目的 探讨临床实施医护一体化伤口治疗模式的效果和前景。 方法 利用调查问卷方式,对2010年12月-2011年12月19个科室临床实施医护一体化伤口治疗模式的效果进行满意度调查。 结果 医生、患者、护士的总体满意度分别达到94.51%、94.56%、91.43%。 结论 医护一体化伤口治疗模式能够明显提高医护患三方满意度,值得在临床推广。
ObjectiveTo investigate multidisciplinary collaborative role in the treatment of patients with lower extremity degloving injury. MethodsThe wound therapy group and the Department of Orthopedics, Nutrition, and Psychology, and the Pain Clinic and Rehabilitation team collaboratively carried out the comprehensive intervention for a patient with lower extremity degloving injury in March 2013 in our hospital. The wound therapist was responsible for correct evaluation, debridement, infection control, maintaining moisture balance, and protecting wound edges of the skin; The wound team members were responsible for the entire treatment and healing process in the form of text and photos; Department of Orthopedics was responsible for secondary skin graft; Nutrition division was responsible for the correction of anemia in patients with hypoalbuminemia; Psychological service was responsible for psychological counseling as treatment accident sometimes brought psychological trauma; Pain outpatient service was responsible for consultation, formulating specific plans to control and relieve the patient's pain, and promote the patient's physical and mental rehabilitation process; Rehabilitation division was responsible for the guidance of patients on lower limb function exercise, prevention of knee joint rigidity, muscle stiffness, to promote the functional recovery of lower limbs. ResultsThe patient's wound was healed completely without scar or joint contracture. Function of lower limbs recovered to normal. Patients was very satisfied to the treatment, and had a total full recovery of body and mind, and returned to the family and society. ConclusionMultidisciplinary collaboration treatment for patients with lower extremity degloving injury can ease pain, eliminate psychological barriers, promote wound healing, and maintain the limb function.
ObjectiveTo evaluate the clinical efficacy of medical absorbent pads in the treatment of wouds. MethodsA total of 200 patients treated in the Wound Treating Center of a first-class hospital between November 2014 and August 2015 were randomly divided into trial group and control group with 100 patients in each. Patients in the trial group received medical absorbent pads, while patients in the control group received the treatment of Zetuvitò E. All patients received four times of treatment. Then we used self-made observation record form to evaluate such indexes as water absorption and compliance of the dressing, patients' pain and secondary trauma scores and the score of skins around the wounds. ResultsThere was no significant difference in water absorption property and compliance between the two groups (P>0.05). But the score of visual analogue scale was lower in the trial group than that in the control group at the fourth dressing changing (P<0.05), and the score of secondary trauma was also lower in the trial group than that in the control group at the third and fourth dressing changing (P<0.05). ConclusionThe medical absorbent pads used in the trial group can relieve pain and promote wound healing. It can be used for the treatment of wounds, and also has better effectiveness and safety.
Objective To investigate the function of a self-designed bilayered negative pressure wound therapy (b-NPWT) in reducing the occurrence of these complications through a clinical randomized controlled trial. Methods We included 72 coronary heart disease patients in our hospital from December 2013 through March 2014. There were 48 males and 24 females aged 38.4±18.6 years undergoing coronary artery bypass graft(CABG) surgery, and great saphenous veins were chosen as grafts. Patients were equally randomized into a trial group and a control group. The patients in the trial group underwent 5 d of b-NPWT for thigh incision and interrupted suture for shank incision after the harvesting of great saphenous veins. Patients in the control group received an interrupted suture for both thigh incision and shank incision after the harvesting of great saphenous veins. We evaluated the function of b-NPWT by reducing the complications arising from the harvesting of great saphenous veins in CABG patients. Results The incidences of early complications, such as lymphedema, incision infection, non-union, and skin flap necrosis of the vascular zone in the trial group were significantly lower than those of the control group. There was no death or new problem in heart or deep venous thrombosis in both groups. No complication occurred in long term. The incidence of lower limb edema was lower in the trial group than that in the control group at the end of 3 months follow-up. Conclusion B-NPWT can effectively prevent lymphorrhagia in CABG patients who underwent the harvesting of great saphenous veins. It can also reduce the incidence of complications and discomfort of the patients.
Objective To explore the knowledge distribution, knowledge clustering, and the trend in development of wound therapy, by revealing the same keywords with multiple statistical method and social network analysis. Methods We searched the CNKI under the term " wound” , " therapy” , and " wound therapy” in February 2016. After the core keywords had been identified by Bicomb and Endnote X6 software in each stage, the co-occurrence matrix was built. Transformation, dimensionality reduction and clustering of the co-occurrence matrix were finished by SPSS 22.0 software, leading the strategic plot to be built. The visualized network images were drawn using Ucinet 6.0 software. Results The visualized domain knowledge-mapping was successfully built, and it directly reflected the structure of knowledge-mapping of the discipline, as well as key clusters. Boost development had been identified in this research. The subject developed own core research areas and clusters, but there was still lack of fitting characteristics. The newly wound therapeutic techniques had limited correlation with other clusters, while provided limited contributions to forward this subject. However, enriched core keywords had been demonstrated, and formed clear domain parts of this subject. Conclusions The analysis demonstrates that wound therapy has developed well, and hot research points follow the direction of medication treatment. The network of wound therapeutic subject has become mature and completed within a short period. Comprehensive therapy and long term follow-up results according to evidence-based nursing have become the domain field. Moreover, the newly therapeutic techniques should be paid more attention to shift the development of this subject. And the interactive research within this subject and among other regions should be enhanced.
The Wound Care Center of West China Hospital of Sichuan University is motivated by professional and high-quality solutions to patients’ wound problems. It has explored and carried out a variety of wound treatment modes, such as wound care specialist clinic, wound nursing expert clinic, integrated wound multiple disciplinary team clinic and complex and difficult wound consultation. Under the multi-mode management of wound treatment, patients with all levels and all types of wounds are treated in an orderly manner, which has effectively promoted the development of wound care to a deeper level, a wider range and a higher level. This paper introduces the experience of multi-mode wound treatment in Wound Care Center from the aspects of organizational structure and treatment process, wound outpatient management, wound treatment competence matching and multi-mode operation. The purpose is to provide a reference for the management of related departments in similar hospitals.
