Objective To evaluate the effectiveness and safety of gabexate mesylate in the prevention of post-endoscopic retrograde cholangiopancreatography (ERCP) pancreatitis (PEP). Methods The literatures and corresponding references of randomized controlled trials (RCT) on gabexate mesylate in the prevention of PEP were searched from Cochrane library (1970 to December 2010),PubMed (1966 to December 2010),EMBASE (1966 to December 2010),and CBM (1978 to December 2010).Literature screening,data extraction and methodological quality assessment were individually performed by two researchers.The software RevMan 4.2 of Cochrane Collaboration was used for statistical analysis. Results A total of 11 trails involving 3 012 patients were included. Meta analysis showed that high dose (≥1 000 mg)of gabexate mesylate with slow intravenous infusion (≥12 h) could effectively prevent from PEP (OR=0.27,95%CI: 0.11-0.64, P=0.003) and post-ERCP abdominal pain (OR=0.44,95%CI: 0.25-0.79,P=0.005) when compared with placebo. The low dose (≤500 mg) of gabexate mesylate with quick intravenous infusion (≤6.5 h) could not effectively prevent from PEP (OR=0.85, 95%CI: 0.59-1.34, P=0.570),post-ERCP hyperamylasemia (OR=0.86,95%CI: 0.69-1.07,P=0.180),and post-ERCP abdominal pain (OR=0.66,95%CI: 0.32-1.35,P=0.250).When compared gabexate mesylate with ulinastatin, there was no statistical difference between them in the prevention of PEP (OR=1.57,95%CI: 0.39-6.24,P=0.520) and post-ERCP hyperamylasemia (OR=1.85, 95%CI: 0.83-4.13,P=0.130). Conclusions The high dose (≥1 000 mg) of gabexate mesylate with slow intravenous infusion (≥12 h) other than low dose (≤500 mg) with quick intravenous infusion (≤6.5 h) is effective to prevent from PEP and post-ERCP abdominal pain,and can also prevent from post-ERCP hyperamylasemia to some extent.The effect which ulinastatin prevents PEP and post-ERCP hyperamylasemia is same to gabexate mesylate.The above conclusions remain to confirm by RCT with large sample and long-term follow-up of high-quality.
In the hospital where centralized treatment was applied for the victims after Yiliang earthquake (on 7th September, 2012), a holistic rehabilitation service mode which centred on supportive psychological intervention promoting was developed by the Yiliang psychological crisis intervention team designated by the National Health and Family Planning Commision of the People’s Republic of China. The pattern takes psychological nurses as liaison, bases on the integrity and continuity of medical service, roots in comprehensive health assessment and key assessment of mental trauma stress, emphasizes on solving realistic problems for patients and their caregivers, provides supportive psychological intervention, and encourages psychological nurses to offer psychological support for earthquake victims with the help of quantitative self-assessment of social psychological support service. In practice, the mode of psychological services, which could be contiuously conducted, is welcomed by hospital managers, work staff, and earthquake victims and their caregivers.
ObjectiveTo explore the relationship between severity of gastrointestinal symptoms and psychological stress after the earthquake. MethodsThe sample was teachers from Baoxing county which was most affected by the earthquake. Post-traumatic stress disorder checklist civilian version (PCL-C) scale was used to assess the symptoms of psychological stress at 2-3 weeks, 2-3 months, and 6 months after the earthquake, and gastrointestinal symptoms were evaluated using three questions of patient health questionare (PHQ)-15 at 2-3 months and 6 months after the earthquake. Spearman rank correlation was used to explore the dynamic relationship between the symptoms of psychological stress evaluated at the three waves and gastrointestinal symptoms evaluated at 2-3 months and 6 months after the earthquake. ResultsPrevalence rates of gastrointestinal symptoms in teachers were 6.7% and 4.7% for stomach and abdominal pain, 6.7% and 5.6% for constipation and diarrhea, and 9.1% and 8.1% for nausea and gastrointestinal flatulence or dyspepsia at 2-3 months and 6 months after the earthquake. The relationships between three gastrointestinal symptoms evaluated at 2-3 months and 6 months after the earthquake and psychological stress symptoms evaluated at the three waves were all significant (all P<0.05). ConclusionGastrointestinal symptoms are common among the teachers. The association between psychological stress symptoms and severity of gastrointestinal symptoms at the same point after the earthquake is most strong, and psychological stress symptoms in the early period can predict the severity of subsequent gastrointestinal symptoms, and gastrointestinal symptoms in the early period can predict symptoms of subsequent psychological stress.
ObjectiveTo evaluate the predictors of generalized anxiety disorder (GAD) among teachers in 3 months after Lushan earthquake. MethodsA prospective cohort study was conducted to diagnostically evaluate the psychological sequelae and GAD during 14-20 days and 85-95 days after the earthquake. The possible predictive factors of psychological sequelae were assessed by a self-made questionnaire and the GAD was assessed by the GAD symptom criterion of M.I.N.I. in 3 months. The univariate and multivariate logistic regression analysis (ULRA, MLRA) were applied to analyze the predictors of GAD after the two-staged assessments. ResultsThere were a total of 319 teachers completed the two-staged assessments. The total response rate was 51.3%. Seventy teachers were diagnosed as GAD and the prevalence of GAD in 3 months was 21.9%. The predictive factors by ULRA included:male, older than 35 years old, having unlivable house, living in tents, sleeping difficulties, easy to feel sad, physical discomfort, loss of appetite, feeling short of social support, unable to calm down for working, feeling difficult for teaching, observing more inattention of students, and wanting to ask for a leave. The independent predictors by MLRA included:male, having unlivable house, feeling short of social support, and feeling difficult for teaching. ConclusionThe teachers have a higher likelihood of GAD after earthquake. It is essential to pay more attention to those male teachers, who feel short of social support and don't have a livable house thus to prevent the GAD at the early stage of post-earthquake.
