Objective To explore the effect of deep muscle stimulation (DMS) combined with exercise therapy on morphological changes of the trapezius muscle assessed by musculoskeletal ultrasound and the function for patients with neck and shoulder myofascial pain syndrome (MPS). Methods One hundred and twenty patients with neck and shoulder MPS admitted to the First People’s Hospital of Shuangliu District, Chengdu between August 2021 and December 2022 were randomly selected and divided into 4 groups. There were 30 cases in each group. Group A received modulated middle frequency electrotherapy, group B received modulated middle frequency electrotherapy combined with DMS, group C received modulated middle frequency electrotherapy combined with exercise therapy, and group D received modulated middle frequency electrotherapy, DMS combined with exercise therapy. Before treatment and after 4 weeks of treatment, shear wave elastography was used to detect the shear wave velocity (SWV) of the four groups, high frequency ultrasound was used to detect the muscle thickness of the trapezius muscle. The Visual Analogue Scale (VAS) scores, Neck Disability Index (NDI), modified Barthel Index (MBI), Instrumental Activities of Daily Living Scale (IADL), World Health Organization Quality of Life-BREF (WHOQOL-BREF), Self-rating Anxiety Scale (SAS), and Self-rating Anxiety Scale (SDS) were also assessed to compare the therapeutic effects among the four groups of patients under different intervention methods. Results Before treatment, there was no significant difference in trapezius muscle SWV, trapezius muscle thickness, VAS, NDI, MBI, IADL, WHOQOL-BREF, SAS or SDS among the four groups (P>0.05). After 4 weeks of treatment, there were significant differences in trapezius muscle SWV, VAS, NDI, WHOQOL-BREF in the physiological domain and psychological domain, SAS and SDS among the four groups (P<0.05), and the improvements of trapezius muscle SWV, WHOQOL-BREF in the physiological domain and psychological domain, and SDS in group D were more obvious than those in other groups (P<0.05). After 4 weeks of treatment, there was no significant difference in MBI, IADL, or WHOQOL-BREF in the society domain and environmental domain among the four groups (P>0.05). Conclusion DMS combined with exercise therapy can significantly improve trapezius muscle elasticity, psychological state and quality of life of patients with neck and shoulder MPS.
ObjectiveTo construct the occupational therapy core items oriented by life return for stroke patients, and provide a reliable tool to promote the comprehensive rehabilitation and successful return for stroke patients.MethodsFrom January to February 2020, the primary items were summarized through literature analysis and research group discussion, and then Delphi method was used to determine which items should be included by two rounds of experts consultation.ResultsA total of 22 experts were selected. After two rounds of experts consultation, 60 items in 6 domains were included. The effective recovery rates of the two rounds of experts consultation were both 100%, the mean expert authority coefficient was 0.87±0.54. In the two rounds of experts consultation, the importance scores of consulted items were 7.60±0.97 and 8.06±0.35, respectively; the variation coefficients were 0.14±0.05 and 0.11±0.03, respectively; the Kendall coefficients of concordance were 0.522 (P<0.001) and 0.578 (P<0.001), respectively.ConclusionThrough the Delphi method, the core items oriented by life return are identified with high recognition and consistency from experts, which can be used as a guideline tool for stroke patients in in-hospital occupational therapy, discharge guidance, and community/home rehabilitation.
Spasm is a common complication of stroke with high morbidity and a high disability rate. Spasticity continues to impair the patient’s motor function, and symptoms such as muscle shortening and joint deformity may occur, which seriously affects the patient’s physical and mental health. Systemic therapy is a key part to control spasticity clinically. However, systematic treatment is conducted on the basis of proper assessment. The tools for assessing spasticity are divided into two categories: subjective and objective, and the subjective assessment tools mainly include some scales, which are subjectively assessed and judged by the therapist. Objective methods mainly include electrophysiology testers, ultrasonic measurement techniques, robots, three-dimensional motion analysis systems, isokinetic muscle strength testers, etc. This paper analyzes and summarizes the methods of post-hemiplegic spasticity assessment through a literature review, reviews the advantages, disadvantages and practicability of various assessment methods, and puts forward prospects for future research directions.
Objective To analyze the injury and dysfunction as well as the rehabilitation status and demand of the 188 Lushan earthquake victims admitted in the hospitals in and around Ya’an city and the West China Hospital of Sichuan University, so as to provide guidance for the rehabilitation work in the following step. Methods By means of the onsite investigation, 122 victims admitted in 7 hospitals and 2 health centers in and around Ya’an city within 1-9 days after Lushan earthquake, and the other 66 victims treated in the West China Hospital of Sichuan University were analyzed. Results The injury categorization of 122 victims in and around Ya’an city was as follows: upper limb fracture (12.30%), lower limb fracture (42.62%), spine fracture (16.39%) (25.00% treated by surgery and 75.00% treated by non-surgery treatment), pelvis fracture (1.64%), rib fracture (4.10%), traumatic brain injury (10.66%), soft tissue contusion (8.20%), and others (4.09%). At the corresponding period, the injury categorization of 66 victims treated in the department of rehabilitation medicine of the West China Hospital of Sichuan University was as follows: fracture (77.27%), traumatic brain injury (3.03%), spinal cord injury (4.55%), and others, including soft tissue injury (15.15%). At the ninth day after earthquake, among the 122 victims in and around Ya’an city, 8 victims (6.56%) were recovered with self-care ability of daily living, and the other 144 (93.44%) still needed the strengthened rehabilitation treatment. At the corresponding period, among the 66 victims in the West China Hospital, one victim (1.52%) was recovered with self-care ability of daily living, and the other 65 (98.48%) still needed the strengthened rehabilitation treatment. Conclusion Early rehabilitation treatment such as active exercise, elevating injured limbs, physical therapy, turning over at regular time, and psychological intervention can help the earthquake victims to return to home and society early.
ObjectiveTo systematically evaluate the efficacy of mirror therapy for phantom limb sensation and phantom limb pain in amputee. MethodsDatabases include PubMed, EMbase, The Cochrane Library (Issue 9, 2015), CBM, CNKI, VIP and WanFang Data were searched to collect randomized controlled trials (RCTs) about mirror therapy for phantom limb sensation and phantom limb pain in amputee from inception to 1st Sept. 2015. Two reviewers independently screened literature, extracted data and assessed the methodological quality of included studies. Then metaanalysis was performed using RevMan 5.3 software. ResultsA total of 5 RCTs involving 129 patients were included. The results of meta-analysis showed that, there was no significant difference between the mirror therapy group and the control group in relieving the phantom limb sensation and phantom limb pain in amputee (MD=-7.29, 95%CI -27.73 to 13.16, P=0.48). ConclusionMirror therapy could improve the control of phantom limb, however, there is no sufficient evidence to support the effect of mirror therapy on pain management in amputee. The long-term effect of mirror therapy is still under exploration. Due to the limited quantity and quality of the included studies, larger-sample, high quality designed RCTs are needed to verify the above conclusion.
Low-level laser therapy (LLLT) as an effective therapy with biochemical change has gained more and more attention in clinical treatment. This review sorts many fundamental experiment studis and clinical applications about LLLT published in recent years, and summarizes the advances of evidence-based proofs of the potential mechanisms of LLLT with different wavelengths in treating osteoarthritis. It shows that LLLT could relieve pain, modulate inflammatory response, protect cartilage, and prevent cartilage degeneration, through increasing cell proliferation, stimulating fibroblasts to produce collagen, bone repair, and regulating inflammatory markers and so on. LLLT with different wavelengths has different effect, and it is important for clinical practice.
ObjectiveTo analyze the state of rehabilitation after Lushan earthquake for improving the rehabilitation and personnel training system. MethodsFrom April 21st to May 5th, 2013, we collected data related to earthquake victims and the staff structure from West China Hospital, Leshan Downtown Hospital, Emeishan Hospital of Traditional Chinese Medicine (TCM), and Ya'an People's Hospital through interview and field investigation. ResultsTwo weeks after the earthquake, 25 220 victims received treatments in Sichuan Province, 6 545 surgeries were conducted, and 947 victims received postoperative early rehabilitation. A total of 392 victims received treatments in West China Hospital, Sichuan University, including 321 in-patients. In Leshan Downtown Hospital, Emeishan Hospital of TCM and Ya'an People's Hospital, the proportion of rehabilitation doctors reached 11.5% among all the doctors, the rehabilitation nurses reached to 8.6% among all nurses, and the rehabilitation therapists with certification reached to 4/5. ConclusionThe construction of the rehabilitation system and discipline, the cultivation of professional personnel, and popularization of rehabilitation concept are essential for development of rehabilitation in Sichuan province.
As an important component of the healthcare system reform, the regional rehabilitation medical alliance aims to integrate medical resources within the region, promote standardization, specialization, and accessibility of rehabilitation medical services, and achieve efficient collaboration and resource sharing among medical institutions at all levels. This article comprehensively and systematically reviews and analyzes the current situation of the construction of regional rehabilitation medical alliance in China. At the same time, combined with the construction practice of West China Airport Hospital of Sichuan University, it summarizes experience and puts forward suggestions, providing a reference for the development of regional rehabilitation medical alliance.
