    远端缺血预处理可以诱导脑缺血耐受形成从而产生脑保护作用,具有很强的临床应用价值。其可通过机体不太重要的器官的缺血预处理来保护重要器官如脑、心脏、肺、小肠、胃、肝等,其脑保护机制可能与内源性一氧化氮、丝裂原活化蛋白激酶通道、Notch信号、热应激蛋白70表达、抗氧化作用、抑制神经元凋亡、神经和(或)体液因素等有关。现对远端缺血预处理对脑缺血保护作用相关机制的研究现状作一综述。

  • The cognition of intraspinal labor analgesia of women in labor and its influencing factors

    Objective To investigate and analyze the cognition of intraspinal labor analgesia (ILA) of women in labor and its influencing factors. Methods A total of 322 women in labor in West China Second University Hospital of Sichuan University between May and September 2015 were investigated by questionnaire, which included the general situation, the pregnancy status, the understanding for labor pain, and the cognition of ILA. Investigator introduced the knowledge of ILA to them after the investigation, and then they were re-investigated for the choice of ILA. Results Only 22.67% of these women in labor knowed ILA clearly, 53.42% heared it but did not understand, and 49.38% liked to use it. The proportion of women in labor who liked to use it increased to 81.89% after introduction, and the difference was significant (P<0.001). Among all factors, the acknowledgement level of ILA and the choice to use it were mainly related to the family awareness concept for labor pain. Conclusions The cognition of ILA of women in labor is not enough, and their using desire is not strong. The family awareness concept is one of the important factors. The education and advertise to the women in labor and their family members after admiting to labor room can increase the choice rate of ILA significantly.

  • Changes in the disease burden of ischemic stroke in China from 1990 to 2019

    Objective To systematically review the epidemic trend and disease burden of ischemic stroke in the Chinese population and to provide references for formulating reasonable prevention and treatment measures and allocating health resources. Methods Based on Global Burden of Disease (GBD) data, we analyzed the morbidity, mortality, disability-adjusted life year (DALY) and normalized rates for ischemic stroke in China from 1990 to 2019 and evaluated the changes in the disease burden by sex and age group. Meanwhile, joinpoint regression model was constructed to analyze the time trend change in each stage during the study period. Results Compared with 1990, the incidence, mortality and DALY rate of ischemic stroke in China increased by 171.68%, 125.60% and 98.60% in 2019, among which the incidence, mortality and DALY rate of males increased by 184.29%, 148.96% and 115.16%, respectively; the morbidity, mortality and DALY rates of females increased by 160.9%, 101.32% and 81.44%, respectively. The age-standardized incidence increased by 34.70%, while the age-standardized mortality and age-standardized DALY rate decreased by 3.33% and 4.02%, respectively; the age-standardized incidence, mortality and DALY rates of males increased by 39.52%, 8.03% and 3.68%, respectively; the age-standardized incidence rate of females increased by 31.40%, while the age-standardized mortality rate and age-standardized DALY rate decreased by 14.02% and 11.53%, respectively. In 2019, both the mortality rate and DALY rate due to ischemic stroke increased with age, and the highest rate was found in the population over 85 years old. Males over 60 years old were significantly than females. In the 55-84 age group, the incidence of ischemic stroke in females was higher than that in males, while in the 85 and above age group, the incidence of ischemic stroke in females was lower than that in males. The AAPC of age-standardized incidence, age-standardized mortality, and age-standardized DALY rates due to ischemic stroke from 1990 to 2019 were 1.06% (95%CI 1.00% to 1.11%), 0.01% (95%CI −0.45% to 0.48%) and −0.16% (95%CI −0.53% to 0.22%), respectively. All indicators of the AAPC for males were higher than those for females. ConclusionThe curvent age-standardized mortality and DALY rate of ischemic stroke in China have decreased slightly compared with 1990. The crude mortality, morbidity and disease burden have significantly increased. All indicators of the AAPC for males were higher than those for females. To reduce the epidemic trend and disease burden of ischemic stroke, reasonable prevention and treatment measures and rational allocation of health resources should be made according to sex and age.

  • Older adult injury mortality in China: a meta-analysis

    Objective To systematically review injury, death, and their causes in elderly people in China from 2000 to 2020 and to prevent and reduce the occurrence of injuries and death. Methods The CNKI, VIP, WanFang Data, PubMed, SinoMed, and Web of Science databases were searched to collect studies on injury and death among elderly people over 60 years of age who resided in China from January 2000 to December 2020. Two reviewers independently screened the literature, extracted data, and assessed the risk of bias of the included studies. A meta-analysis was then performed using R 4.1.2 software. Results A total of 41 studies with 187 488 subjects were included, including 125 million elderly individuals. The pooled injury mortality rate was 135.58/105 (95%CI 113.36/105 to 162.14/105, P<0.001). Subgroup analysis showed that male injury death (146.00/105, 95%CI 116.00 to 183.74, P=0.001) was significantly higher than that of females (127.90/105, 95%CI 102.31 to 159.88, P=0.001) and that overall injury mortality increased exponentially with age (R2=0.957), especially in those over 80 years old. The spatial distribution showed that the injury death rate in the central region was higher than that in the east and west and higher in the countryside than in the city. The time of death distribution showed that after China became an aging society (2000-2020), the time of death was significantly later than before (1990-2000). There were more than 12 types of injuries that caused death, the top three of which were falling, traffic accidents, and suicide. Conclusion From 2000 to 2020, the injury mortality rate of the elderly people in China initially increase and then slightly decrease. The phenomenon affects more men than women, especially those beyond the age of 80. Regional differences are identified, and the types of injuries that cause death are mainly falls, traffic accidents, and suicide. Due to the limited quantity and quality of the included studies, more high-quality studies are needed to verify the above conclusion.
