  • 特发性黄斑裂孔手术后黄斑形态功能评估研究现状与进展

    随着黄斑区形态、功能检查手段不断丰富,特发性黄斑裂孔(IMH)手术后效果评估也愈加全面。光相干断层扫描用于IMH手术后视网膜分层结构细微变化观察、黄斑中心凹位置以及视网膜体积测量,发现外界膜、光感受器内外节连接、锥体外节尖端等外层视网膜结构的连续性以及黄斑中心凹位置和视网膜体积变化与手术后视力及视敏度改善有一定相关性。眼底自身荧光(FAF)强弱除了可直观反映裂孔闭合状态外,也与视功能密切相关。FAF呈强荧光患眼较正常荧光或弱荧光患眼手术后视力、外层视网膜连续性均差。自适应光学激光扫描检眼镜能够分辨个体视锥细胞异常,由此得到的中心凹中央处暗区像素与总体像素比值这一锥细胞丢失指数可以作为衡量锥细胞丢失水平的客观标准。锥细胞丢失指数越高,手术后视力、视敏度越差。深入了解IMH玻璃体切割手术后黄斑形态、功能改变及其之间的关系,有助于提高对IMH愈合状态的认识,促进IMH玻璃体切割手术治疗技术进步。

  • Effects of cytokines on collagen synthesis in human retinal pigment epithelial cells

    Objective To investigate the effects of transformin growth factor-beta (TGF-beta;) and interferon-gamma(IFN-gamma;)on collagen synthesis in human retinal pigment epithelial cells(RPE). Methods TGF-beta;(0.01~10 ng/ml),recombinant IFN-gamma;(100~10000 U/ml)or a combination of two were added to cultures of RPE and collagen synthesis of the cells were measured by3 H-proline incorporation assay,indirect immunofluorescence staining and dot-blot hybridization. Results TGF-beta; at 10 ng/ml increased cell uptake of 3 H-proline to 130.87% of controls.It intensified Type IV,I and Ⅲ collagen fluorescent staining as well as mRNA expression.IFN-gamma; at 10000 U/ml caused 54.72% inhibition of 3 H-proline uptake by RPE,and decreased TypeⅣ collagen fluorescent staining as well as mRNA expression of Type Ⅳ,I and Ⅲ collagens. Conclusion TGF-beta; and IFN-gamma; stimulated and inhibited collagen synthesis of human RPE,respectively.The combination of two had antagonistic effects.IFN-gamma; can be used for inhibition of collagen synthesis of RPE.   • Radioimmunoassay of proco!!agen m in the vitreous of rabbit eyes with experimental proliferative vitreoretinopathy

    Objective:To detect collagen I synthesis activity in the vitreous of PVR induced by macrophages in rabbits. Methods:PC Ⅲ (Procollagen Ⅲ ) concentrations were measured by radioim- munoassay in the vitreous samples of 14 rabbit eyes with experimental PVR and 14 control eyes. Results:The mean PC Ⅲ concentration on the 7th day after macrophage injection as 257.58mu;g/L(range,236.04~266.88mu;g/L,n= 4)and significantly increased on the 14th day later. On the 28th day the mean concentration of PC Ⅲ as 912.23mu;g/L (range, 881.36~943.10mu;g/L ;n= 2). There was a significant difference between the 7th and the 14th, 21st of 28th day statistically(P<0.05). PC Ⅲ was not detected in control eyes. Conclusion:The PC Ⅲ level in the vitreous of rabbit eyes with experimental PVR increased significantly from the 7th to the 28th day after macrophages injection and is well consistent with the time course of scarring and the development of traction retinal detachment in the PVR model.   • Retinal vein occlusion with vitreous hemorrhage,neovascular membranes and traction retinal detachment treated with vitreous surgery and endolaser

    Purpose To evaluate the efficacy of vitreous surgery and endolaser in a series of patients with retinal vein occlusion(RVO)with vitreous hemorrhage,neovascular membranes(NVM) and/or traction retinal detachment(TRD). Methods Clinical records were reviewed on 37 consecutive patients(38 eyes)who underwent vitreous surgery and endolaser for RVO with persistent vitreous hemorrhage,NVM and/or TRD.There were 19 patients(20 eyes)with retinal branch vein occlusion (BRVO)and 18 patients(18 eyes)with central retinal vein occlusion(CRVO). Results NVM and TRD were confirmed during operation in 27 and 23 eyes,respectively.Visual acuity improved postoperatively in 34 eyes(89.5%)including 22 eyes with 0.1 or better vision,and 4 eyes remained unchanged.CRVO group had longer history and less visual improvement after surgery. Conclusions Vitreous surgery and endolaser photocoagulation can improve the outcome in the majority of patients with RVO with vitreous hemorrage,NVM and/or TRD. (Chin J Ocul Fundus Dis,1998,14:3-6)

  • Combination therapy for thick submacular hemorrhage

    ObjectiveTo observe the efficacy and safety of vitrectomy combined with submacular injection of tissue-type plasminogen activator (t-PA), gas filling and anti-VEGF drugs (multiple therapy) for thick submacular hemorrhage.MethodsA retrospective case study. From January 2014 to June 2018, 24 patients (24 eyes) with thick submacular hemorrhage who received multiple therapy at the Department of Ophthalmology of Peking University Third Hospital were included in the study. Among them, there were 15 males and 9 females with the average age of 69.05±8.86 years. The average submacular bleeding time was 17.15±10.30 days, the average bleeding area was 13.85±8.63 DD. Seventeen eyes showed hemorrhagic RPE detachment. The international standard visual acuity chart was used to BCVA examination, which was converted to logMAR visual acuity in statistics. The frequency domain OCT was used to measure the height of the foveal elevation. The average logMAR BCVA of the affected eye was 1.37±0.38. The average height of the macular fovea was 824.94±294.38 μm. All eyes underwent 23G or 25G vitrectomy. During the operation, 0.1-0.5 ml t-PA (10 μg/0.1 ml) was injected under the macula. The vitreous cavity was filled with 15% C3F8 after gas-liquid exchange in 13 eyes, and 11 eyes were filled with sterilized air. Eleven eyes were injected with anti-VEGF drugs at the end of the operation, and anti-VEGF drugs were given PRN treatment according to the activity of the lesion. The average follow-up time after treatment was 27.90±22.21 months. The absorption of bleeding under the macula, the improvement of vision, the occurrence of rebleeding and treatment-related complications were observed and recorded. The Wilcoxon rank sum test was performed to compare the BCVA and the height of foveal elevation before and after treatment.ResultsOne month after the treatment, the blood in the fovea of all affected eyes was cleared. At the last follow-up, the logMAR BCVA and macular foveal elevation were 0.82±0.28 and 253.88±71.75 μm, respectively. Compared with those before treatment, the difference was statistically significant (Z=-3.727, -3.234; P<0.001, <0.001). The average intravitreal injection of anti-VEGF drugs was 1.08 times. During the operation, a tiny hole was formed in the center of the macula when t-PA was injected under the retina. Two eyes showed mild vitreous hemorrhage early after the operation. During the follow-up period, bleeding recurred in 2 eyes.ConclusionsVitrectomy combined with submacular injection of t-PA, gas filling, and anti-VEGF drugs is an optimal combination for the treatment of thick submacular hemorrhage. It can effectively remove submacular hemorrhage, improve vision, reduce foveal elevation with good safety.
