Objetive To investigate the clinical characteristics, diagnosis, and treatment for diffuse alveolar hemorrhage. Methods The clinical data of 17 patients admitted to respiratory ICU with diffuse alveolar hemorrhage from July 1998 to May 2008 were reviewed. Results In the 17 cases, Wegener granulomatosis accounted for 6 cases, microscopic polyangiitis for 3 cases, systemic lupus erythematosis for 4 cases,mitral stenosis for 2 cases, and idiopathic pulmonary hemosiderosis for 1 cases. Clinical presentations included fever ( 76. 4% ) , hemoptysis( 47% ) , anaemia ( 100% ) , dyspnea ( 100% ) , hypoxaemia ( 100% ) ,and elevated total count of white blood cells ( 76. 4% ) . The titer of antineutrophil cytoplasmic antibody ( c-ANCA, p-ANCA) was 1∶( 46. 0 ±3. 7) , 1 ∶( 108. 0 ±16. 1) , respectively. Hemoglobin was ( 78. 0 ±2. 4) g/L. Bronchoalveolar lavage fluid presented with blood accounted for 35. 3% . Siderophages was found in all cases. Computed tomography showed areas of consolidation interspersed with areas of ground-glass attenuation and reticular interstitial opacities. Mortality was 29. 4% . Conclusions Diffuse alveolar hemorrhage is an acute life-threatening event. The clinical presentations of hemoptysis, dyspnea, and anaemia, and chest imaging studies provide clues to diagnosis of diffuse alveolar hemorrhage. Corticosteroids and immunosuppressive agents have proven effective in diffuse alveolar hemorrhage.
Objective To evaluate the outcome and explore the mechanism of coronary vein bypass grafting (CVBG) performed by anastomosing the right internal mammary artery with the middle cardiac vein via off-pump surgery. Methods Twelve Chinese experimental miniswines (either male or female, age from 7 to 10 months, body weight 40±5 kg) with severely diffuse stenosis in the right coronary artery were randomly divided into control group and experiment group with 6 miniswines in each group, using a random number table method. CVBG was performed in the experiment group and sham surgery was performed in the control group. To assess cardiac function, graft flow, graft patency and micro-circulation reperfusion of ischemia myocardium, following measurements were conducted. Eight weeks after right coronary endarterectomy, transthoracic echocardiography was performed for both groups. Coronary angiography, graft flow and echocardiography were performed or measured 6 hours and 3 months after CVBG or sham surgery. Measurement of myocardial blood flow with non-radioactive colored microspheres was also conducted 3 months after surgery for two groups. Results There was no statistical difference in cardiac function 8 weeks after right coronary endarterectomy between the two groups. There were significant improvements in cardiac systolic and diastolic function (ejection fraction 3 months after operation: 52%±6% vs. 44%±5%, t=-2.500, P=0.031) in the experiment group after CVBG compared with the control group. Graft flow of the experiment group 6 hours and 3 months after CVBG were 44.50±5.86 ml/min and 43.33±5.01ml/min respectively (P=0.718), and pulsatility index (PI) was 0.73±0.14 and 0.80±0.14 respectively(P=0.858). Internal mammary artery grafts and the anastomoses were all patent without stenosis, documented by coronary artery angiography for the experiment group 6 hours and 3 months after CVBG. Myocardial flow in all aspects especially in the subendocardial layer, estimated by non-radioactive colored microsphere injection, was significantly higher in the experiment group after CVBG than that of the control group, transmural flow was 0.33±0.05ml/(g • min) vs. 0.19±0.03 ml/(g • min) (P<0.05). Conclusion Ischemic conditions of the myocardium can be relieved by CVBG using internal mammary artery in a short-term to medium-term period. The mechanism may be due to improvement of the myocardial micro-circulation.
Objective To explore the methods of evidence-based individualized treatment for a patient with Graves’ disease. Method We searched The Cochrane Library (Issue 3, 2006), SUMSEARCH (Jan.1980 to Mar. 2006), PubMed (1980 to Mar.2006), CNKI (Jan.1980 to Mar. 2006) and VIP (Jan.1980 to Mar.2006) to identify the best evidence for antithyroid drugs, iodine radioisotopes and thyroidectomy for patients with Graves’ hyperthyroidism and evaluate the quality of available evidence. Results We identified 1 clinical guideline, 1 Cochrane systematic review, 1 meta-analysis and 15 randomized controlled trials. There was no significant difference between the titration regimen and the block-replace regimen in the relapse of hyperthyroidism, while the block-replace regimen was associated with more adverse effects. Prescribing replacement thyroxine, either with or after the anti-thyroid drug treatment, had no significant effect on relapse. Methimazole was more effective than propylthiouracil in the induction of euthyroidism. There was no significant difference in the quality of life between the drugs, 131I and the thyroidectomy therapy, and the relapse was lower with thyroidectomy therapy but the cost was higher. Given the current evidence together with our clinical experience and considering the patient and her family’s values and preferences, methimazole (10 mg tid) was administered to her and then gradually reduced. No obvious adverse effects occurred within 4 months’ follow-up. Conclusion Drugs, radioactive iodine and thyroidectomy are all effective in the treatment of Graves’ hyperthyroidism. Methimazole is an effective and well tolerated drug for the treatment of Graves’ hyperthyroidism and further research into side effect are needed. Prescribing replacement thyroxine has no significant effect on relapse.
Objective Methods of evidence-based medicine were used to make an individualized treatment plan concerning newly diagnosed diffuse large-B-cell lymphoma in elderly patients. Methods After clinical problems were put forward, evidence was collected from MEDLINE (Jan. 1990 to Dec. 2004) and http://sumsearch.uthscsa.edu/searchform4.htm. Subject words were: NHL; aggressive non-Hodgkin’s lymphoma; diffuse large-B-cell lymphoma; chemotherapy; CHOP; rituximab; RCT; economic evaluation; older patient. Results A total of 11 randomized controlled trials and 8 systematic reviews about chemotherapy regimen, and 1 systematic review about economic evaluation were identified. A rational treatment plan was made upon a critical evaluation of the data. After 5 months follow-up, the plan was proved optimal. Conclusions The treatment effectiveness in newly diagnosed diffuse large-B-cell lymphoma in the elderly has been improved by an individual treatment plan according to evidence-based methods.
目的 探讨弥漫性轴索损伤(diffuse axonal injury,DAI)的发病机制、临床特点、诊断及治疗方法,以提高治愈率,降低致残、致死率。 方法 回顾性分析2006年6月-2010年3月间65例临床诊断为DAI患者的受伤机制、临床特征、影像学表现、治疗及预后。 结果 DAI最常见原因为车祸伤70.7%,其次为坠落、坠物伤(10.7%),其他(18.6%)。按格拉斯哥昏迷分级(GCS)评分结果3~5分18例,6~8分15例,9~12分32例;治愈43例,轻残15例,中残8例,重残或植物生存7例,死亡7例。 结论 DAI具有诊断、治疗困难,预后差等特点,交通事故是导致DAI的主要原因,格拉斯哥昏迷分级(GCS)评分、昏迷时间和瞳孔变化是判定预后的重要指标。目前尚无特效治疗方法,由于80%以上患者往往是多发伤,故早期气管切开、呼吸机辅助呼吸、促醒、亚低温治疗、防治并发症、钙离子拮抗剂应用等综合治疗可显著改善预后。
Objective To introduce the current value of multi-detector row CT (MDCT) and magnetic resonance imaging (MRI) techniques on evaluation of diffuse hepatic parenchymal diseases.Methods By literature review, the application and recent advances of various kinds of MDCT and MR imaging techniques in evaluating diffuse hepatic parenchymal diseases were summarized. Results There were three kinds of diffuse liver parenchyma diseases, which were the diseases of storage, vascular and inflammatory. The morphology changes of diffuse liver parenchyma diseases could be demonstrated well by MDCT. MRI, especially MR functional imaging could reflect the morphology changes, and cellular metabolic activity of the liver, which provided qualitative and quantitative information for the diagnosis and evaluation of therapeutic effect on diffuse liver parenchyma diseases. Conclusion MR imaging techniques, especially those functional techniques, developed rapidly and had practical value in both the diagnosis and severity assessment of hepatic fibrosis.
Objective To review the examination techniques and the current research progress of the magnetic resonance diffusion weighted-imaging (DWI) used in liver. Methods The recent and relevant literatures about the principles and the current study situation of liver DWI were scrutinized and analyzed retrospectively. In addition, the existing problems of liver DWI were discussed. Results DWI could demonstrate the normal and abnormal structure and function through measuring the diffusion motions of water molecule in the liver. With the improving technology and better understanding of diffusion dynamics, DWI has been used for the diagnosis and differential diagnosis for hepatic diseases. Conclusion DWI as a non-invasive examine method, may provide valuable functional information for clinical diagnosis and treatment.
Objective To analyze the clinical symptoms, imaging, laboratory tests, efficacy and other indicators of patients diagnosed as diffuse parenchymal lung disease ( DPLD) , in order to provide a reference for differential diagnosis of secondary DPLD and idiopathic interstitial pneumonia ( IIP) .Methods Sixtytwo patients diagnosed as DPLD were retrospectively analyzed. Results In 62 patients with DPLD, 19 patients ( 30. 6% ) were diagnosed as secondary DPLD, 42 cases ( 67. 7% ) as IIP, 1 case ( 1. 6% ) as Langerhans cell histiocytosis. The smoking rate of the DPLD patient was 33. 8% , which was higher than general population ( 29. 7% ) . 94. 7% of the secondary DPLD patients had cough, which was higher than the IIP patients ( 71. 4%) . The average age of onset of the secondary DPLD and IIP was ( 45. 9 ±16. 8) years and ( 60. 5 ±7. 7) years respectively, without significant difference ( P gt; 0. 05) . Etiological factors of secondary DPLD were dust, pets, drugs, pesticides, decoration material, etc. The secondary DPLD patients had higher response rate to steroid therapy, but had no statistical difference compared with the IIP patients ( 46. 2% vs. 37. 5% , P gt;0. 05) . Conclusions As a group of diseases of known causes, history taking is very important for DPLD diagnosis and differential diagnosis. Clinical symptoms, imaging, and laboratory tests may provide reference for differential diagnosis of secondary DPLD and IIP.
