  • Diagnosis and Treatment of Hepatic Focal Nodular Hyperplasia (Report of 21 Cases)

    ObjectiveTo explore the clinical features and sum up the laws of the hepatic focal nodular hyperplasia (FNH) in its diagnosis and treatment. MethodsFNH was an uncommon benign hepatic tumor that often posed diagnostic dilemmas. We analyzed retrospectively the clinical, imaging of ultrasound, imaging of computed tomography (CT) and magnetic resonance images (MRI), and pathological materials of 21 patients with FNH proven by the pathological diagnosis during 5 years from April 1996 through April 2001 in two hospitals. ResultsThe diagnosis of FNH remained a challenge for clinicians and surgeons. Rate of correct diagnosis of FNH was low preoperatively (19.0%). The lesions of FNH were seen in males and females (m/f: 14/7). Only three female patients (3/7) had the history of taking oral contraceptive. Patients with FNH were largely young and middle age persons (81.0% under 50 years), discovered by accident (57.1%), without infection of the hepatitis B virus (95.2%) and with normal liver functions (100%) and serum AFP levels (100%). Color Doppler ultrasound showed blood vessels passing through the lesion (80.0%) and there was abundant in blood (66.7%). CT scan showed that the lesion had transient immediate enhancement in 60.0% of patients and had homogeneous signal in 60.0% after bolus injection. MR imaging demonstrated early vigorous enhancement (64.3%), homogenous signal (57.1%) and having central scar (35.7%) in the lesion. The demonstration of a central scar in the lesion was very helpful for the diagnosis of FNH. MRI was more helpful for the diagnosis of FNH using liver specific contrast agents: superparamagnetic iron oxide(SPIO). All patients underwent focus resection (18 cases) or segmentectomy (2 cases), except one having no treatment. ConclusionFNH shows some typical clinical and imaging features. We could increase the rate of correct diagnosis by comprehensively analyzing the clinical and imaging materials. It is very important and necessary to determine a definite diagnosis of FNH, hepatic adenoma (HA) and primary liver cancer (PLC) preoperatively, because the HA and PLC must be surgically resected, FNH can only be followed up.

  • Analysis of MRI Manifestations and Pathology Characteristic of Hepatic Focal Nodular Hyperplasia

    摘要:目的: 分析肝脏局灶性结节增生(FNH)的MRI表现和病理特点,探讨两者的相关性,提高FNH诊断的准确率。 方法 :回顾性分析23例(共28个病灶)经手术切除病理证实为FNH的MRI平扫及增强表现,与其病理特点进行对照。 结果 :25个病灶在平扫T1WI上呈等或稍低信号,T2WI上呈等或稍高信号,3个病灶在平扫T1WI及T2WI上均呈稍高信号,增强后所有28病灶动脉期可见明显强化,门脉期及延迟期呈稍高、等或稍低信号,其中12个病灶可见中心纤维瘢痕延迟强化。FNH组织病理上表现为富血供的实质性肿块,肿物内部组织较均匀,没有异型细胞,中心可见纤维瘢痕。 结论 :肝脏MRI平扫及增强检查能很好的反映FNH的组织病理及血供特点,能为FNH的诊断及鉴别诊断提供可靠证据。Abstract: Objective: To analyze the MRI manifestations and pathology characteristic of hepatic focal nodular hyperplasia(FNH),and to investigate their correlation. Methods : A retrospective analysis was made on the unenhanced and dynamic enhanced MR images of 23 patients (totally 28 lesions) with surgical and pathological proved FNH. Results :25 FNH lesions demonstrated isointensity or slightly hypointense on unenhanced T1WI,and isointensity or slightly hyperintense on unenhanced T2WI,3 FNH demonstrated hyperintense on both unenhanced T1WI and T2WI. 28 FNHs were markediyhyperintense in the arterial phase of dynamic contrast enhanced MRI, and isointense or slightly hyperintense in the portal and delayed phase.The seals were shown in 12 lesions, and enhanced in delayed phase.FNH is solid mass with vast blood supply,its inside structure is homogemeous. Typical seal can be found by microscopic examination.〖WTHZ〗Conclusion : MRI could disclose the pathologic features of FNH and its blood supply and improve the accuracy 0f its diagnosis.

  • Value on The Differential Diagnosis of Ultrasound Contrast in Hepatocellular Carcinoma and Hepatic Focal Nodular Hyperplasia

    ObjectiveTo investigate the value of ultrasound contrast in the differential diagnosis for hepatocellular carcinoma (HCC) and hepatic focal nodular hyperplasia (FNH). MethodsTwenty-three HCC patients and 23 cases hepatic FNH patients from January 1, 2012 to January 1, 2014 in our hospital were selected, all of them were underwent ultrasound contrast examination before operation. The ultrasound contrast results of 2 groups patients were comparative analyzed. ResultsIn HCC group, the contrast arrival time was (3.8±1.9) s, peak time was (21.8±11.9) s, and peak strength was (28.8 ±3.1) dB; in hepatic FNH group, these indicators was (2.3±1.4) s, (35.3±14.8) s, and (22.3±7.9) dB, respectively. These indicators of HCC group was longer or shorter or higher than those of hepatic FNH group, respectively (P < 0.05). The sensitivity, specificity, and accuracy of ultrasound contrast diagnosed HCC was 91.3% (21/23), 87.0% (20/23), and 95.7% (22/23), and diagnosed hepatic FHN was 87.0% (20/23), 91.3% (21/23), and 91.3% (21/23), respectively, the difference were not statistically significant between the two groups (P > 0.05). ConclusionUltrasound contrast has very important application value in the differential diagnosis of HCC and hepatic FNH, can be used in clinical application.
