation group on 360 d after operation or treatment (5.3% vs. 97.2%, P=0.015). ConclusionThe potaried technique with Trivex system can be used as one of the surgical treatment methods for venous skin ulcer with DVTS of lower limbs.

  • Combinative Use of Radiofrequency and Transilluminated Powered Phlebectomy in Treatment of Varicose Veins of Lower Extremity

    Objective To evaluate the efficiency and safety of combinative use of radiofrequency and transilluminated powered phlebectomy for treatment of varicose veins of lower extremity. Methods Three hundred and twenty-one cases of varicose veins of lower extremity were randomly divided into three groups: group A (102 cases): endovenouser whole range closure in situ for great saphenous vein by radiofrequency and transilluminated powered phlebectomy for varicose veins of leg; group B (97 cases): endovenouser whole range closure in situ for upper leg part of great saphenous vein by radiofrequency and transilluminated powered phlebectomy for varicose veins of leg; 122 conventional treatment cases, which high ligation and stripping of great saphenous vein, blood vessel branches and communicating branches resection and ligation were used, were selected as controlled group (group C). Consequences and complications of operation were checked. Results In all cases, the symptoms of swelling in lower extremity, soreness and infirmity disappeared within one month after operation, skin pigmentation was alleviated within one month after operation, and disappeared gradually within three months. Contrasting three groups in complications of skin numbness of legs, ecchymosis of upper leg, wound hematoma, and swelling below ankle, there was no statistical difference between group A and B, and there was statistical difference between group A and C, and B and C. There were more complications in conventional group than those in mini-invasive groups. There was no statistical difference in leg ecchymosisa between group B and C. No deep vein thrombosis of lower extremity and incision infection was detected in all cases. Conclusion It is simple and definitely efficient to combine radiofrequency with transilluminated powered phlebectomy for treatment of varicose veins of lower extremity. Powered phlebectomy is efficient in removal of varicose vein, but not suitable for phlebectomy for trunk of great saphenous vein.

  • Assessment of Combined Management for Lower Limb Chronic Venous Insufficiency According to CEAP Classification

    Objective To assess the combined management of lower limb chronic venous diseases according to the CEAP classification. Methods One hundred and twenty patients were classified according to the CEAP classification. Based on clinical presentation and image study, all patients were treated with combined management plan including oppression, medication and surgery. Results All 120 patients (135 limbs) were followed up in clinic, the local recurrence rate was 18.52%(25/135). Conclusion CEAP classification expounds the developing process of lower limb chronic venous diseases. With CEAP, we can avoid the blind spot in the treatment and expand the extent of combined therapy. Accordingly, CEAP classification is useful in the treatment and diagnosis of chronic venous diseases.

  • Level and Significance of Inflammatory Cytokines in Patients with Acute Deep Venous Thrombosis of Lower Extremity

    Objective To study the significance of the levels of plasma inflammatory cytokines (IL-6,IL-8,IL-10 and TNF-α) in patients with acute deep venous thrombosis (DVT) of lower extremity. Methods Forty untreated DVT cases were selected as the subjects in the DVT group, while thirty healthy subjects, whose ages and genders showed no significant difference with the DVT patients, were collected as the control group. The plasma levels of IL-6, IL-8 and TNF-α were detected by radioimmunoassay (RIA), and the plasma level of IL-10 was measured by enzyme-linked immunosorbent assay (ELISA). Correlation analysis was used to investigate the relationships between the levels of different inflammatory cytokines within DVT group. Results The levels of plasma cytokines in the DVT group were all significantly higher than those in control group (P<0.001). The results of the correlation analysis showed that there were positive correlations between IL-6 and TNF-α (r=0.383, P<0.05), IL-10 and TNF-α (r=0.390, P<0.05), respectively, within the DVT group; whereas there were no correlations between IL-6 and IL-8, IL-6 and IL-10, IL-8 and IL-10, and IL-8 and TNF-α. Conclusion The levels of plasma cytokines increased significantly in patients of DVT. Inflammatory cytokines may play an important role in acute DVT by accelerating the pace of thrombosis, intensifying the inflammatory reaction around thrombus and aggravating the injured blood vessel.

  • Thrombolytic Therapy of Acute Deep Vein Thrombosis of Lower Extremity(Report of 126 Cases)

    Objective To explore the results of thrombolytic therapy of acute deep vein thrombosis of lower extremity. Methods The clinical data of 126 patients with acute deep vein thrombosis of lower extremity treated conservatively from Dec.1999 to Feb.2004 were analyzed retrospectively. Results All patients were divided into three groups: 26 of central, 33 of surrounding and 67 of combined by Doppler sonography. Combined medication was adopted in all cases, in which 85 cases (67.5%) were clinically cured, 34 cases (27.0%) had good results, and 5 cases (4.0%) recovered partially. Two cases (1.6%) didn’t get better. The total effective rate was 98.4% (124/126). Conclusion Most patients with acute deep vein thrombosis of lower extremity can get satisfied results with conservative treatment including thrombolysis and anticoagulation. Early diagnosis and appropriate medication are crucial.
