  • 心脏手术围术期辅助循环的应用与管理

    目的总结心脏手术围术期应用辅助循环的经验. 方法 13例患者(左心室射血分数平均0.37±0.06)中行冠状动脉旁路移植术10例,机械瓣膜置换术2例,马方综合征(Marfan syndrome)患者行Bentall手术1例.术后不能脱离心肺转流(CPB)9例,术后并发心肌梗死、严重室性心律失常和心脏骤停4例,分别采用主动脉内球囊反搏( IABC)、转子泵或离心泵左心转流和体外膜肺氧合等辅助循环治疗. 结果 6例存活出院,平均辅助循环时间44.0±32.5小时.不能脱离 CPB的 9例患者,经辅助循环支持后7例脱离CPB,4例存活;在ICU行辅助循环的 4例患者中,2例存活. 结论辅助循环是治疗心脏手术后严重心力衰竭和室性心律失常的有效方法.

  • Effects of Pulmonary Rehabilitation on Exercise Capacity and Quality of Life in Patients with Stable Chronic Obstructive Pulmonary Disease

    ObjectiveTo investigate the effects of pulmonary rehabilitation on the exercise capacity and quality of life in patients with stable chronic obstructive pulmonary disease (COPD) for a optimal strategy for pulmonary rehabilitation. MethodsOne hundred and six patients with COPD in stable stage were divided into group B (n=37), group C (n=36), and group D(n=33) based on GOLD 2011.Each group of patients were randomly subdivided into a control group(usual care), a pulmonary rehabilitation strategy group 1 (breathing training), and a pulmonary rehabilitation strategy group 2 (breathing training and exercise training), and they were intervened for 24 weeks.Pulmonary function(FEV1%pred), COPD Assessment Test (CAT), modified British Medical Research Council dyspnea scale(mMRC), BODE index and 6-minute walking distance(6MWD) were compared before and after intervention. ResultsAfter pulmonary rehabilitation intervening for 24 weeks, in group B and group C, pulmonary rehabilitation strategy group 2 showed the best effect, CAT, mMRC, BODE index, and 6MWD were proved significantly different before and after pulmonary rehabilitation (P < 0.05).In group D, all indexes had no significant difference between pulmonary rehabilitation strategy group 1 and group 2 before and after pulmonary rehabilitation (P > 0.05), but they were better than those of the control group.Correlation analysis showed that CAT score had significant correlation with FEV1 % pred, mMRC, BODE index and 6MWD (P < 0.01). ConclusionPatients with different subgroup of COPD based on GOLD 2011 may take different pulmonary rehabilitation strategies to achieve the optimal effect.

  • Effect of SU5416 on the inflammation of pulmonary fibrosis in mice

    Objective To explore the effects of SU5416,a vascular endothelial growth factor receptor (VEGFR) inhibitor,on inflammation in mice with bleomycin-induced pulmonary fibrosis.Methods C57BL/6 mice were randomly divided into four groups,ie.a control group,a bleomycin-induced pulmonary fibrosis (BPF) group,early intervention with SU5416 group (days 1-14,SE),and delayed intervention with SU5416group (days 15-28,SD).Bleomycin (10 mg/kg) was instilled into the trachea of the mice to induce pulmonary fibrosis.After the instillation,SU5416 (50 mg/kg) was injected into the peritoneal cavities of the mice twice a week.The mice treated with bleomycin alone or with bleomycin followed by SU5416 were sacrificed on the day 14 and day 28 after the instillation of bleomycin.Lung tissues were taken and processed for determination of hydroxyl proline content.Bronchoalveolar lavage fluid (BALF) was collected for total and differential cell counts and measurements of lactic dehydrogenase (LDH) levels.Results It was found that the hydroxyl proline contents,the number of macrophages,and the LDH contents from the mice treated with bleomycin alone on day 14 were greatly increased.However,only the hydroxyl proline showed a significant increase in the mice treated with bleomycin alone on day 28.The number of macrophages,LDH contents,and hydroxyl proline contents were greatly reduced in the mice treated with bleomycin and early administration of SU5416 on day 14 when compared with those treated with bleomycin alone.But there were no significant differences in the above parameters of the mice treated with bleomycin and delayed administration of SU5416 on day 14.Conclusion This study indicates that early intervention by SU5416 might attenuate the fibrosis through inhibiting early inflammation in animal model of pulmonary fibrosis induced by bleomycin.

  • Screening and analyzing risk factors for obstructive sleep apnea hypopnea syndrome in patients with snoring

    ObjectiveTo establish a screening model for obstructive sleep apnea hypopnea syndrome (OSAHS) through data analysis, and explore the risk factors of OSAHS. MethodsA total of 558 patients who underwent polysomnography in the Sleep Monitoring Room of Zigong Fourth People’s Hospital were recruited in the study. Among them there were 163 cases in a snore group and 395 cases in an OSAHS group. Risk factors of OSAHS were screened by both univariate analysis and multivariate analysis, then the model was established by means of binary logistic regression analysis. Finally, the screening model was evaluated by receiver operating characteristic (ROC) curve of the combined predictive factor. ResultsThe screening model of OSAHS was established as: X=–10.286+0.280×body mass index+1.057×snoring degree+1.124×sex+0.085×Epworth score+0.036×age. In this equation, sex value was 1 for men and 0 for women. If the value of X is higher than 1.123, it is likely that OSAHS would occur, and the probability (P)=ex/(1+ex). The sensitivity of the screening model was 77.70%, the specificity was 85.89%, the area under the ROC curve was 0.890, and the 95% confidence interval ranged from 0.862 to 0.918. ConclusionThis study demonstrates that a screening model based on the snoring degree, Epworth score, and body measurement data is a valuable tool to predict and screen OSAHS in patients with snoring, and the screening model could be useful in clinical diagnosis of OSAHS.
