  • Effects of Ulinastatin Therapy on Acute Lung Injury after Seawater Drowning in Rats

    Objective To investigate the protective effects of ulinastatin on acute lung injury ( ALI)induced by seawater drowning in rats. Methods Thirty male SD rats were randomly divided into three groups, ie. a control group, a model group, and an ulinastatin treatment group. The rats in the model group and the ulinastatin treatment group received intratracheal artificial seawater ( 4 mL/kg) instillation. Then the ulinastatin treatment group received ulinastatin ( 100 000 U/kg) injection after infusion of seawater while the model group received an injection of same amount of saline. The rats were sacrificed at 4 hours after instillation. The pathological changes of lung were evaluated by hematoxylin-eosin stain under light microscope. Lung wet/dry weight ratios were measured to assess the level of pulmonary edema.Concentrations of tumor necrosis factor ( TNF) -α, interleukin ( IL) -1β, IL-6, and IL-10 in bronchoalveolar lavage fluid ( BALF) were detected by enzyme-linked immunosorbent assay ( ELISA) . The myeloperoxidase activity in lung tissue homogenates were measured by colorimetric method. Results Ulinastatin treatmentsignificantly relieved the decline of PaO2 and lung pathological changes, inhibited myeloperoxidase activity,and reduced lung wet/dry weight ratios. Ulinastatin also inhibited the release of TNF-α, IL-1β, and IL-6,whereas increased the expression of IL-10 simultaneously. Conclusion Ulinastatin attenuates seawater induced ALI, which may be related to its inhibitory effects on inflammation reaction through regulating cytokine secretion.

  • Epidemiological Study of Prevalence and Risk Factors for Hypertension in Urban Areas of Chengdu City

    ObjectiveTo investigate the prevalence of hypertension and to find the cardiovascular risk factors in the urban residents of Chengdu city. MethodsBy cluster sampling, a population of 994 inhabitants were selected from 14 urban communities in Chengdu city between February and October 2010. They were 35-70 years old and had resided in the area for over 2 years. Hypertension questionnaire was used and physical examinations were taken to investigate. The definition of hypertension was determined by the Guidelines of Hypertension Prevention and Control made by National Revision Committee in 2010. Logistic regression model was used to define the risk factors for hypertension. ResultsThe prevalence rate of hypertension was 44.87%, and the standardized prevalence rate was 39.21% (male:41.07%, female:38.20%). The difference of prevalence rate between males and females was not significant (P>0.05). The prevalence rate of hypertension increased significantly with age. By multi-factor logistic regression analysis, age (OR=1.103, P<0.001), serum uric acid (OR=1.003, P=0.001), heart rate (OR=1.014, P=0.027), and waist circumference (OR=1.624, P<0.001) were the risk factors for hypertension. ConclusionThe prevalence rate of hypertension is high in urban communities of Chengdu city, and age, serum uric acid, heart rate, and waist circumference are the risk factors for hypertension.

  • Epidemiological Study of Dyslipidemia with Cardiovascular Risk Factors in Urban Communities of Chengdu City

    ObjectiveTo investigate the prevalence status and characteristics of dyslipidemia and cardiovascular risk factors in urban communities of Chengdu city. MethodsBy cluster sampling, a population of 994 inhabitants were selected from 14 urban communities in Chengdu city between February and October 2010. We investigated patients between 35-70 years old who lived in the area over 2 years, by using questionnaires survey, physical examination, and laboratory tests. According to the age, the respondents were divided into three groups: young group ( ≤ 44 years old), middle-aged group (45-59 years old) and elderly group ( ≥ 60 years old). We further investigated the major cardiovascular risk factors for hyperlipidemic patients. ResultsThe prevalence of dyslipidemia in Chengdu urban area was 28.47% (283/994), and standardized prevalence rate was 24.65% (male, 24.68%; female, 30.91%). The prevalence rate between male and female had significant diTherence (χ2=4.513, P=0.034). Female elderly group and middle-aged group had a significantly higher prevalence than the young group (P<0.05), while the male prevalence had no statistical diTherences among all age groups (P>0.05). Hypertension was most common in patients with dyslipidemia (male, 50.04%; female, 52.41%) followed by diabetes and coronary heart disease. There was no difference between male and female in the prevalence of dyslipidemia accompanied hypertension, history of stoke and coronary heart disease, abdominal obesity,or obesity (P>0.05). Higher prevalence with smoking and drinking was in male patients (P<0.05). ConclusionThe prevalence of dyslipidemia is relatively high in urban communities of Chengdu city. According to the region of hyperlipemia epidemiological characteristics and risk factors, by using comprehensive intervention measures, we can reduce the prevalence of dyslipidemia.

  • 日间手术模式接受意愿及满意度调查分析

    目的 调查日间手术患者对日间手术模式的接受意愿及其原因分析。 方法 采用问卷调查法,对2015年6月-8月日间手术的患者实施调查。由调查者于床旁亲自发放问卷,问卷当场回收。 结果 调查共发放问卷110份,回收有效问卷110份,有效率为100%。10例(9.1%)表示非常了解日间手术模式,34例(30.1%)表示了解日间手术模式,12例(10.7%)表示对日间手术模式了解一般,14例(12.5%)表示对日间手术模式不了解,40例(36.6%)表示对日间手术模式非常不了解。91例(82.7%)表示通过医生介绍知道日间手术模式的特点,28例(25.5%)通过亲朋好友的介绍了解一些日间手术模式的特点。98例(89.1%)表示通过住院知晓了日间手术模式的特点和相关信息,12例(10.9%)表示未获得日间手术模式的特点和相关信息。104例(94.5%)表示愿意接受日间手术模式。大部分患者愿意接受日间手术模式的原因是缩短了住院等候时间和治疗时间;不愿意接受日间手术模式的患者主要是担心出院后无医务人员的治疗和护理,影响术后的恢复。患者对就医过程、治疗效果、医护人员服务态度、出院指导均有较高的满意度,分别为96.4%、95.5%、99.1%和90.9%。 结论 日间手术模式是一种安全、快捷的手术模式,患者虽了解程度较低,但亲自体验后仍给予了高度认可。

  • Evaluation of Safety and Quality of 5 520 Cases in Day Surgery

    ObjectiveTo evaluate the safety and quality of patients in day surgery. MethodsThe clinical data of patients in day surgery from March to December 2014 in this hospital were collected. The incidence of complications, delayed discharge rate, rate of readmission to hospital, satisfaction, and so on, were analyzed. ResultsOf 5 520 patients were in day surgery, including laporoscopic cholecystectomy, hernia repair surgery, vocal cord polyps resection, breast minimally invasive surgery, varicose vein of lower limb, choledochoscopy, gastrointestinal polyposis, and so on. No patient was loss of follow-up, unplanned reoperation or death after day surgery. There were 39.95%(2 205/5 520) of wound pain and 0.51%(28/5 520) of postoperative nausea and vomiting in the complications. The delayed discharge rate was 0.62%(34/5 520), the rate of unplanned readmission to hospital was 0.49%(27/5 520), and the satisfaction rate was 98.99%(5 464/5 520). ConclusionFor the above surgery types in this study, the day surgery mode is safe and effective.

  • Analysis of the Causes of Delay Discharge Patients after Day Surgery in West China Hospital of Sichuan University

    ObjectiveTo analysis the reasons of delayed discharge in day surgery ward, so as to provide the basis for discharged quality monitoring of ambulatory surgery management. MethodsA retrospective survey was conducted on patients admitted in Department of Day Surgery Center of West China Hospital of Sichuan University from April 2012 to August 2014 after day surgery. The characteristics of patients classified as "delayed discharge patients" were described and the factors associated with delayed discharges were analyzed by using SPSS 19.0 software. ResultsA total of 14 560 patients were included, of which 81 patients were classified as "delayed discharge", and the rate of delayed discharge was 0.56% (81/14 560). Single disease delayed discharge rates were as follows: gallstone 2.13% (37/1 737), operation for varicose veins of lower limbs 1.91% (11/576), gastrointestinal polyps resection 0.33% (11/3 325), inguinal herniorrhaphy 0.63% (9/1 424), polyp of vocal cord resection 0.21% (4/1 879), breast package block minimally invasive resection 0.11% (2/1 761), choledochoscopy 0.06% (1/1 563) and other surgical 0.26% (6/2 295). No significant differences were found between the delayed discharge patients and normal discharge patients in age and gender (P>0.05). Compared with the normal discharge patients, there were significant differences in the four aspects of operation mode changes, postoperative complications, anesthesia factors and patient's own factors of delayed discharge patients (P<0.05). ConclusionThe changes of operation mode, postoperative complications, anesthesia factors and patient's own factors are related to the delay of hospital discharge. Strictly grasping the indications for ambulatory surgery and anesthesia patients, strengthening the admission education, letting the patients accept day surgery fully in psychological and preventing the possible complications and ensure the quality and safety after day surgery.

  • Effects of physical exercises on the vision of students in China: a meta-analysis

    ObjectivesTo evaluate the effects of physical exercises on the vision of students in China.MethodsTrials of the effects of physical exercises on students' vision were retrieved in databases of CNKI, WanFang Data, VIP, Web of Science, EBSCO, PubMed and The Cochrane Library from inception to September 2019. Two reviewers independently screened literature, extracted data and assessed risk of bias of included studies. Meta-analysis was carried out with RevMan 5.3 and Stata 13.0 software.ResultsA total of 8 studies including 681 students in the experimental group and 679 in the control group were included. The results of meta-analysis showed that physical exercises were beneficial to improving students' vision (SMD=1.02, 95%CI 0.90 to 1.13, P<0.000 01). The results of subgroup analysis showed that physical exercises in the elementary school students group (SMD=1.361, 95%CI 0.873 to 1.850, P<0.000 01), long period (>24 weeks) (SMD=1.502, 95%CI 0.984 to 2.019, P<0.000 01), medium frequency (3 times/week) (SMD=1.631, 95%CI 1.022 to 2.240, P<0.000 01), and short time (<90 min) (SMD=1.410, 95%CI 0.758 to 2.062, P<0.000 01) had superior results.ConclusionsPhysical exercises have beneficial effects on improving vision. Limited by quantity and quality of subjects, the above conclusions requires verification by more high-quality studies.

  • Influencing factor analysis of day surgery developing towards same-day surgery based on structural equation modeling

    Objective To analyze the factors influencing day surgery developing towards same-day surgery, and provide a reference for the promotion of same-day surgery. Methods A questionnaire on influencing factors of same-day surgery was prepared based on structural equation modeling (SEM), including 6 dimensions and 23 items. From January to October 2021, at the Day Surgery Center of West China Hospital, Sichuan University, the random sampling method was used to select the research subjects for a questionnaire survey, and SEM was used to analyze the factors impacting the iterative development of day surgery to same-day surgery service model. Results A total of 200 questionnaires were distributed, and 192 valid questionnaires were recovered, with an effective recovery rate of 96.0%. The Cronbach’s α coefficient of the questionnaire was 0.857, and the Cronbach’s α coefficient of each dimension was from 0.832 to 0.934. The KMO test value was 0.822, and the result of Bartlett sphericity test was χ2=4568.330, P<0.001. The model fit indexes met the standard requirements well. The result of SEM revealed that the standard path coefficients of “the hospital’s preliminary preparation for day surgery developing towards same-day surgery” impacting “the guarantee system related to surgical quality and safety”, “the guarantee system related to surgical quality and safety” impacting “the public’s awareness of day surgery developing towards same-day surgery”, “the guarantee system related to surgical quality and safety” impacting “the ‘hospital-community’ integrated collaboration network”, “the public’s awareness of day surgery developing towards same-day surgery” impacting “the ‘hospital-community’ integrated collaboration network”, and “the ‘hospital-community’ integrated collaboration network” impacting “the iterative upgrade of day surgery” were all larger than 0.5, indicating strong influences between these factors, but the absolute value of standard path coefficient of the “hospital’s development brought by the transform of day surgery to same-day surgery” impacting “the guarantee system related to surgical quality and safety” was smaller than 0.3. Conclusion The hospital’s preliminary preparation, the guarantee system related to surgical quality and safety, the development of the “hospital-community” integrated collaboration network, and the public’s awareness are the probable factors influencing the iterative development of day surgery to same-day surgery.

    The International Society on Thrombosis and Hemostasis (ISTH) recently released the first ISTH guideline for antithrombotic treatment of COVID-19, which provides recommendations on anticoagulant and antiplatelet agents for patients with COVID-19 in different clinical settings. The target audience includes clinicians in internal medicine, intensive care, infectious diseases, hematology, vascular medicine, residents, family physicians, and other health care providers providing inpatient or outpatient care to COVID-19 patients. This article interprets the important parts of ISTH guideline.
