【摘要】 目的 了解行肾切除手术疾病谱、疾病的临床表现及诊治方法的演变,探讨避免肾脏切除保留肾单位的术式,以期提高对肾脏疾病的诊治水平。 方法 回顾性分析泌尿外科1955年1月-2001年12月收治入院22 603例患者的临床资料,对其中行肾切除手术的1 952例进行分析。根据肾切除手术病因,将疾病分为3类:肾肿瘤、肾结核、其他疾病。将47年分为5个时段:50、60、70、80、90年代。应用平均值、构成比、中位数、率等对各项指标进行统计学分析。 结果 5个时段年平均入院人数与年均切肾率呈上升趋势。疾病谱构成中肾肿瘤共计663例,占33.97%;肾结核共计599例,占30.69%;其他疾病共计690例,占35.35%。肾肿瘤与其他疾病的例数及构成比分别随着年代的推进不断增加。而肾结核则未显示出该特点。各年代3类疾病发病年龄(中位数)在肾肿瘤、肾结核与其他疾病亦沉陷;体检发现疾病自70年代分别为1.10%、5.10%、8.80%。 结论 90年代后,肾切除手术的术式更加标准和成熟,保留肾单位的肾脏手术正在受到临床的重视和推广。【Abstract】 Objective To study the spectrum of diseases subjected to nephrectomy, to find out the clinical manifestations of the diseases, to summarize the evolvement of operational technology of kindney removal, to discuss the method of nephron sparing surgery in the purpose of avoiding nephrectmoy and reducing the operational risk, and to promote the level of diagnosing and treating nephropathy. Methods There were 1 952 cases of nephrectomy selected in the total 22 603 cases treated in the Department of Urology from January 1955 to December 2001. According to the diseases subjected to the surgery, these 1 952 cases were assigned into 3 groups: renal tumor group, renal tuberculosis group and other diseases group. The 47 years from 1955 to 2001 were divided into 5 periods: the 1950s (1955-1960), 60s (1961-1970), 70s (1971-1980), 80s (1981-1990) and 90s (1991-2001). Indexes such as average, proportion, median, and ratio were analyzed statistically. Results From the 1950s to 1990s, the number of patients undergoing nephrectomy increased from period to period. Among the diseases causing the surgery, the number of renal tumor cases was 663 (33.97%), the number of renal tuberculosis cases was 599 (30.69%), and the number of other disease cases was 690 (35.35%). The number and proportion of renal tumor and other disease cases increased from year to year, while the nuber of renal tuberculosis cases did not show this feature. The median age of patients treated with nephrectomy increased year by year, and the patient’s age of renal tumor was older than that of renal tuberculosis and other diseases. The proportion of patients whose diseases were found out by physical examination since the 1970s was respectively 1.10%, 5.10% and 8.80%. Conclusion Since the 1990s, the technology of nephrectomy has become maturer and more standardized, and nephron sparing surgery has caught more attention and has been applied more in clinics.
ObjectiveTo compare the clinical efficacy of transurethral plasmakinetic resection of bladder tumors (PKRBT) and holmium laser resection of bladder tumors (HOLBT), and discuss the effcacy, safety, indication, and complications of PKRBT for the treatment of bladder tumors compared with HOLBT. MethodsA hundred patients with bladder tumors were divided into two groups randomly, who were selected from patients in the Department of Urology of West China Hospital from March 2011 to March 2013. Among all the 100 cases, half of them were treated with PKRBT, and all others treated with HOBLT. The significant markers in both groups were recorded and evaluated, including the situation of before operation, during operation and after operation. The data recorded consisted of the general records of patients' medical background, concomitant disease, laboratory examination, and the position, amount, pathology of the tumor, total operative duration, the time of gross hematuria, the time of postoperative bladder irrigation and catheterization, the length of stay, postoperative complications and patients' conditions at month 3, 6, and 12 during the follow-up. ResultsAll operations were successfully performed, and there was no significant diTherences between the two groups in preoperative indexes (P>0.05). No abnormalities were detected in the postoperative laboratory examinations. The diTherences in operatative duration, time of bladder irrigation, duration of indwelling catheter, and postoperative length of stay between the two groups were not significant (P>0.05). But the mean time of gross hematuria was significantly shorter after operation in the HOLBT patients [(6.1±7.6) hours] than in those treated with PKRBT [(15.3±17.2) hours] (P<0.05). There was no significant diTherence between the two groups in the recurrence rate 3, 6, and 12 months after operation (P>0.05). ConclusionHOLBT can be used safely and effectively in treating bladder tumors, and it is easy for clinical manipulation. HOLBT is as effective and safe as PKRBT with similar adverse side-effect rate within and after operation.
ObjectiveTo investigate the clinical value of serum proadrenomedullin (pro-ADM) for diagnosis of ventilator-associated pneumonia(VAP). MethodsA prospective study was carried out in eighty-nine patients with clinically suspected diagnosis of VAP who underwent invasive mechanical ventilation between June 2014 and July 2015.The patients were divided into a VAP group (n=52) and a non-VAP group (n=37) according to clinical and microbiological culture results.The levels of serum pro-ADM were measured by sandwich ELISA on 1st, 3rd and 5th day of VAP suspicion.The diagnostic value of pro-ADM for VAP was assessed by receiver operating characteristic (ROC) curve analysis. ResultsOn 1st day, 3rd day and 5th day, the pro-ADM levels [3.10(2.21, 4.61) nmol/L, 3.01(2.04, 4.75)nmol/L and 1.85(1.12, 3.54)nmol/L, respectively] in the VAP group were significantly higher than those in the non-VAP group [1.53(1.07, 2.24)nmol/L, 1.52(1.05, 2.17) nmol/L and 1.26(1.02, 2.17) nmol/L, respectively] (all P < 0.05).For diagnosis of VAP, the area under the ROC curve (AUC) for pro-ADM on 1st, 3rd and 5th were 0.896 (95%CI 0.799-0.940), 0.863(95%CI 0.791-0.935) and 0.651 (95%CI 0.538-0.765), respectively.When using 2.53 nmol/L as the best cutoff on 1st day, pro-ADM had 84.6% sensitivity and 86.5% specificity.When using 2.40 nmol/L as the best cutoff on 3rd day, pro-ADM had 82.7% sensitivity and 83.8% specificity. ConclusionSerum level of pro-ADM in the diagnosis of VAP has good sensitivity and specificity, which may be used as a marker to diagnose VAP early.
ObjectiveTo evaluate the role of CD3+CD4+T cells in patients with nosocomial infection in ICU. MethodsOne-hundred and eleven patients who admitted in ICU and in respiratory department from March to December in 2014 were recruited in the study.There were 33 patients with community-acquired pneumonia (CAP group), 31 patients without nosocomial infection (NNI group), and 47 patients with hospital-acquired pneumonia (HAP group).The counts of T cells, B cells, CD3+CD4+ T cells, CD3+CD8+ T cells, and NK cells were compared among three groups. ResultsThe comparison among the groups had no statistical significance in sex and age(P > 0.05).The three groups had statistical significance in APACHEⅡscore, CD3+CD4+T cells, T cells and B cells, but had no statistical significance in CD3+CD8+T cells, CD3+CD4+/CD3+CD8+ T cells, NK cells, white blood cells, neutrophils, procalcitonin or C reactive protein.CD3+CD4+T cells of HAP group were less than other two groups.The area under the ROC curve (AUC) was 0.660, with a threshold of 29.96%, a sensitivity of 93.8%, and a specificity of 40.4%. ConclusionCD3+CD4+ T cell is an independent predictor for nosocomial infection.
ObjectiveTo analyze the difference of location identification of pulmonary nodules in two dimensional (2D) and three dimensional (3D) images, and to discuss the identification methods and clinical significance of pulmonary nodules location in 3D space.MethodsThe clinical data of 105 patients undergoing sublobectomy in the Department of Thoracic Surgery, the First Affiliated Hospital with Nanjing Medical University from December 2018 to December 2019 were analyzed retrospectively. There were 28 males and 77 females, with an average age of 57.21±13.19 years. The nodule location was determined by traditional 2D method and 3D depth ratio method respectively, and the differences were compared.ResultsA total of 30 nodules had different position identification between the two methods, among which 25 nodules in the inner or middle zone of 2D image were located in the peripheral region of 3D image. The overall differences between the two methods were statistically significant (P<0.05). The diagnostic consistency rates of two methods were 66.67% in the right upper lung, 83.33% in the right middle lung, 73.68% in the right lower lung, 75.76% in the left upper lung, and 64.71% in the left lower lung. In each lung lobe, the difference between the two methods in the right upper lung (P=0.014) and the left upper lung (P=0.019) was statistically significant, while in the right middle lung (P=1.000), right lower lung (P=0.460) and left lower lung (P=0.162) were not statistically significant.ConclusionThe 3D position definition of lung nodules based on depth ratio is more accurate than the traditional 2D definition, which is helpful for preoperative planning of sublobectomy.
Objective To retrospectively analyze the clinical information of a series of patients with atypical adrenal phaeochromocytoma in order to improve the diagnosis of atypical phaeochromocytoma. Methods Ninety patients diagnosed pathologically as adrenal phaeochromocytoma from January 1998 to December 2004 in the Affiliated Hospital of Inner Mongolia Medical College were included in the study. Patients with phaeochromocytoma were classified into the typical group and atypical group based on their clinical manifestations. The differences were analyzed between the two groups in terms of patient age, tumor location and maximal diameter, incidence of hypertension, and plasma catecholamine level. Results About 24.4% (22/90) of the patients with phaeochromocytoma were classified as atypical. There was no significant difference in the maximal tumor diameter between the two groups. The plasma norepinephrine level of the atypical group was significantly lower than that of the typical group (P=0.001), and the positive rate of plasma norepinephrine of the atypical group was also lower than that of the typical group (P=0.003). Conclusion Compared with typical phaeochromocytoma, atypical phaeochromocytoma is associated with a lower plasma norepinephrine level. The size of adrenal tumor cannot contribute to the diagnosis of atypical phaeochromocytoma. Based on atypical manifestations, CT examination results, and plasma catecholamine level, most atypical phaeochromocytoma should be diagnosed correctly.
Objective To explore of a surgical approach of posterior pericardial ascending-to-descending aortic bypass through a median sternotomy for complex coarctation and interrupted aortic arch adult patients with coexistent cardiac disorder. Methods We retrospectively reviewed the clinical data of 2 adult patients with complex coarctation and 1 adult patient with interrupted aortic arch and all with coexistent cardiac disorder who underwent ascending-to-descending aortic bypass in our hospital between April 2010 and January 2015. There were 2 males and 1 female with age of 35.6 (27-46) years. One patient was with complex coarctation, and prolapse of anterior mitral leaflet with moderate regurgitation. One patient was with complex coarctation, and bicuspid aortic valve with severe aortic regurgitation, and ascending aortic aneurysm. One patient was with interrupted aortic arch (type A), and bicuspid aortic valve with mild stenosis, and secundum atrial septal defect. The surgical approach used in all patients was the median sternotomy. After aorta, femoral artery and bicaval cannulation, hypothermic cardiopulmonary bypass was established. With posterior pericardial ascending-to-descending aortic bypass procedure for repair of complex coarctation and interrupted aortic arch with coexistent cardiac disorder. Results There was no death. The symptoms of the patients obviously improved. All the patients were alive with ascending-to-descending aortic bypass procedure at a mean follow-up ranged from 2 to 59 months. Except that one patient had residual upper-extremity hypertension, and needed antihypertensive medications taken postoperatively, other patients’ systolic blood pressure returned to normal level. All patients’ lower-extremity fatigability resolved. Postoperative computed tomography angiography (CTA) of the patients showed that dacron graft was unobstructed with no graft-related complications of kinking and narrowing, development of false aneurysms or other complications. Conclusion The surgical management of adult patients’ complex coarctation and interrupted aortic arch with coexistent cardiac disorder, a one-stage approach using pericardial ascending-to-descending aortic bypass through a median sternotomy is an alternative surgery.
Differential diagnosis of benign and malignant ground glass nodule (GGN) is of great significance to the early detection, diagnosis and treatment of lung cancer. Increasing attention has been paid to radiomics technology application in early diagnosis of benign and malignant GGN, which can analyze the characteristic appearances of GGN in non-invasive manner. This article reviews the latest research progress of radiomics in the diagnosis of GGN.
