  • Trends and age-period-cohort model analysis of incidence and mortality of falls among elderly in China from 1990 to 2019

    ObjectiveTo analyze the incidence and mortality trends of falls among elderly in China from 1990 to 2019, and to evaluate the impact of age, period, and cohort on their long-term trends. MethodsThe joinpoint regression model and age-period-cohort model were used to analysis. The annual percent change (APC) and the average annual percent change (AAPC) were used to analyze the incidence and mortality of falls among elderly in China from 1990 to 2019. The influence of age, period and cohort on the incidence and mortality of falls were analyzed. ResultsThe standardized incidence and mortality rates of falls among the elderly in China showed fluctuating upward trends from 1990 to 2019 (AAPC=1.929%, 1.535%, P<0.001), The increase rate of falls incidence was higher in men than that in women (AAPC=1.928%, 1.923%, P<0.001), and the increase rate of falls mortality was lower in men than that in women (AAPC=1.407%, 1.562%, P<0.001). The results of the age-period-cohort model showed that the age effect and period effect coefficients of incidence and mortality of falls in Chinese elderly men and women showed an overall fluctuating upward trend, while the cohort effect coefficients showed an overall downward trend. ConclusionFrom 1990 to 2019, the age standardized incidence and mortality rate of falls among the elderly in China show fluctuating upward trends. The incidence of falls in elderly women is higher than that in men, and the mortality rate of falls in elderly men is higher than that in women. It is recommended to strengthen the prevention work for falls in the oldest old, the incidence of falls in elderly women, and the mortality of falls in elderly men.

  • Establishment of Predictive Model for Survival in Patients with Hilar Cholangiocarcinoma

    ObjectiveTo establish a predictive model for survival and study it’s clinical value by reviewing the information of patients with hilar cholangiocarcinoma. MethodsMedical record of 196 patients with hilar cholangiocarcinoma were analyzed retrospectively. Seventeen possible clinicopathologic factors were selected. Cox model was used for univariate and multivariate analysis. Prognostic index (PI) was calculated based on the results of multivariate analysis. Patients with different PI were divided into three different risk level groups in order to compare the survival rate. Individual expected survival rate was calculated based on the median PI. Log cumulative hazards function plot was used to test Cox model proportional hazards assumption (PH assumption). ResultsThe significant prognostic factors influencing the survival rate were surgical procedure, surgical margin, and preoperative total bilirubin level (Plt;0.05). The predictive formula was PI=0.815×preoperative total bilirubin level+0.580×surgical margin-0.713×surgical procedure. According to the value of PI, all patients were divided into 3 groups, low risk group (PI≤-0.642), middle risk group (-0.642lt;PIlt;1.364), high risk group (PI≥1.364), and survival rate declined between groups and in groups with statistically significant difference (Plt;0.05). ConclusionThis model for survival can predict the prognosis of patients with hilar cholangiocarcinoma individually and help to conduct individual clinical therapy.

  • Effects of Estrogen Receptor Subtypes on Biological Behaviors of Human Breast Cancer Cell Line MCF-7

    ObjectiveTo establish MCF-7 cell lines with different ERα/ERβ expression level and observe their biological behaviors. MethodsERα or ERβ gene were silenced by RNA interference, and the cell lines with different ERα or ERβ expression level were obtained in MCF cell lines. MTT assay, flow cytometry, RT-PCR, double layer softagar clony formation test, and Matrigel adhesion assay were used to detect the abilities of cell proliferation, apoptosis, tumorigenesis, and adhesion. ResultsThe stable expression cell lines with ERαlow/ERβhigh or ERαhigh/ERβlow were established successfully. After ERα gene knockdown, MCF-7 grew slowly and was arrested at phase G0-G1. Apoptosis of MCF-7 was induced and the capacity of tumorigenesis and adhesion in vitro were weakened. However, the characteristics mentioned above except for adhesion changed to the opposite sides after ERβ gene knockdown. ConclusionsThe ERα gene silence can inhibit the formation of tumor, however the ERβ gene silence can promote tumor growth, invasion, and metastasis. Therefore, may be a useful approach on the breast cancer therapy.

  • 刺激诱发出的脑电异常对重症患者脑功能的临床意义

    刺激诱发出的节律性、周期性或者发作期放电(Stimulus-induced rhythmic,periodic,or ictal discharges,SIRPIDs)是Hirsch等在2004年首次命名的一种脑电现象,在意识障碍或者昏迷患者的长程脑电监测中所得。SIRPIDs机制可能与皮层下-皮层功能异常相关,尤其与丘脑皮层回路功能异常相关。但目前研究对于这一现象的病理生理学机制不甚明确。部分研究认为出现这一脑电现象提示患者预后较差,但从现有研究来看,该异常脑电活动能否引起神经损伤,或者这些异常是否仅仅为脑损伤严重的表现,以及是否需要药物干预治疗,临床中尚无法确定。文章目的是综述现有的文献了解SIRPIDs的概念、病理生理学机制、临床相关性,以及其对于神经重症患者脑功能的提示。

  • Clinical study of autoimmune optic neuropathy

    Objective〓〖WTBZ〗To observe the clinical features of autoimmune optic neuropathy (AON). 〖WTHZ〗Methods〓 〖WTBZ〗The clinical data of 58 patients with AON from Jan. 2006 to Dec. 2007 were retrospectively analyzed. The patients had undergone routine ophthalmological, neurological examination, visual field test, all set of autoimmune antibody test, brain MRI. 〖WTHZ〗Results〓〖WTBZ〗In 93 eyes of 58 patients with AON, the lowest best corrected visual acuity (BCVA) was lt;01 in 68 eyes (731%), 10 patients (172%) had other symptoms of nervous system, 14 patients (241%) had lesions of nonneurological system. Positive antinuclear antibody was found in 43 patients (796%); other abnormal antibodies were also found, including antiSSA/SSB, antidsDNA, antihistonic, anticardiolipin, and antihuman leukocyte antigen B27 antibodies. Systematic connective tissue disease presented in 20 patients (345%), such as sicca syndrome, systemic lupus erythematosus, and Behcet disease. 32 patients (552%) had abnormal brain MRI, and the BCVA of 49 eyes (777%) improved significantly after hospitalization. 〖WTHZ〗Conclusion〓〖WTBZ〗Patients with AON always have poor visual function, some of whom associate with other systems, as well as damages to other parts of the nervous system. While some AON patients are secondary to systemic connective tissue disease involving the optic nerve, the majority of these patients are isolated autoimmune optic neuropathy.

  • A visual analysis of the current research status of evidence-based social sciences

    Objective To analyze the research status and development trend of evidence-based social sciences, and to explore the synergistic relationship among branches of various fields. Methods A comprehensive search for research related to evidence-based social sciences published between the establishment of the database and September 2022 was conducted on the Web of Science, China Science Citation Database (CSCD), and Chinese Social Sciences Citation Index (CSSCI). The VOSviewer software was used to analyze the author and keywords and to perform the visualization. Results A total of 6 969 papers were included, 195 of which were in Chinese. The first paper, published in 1995, was on evidence-based management. The number of papers published on evidence-based social science research presented a trend of increasing continuously. The country with the most papers published was the United States; the institution outside China with the most papers published was McMaster University; and the institution in China with the most papers published was Sichuan University. The analysis of keywords showed that evidence-based policy, evidence-based practice, evidence-based management, and evidence-based decision-making appeared more frequently; research questions focused on evidence-based health policy development, application of big data in the context of COVID-19, evidence of climate change-induced disease, and real world research. Conclusion All the fields of evidence-based social sciences are closely related to each other in terms of research content and methods, and it presents a multi-level and multi-field crossover with evidence-based medicine. However, evidence-based social sciences in China are still in their infancy. Therefore, it is necessary to improve the methodological system of evidence-based social sciences, enrich the research content of social sciences, speed up the filling of evidence gaps in various fields, and promote the improvement and sustainable development of evidence-based social sciences.

  • Effect of NLRP3 gene silencing on expression of proinflammatory agents-induced inflammatory factors in rat brain microvascular endothelial cells

    Objective To study the effect of silencing the NOD-like receptor family, pyrin domain containing protein 3 (NLRP3) gene on the production of inflammatory factors induced by lipopolysaccharide (LPS) and adenosine triphosphate (ATP) in rat brain microvascular endothelial cells (BMECs), and whether NLRP3 inflammasome signaling pathway plays a role in the BMEC model of cerebral small vessel disease induced by proinflammatory agents. Methods BMECs from male Wistar rats were extracted in vitro and the morphology and purity of endothelial cells were identified. BMECs in normal culture were divided into blank control group and LPS+ATP group. The expression levels of NLRP3 inflammasome and downstream inflammatory factor Caspase-1 were detected by Western blot and real-time polymerase chain reaction, and compared by student’s t test between the two groups. Small interfering RNA (siRNA) was used to silence the specific gene NLRP3 in BMECs. After transfection of siRNA NLRP3 and siRNA plasmid negative control into BMECs, the transfected cells were divided into four groups, namely, siNC group (non silenced target gene), siNLRP3 group (silenced target gene), siNC+LPS+ATP group (non silenced target gene and added proinflammatory agents) and siNLRP3+LPS+ATP group (silenced target gene and added proinflammatory agents). The expression levels of NLRP3 and Caspase-1 were detected by Western blot and real-time polymerase chain reaction, and analyzed by analysis of variance for 2-factor factorial design. Results The microvascular segments of rat BMECs were “beaded” after 24 h of isolation and culture; after 48 h, “island” cell clusters were formed; after 72 h, “paving stone” like monolayer cells adhered to the wall and grew. After that, the cells gradually became dense and reached the convergence degree of 80%. The positive rate of BMECs detected by immunofluorescence staining was 96%. In the normally cultured cells, the protein and mRNA expression levels of NLRP3 and Caspase-1 in the LPS+ATP group were higher than those in the blank control group (P<0.05). In the RNA interference cultured cells, the protein and mRNA expression levels of NLRP3 and Caspase-1 in the siNLRP3 group were lower than those in the siNC group, and those expression levels in the siNLRP3+LPS+ATP group were lower than those in the siNC+LPS+ATP group (P<0.05); the protein and mRNA expression levels of NLRP3 and Caspase-1 in the siNC+LPS+ATP group were higher than those in the siNC group, and those expression levels in the siNLRP3+LPS+ATP group were higher than those in the siNLRP3 group (P<0.05). Plasmid transfection and proinflammatory agents intervention had statistically significant interaction effect on the mRNA expression of NLRP3 and Caspase-1 (P<0.05). Conclusions LPS and ATP can promote the release of NLRP3 and Caspase-1 in BMECs. Silencing NLRP3 gene expression can reduce the induction of proinflammatory agents. NLRP3 inflammasome signaling pathway may play a role in the cerebral small vessel disease cell model of rat BMECs induced by proinflammatory agents.

  • A prospective randomized clinical trial on intravitreous injection of Conbercept combined with laser photocoagulation for macular edema secondary to branch retinal vein occlusion

    ObjectiveTo investigate the effects and safety of intravitreous injection of Conbercept combined with laser photocoagulation for macular edema secondary to branch retinal vein occlusion. MethodsAll subjects were assigned randomly to 3 groups:intravitreal Conbercept combined with laser photocoagulation group (CL group), intravitreal triamcinolone combined with laser photocoagulation group (TL group), and photocoagulation group (L group). The best-corrected visual acuity (BCVA), central macular thickness (CMT), fundus oculi and fundus fluorescein angiography (FFA), intraocular pressure (IOP), slit lamp were observed before and 1 day, 1 week, 1 month, 3 months after treatment. The changes of post-treatment BCVA and CMT in pre-therapy and post-treatment were compared, and related complications were recorded. ResultsThere were significant differences of BCVA (χ2=9.754, 12.430, 17.424, 13.189) and CMT (F=10.614, 4.099, 4.927, 8.99) between 3 groups in post-treatment 1 day, 1 week, 1 month and 3 months. The numbers of subjects of improving and stabilizing BCVA in CL group were remarkably more than that in L group in every post-treatment follow-up time (P < 0.01), whereas the CMTs in CL group were significantly less than that in L group in every post-treatment follow-up time (P < 0.05). The CMTs in post-treatment 1 day, 1 week, 1 month, 3 months were thinner than that in pretreatment in CL group and TL group (P < 0.05). Meanwhile, there was no significant difference (P > 0.05)between any two CMTs in post-treatment 1 day, 1 week, 1 month and 3 months in CL group. Yet, the CMT in post-treatment 3 months was thicker than those in post-treatment 1 day, 1 week and 1 month in TL group(P < 0.05). And there was no significant difference(P > 0.05)between any two CMTs in post-treatment 1 day, 1 week and 1 month in TL group. There was no conspicuous difference in CMTs(P > 0.05)between in CL group and in TL group in every viewing time, except for that in post-treatment 3 months(P < 0.05). There was only 1 case of intraocular hypertension in post-treatment in TL group. ConclusionsIntravitreous injection of Conbercept combined with laser photocoagulation for macular edema secondary to BRVO is effective, safe, and superior to laser photocoagulation only. Also it had a longer effective duration and less complications than intravitreal triamcinolone combined with laser photocoagulation.

  • Feature of the angulation between left pulmonary artery and main pulmonary artery and its relationship to pulmonary artery development in patients with tetralogy of Fallot

    ObjectiveTo investigate the feature of the angulation between left pulmonary artery (LPA) and main pulmonary artery (MPA) and its relationship to pulmonary artery development in patients with tetralogy of Fallot (TOF).MethodsA total of 101 TOF patients in West China Hospital from 2014 to 2018 were enrolled in a TOF group, including 62 males and 39 females, aged 6.8 (0.3-45.8) years, and another 20 patients without basic cardiac diseases at the same stage were enrolled in a control group, including 10 males and 10 females, aged 6.9 (0.3-54.0) years. Diameters of LPA, right pulmonary artery (RPA) and MPA, the angulation between LPA and MPA (MPA-LPA), McGoon ratio, and Nakata index were measured and compared between the two groups. The relationship between the above data and MPA-LPA angulation was also analyzed.ResultsThe average MPA-LPA angulation was smaller in the TOF group than that in the control group (113.63° vs. 128.45°, P=0.001 8). The MPA Z score was also smaller in the TOF group than that in the control group (0.46 vs. 2.75, P=0.000 4). No relationship was found by correlation analysis between the MPA-LPA angulation and MPA Z score or LPA Z score in the control group (P=0.239 6, 0.114 7) and the TOF group (P=0.759 3, 0.242 7). The McGoon ratios (2.22±0.72, 2.43±0.94, P=0.340 0) and Nakata index (359.3±294.24, 395.52±329.31, P=0.650 0) were not significantly different between the two groups.ConclusionThe angulation of LPA-MPA and MPA Z score are smaller in the TOF group than those in the control group. There is no relationship between MPA-LPA angulation and pulmonary artery diameters. The LPA-MPA angulation should not be considered as an influence factor for LPA development and trans-annular patch surgery.
