  • Nuclear Factor Kappa B and Liver Injury

    Objective To summarize the role of nuclear factor kappa B (NF-κB) in the occurrence and progression of various sorts of liver injury. Methods Literatures on the structures, property of molecular biology and function of NF-κB, as well as its relationships with liver injury were collected and reviewed. Results NF-κB was an important nuclear factor existed in cells widely distributed in most cell types. The activation of NF-κB was induced by various sorts of liver injury. The activated NF-κB could affect the liver injury by regulating cytokines, adhesion molecules, and activating factor involving in immunologic reaction, inflammatory reaction and the apoptosis. Conclusion  NF-κB plays an important role during the occurrence and progression of liver injury, and may become a new target in the treatment of liver injury.

  • Cyclooxygenase-2 Expression and Its Correlation with Vascular Endothelial Growth Factor in Pancreatic Adenocarcinoma

    ObjectiveTo investigate the expression of cyclooxygenase-2 (COX-2) and vascular endothelial growth factor (VEGF) in human pancreatic adenocarcinoma and their correlation with clinicobiological behavior.MethodsThe expression of COX-2 and VEGF in 51 cases of human pancreatic ductal adenocarcinoma were detected with immunohistochemistry of Envision.ResultsExpression of COX-2 and VEGF in pancreatic ductal adenocarcinoma were 74.5% and 68.6%, respectively; no expression of COX-2 and VEGF in adjacent normal tissue was detected. Both COX-2 and VEGF expression in clinical stage Ⅲ-Ⅳ were much higher than those in clinical stage Ⅰ-Ⅱ, and also higher in positive group of lymph node metastasis than in negative group as well (Plt;0.05). None of them had relation with histological grades, age, sex, tumor size and location. The expression of COX-2 was closely correlated with VEGF (r=0.411, Plt;0.01).ConclusionCOX-2 and VEGF may play a pivotal role in tumorigenesis and tumor progression in pancreatic cancer, they may provide new targets for therapy of pancreatic cancer.

  • Effects of Emodin on the Expression of Hypoxia Inducible Factor-1α Protein in Rats with Severe Acute Pancreatitis-associated Renal Injury

    ObjectiveTo investigate the effect of emodin on the expression of hypoxia inducible factor (HIF)-1α protein in rats with severe acute pancreatitis-associated renal injury and explore the possible mechanisms. MethodsA total of 72 rats were randomly divided into sham-operated group (n=24), severe acute pancreatitis with renal injury group (injury group, n=24), and treatment group (n=24). The sham-operated and injury groups were given 1.5 mL saline through intragastric administration before operation while the treatment group was fed with the same amount of 50 mg/kg emodin diluent. The pancreas and pancreatic tail-segment was dissociated and the head of pancreas was occluded in rats to form the model, and blood vessel forceps were loosed after three hours. All the rats were sacrificed 12, 24 and 36 hours after modeling. The level of ascites, serum amylase, creatinine, blood urea nitrogen were detected. Hematoxylin-eosin staining was used to observe the pancreatic and renal pathological changes, and immunohistochemical method was used to detect the expression of HIF-1α protein level in the kidney. ResultsCompared with the sham-operated group, the level of ascites, serum amylase, creatinine, blood urea nitrogen and the expression of HIF-1α protein level increased significantly. The tissue damage of pancreas and the kidney became more serious. Compared with the injury group, the kidney and pancreas function of the treatment group had a better performance. HIF-1α protein level significantly increased in the treatment group, and the difference had a statistical significance (P<0.05). ConclusionEmodin has a good protective effect on severe acute pancreatitis-associated renal injury. It may function through up-regulation expression of HIF-1α protein level to improve the ability of the kidney to tolerate hypoxia, and then reduce the cell apoptosis and necrosis of the kidney.

  • 非手术治疗胰腺颈部断裂伤 1 例并相关文献复习

    目的 提高胰腺颈部断裂伤治疗的新认识。 方法 对笔者所在医院收治的 1 例胰腺颈部断裂伤患者的临床资料进行分析并复习相关文献。 结果 患者经过禁饮食,持续胃肠减压,质子泵抑制剂抑制胃酸分泌,静脉补充水、电解质,广谱抗生素抗感染,静脉泵入生长抑素抑制胰液分泌,行肠外营养支持,生大黄泡水后低压灌肠,肠道功能恢复后经鼻空肠营养管行肠内营养支持,然后行经皮胰周穿刺置管引流治疗,患者病情好转趋于愈合。出院后 3 个月随访,腹腔引流管引流液量为 5~10 mL/d,患者可以经口进饮食,无发热、腹痛、腹胀等临床症状。 结论 胰腺颈部断裂伤除了可以采用手术治疗外,在患者身体状况平稳的条件下也可以选择非手术治疗。

  • Protective and Therapeutic Effects of Curcumin Combined with Rhodiola on Rats with Severe Acute Pancreatitis Associated Renal Injury

    ObjectiveTo investigate the effect of Curcumin combined with Rhodiola on rats with severe acute pancreatitis (SAP) associated renal injury and explore the possible mechanisms. MethodsA total of 24 rats were randomly divided into SAP with renal injury group (SAP group, n=8), Curcumin group (n=8), Curcumin combined with Rhodiola group (n=8).The SAP group was given 1.5 mL saline through intragastric administration before operation while the Curcumin group was fed with same amount of Curcumin diluent.The Curcumin combined with Rhodiola group was given 1.5 mL Curcumin diluent through intragastric administration and 6 g/kg Rhodiola diluent through intraperitoneal injection before operation.The pancreas and pancreatic tail-segment was dissociated and the head of pancreas were occluded in rats to make the model, blood vessel forceps was loosed after three hours.All the rats were sacrificed at 18 h after modeling.The levels of serum amylase, creatinine, blood urea nitrogen were detected and pathological changes of pancreas and the left kidney were observed under the light microscope.The cell apoptosis was analyzed using TUNEL staining.The serum levels of interleukin (IL)-1β, IL-6, and IL-10 among the three groups were detected by enzyme-linked immunosorbent assay.The expression of inducible nitric oxide synthase (iNOS) mRNA in the right kidney was detected by real-time polymerase chain reaction.The superoxide dismutase (SOD) activity of the renal tissue was determined by hydroxylamine method. ResultsCompared with the SAP group, the levels of serum amylase, creatinine, blood urea nitrogen, IL-1β, IL-6, the cell apoptosis index, and the expression of iNOS mRNA were significantly decreased, the serum level of IL-10 and the activity of SOD were significantly increased (P < 0.05), the pancreas and the kidney damaged more slightly in the Curcumin group and Curcumin combined with Rhodiola group.Compared with the Curcumin group, the above situations were more better in the Curcumin combined with Rhodiola group. ConclusionsCurcumin combined with Rhodiola has a better protective effect on SAP associated renal injury.It might be through inhibiting the expressions of IL-1β, IL-6, stimulating the expression of IL-10, down-regulating the expression of iNOS mRNA, and improving the activity of SOD.It could reduce the cell apoptosis and necrosis of the kidney and improve the ability of the kidney to tolerate hypoxia.

  • Changes of Peripheral Blood Lymphocyte Subsets of Patients with Primary Hepatocellular Carcinoma Treated with Radiofrequency Ablation

    ObjectiveTo observe the changes of peripheral blood lymphocyte subsets of patients with primary hepatocellular carcinoma (PHCC) treated with radiofrequency ablation (RFA). MethodsThe data of 70 cases of hospitalized patients with PHCC that voluntary accepted RFA diagnosed by clinical and pathological in our hospital between July 2011 and December 2014 were collected. According to the numbers of HCC lesions, 70 cases were divided into single focus group (n=41) and multifocal group (n=29). The changes of their immune parameters before and after RFA were analyzed. Results①The ratioes of peripheral blood CD3+/CD19-, CD3+/CD4+, CD4+/CD8+, and NK cells on 7 days and 14 days after RFA treatment of 70 cases were significantly higher than those on 1 day before RFA treatment (P < 0.05). The ratio of CD3+/CD8+ T cells reduced from 1 day before RFA treatment to 14 days after RFA treatment, but the difference was no statistically significant (P > 0.05).②The changing trend of peripheral blood lymphocyte subsets before and after RFA treatment in single focus group and multifocal group were similar to the above.③Compared with single focus group, the ratioes of peripheral blood CD3+/CD19-, CD3+/CD4+, CD4+/CD8+, and NK cells before and after RFA treatment in multifocal group were lower, and the ratio of CD3+/CD8+ T cells was higher, but the difference were not statistically significant (P > 0.05). ConclusionRFA can not only destroy small PHCC foci, but also to significantly improve immune function and enhance the anti-tumor effect.

  • Relation Between Clinicopathologic Factors and Prognosis of Pancreatic Head Cancer Following Radical Resection

    ObjectiveTo investigate the correlation with clinicopathologic factors and prognosis in pancreatic head cancer following radical resection. MethodsThirteen-eight patients with primary pancreatic head cancer admitted into Urumuqi General Hospital of Lanzhou Military Region from January 2000 to December 2007 were treated by radical resection, the clinicopathologic factors were retrospectively analyzed, the SPSS 17.0 analysis software was used to analyze the relation between clinicopathologic factors and prognosis. Results①The follow-up rate was 81.6%(31/38), the 1-and 2-year overall survival was 38.7% and 12.9%, respectively.②Spearman analysis showed that the distant metastasis was significantly correlated with the tumor size(rs=0.421, P=0.008)and TNM stage(rs=0.746, P=0.000); the lymph node metastasis was significantly correlated with TNM stage(rs=0.543, P=0.001)and was not correlated with tumor differentiation degree(rs=0.074, P=0.695), the tumor size was also significantly correlated with TNM stage (rs=0.475, P=0.003).③Univariate Chi-Square test revealed that the gender(P=0.045), tumor size(P=0.004), tumor differentiation degree(P=0.003), TNM stage(P=0.000), distant metastasis(P=0.002), and lymph node metastasis(P=0.001)were correlated with the prognosis of pancreatic head cancer. However, when the interactive effects of those factors were taken into account, TNM stage(P=0.001), differentiation degree(P=0.003), and lymph node metastasis(P=0.007)were selected as the most significant prognostic factors in a multivariate analysis by using the Cox proportional hazard regression model.④The survival rate of patients with low differentiation degree was significantly shorter than that of moderate/high differentiation degree(P=0.003), of patients withⅠ+Ⅱstages was significantly longer than that ofⅢ+Ⅳstages(P=0.000), of patients with distant metastasis or lymph node metastasis was significantly lower than those patients without distant metastasis or lymph node metastasis(P=0.002, P=0.001). ConclusionsThe lymph node metastasis is significantly associated with TNM stage, and is not associated with differentiation degree. The lymph node metastasis, differentiation degree, and TNM stage are related with survival, and they are also independent prognostic factors of pancreatic head cancer.

  • Effects of Curcumin on Expression of iNOS and Investigation The Protective Effects on Rats with Severe Acute Panceratitis Associated Renal Injury

    ObjectiveTo investigate the effects of curcumin on expression of iNOS mRNA, study the protective and therapeutic effects on rats with severe acute panceratitis associated renal injury, and explore the possible mechanisms. MethodsA total of 24 rats were randomly divided into control group (n=8), severe acute panceratitis with renal injury group (injury group, n=8), and curcumin-treated group (treated group, n=8).The rats of control group and injury group were given 1.5 mL saline through intragastric administration at 3 h before operation, while the rats of treated group were fed with same amount of curcumin diluent at 3 h before operation.The rats of injury group and treated group with pancreatic head clamping method to establish the model of severe acute pancreatitis.At 12 h after modeling, rats in control group and injury group were perfused with 1.5 mL saline, and rats in treated group were intragastrically administrated to the same volume of curcumin diluent.All the rats were sacrificed at 18 h after modeling.The levels of serum amylase, creatinine, and blood urea nitrogen were detected and pathological chang of pancreas and the left kidney were observed under light microscope.The renal cell apoptosis were analyzed using TUNEL staining in three groups.The serum levels of interleukin-1β(IL-1β), interleukin-6 (IL-6), and interleukin-10 (IL-10) in three groups were detected by enzyme-linked immunosorbert assay (ELISA).The expressions of inducible nitric oxide synthase (iNOS) mRNA in the right kidney were detected with realtime polymerase chain reaction (RT-PCR) in three groups. ResultsCompared with the control group, the levels of serum amylase, creatinine, blood urea nitrogen, IL-1β, IL-6, and IL-10, the cell apoptosis and the expressions of iNOS mRNA in injury group and treated group were significantly increased (P < 0.05).In the treated group, the level of IL-10 was higher than the injury group (P < 0.05), and the difference of other indexes were lower than the injury group (P < 0.05). ConclusionsCurcumin has a better protective effect on severe acute pancreatitis associated renal injury.It may be through inhibited the expressions of IL-1βand IL-6, stimulated the expression of IL-10, and down-regulation of iNOS mRNA expression to reduce the generation of oxygen free radicals and NO damage to cell and reducing the cell apoptosis and necrosis of kidney.

  • Radiofrequency Ablation in Treatment for Hepatocellular Carcinoma(Report of 58 Cases)

    目的 探讨射频消融术在无法实施手术的原发性肝癌患者中的临床应用效果。方法 回顾性分析2007年9月至2011年3月期间我院收治的58例由于各种原因无法手术治疗的大肝癌(>5cm)并接受射频消融治疗患者的临床资料,射频消融前后结合超声造影的方法,术后随访并定期复查血清甲胎蛋白水平和超声造影或者肝脏增强CT。结果 58例中无一例发生与射频消融相关的死亡,22例达到完全消融(37.9%,22/58)。共51例(87.9%,51/58)获得随访,随访至2012年5月,随访时间(12.6±6.4)个月(2~21个月),随访12个月以上患者有20例(39.2%,20/51),有10例(19.6%,10/51)无肿瘤复发或者转移的迹象。22例达到完全消融的患者无瘤生存时间为(13.6±10.4)个月(3~21个月);36例未能达到完全消融的患者中,有19例随访期间死亡,生存时间(8.3±6.1)个月(4~16个月)。结论 由于各种原因不能接受手术的大肝癌患者,射频消融作为一种有效、安全治疗方法,部分能达到完全消融的效果,而部分作为姑息性治疗手段,能一定程度上减轻患者的疼痛,达到提高生活质量的目的。

  • Cholecystolithotomy by Combination of Laparoscopy and Choledochoscopy for Cholecystolithiasis (Report of 95 Cases)

    目的 探讨腹腔镜联合胆道镜保胆取石术的临床价值。方法 回顾性分析我院2009年2月至2010年2月期间95例腹腔镜联合胆道镜保胆取石术患者的临床资料。结果 95例患者均顺利完成保胆取石术,平均手术时间49 min,术中平均出血量9 ml,除8例发生切口脂肪液化外无其他手术并发症发生,1年内随访无复发。结论 在严格掌握适应证的前提下,对胆囊结石患者有选择性地采用腹腔镜联合胆道镜保胆取石术,是一种可供选择的安全的微创治疗方法。
