  • Analysis of Fibrinogen Level in Patients Injured in Lushan and Wenchuan Earthquake

    ObjectiveTo explore fibrinogen (Fbg) variety in the patients wounded in Lushan and Wenchuan earthquake. MethodsAs the research subjects, 276 Lushan earthquake victims (from April 20th to 24th, 2013) and 503 Wenchuan earthquake victims (from May 13th to 17th, 2008) were divided into five groups according to the admission date, and then the Fbg variety in those earthquake victims were analyzed. We carried out pairwise comparison among these groups in the Lushan earthquake and Wenchuan earthquake respectively on the Fbg variety. Ninety-one fracture patients in the Lushsan earthquake and 130 fracture patients in the Wenchuan earthquake were divided into two groups according to clinical diagnosis:multiple fracture and single fracture, and then we evaluated the Fbg values in patients with different degrees of disability. ResultsThe whole Fbg level[(2.70±1.15) g/L] in Lushan earthquake was below the level[(4.47±1.94) g/L] in Wenchuan earthquake, and the difference was statistically significant (P<0.01). The Fbg level in the patients whose admission date was within 48 hours was significantly different with that in patients whose admission date was more than 48 hours (P<0.01). The Fbg level in Lushan earthquake was below the level in Wenchuan earthquake, not only in the multiple fracture group but also in the single fracture group[in Lushan earthquake, the former was (2.21±0.76) g/L, and the latter was (1.98±0.85) g/L; in Wenchuan earthquake, the former was (3.35±1.48) g/L, and the latter was (3.11±1.05) g/L], and the difference was statistically significant (P<0.01). ConclusionBlood coagulation, especially Fbg level, has different degrees of changes in acute stress caused by emergency and in different treatment times, and it is better to take preventive measures.

  • Correlation of Warfarin Dosage and Genetic Polymorphism of Han-patients after Heart Valve Replacement

    ObjectivesTo investigate the correlation of warfarin dose genetic and polymorphism of Han-patients after heart valve replacement, to forecast the anticoagulation therapy with warfarin reasonable dosage, and to realize individualized management of anticoagulation monitoring. MethodsWe selected 103 patients between January 1, 2011 and December 31, 2012 in West China Hospital of Sichuan University who were treated by oral warfarin after heart valve replacement with monitoring anticoagulation by international normalized ratio (INR) in Anticoagulation Therapy Database of Chinese Patients after Heart Valve Replacement. There were 32 males and 71 female at age of 21-85 (48.64± 11.66) years. All the patients' CYP2C9 and VKORC1 genetic polymorphisms were detected by using polymerase chain reaction-restriction fragment length polymorphism (PCR-RELP) method and gene sequencing technology. Warfarin concentration in plasma was determined by high performance liquid chromatography (HPLC) method. The activity of coagulation factorⅡ, Ⅶ, Ⅸ, Ⅹwas determined by Sysmex CA7000 analyzer. ResultsThe multivariate linear regression analysis showed that age, body surface area, and coagulation factor activity had no significant effect on warfarin dosage. While the gene polymor-phisms of CYP2C9 and VKORC1, warfarin concentration, and age had significant contributions to the overall variability in warfarin dose with decisive coefficients at 1.2%, 26.5%, 43.4%, and 5.0% respectively. The final equation was:Y=1.963-0.986× (CYP2C9* 3) +0.893× (VKORC1-1639) +0.002× (warfarin concentration)-0.019× (age). ConclusionMultiple regression equation including gene polymorphisms of CYP2C9 and VKORC1, non-genetic factors of coagulation factor activity, warfarin concentration, age, and body surface area can predict reasonable dosage of warfarin for anticoagulation to achieve individualized management of anticoagulation monitoring and reduce the anticoagulation complications.

  • Correlation analysis between platelet-related parameters and inflammatory indicators in patients with acute pancreatitis

    ObjectiveTo find out the diagnostic power and correlation between platelet-related parameters and inflammatory indicators in acute pancreatitis patients with different severity.MethodsA total of 88 patients with acute pancreatitis diagnosed in West China Hospital of Sichuan University from January 2019 to August 2019 were enrolled in the retrospective study, including 58 mild acute pancreatitis patients and 30 severe acute pancreatitis patients. The patients’ platelet-related parameters and inflammatory indicators were collected. The platelet parameters and inflammatory indicators in different severity acute pancreatitis groups were compared and the diagnostic power and correlation were analyzed.ResultsThe platelet count and plateletcrit of the severe group were lower than those of the mild group, the difference was statistically significant (Z=–5.502, –3.673; P<0.001). The immature platelet fraction, platelet distribution width, mean platelet volume and platelet-large cell ratio levels of the severe group were higher than those of the mild group (Z=–4.217, –2.998, –3.754, –3.816; P<0.05). Platelet-related parameters platelet count, immature platelet fraction, platelet distribution width, mean platelet volume, platelet-large cell ratio, plateletcrit and inflammatory indicators procalcitonin, interleukin-6 showed good diagnostic power with areas under under receiver operating characteristic curves greater than 0.75 in differentiating the severity of acute pancreatitis. Correlation analysis showed that immature platelet fraction was positively correlated with procalcitonin and interleukin-6 levels in patients with acute pancreatitis (rs=0.457, 0.385; P<0.05).ConclusionsImmature platelet fraction is correlated with the severity of acute pancreatitis, and positively correlated with the levels of inflammatory indicators procalcitonin and interleukin-6.
