  • Videoassisted Thoracoscopic Box Lesion Bipolar Radiofrequency Ablation of Atrial Fibrillation

    Abstract: Objective To explore a new videoassisted thoracoscopic surgical treatment for lone atrial fibrillation, in order to seek better efficacy, reduce invasiveness, and devise an easiertooperate surgical treatment for atrial fibrillation. Methods In June 2011, 3 women aged 40 years, 60 years, and 66 years with lone atrial fibrillation were treated in the Cardiovascular Surgery Department of West China Hospital. The patients underwent a videoassisted thoracoscopic “Box Lesion” bipolar radiofrequency atrial fibrillation therapy (bilateral pulmonary vein + left atrial posterior wall isolation), including three 5 to 10 mm small incisions on each side of the chest wall. The complications and sinus rhythm maintenance of the patients were observed. Results The operative times were 140 min, 170 min, and 155 min. The three patients were in sinus rhythm immediately after the surgery. Mean blood loss was approximately 80 ml, mean intensive care unit (ICU) stay was 1 day, and average hospital stay was 7 days. No deaths and serious complications occurred. The three patients were still in sinus rhythm one week and one month after the operation, as measured by electrocardiogram. Conclusion Box Lesion bipolar radiofrequency treatment for atrial fibrillation therapy shows fast postoperative recovery. It is a promising procedure in atrial fibrillation treatment and is worthy of further study.

  • Bi-polar radiofrequency ablation for rheumatic heart disease with atrial fibrillation

    Objective To evaluate the clinical effects of bi-polar radiofrequency ablation (BRFA) for the surgical treatment of atrial fibrillation (AF) in rheumatic heart disease (RHD) patients. Methods We involved 96 RHD patients who underwent valve replacement in Department of Cardiovascular Surgery, West China Hospital from July 2014 through May 2015. There were 74 patients with AF, 22 in sinus rhythm. All patients with AF were fully informed of BRFA, in which 40 of the 74 patients received while the other 34 patients refused. The 40 patients with AF who received BRFA were classified into a treatment group and the other 34 patients were classified into a control group. The 20 patients with sinus rhythm were classified into a blank group as well. The clinical effect and quality of life of the patients were analyzed. Results Left atrial diameter (LAD), pulmonary arterial systolic pressure (PASP), and conversion rate of sinus rhythm at discharge, maintenance rate of sinus rhythm during follow-up in patients with AF were significantly higher than those in the patients in sinus rhythm. No difference was found in terms of survival rate, mortality, major complications among the three groups. The treatment group experienced higher scores in physical functioning (PF), role-physical (RP), general health (GH), vitality (VT), social functioning (SF), mental health (MH), physical component summary (PCS), mental component summary(MCS) and total score compared with the control group both at postoperative 3-month and 6-month . At postoperative 12-month, the treatment group experienced higher scores in PF, RP, GH, PCS, and total score compared with the control group. Conclusion AF would enhance the negative impact on structure remodeling in RHD patients. BRFA is beneficial for the conversion and maintaining of sinus rhythm in RHD patients and the decreasing of LAD & PASP parameter as well. BRFA is more beneficial for the improvement of quality of life in RHD patients with AF during one-year follow-up.

  • 应用全 3D 经食管超声心动图精确引导房间隔缺损经皮封堵治疗的可行性研究

    目的探讨应用全 3D 经食管超声心动图(TEE)在精确引导房间隔缺损(ASD)经皮封堵治疗中的应用。方法纳入 2017 年 7 月至 2018 年 6 月间诊断 ASD,并于我院行全 3D-TEE 引导经皮介入封堵患者 17 例,男 4 例、女 13 例,平均年龄(27.1±8.0)岁。手术过程采用全 3D-TEE 引导经皮 ASD 封堵技术。记录手术时间、成功率、3D 超声图像质量、封堵器更换率及手术相关并发症等。结果经 TEE 示房间隔缺损直径(15.8±2.7)mm,3D-TEE 测量房间隔缺损前后径(17.4±2.4)mm,上下径(17.9±5.3)mm,缺损周长折算径(18.0±3.5)mm。手术成功率 100.0%,平均 ASD 封堵器型号(23.1±3.5)mm,无封堵器脱落,明显残余分流,心内结构损伤等严重不良事件,平均导管操作时间(30.7±5.5)min,封堵器换伞率 11.8%(2/17)。15(88.2%)例患者图像佳并采用全 3D-TEE 引导。结论应用全 3D-TEE 精确引导房间隔缺损经皮封堵治疗安全、有效,具有无辐射及引导精准的优势。

  • 3D打印辅助复杂胸主动脉血管病变分支支架腔内修复手术治疗的临床分析

    目的探讨 3D 打印辅助复杂胸主动脉血管病变 Castor 分支支架腔内修复手术治疗的可行性。方法纳入 2018 年 9~12 月急性 Stanford B 型主动脉夹层或主动脉弓部血管病变拟行腔内介入支架手术的 4 例患者,年龄分别为 65、55、45、39 岁。患者均缺乏足够的左锁骨下近端锚定区,或病变累及弓部血管无法采用常规单纯支架置入术。手术均采用 Castor 分支支架,术前采用三维 CT 血管造影(3D-CTA)及 3D 打印技术对主动脉弓部血管解剖进行精确刻画,并辅助手术策略制定及进行器械模拟植入。结果3 例为累及左锁骨下动脉的 Stanford B 型夹层(1 例合并升主动脉瘤),1 例为主动脉弓部穿通性溃疡。4 例患者手术顺利,1 例患者同期行体外循环升主动脉置换。患者术后 CTA 提示效果满意,无内漏、支架移位及卒中等严重并发症出现,30 d 随访患者状态良好,无不良严重并发症出现。结论3D 打印辅助复杂胸主动脉血管病变分支支架腔内修复手术具有很好的可行性及诸多潜在优势。
