  • Clinical Application of Binding Pancreaticojejunostomy on Prevention of Pancreatic Fistula after Pancreatoduodenectomy

    目的 探讨捆绑式胰肠吻合术在预防胰十二指肠切除术后胰肠吻合口漏的可行性及临床应用价值。 方法 我院在2002年3月至2006年10月期间对32例胰十二指肠切除术患者采用捆绑式胰肠吻合术。结果 32例患者手术均顺利,无一例发生胰瘘,无手术死亡病例。 术后发生胆瘘2例,经腹腔引流治愈。结论 捆绑式胰肠吻合术操作简便,预防胰瘘效果可靠,值得推广。

  • Changes of Platelet MKK3 Phosphorylation Level in Acute Respiratory Distress Syndrome Rats

    Objective To explore the molecular mechanism of pathogenesis and signal pathway of platelet activation in acute respiratory distress syndrome (ARDS). Methods Thirty healthy Sprague-Dawley rats were randomly divided into 5 groups. Four groups were intravenously injected with oleic acid (OA, 0.25 ml/kg) to establish ARDS rat model. One group was intravenously injected with normal saline (NS) in same dose as control group. After injection of oleic acid for 2 h, 6 h, 24 h, 72 h in four OA groups, and injection of saline for 2 h in the control group, the rats were sacrificed. Blood was sampled from the abdominal aorta, then platelets were separated for abstracting platelet protein. The mitogen-activated protein kinase kinase 3 (MKK3) phosphorylation level in platelet was detected by Western blot method, to explore the changes of platelet mitogen activated protein kinase (MAPKs) signal transduction pathway in ARDS, and the relationship between the changes and the pathogenesis of ARDS. Results Platelet MKK3 phosphorylation level significantly increased 6-72 h after injection of oleic acid (P<0.05). It was 2.4 times that of the control group in 6 h group (0.50±0.09vs. 0.21±0.05), peaked and 3.7 times that of the control group in 24 h group (0.78±0.06), then fell slightly but still significantly higher than the control group in 72 h group (0.75±0.13). Conclusion The activation process of platelets is related with MKK3-p38 MAPK signaling pathway in ARDS.

  • Application of pancreatic duct stent in common pancreas diseases

    ObjectiveTo investigate the application scope and effects of pancreatic duct stent in common pancreas diseases.MethodThe literatures on application of pancreatic duct stent in common pancreas diseases at domestic and foreign were reviewed.ResultsPancreatic duct stent had been used in acute and chronic pancreatitis and pancreatic tumor, which could the relieve symptoms, reduce the complications of pancreatitis, improve the prognosis, shorten the hospital stay, and reduce the hospitalization costs. Meanwhile it had also been used in pancreatic tumor surgery, palliative treatment, and brachytherapy that some achievements had been made.ConclusionAlthough application of pancreatic duct stent in some common pancreatic diseases is still controversial, effect is better and has some advantages with small trauma and low incidence of complications, which also provides a new treatment option for some common pancreatic diseases.

  • Research of Continuous Regional Arterial Infusion of Dexamethasone Treatment for Severe Acute Pancreatitis

    ObjectiveTo study the effects of continuous regional arterial infusion (CRAI) of dexamethasone on plasma inflammatory factors of severe acute pancreatitis (SAP) rabbits. MethodsTwentyfour rabbits were randomly divided into sham operation group, SAP group, intravenous infusion of dexamethasone group and CRAI of dexamethasone group (each group 6 rabbits) by random number table. The serum tumor necrosis factor (TNF)-α, interleukin (IL)-6, IL-10, and amylase (AMY) levels in rabbits were tested at hour 0.5, 3, 6, 9, and 12 after modeling succeed. The pathological changes of pancreas and the survival were observed on day 3 after modeling succeed. ResultsCompared with the sham operation group, the serum levels of IL-6 significantly increased at 3 h and reached the peak at 6 h, decreased at 9 h (all Plt;0.05); levels of IL-10 significantly increased at 6 h, continuously elevated at 9 h and 12 h (all Plt;0.001); levels of TNF-α significantly increased at 0.5 h (Plt;0.001), reached the peak at 6 h (Plt;0.001) and decreased at 9 h (Plt;0.05); levels of AMY significantly increased at 9 h, continuously elevated at 12 h (all Plt;0.05) in the SAP group. Compared with the SAP group, the serum levels of IL-6 and IL-10 in the CRAI of dexamethasone group all significantly decreased at 6 h, 9 h, and 12 h (Plt;0.001); levels of IL6 significantly decreased only at 6 h in the intravenous infusion of dexamethasone group; levels of TNF-α in the CRAI of dexamethasone group significantly decreased at 3 h, 6 h, 9 h, and 12 h (all Plt;0.001), which in the intravenous infusion of dexamethasone group significantly decreased only at 6 h (Plt;0.05); levels of AMY in the CRAI of dexamethasone group and intravenous infusion of dexamethasone group all significantly decreased at 12 h (Plt;0.05). Compared with the intravenous infusion of dexamethasone group, the serum levels of IL-6 and IL-10 in the CRAI of dexamethasone group all significantly decreased at 6 h (Plt;0.05) and 12 h (Plt;0.001); levels of TNF-α all significantly decreased at 3 h, 6 h, 9 h, and 12 h (all Plt;0.001); levels of AMY were not significantly different (Pgt;0.05). The pathological changes of pancreas in the CRAI of dexamethasone group were obvious, the death of rabbits reduced on day 3 after modeling succeed. ConclusionCRAI dexamethasone can effectively reduce the systemic inflammatory response and pancreatic inflammation, and reduce mortality.

  • Effect of Early Coronary Artery Bypass Graft Surgery on Expression Level of Ionophorous Protein at Infracted Border Zone in Dog with Acute Myocardial Infarction

    ObjectiveTo investigate the effect and significance of early coronary artery bypass graft (CABG) on the expression level of ionophorous protein at infracted border zone (IBZ) in dog with acute myocardial infarction. MethodsThe anterior descending coronary artery of all thirty healthy mongrel dogs were ligated into myocardial infarction model, whose successful criteria was that the regional myocardium supplied by ligated coronary artery became darker. Coronary artery bypass surgery performed at different time points after myocardial infarction (in the 1st week, the 2nd week, the 4th week, the 6th week respectively) was as an experimental group. While myocardial infarction without coronary artery bypass surgery was set up as a control group. Myocardial tissue without ligation of coronary artery was as a normal group. After 8 weeks, myocardial specimens were cut out in the experimental group and the control group. The local expression levels of ionophorous proteins such as Cav1.2, Kv4.3 and KchIP2 mRNA were detected by means of reverse transcription- polymerase chain reaction (RT-PCR) at normal myocardium and IBZ of the experimental group and the control group. ResultsFour dogs in every experimental group and all dogs in the control group survived to the end of the study. Three myocardial ion channel proteins expression in the control group were lower than those of the normal group or the experimental group significantly (P<0.01). Cav1.2 mRNA expression in the experimental group in the 4th week or the 6th week was lower than that in the normal group significantly (P<0.05). Kv4.3 and KchIP2 mRNA expression in the experimental group in the 4th week or the 6th week were lower than those in the normal group and the experimental group significantly in the 1st week or the 2nd week (P<0.05). ConclusionEarly CABG surgery for acute myocardial infarction could lessen the changes of expression level of ionophorous protein at infracted border zone (IBZ) of dog with acute myocardial infarction. Especially, CABG surgery among two weeks could improve expression level of ionophorous protein, and reduce the effect of ischemia for ionophorous protein and myocardial electrophysiology at IBZ.
