  • 改良迷宫术治疗慢性心房颤动

    目的 为简化迷宫手术程序,缩短手术时间,避免出血,增加手术的安全性.方法 对18例风湿性心脏二尖瓣病变合并慢性心房颤动患者实施了改良迷宫术加心脏二尖瓣置换术.我们在Cox迷宫术的基础上从三个方面加以了改进:(1)用热凝代替左心房切口;(2)保留窦房结动脉,提高窦性心律恢复率;(3)应用无水酒精注射左、右房室环和房顶部,避免冠状血管和房室结损伤.结果 18例全部存活,随访4~46个月,心房颤动全部消失,其中14例维持窦性心律,心房颤动复发4例,服用乙胺碘呋酮恢复窦性心律.无房室传导阻滞,无出血.结论 改良迷宫术手术时间短,效果好,并发症少.

  • 双极射频消融迷宫术后同步电复律治疗心房颤动

    目的 总结双极射频消融迷宫术(CoxⅣ)术后同步电复律治疗心房颤动(AF)的临床经验。 方法 2006年7月至2009年7月沈阳军区总医院对223例AF患者行CoxⅣ治疗,其中13例患者(包括男4例,女9例;年龄41~69岁;风湿性心脏病7例,退行性心瓣膜病5例,继发孔型房间隔缺损合并三尖瓣关闭不全1例)于出院后仍为AF而药物转复不佳,行同步电复律治疗。电复律后观察心率和心律情况,监测生命体征,出院后继续口服胺碘酮200 mg,1次/天,定期门诊随访。 结果 CoxⅣ手术后6个月内行同步电复律4例,即刻转复窦性心律3例;6个月以上行同步电复律9例,即刻转复窦性心律4例、AF 2例、交界性心律3例。CoxⅣ术后1年以上行电复律3例,转复窦性心律2例。13例均获随访,随访时间6个月~1年。随访期间窦性心律8例(61.54%)、AF4例(30.77%)、交界性心律1例(7.69%)。7例电复律后即刻转复为窦性心律的患者中有1例复发,窦性心律维持率为85.71%(6/7);6例即刻未转复为窦性心律患者中有2例转为窦性心律。窦性心律患者复律后左心房内径(LAD)较复律前明显减小(39.00±5.15 mm vs. 54.50±3.63 mm, t=6.958, P=0.000),而AF患者LAD在复律前后无明显改变(51.00±5.72 mm vs. 48.00±5.89 mm,t=0.731,P=0.493)。 结论 CoxⅣ手术后电复律治疗AF安全有效,成功率高,是CoxⅣ术后控制AF的重要治疗手段。复律时机以术后6个月以内为宜,即刻转复窦性心律成功率较高,维持率也高。LAD明显缩小者电复律效果较好。

  • Relationship Between Serum Apelin Level and Inflammation in Patients with Atrial Fibrillation

    ObjectiveTo study on the relationship of serum apelin level with inflammation in patients with atrial fibrillation (AF). MethodsWe recruited 58 patients with valvular heart disease who admitted in our hospital between October 2014 and December 2014 and planned to undergo surgery, including 29 patients with persistent AF (an AF group) and 29 patients with sinus rhythm (a SR group). There were 14 males and 15 females in the AF group at an average age of 57±8 years. There were 20 males and 9 females in the SR group at an average age of 54±10 years. The left atrial diameter (LAD) and ejection fraction (EF) were detected by echocardiography. The levels of serum apelin and interleukin-6 (IL-6) were measured by enzyme linked immuno sorbent assay, and the level of high-sensitivity C-reactive protein (hs-CRP) were determined by turbidimetric inhibition immuno assay. ResultsCompare with the SR group, the serum apelin level (201.94±71.96 pg/ml vs. 286.72±129.33 pg/ml) and EF (54.52%±3.94% vs. 56.41%±2.85%) were significantly lower in the AF group, while the hs-CRP (5.58±12.90 mg/L vs. 1.89±3.55 mg/L), IL-6 (2.59±0.64 pg/ml vs. 2.26±0.55 pg/ml) and LAD (57.10±11.69 mm vs. 43.07±5.31 mm) were significantly higher in the AF group (P<0.05). Correlation analysis showed that the apelin level was negatively correlated with hs-CRP and LAD (r=-0.308, P=0.019; r=-0.313, P=0.017), and were positively correlated with EF (r=0.265, P=0.044). ConclusionSerum apelin level is significantly lower in patients with AF and levels of inflammation makers are significantly higher. Apelin may be closely related to AF and inflammation, and may take part in the occurrence and maintenance of AF through the regulation of inflammatory processes.

  • A Meta-analysis on Randomized Controlled Trials of Statins to Prevent Atrial Fibrillation

    Objective To synthesize the available evidence on the efficacy of using statins in the prevention of recurrent and new-onset atrial fibrillation (AF). Methods We searched PubMed, EMbase, EMB Reviews-Cochrane Central Register of Controlled Trials (Issue 3, 2007), CBMdisc, VIP, and CNKI databases from establishment to 15th Sep. 2007 to identify randomized controlled trials (RCTs) covering the use of statins for the patients with persistent AF after electrical cardioversion, paroxysmal and postoperative AF. Meta-analysis was performed using RevMan 4.3 software after the strict evaluation of the methodological quality of the included RCTs. Results Five RCTs including 470 patients were included. Significant heterogeneity was found when the data were pooled, so a random effect model was used for metaanalysis. Compared with placebo or no use of statins, the statins decreased risk of AF recurrence and postoperative AF (RR=0.61, 95%CI 0.43 to 0.88, P=0.008). Sensitivity analysis showed that the result was stable. The fail-safe number was 52.91. Conclusion The statins may decrease incidence of AF recurrence and postoperative AF. Because of the low quality and the small number of included studies, larger sample-size, randomized, double-blinded controlled trials are needed.

  • Recovery rule of atrial contractility after Maze Ⅳ in patients with valvular atrial fibrillation

    ObjectiveTo analyze the recovery rule of atrial contractility (AC) function after Maze Ⅳ procedure of valvular atrial fibrillation (AF).MethodsIn our hospital from March 2016 to April 2018, 103 patients who underwent cryoablation Maze Ⅳ procedure due to mitral valve lesions associated with persistent or long-term persistent AF were enrolled. There were 42 males and 61 females, with an average age of 58.5±9.1 years. Electrocardiogram and echocardiography were followed up at discharge and 1, 3, 6, 12 months after procedure. A multivariate Cox analysis of predictive factors for AC recovery was applied.ResultsAll the 103 patients were followed up for 1 year. The recovery rate of AC increased gradually after operation. It was not until 3 months after procedure that most of the right atrial contractility (RAC) was accompanied by synchronous recovery of the corresponding left atrial contractility (LAC, Kappa coefficient≥0.40, P<0.05). However, the coexistence of sinus rhythm (SR) and bilateral AC was not consistent well until 1 year after operation (Kappa coefficient≥0.40, P<0.05). One year after procedure, the recovery rates of SR and bilateral AC were 86.4% (89/103) and 66.0% (68/103) respectively. By Cox multivariate regression analysis, longer preoperative AF duration (P=0.040), larger preoperative left atrial diameter (LAD, P=0.003), and AC deletion 3 months after surgery (P=0.037) were predictive factors for AF recurrence in the middle and advanced stages (>3 months) after Maze surgery. At the same time, longer preoperative AF duration and larger preoperative LAD were also negative predictors of middle and late recovery of LAC and bilateral AC (All P<0.05). Receiver operating characteristic curve analysis showed that the best critical value of preoperative AF time and preoperative LAD for prediction of AC recovery was 37 months (sensitivity 99.6%, specificity 76.3%) and 60.5 mm (sensitivity 98.5%, specificity 78.9%), respectively.ConclusionThe recovery of AC after Maze procedure is a dynamic improvement process. Early recovery of AC is beneficial to the stable maintenance of SR in the future. Prolonged duration of AF and enlarged LAD have adverse effects on the outcome of Maze Ⅳ procedure.

  • Correlation between Atrial Fibrillation and Prognosis of Patients with Ischemic Stroke after Intravenous Thrombolysis: A Meta-Analysis

    ObjectiveTo systematically review the correlation between atrial fibrillation and prognosis of patients with ischemic stroke after intravenous thrombolysis. MethodsLiterature search was carried out in PubMed, EMbase, Web of Science, The Cochrane Library (Issue 4, 2014), CBM and WanFang Data up to April 2014 for the domestic and foreign cohort studies on atrial fibrillation and prognosis of patients with ischemic stroke after intravenous thrombolysis. Two reviewers independently screened literature according to inclusion and exclusion criteria, extracted data, and assessed methodological quality of included studies. Then meta-analysis was performed using RevMan 5.2. ResultsA total of 7 cohort studies were finally included involving 69 017 cases. The results of meta-analysis showed that, compared with patients without atrial fibrillation, atrial fibrillation reduced 3-month favourable nerve function of patients with atrial fibrillation (OR=0.85, 95%CI 0.73 to 0.98, P=0.03) but did not influence the risk of death after intravenous thrombolysis (OR=1.47, 95%CI 0.75 to 2.86, P=0.26); and increased the risks of intracranial haemorrhagic transformation (OR=1.36, 95%CI 1.26 to 1.47, P < 0.001) and symptomatic intracranial hemorrhage after intravenous thrombolysis (OR=1.43, 95%CI 1.02 to 1.99, P=0.04). ConclusionFor patients with ischemic stroke, atrial fibrillation does not influence the risk of death, but it increases the risks of intracranial hemorrhage, and worsens 3-month favourable nerve function of after intravenous thrombolysis. For those patients, more assessment before intravenous thrombolysis and more monitoring after intravenous thrombolysis are necessary. Due to limited quality and quantity of the included studies, the abovementioned conclusion still needs to be verified by conducting more high quality studies.

  • Videoassisted Thoracoscopic Box Lesion Bipolar Radiofrequency Ablation of Atrial Fibrillation

    Abstract: Objective To explore a new videoassisted thoracoscopic surgical treatment for lone atrial fibrillation, in order to seek better efficacy, reduce invasiveness, and devise an easiertooperate surgical treatment for atrial fibrillation. Methods In June 2011, 3 women aged 40 years, 60 years, and 66 years with lone atrial fibrillation were treated in the Cardiovascular Surgery Department of West China Hospital. The patients underwent a videoassisted thoracoscopic “Box Lesion” bipolar radiofrequency atrial fibrillation therapy (bilateral pulmonary vein + left atrial posterior wall isolation), including three 5 to 10 mm small incisions on each side of the chest wall. The complications and sinus rhythm maintenance of the patients were observed. Results The operative times were 140 min, 170 min, and 155 min. The three patients were in sinus rhythm immediately after the surgery. Mean blood loss was approximately 80 ml, mean intensive care unit (ICU) stay was 1 day, and average hospital stay was 7 days. No deaths and serious complications occurred. The three patients were still in sinus rhythm one week and one month after the operation, as measured by electrocardiogram. Conclusion Box Lesion bipolar radiofrequency treatment for atrial fibrillation therapy shows fast postoperative recovery. It is a promising procedure in atrial fibrillation treatment and is worthy of further study.

  • Effects of selectively resecting the lower half of stellate ganglion on fast ventricular rate in canines with persistent atrial fibrillation

    ObjectiveTo determine the effects of resecting the lower half of left stellate ganglion (LSG) on fast ventricular rate (VR) in persistent atrial fibrillation (AF) and its mechanism. MethodsTwelve mature healthy male beagle dogs (15–25 kg) were studied. They were randomly divided into two groups (an experimental group and a control group, 6 dogs in each group). The control group were merely performed with rapid left atrial pacing to induce persistent AF. The experimental group were disposed with rapid left atrial pacing and received resection of the lower half of LSG after the persistent AF was documented. Simultaneously the ventricular rates were monitored separately before anesthesia, after anesthesia, 30 minutes and one month after LSG resection. The forward passing effective refractory period (ERP) of the canine atrioventricular node (AVN) was also measured. ResultsEach dog was documented with persistent AF after 3–6 weeks’ left atrial pacing. After resecting the lower half of LSG for 30 minutes (the control group was only observed for 30 minutes without LSG resection), the average VR of the control group attained 144.5±4.2 beats/min, while that of the experimental group was 121.5±8.7 beats/min (P<0.001). After resecting the lower half of LSG for one month (the control group was observed for one month without LSG resection), the average VR of the control group was 139.2±5.6 beats/min, while that of the experimental group was 106.5±4.9 beats/min (P<0.001). Meantime, the forward passing ERP of AVN of the experimental group was significantly prolonged than that of the control group (265.6±7.8 ms vs. 251.1±4.6 ms, P=0.003). ConclusionResection of the lower half of LSG is efficient in reducing VR in canines with persistent AF, one of the mechanisms of which may be prolonging the forward passing ERP of AVN.

  • Prediction and risk factors of recurrence of atrial fibrillation in patients with valvular diseases after radiofrequency ablation based on machine learning

    ObjectiveTo use machine learning technology to predict the recurrence of atrial fibrillation (AF) after radiofrequency ablation, and try to find the risk factors affecting postoperative recurrence. MethodsA total of 300 patients with valvular AF who underwent radiofrequency ablation in West China Hospital and its branch (Shangjin Hospital) from January 2017 to January 2021 were enrolled, including 129 males and 171 females with a mean age of 52.56 years. We built 5 machine learning models to predict AF recurrence, combined the 3 best performing models into a voting classifier, and made prediction again. Finally, risk factor analysis was performed using the SHApley Additive exPlanations method. ResultsThe voting classifier yielded a prediction accuracy rate of 75.0%, a recall rate of 61.0%, and an area under the receiver operating characteristic curve of 0.79. In addition, factors such as left atrial diameter, ejection fraction, and right atrial diameter were found to have an influence on postoperative recurrence. ConclusionMachine learning-based prediction of recurrence of valvular AF after radiofrequency ablation can provide a certain reference for the clinical diagnosis of AF, and reduce the risk to patients due to ineffective ablation. According to the risk factors found in the study, it can provide patients with more personalized treatment.
