Objective To summarize the visual outcome of patients with severe ocular trauma treated with vitreous surgery. Methods Clinical data of 188(191 eyes) with severe ocular trauma treated with vitreous surgery in a period from November 1996 to April 1998 were analysed retrospectively. Results The study included penetrating injury in 56 eyes, foreign bodies in the posterior segment in 70 eyes, blunt injury in 41 eyes , and globe rupture in 24 eyes. Main complications included endophthalmitis in 35 eyes, choroidal bleeding in 20 eyes, retinal detachment in 60 eyes, and vitreous hemorrhage in 97 eyes. Post-opera-tively, out of 188 eyes, except for 3 of patients too young to examine, visual acuity improved in 133(70.7%), including 85(45.2%) with visal acuity 0.02-1.0, 46(24.5%) remained unchanged; and 9(4.8%) had worse vision. Among 34 with no-light-perception, 12 had light-perception or over. Conclusion A majority of severe trauma eyes can be salvaged with considerable visual recovery after adequate and timely vitreous surgery. (Chin J Ocul Fundus Dis,1999,15:4-6)
【摘要】 目的 研究千伏级锥形束CT(kV-cone beam CT,kV-CBCT)影像用于鼻咽癌调强放射治疗计划剂量计算的可行性和精确度。 方法 2010年7-9月7例鼻咽癌患者 ,获取每例患者的第1天放射治疗时的kV-CBCT影像。用CIRS062密度模体和患者自身特定区域亨氏单位值(hounsfield unit,HU)映射的两种方法重新刻度亨氏单位值-相对电子密度(HU-RED)表,分别进行剂量计算,并与在传统扇形束CT(FBCT)影像上的原放射治疗计划结果进行对比,包括辐射剂量分布、靶区和危及器官的剂量体积直方图(DVH)。 结果 kV-CBCT影像的治疗计划和原治疗计划在剂量分布和DVH上有较好的一致性。在剂量分布的比较上采用了γ分析(2%/2 mm标准的通过率),用基于模体的HU-RED表得到的治疗计划与原治疗计划对比,在经过等中心冠状面、矢状面和横断面的通过率分别为92.7%±3.5%、95.1%±3.1%和95.7%±3.4%,用基于患者的HU-RED表得到治疗计划与原治疗计划对比的通过率分别为94.8%±2.7%、96.6%±2.9%和97.4%±2.7%。DVH的统计数据表明,两种方法得到的kV-CBCT治疗计划和原治疗计划相比较,靶区和危及器官剂量偏差大多数在2%以内。有1例因在横断面发生了明显的旋转误差,导致在横断面的通过率很低,DVH统计数据较原计划偏差较大。 结论 kV-CBCT影像可以用来做辐射剂量计算,基于患者自身影像生成的HU-RED表的治疗计划较原治疗计划有更高的符合度。【Abstract】 Objective To evaluate the feasibility and accuracy of dose calculation based on cone beam CT (CBCT) data sets for intensity modulated radiation therapy (IMRT) planning of nasopharyngeal cancer (NPC). Methods Seven NPC patients were selected. The kV-CBCT images for each patient were acquired on the first treatment day. Two correction strategies were used to generate the cone beam HU value vs relative electron density calibration tables which named CIRS062 phantom based HU-RED tables and patient specific HU-RED tables respectively for dose calculation. The dose distributions and dose volume histograms (DVHs) of the target and organs at risk (OAR) based on kV-CBCT images were compared to the plans based on the fan-beam CT (FBCT). Results The DVH and dose distribution comparison between plans based on the FBCT and those on the CBCT showed good agreements. The γ analysis with a criterion of 2 mm/2% was used for the comparison of dose distribution at the coronal plane, sagital plane and cross plane through the isocenter point. The passing rate from phantom based HU-RED tables were (92.7±3.5) %, (95.1±3.1) %, and (95.7±3.4)%, respectively. The passing rates from the patient specific HU-RED tables were (94.8±2.7) %, (96.6±2.9) %, and (97.4±2.7) %, respectively. The dose difference between plans based on CBCT and those based on FBCT was within 2% at most patients by analyzing DVH based parameters. Only one patient who had significant rotation setup error resulted in the low passing rate and disagreement in DVH. Conclusion The CBCT images can be used to do dose calculation in IMRT planning of NPC. The differences between plans based on HU-RED tables generated by specific patient and the original plans are less than those between plans based on CIRS062 phantom based HU-RED tables and the original plans.
目的:对糖尿病足溃疡中面积和体积计算的方法学的临床应用进行评价。方法:广泛查阅文献,并结合我们的实际,对计算糖尿病足溃疡中面积和体积的各种方法效果进行综合分析。结果:目前常用于计算糖尿病足溃疡面积的方法有公式法、数码照像计算法、无菌薄膜勾边法等,但以数码照像计算法更准确,更易被患者接受;目前常用于计算糖尿病足溃疡体积的方法有公式法、媒介填充法、CT足部扫描法等,但以媒介填充法更经济易行。结论:数码照像计算法是目前较好的用于测量糖尿病足溃疡面积的方法,而媒介填充法是较简便易行的测量糖尿病足溃疡体积的方法。
Objective To establish different kinds of reduced size liver transplantation model of rats, and to explorethe optimal marginal size of liver graft in orthotopic liver transplantation, in purpose of providing a kind of animal modelfor the study about mechanism and prevention measures of small-for-size syndrome. Methods One hundred and ninety-two rats were randomly divided into whole liver graft transplantation group (underwent whole liver graft transplantation),half liver graft transplantation group (the median lobe and right lobe of the liver were selected to be the graft), small size liver graft transplantation group (the median lobe of the liver was selected to be the graft), and extra-small size liver graft transplantation group (the median lobe and left lobe of the liver were reduced, and remained lobes were selected to be the graft), each group enrolled 48 rats. After liver graft transplantation, 24 rats of each group were selected to observe the survival situation, 12 rats of each group were selected to measure portal venous pressure at time point of before operation,and 5, 15, 30, 45, and 60 minutes after transplantation. The other 12 rats of each group were test the level of alanine aminotransferase (ALT). Results Seven-day survival rate of the whole liver graft transplantation group, half liver graft transplantation group, small size liver graft transplantation group, and extra-small size liver graft transplantation group was 100% (24/24), 87.5% (21/24), 37.5% (9/24), and 0 respectively. Portal venous pressure of whole liver graft trans-plantation group was stable after opening the portal vein, although there was slight increase at prophase in half liver graft transplantation group, and then the portal venous pressure would let down, keeping stable at the later stage. But in small size liver graft transplantation group and extra-small size liver graft transplantation group, the portal venous pressure incr-eased and got the top at 15 minutes after opening the portal vein, and then induced, keeping stable during the 45-60 minutes.Portal venous pressure at the point of 5 (r=-0.942), 15 (r=-0.947), 30 (r=-0.900), 45 (r=-0.825), and 60 (r=-0.705)minutes after opening the portal vein were significantly related to liver graft size (P<0.001). The levels of ALT in wholeliver graft transplantation group and half liver graft transplantation group were both lower than those of small size livergraft transplantation group and extra-small size liver graft transplantation group (P<0.05), and levels of ALT in small size liver graft transplantation group was lower than extra-small size liver graft transplantation group too (P<0.05). Levelof ALT at 24 hours after transplantation were significantly related to liver graft size (r=-0.685, P<0.001). Conclusions The minimum graft volume/standard liver volume (GV/SLV) in reduced size liver transplantation in rat is 50%. The liver graft whose GV/SLV is 30%-35% should be considered as marginal size liver graft, and the liver graft whose GV/SLV less than 30% should be considered as extra-small size liver graft in the rat.
Objective To explore the correlation between liver volume variation of posthepatitic cirrhosis patients and the severity of the disease. Methods One hundred and eleven patients with normal livers and 74 posthepatitic cirrhosis patients underwent volume CT scan. The relation between normal liver volume and body height, body weight and body surface area was studied by linear regression and correlation method, the standard liver volume equation was deduced. The change ratio of liver volume in cirrhotic patients was calculated and compared with Child classification. Results The mean normal liver volume of Chinese adults was (1 225.15±216.23) cm3, there was a positive correlation between liver volume and body height, body weight 〔liver volume (cm3)=12.712×body weight (kg)+450.44〕 and body surface area 〔liver volume (cm3)=876.02×body surface area (m2)-297.17〕. The mean liver volume of Child A, B and C patients were (1 077.77±347.01) cm3, (1 016.35±348.60) cm3 and (805.73±208.85) cm3 respectively. The liver volume and liver volume index was significantly smaller in Child C patients than those in Child A and B patients (P<0.05); while liver volume change ratio was higher in Child C patients (P<0.05). Conclusion Liver volume variation of cirrhotic patients can be quantitatively assessed by 16 slices helical CT volume measurement and standard liver volume equation. The change of the liver volume is correlated with the severity of liver cirrhosis.
Objective To study the value of the clinical application of stereography and measurement of liver volume in cirrhotic patients with portal hypertension. Methods By use of the personal computer and the software of threedimensional reconstruction and measurement system of liver faultage photograph, the liver volume in vivo was successfully measured and the threedimensional image of the liver rebuilt in 46 posthepatitic cirrhotics who were selected for orthotopic liver transplantation and in 30 noncirrhotic controls, and comparison with the reference volume of recipient liver was obtained by means of water deplacement after transplantation. Results The liver volume of cirrhotic patients with portal hypertension measured by software and water deplacement was (983.33±206.11) cm3, and (904.93±209.56) cm3 respectively. Comparison by means of linear regression analysis between volume measurement on threedimensional reconstruction software and reference volume showed a nearly ideal correlation coefficient(r=0.969,P<0.01), the average error was 8.66%.The average of liver volume in controls was (1287.00±96.18) cm3, and was positively correlated to the height,weight and body surface area (r=0.845,0.833,0.932 respectively,P<0.01),and was different from that of cirrhotics. Liver volume of cirrhoitic patients with portal hypertension was related to their ChildPugh classification. The liver volume of patients in Child C group was significantly smaller than that of patients of Child B group, and was significantly correlated with Alb (r=0.496, P<0.01) and TBIL(r=-0.493, P<0.01),PT(r=-0.517, P<0.01), but was not significantly correlated with ALT(r=0.206,Pgt;0.05),portal pressure(r=-0.093,P=0.539) and portosystemic shunt index (r=0.044,P=0.769). Conclusion The volume measurement of the liver by the threedimensional software is relatively accurate. Liver volume of cirrhotic patients with portal hypertension was significantly related to their liver function,and can reflect the liver reserve function.
Objective To test the effects of large amount of blood in the vitreous on electrophysiological examination. Methods The reductions of transmission of flash light through a serial dilution and depth of whole blood were measured.An experimental model of vitreous hemorrhage in rabbits was established by injecting 0.5ml autologous uncoagulated whole blood into the vitreous cavities after compression with an expanding perfluoropropane gas bubble.Pars plana vitrectomy was performed to clear the blood clots 2 weeks after blood injection.Ganzfeld and bright-flash electroretinography were performed through six-week observation period. Results Blood reduced remarkably the transmission of reduced remarkably the transmission of bright-flash light.Massive vitreous hemorrhage had a dense filtering effect and extinguished the Ganzfeld but not the bright-flash electrotetinogram.About 3.5log units higher of the intensities of bright- flash light than that of conventional method could stimulate the responses of ERG-B waves in blood injected eyes.Slow recovery of Ganzfeld ERG-b waves after vitrectomy were noted within 2 weeks (Plt;0.05),AND ERG-b wave reached at 80-90% of normal level during the third week. Conclusion The ERG-b waves,which become undetectable because of absorption of the dense opacities of the absorption of the dense opacities of the vitreous in eyes with a large amount of vitreous hemorrhage,can be recorded in bright-flash light conditions with nearly nearly normal amplitudes.This result indicates that functions of retina were not severely damaged by the large amount of vitreous hemorrhage. The injection of large amount of blood into vitreous cavities may cause a transient reduction of the amplitudes of ERG-b waves. (Chin J Ocul Fundus Dis,1998,14:104-107)
