  • A Combined Optical Positioning Method Based on 1-D and 2-D Cameras

    Optical positioning system has a wide range of application in many fields. This paper presents an optical positioning method which combines linear and plane cameras. 1-D image signal of linear camera is used to get 1-D coordinates of the targets, which can be processed quickly. We can use it to assist processing the 2-D image signals of plane camera under some constraints. Linear camera can reduce the amount of position searching and calculation in coordinates extracting of targets in 2D image, and it can also help identify multiple targets. The results showed that our method could reach an accuracy of 1.608 mm, which was about one thousandth of the measurable range, and the reconstruction time for 4 targets without geometric constraints is 23.87 ms, namely 41.9 fps.

  • Diagnosis and Surgical Intervention for Insulinoma:A Single-Institution Study

    Objective To analysis the characteristics of clinical manifestation, diagnosis, and surgical strategy of patients with insulinoma and to summarize the experience in diagnosis and treatment of insulinoma. Methods The medical records of 88 patients with a clinical and pathologic diagnosis of insulinoma in West China Hospital from Oct. 2003 to Jan. 2010 were reviewed and the basic informations and therapeutic data were collected. Results Among the 88 patients, 63 cases (71.6%) were female and 12 cases were multiple endocrine neoplasia Type I (MEN-1) tumors. Eighty-eight patients’ age was (38.59±11.95) years old, body mass index was 27.78±5.86, and tumor diameter was(1.62±0.70) cm. Eighty-six point four percent of the patients had Whipple’ triad and 79.5% of the patients had a overnight fasting ratio of plasma insulin to glucose greater than 0.3. Diagnostic sensitivity of transabdominal ultrasonography, computed tomography scan, magnetic resonance imaging, and intraoperative ultrasonography were 30.8% (24/78), 74.6% (53/71), 82.5% (47/57), and 100% (59/59), respectively. Distal pancreatectomy (28 cases) and enucleation(64 cases) were performed in those patients, and pancreatic fistula rate were 14.3% (4/28) and 37.5% (24/64) respectively(P>0.05). The total duration of hospitalization and postoperative hospitalization time of patients that performed the distal pancreatectomy or enucleation were 28 d and 16 dvs. 29 d and 13 d, respectively (P>0.05). Conclusions Clinical diagnosis of insulinoma can rely on Whipple’ triad and plasma insulin/glucose value of fasting for 15 hours. Modern imaging has a high sensitivity of localization to avoid blind pancreatic resection. Complete surgical resection is the treatment of choice and enucleation acts as safe as distal pancreatectomy .

  • The feasibility study of non-nasogastric tube placement throughout the perioperative period of classical Whipple pancreaticoduodenectomy

    ObjectiveTo evaluate the safety and feasibility of non-nasogastric tube throughout the perioperative period of classical Whipple pancreaticoduodenectomy. MethodsA retrospective analysis was conducted on the clinical data of 63 patients who underwent Whipple pancreaticoduodenectomy at West China Hospital of Sichuan University from June 2020 to August 2023. According to whether nasogastric tube was placed throughout the perioperative period, the patients were divided into nasogastric tube group (NGT group, n=32) and no nasogastric tube group (no NGT group, n=31). The intraoperative indicators such as gastrointestinal anastomosis mode and outcome measures such as postoperative hospital stay were compared between the two groups. Results① Preoperative baseline data: the proportion of benign diseases (P=0.005) and preoperative pancreatitis (P=0.003) of the no NGT group were higher than those of the NGT group, and other preoperative baseline data, such as age and sex, were not statistically significant (P>0.05). ② Intraoperative index: the proportion of circular stapler (P=0.009) was smaller and the operation time was shorter (P<0.001), but there was no significant difference in the anastomosis, intraoperative blood transfusion and intraoperative bleeding (P>0.05). ③ Postoperative outcome measures: the postoperative length of hospital stay (P<0.001) and the incidence of delayed gastric emptying (P<0.001) of the no NGT group were lower than those in the NGT group, but the postoperative time of food intake, incidence of pancreatic fistula, incidence of postoperative bleeding, and Clavien-Dindo grade of complications were not significant (P>0.05), and on one died within 30 d after operation. ConclusionsThe results of this study preliminarily show that, it is safe and feasible not to place nasogastric tubes throughout the perioperativeperiod in patients undergoing classical Whipple pancreaticoduodenectomy. Compared with placing nasogastric tube, it can shorten the postoperative hospital stay, reduce the incidence of postoperative delayed gastric emptying.

  • The Preliminary Discussion of The Effect of PCT on Severe Acute Pancreatitis

    Objectives To identify the effect of procalcitonin (PCT) on the choice of operating time for severe acute pancreatitis and assessment of postoperative complications by monitoring the level of serum PCT and recording the incidence of postoperative complications. MethodsNinety-three patients who underwent debridement and drainage of infected necrosis from Sep. 2009 to Dec. 2011 were included. Serum PCT was tested on the day of admission (0 day), one week after admission, one day before surgery, one day after surgery, and one week after surgery. According to the level of serum PCT, all the 93 patients were divided into 3 groups:low level group (PCT < 2 ng/mL), middle level group (PCT 2-10 ng/mL), and high level group (PCT > 0 ng/mL). Postoperative complications were also recorded. ResultsThe incidence of postoperative complications was 19.4% (18/93). There were postoperative bleeding in 13 patients, intestinal fistula in 14 patients, pancreatic fistula in 18 patients, pulmonary infection in 4 patients, residual abscess in 10 patients; re-operation in 15 patients, and death in 8 patients (the death rate was 8.6%). Besides the pulmonary infection, and grade A and C of pancreatic fistula, the incidence of various complications in middle level group and high level group were lower than low level group (P < 0.05). Patients with complications had significantly higher PCT value in one week after admission than onset (P < 0.05). PCT value was higher in the patients with intestinal fistula and residual abscess on one day after operation than on one day before operation (P < 0.05). Patients who died or with re-operation had significantly higher PCT value in one week after operation than on one day after operation (P < 0.05). In the dead patients, the serum PCT sustained at a high level after operation. ConclusionsThe level of serum PCT is correlated with the incidence of postoperative complications. Dynamic monitoring the level of serum PCT can guide the choice of operative time and assessment of postoperative complications including intestinal fistula, pulmonary infection, and death, which can greatly improve the prognosis of SAP.

  • Effect of Lymph Node-Targeted Chemotherapy with Carbon Nanoparticles Absorbing 5-Fluorouracil on Expressions of bcl-2, bax and Caspase-3 in Gastric Cancer Tissues and Metastatic Lymph Nodes

    Objective To determine whether lymph node-targeted chemotherapy with carbon nanoparticles absorbing 5-FU affects expressions of bcl-2, bax and caspase-3 in gastric cancer tissues, metastatic lymph nodes and normal gastric mucosa. Methods Twenty-eight patients with gastric cancer in our department were divided into lymph node-targeted chemotherapy (LNTC) group and control group from October 2005 to August 2006. The patients were treated with carbon nanoparticles absorbing 5-FU before operation in LNTC group and those were operated directly in control group. The gastric cancer tissues, metastatic lymph nodes and normal gastric mucosa were collected after operation. The expressions of bcl-2, bax and caspase-3 in those tissues were determined by immunohistochemical technique. Results In LNTC group, the positive expression rate of bcl-2 in gastric cancer tissues and metastatic lymph nodes was significantly lower than those in control group (28.6% vs . 78.6% , 25.0% vs . 70.0% , P < 0.05), the positive expression rate of bax (85.7% vs . 28.6% , 80.0% vs . 30.0% ) and caspase-3 (57.1% vs . 14.3% , 55.0% vs . 15.0% ) in gastric cancer tissues and metastatic lymph nodes was significantly higher than those in control group ( P < 0.05). The positive expression rate of bcl-2, bax and caspase-3 in normal gastric mucosa was not significantly different between two groups ( P > 0.05). Conclusion The lymph node-targeted chemotherapy with carbon nanoparticles absorbing 5-FU can down-regulate the expression of bcl-2 and up-regulate the expression of bax and caspase-3 in gastric cancer tissues and metastatic lymph nodes, and therefore by affecting the expression levels of these apoptosis molecules may be one of the ways to induce tumor cell apoptosis.

  • Clinical Analysis of Diagnosis and Treatment for 45 Cases ofmall Bowel Obstruction Due to Bezoars

    目的 总结对粪石性小肠梗阻患者的诊治经验。方法 对我院2000~2012年期间收治的45例成人粪石性小肠梗阻患者的病史、X射线表现、CT表现、治疗情况等进行回顾性分析。结果 本组有22例患者发病前曾进食柿子、山楂或黑枣,7例有胃部手术史,9例患有糖尿病。39例行腹部CT,其中35例可见小肠扩张与萎陷肠管移行区椭圆形斑点状团块。22例经非手术治疗缓解,随访2~3个月无肠梗阻复发;23例行剖腹探查,其中1例于术后21 d死于急性呼吸窘迫综合征,其余均治愈。结论 进食富含鞣酸食物史、胃部手术史、糖尿病史、典型螺旋CT表现是诊断的关键因素,部分患者可经保守治疗缓解,若保守治疗无效,则行手术探查,手术应先试行手法碎石,并全程探查胃肠道以避免多发粪石残留。

  • Preoperative Concurrent Chemoradiotherapy Combined with Total Mesorectal Excision in Treatment for Locally Advanced Lower Rectal Cancer

    Objective To evaluate the efficacy of preoperative concurrent chemoradiotherapy combined with total mesorectal excision (TME) in treatment for locally advanced lower rectal cancer. Methods The clinical data of 31 patients with locally advanced lower rectal cancer received concurrent chemoradiotherapy from January 2009 to December 2011 in this hospital were analyzed retrospectively. Conventional fraction radiotherapy with total dose 50 Gy and chemotherapy with mFOLFOX6 or CapeOX regimen were taken. The efficacy was assessed by recording results of clinical and pathological examination. The function of sphincter was also recorded. Results All 31 patients underwent TME operation. The complication morbidity and mortality was 12.9% (4/31) and 3.2% (1/31),respectively. As a result of the preoperative management,the tumor was reduced by an average of 21.9%, down-regulation of T stage was observed in 48.4% (15/31) patients,the frequency of lymph node metastasis decreased from 83.9% (26/31) to 38.7% (12/31). Pathological complete response was observed in 5 patients (16.1%) and the total response rate was 74.2% (23/31),grade 3/4 toxicity was occurred in 2 (6.5%) patients. 84.6% (22/26) of patients underwent sphincter preservation surgery reserved good function of sphincter. Conclusions Preoperative concurrent chemoradiotherapy combined with TME in treatment for locally advanced lower rectal cancer is effective and safe,which can lead to pathological complete response,decrease the tumor stage and the rate of lymph node metastasis,and can also increase the efficacy of operation.

  • Improve The Survival and Function of Islets by Combined Transplantation of Human Endothelial Cells in Peripheral Blood and Islet of Rats

    ObjectiveTo explore the endothelial cells from human peripheral blood and islet of rat co-transplantation under the renal capsule of diabetic nude mice to improve the survival and function of transplanted islet. MethodsThe endothelial cells from human peripheral blood(5×105)and freshly isolated rat islet cells were co-transplanted under the renal capsule of diabetic nude mice model, then the fasting blood glucose, body weight, peripheral blood C-peptide level, and intraperitoneal glucose tolerance test(IPGTT) were measured to evaluated the islet graft survival and function. ResultsCompared with the control group, the fasting blood glucose level significantly decreased(P < 0.01), peripheral blood C-peptide level rised(P < 0.01), and body weight increased(P < 0.01) of receptor nude mice in experience group, the IPGTT also improved. ConclusionThe endothelial cells from human peripheral blood and islet of rat co-transplan-tation can obviously improve the survival and function of transplanted islet of nude mice.

  • An optical coherent imaging system for measuring the strain of blood vessels

    We aimed to establish an optical coherence tomography (OCT) system to measure the strain of blood vessels. A general OCT system was constructed firstly and its reliability was confirmed by comparing the OCT imaging of the porcine coronary and the corresponding histological slices. The strain of the porcine coronary was induced by static flow pressure and correlation algorithm was used to calculate the strain field of blood vessels within OCT images. The results suggest that bright-dark stratification of blood vessels displayed in OCT images is consistent with the intima and media layers of histological image. Furthermore, the strain of media layer is greater than that of the intima layer under the same static pressure. The optical coherence imaging system could not only measure the histological structure of the blood vessels, but also qualify the vessel strain under flow pressure.
