目的 探讨严重肝损伤的有效治疗方法。方法 回顾性分析我院1996~2002年期间43例严重肝损伤患者的外科治疗方法。结果 术后治愈32例(74.4%),死亡11例(25.6%),其中10例死于失血性休克、多脏器功能衰竭,1例死于术后急性大出血、心跳骤停,术后9例发生并发症。结论 外伤性肝损伤应及时手术止血,同时对严重的合并伤进行处置,手术方式应根据伤者具体情况作出正确选择。
Objective To explore the clinical value of early enteral nutrition in severe acute pancreatitis (SAP) by percutaneous endoscopic gastrostomy/jejunostomy (PEG/J).Methods Treatment condition of nighty patients with SAP were retrospectively analysed.The 90 patients were collected peripheral venous blood respectively on 1, 12, and 18 d after admission to hospital.Forty-five of them were in PEG/J group, the others were in control group. Serum IL-6,TNF-α and endotoxin were detected by enzyme-linked immunosorbent assay (ELISA),CD4 /CD8 was determinated by indirect immunofluorescence staining method (FITC-labeled).Results On 12 d and 18 d,the levels of serum IL-6, TNF-α, and endotoxin in PEG/J group were lower than those in control group (P<0.01).The CD4 /CD8 was significantly higher than that in control group (P<0.01).In control group, 2 cases complicated upper gastrointestinal haemorrhage,4 cases complicated pancreatic pseudocysts, and 2 cases complicated double infection, the temperature became normal after about 13.5 d.In PEG/J group, there were not upper gastrointestinal haemorrhage and double infection,but 2 cases also complicated pancreatic pseudocysts, the temperature became normal after about 10.5 d.Conclusion The clinical effectiveness of early enteral nutrition in SAP by PEG/J is satisfactory.
Objective To study the effect of Fe 3+ -modified carborymethyl celluiose (Fe 3+ -CMC ) on preventing postoperative adhesion and inhibiting the expressions of tumor necrosis factor-α (TNF-α) and fibroblast growth factor (FGF) in the injured parts of postoperative peritoneum. Methods Fourty Wistar mice were divided into 2 groups randomly, and abdominal adhesion models were made, then 0.9% NaCl (control group) and Fe 3+ -CMC (experimental group) were sprayed into the wound surface of abdominal cavity. All mice were killed to observe the adhesion condition on day 14 after operation. Another 120 Wistar mice were divided into 2 groups randomly, and abdominal adhesion models were made as mentioned above. Ten mice were killed which were chosen randomly from 2 groups on day 1, 3, 5, 7, 14 and 60, respectively. The expressions of TNF-α and FGF in the peritoneal injured and adhesion tissues were observed by immunohistochemistry technique. Results The adhesion grade in experimental group was much lower than that in control group ( P < 0.01). The expression of TNF-α (day 3-7 after operation) and FGF (day 5-7 after operation) in experimental group were lower than those in control group ( P < 0.05).Conclusion Fe 3+ -CMC can decrease postoperative adhesion grade and prevent the expressions of TNF-α and FGF in injured parts of postoperative peritoneum.
ObjectiveTo investigate the regularity of myelin degeneration and regeneration and the difference of axonal density between tibial nerve and common peroneal nerve after sciatic nerve injury repair in rhesue monkey. MethodsNine adult rhesue monkeys (male or female, weighing 3.5-4.5 kg) were selected to establish the model of rat sciatic nerve transaction injury. The tibial nerve and common peroneal nerve of 5 mm in length were harvested at 5 mm from injury site as controls in 3 monkeys; the distal tibial nerve and common peroneal nerve were repaired with 9-0 suture immediately in the other 6 monkeys. And the gross observation and neural electrophysiological examination were performed at 3 and 8 weeks after repair respectively. Then, distal tibial nerve and common peroneal nerve at anastomotic site were harvested to observe the myelin sheath changes, and to calculate the number of axon counts and axonal density by staining with Luxol Fast Blue. ResultsAtrophy of the lower limb muscle and various degrees of plantar ulcer were observed. Gross observation showed nerve enlargement at anastomosis site, the peripheral connective tissue hyperplasia, and obvious adhesion. The compound muscle action potential (CMAP) of tibial nerve and common peroneal nerve could not be detected at 3 weeks; the CMAP amplitude of common peroneal nerve was less than that of the tibial nerve at 8 weeks. Different degrees of axonal degeneration was shown in the tibial nerve and common peroneal nerve, especially in the common peroneal nerve. The average axonal density of common peroneal nerve was lower than that of tibial nerve at 3 weeks (13.2% vs. 44.5%) and at 8 weeks (10.3% vs. 35.3%) after repair. ConclusionThe regeneration of tibial nerve is better and faster than that of common peroneal nerve, and gastrocnemius muscle CMAP recovers quicker, and amplitude is higher, which is the reason of better recovery of tibial nerve.
