Objective To evaluate the effect of sensory disturbance after the sagittal split ramus osteotomy (SSRO)on quality of life. Methods From September2004 to September 2005, 21 patients undergoing SSRO were studied by using questionnaires. There were 12 males and 9 females, aging 1927 years(mean 22.6 years).The patients were followed up at 4 weeks and 24 weeks after operation. The subjective questionnaire was completed by patient to evaluate the degree of living quality descending. Results At 4 weeks,19(90.48%) patients’ living quality descended because of postoperative sensory disturbance. Among them, 7(33.33%) patients had severe descending of living quality. The average effecting time of living accounted for total time 26.9%(about 6.5 h/d). At 24 weeks, 12(6316%) patients’ living quality descended because of postoperative sensory disturbance, 7(36.84%) patients could achieve preoperative living quality. The average effecting time of living accounted for total time 15.5%(about 3.7 h/d). Conclusion Most patients have descending of living quality after SSRO because of sensory disturbance. However, this condition can be improved during the followup and more than 1/3 patients canachieve their preoperative living quality.
Objective To explore the safety and clinical efficacy of right chest minithoracotomy for left atrial myxoma resection. Methods We retrospectively analyzed clinical data of 32 patients with left atrial myxoma resection by right chest minithoracotomy (a small incision group, 9 males, 23 females at age of 59.1±9.5 years) in our hospital from July 2011 through March 2015. Meanwhile, we selected 17 patients with left atrial myxoma treated by conventional chest median sternotomy as a control group (7 males, 10 females at age of 60.0±9.0 years). Clinical results of the two groups were compared. Results There was no statistical difference in preoperative clinical data of the patients between the two groups. All the patients were successfully operated. Patients in the small incision group had longer aortic clamping time than that in the control group. But there were shorter postoperative mechanical ventilation time (9.5±4.9 h), shorter ICU stay time (18.6 ± 6.2 h), less amount of thoracic cavity drainage 24 h after drainage (103.8±19.4 ml), lower bleeding reoperation rate (0.0), less blood transfusion after surgery (1.4±1.1U), shorter ambulation time (38.5±6.9 h), shorter hospital stay (8.1 ± 0.9 d), lower postoperative complication rate (0.0) than those of the control group (P<0.05). Conclusion Right chest minithoracotomy left atrial myxoma resection is feasible, safe and effective, is worth promoting.
ObjectiveTo explore the effect of family-school-hospital application in continuous nursing care for children with epilepsy. Methods120 children with epilepsy admitted to Children's Hospital Affiliated to Jiangnan University from January 2021 to October 2022 were randomly divided into two groups, each with 60 cases. The control group received routine care, while the experimental group received family-school-hospital continuous care. Compare the awareness of epilepsy knowledge, disease control effectiveness, medication compliance, negative emotions, physical and mental status, and quality of life before and after nursing between the families of two groups of children with epilepsy. ResultsAfter 2 months of nursing care, the scores of family members' knowledge of epilepsy in the experimental group were higher than the control group (P<0.05). The effect of disease control in the experimental group was better the control group (P<0.05). The drug compliance of the experimental group was higher than the control group (P<0.05). The quality of life score in the intervention group was higher than the control group (P<0.05). ConclusionThe application of family-school-hospital in the continuous care of children with epilepsy can improve their family members' awareness of epilepsy knowledge, effectively control the disease, improve medication compliance, improve negative emotions and physical and mental conditions, and thus improve the quality of life of children.
ObjectiveTo comprehensively analyze current status and progress of vagus nerve-preserving radical gastrectomy for gastric cancer at domestic and foreign. MethodRelevant literatures about vagus nerve-preserving radical gastrectomy at domestic and foreign since 1996 were collected and reviewed. ResultsVagus nerve-preserving radical gastrectomy for gastric cancer had been recognized by many scholars both at home and abroad, which had a higher operation safety and feasibility, could obviously reduce the occurrence of postoperative acute inflammation, maintain the stability of gastrointestinal function to the greatest extent, significantly reduce the incidence of postoperative complications such as gallbladder stone and diarrhea, improve the postoperative quality of life. ConclusionUnder premise of strictly controlling operation indications and ensuring curative effect, vagus nerve-preserving radical gastrectomy is of great significance to improve prognosis of patient with gastric cancer.
Objective To explore the effects on quality of life (QOL), the targeted rates of metabolic parameters and cost-effectiveness in newly diagnosed type 2 diabetic patients who underwent multifactorial intensive intervention. Methods One hundred and twenty seven cases in an intensive intervention and 125 cases in a conventional intervention group were investigated by using the SF-36 questionnaire. The comparison of QOL and the targeted rates of metabolic parameters between the two groups were made. We assessed the influence factors of QOL by stepwise regression analysis and evaluated the efficiency by pharmacoeconomic cost-effectiveness analysis. Results The targeted rates of blood glucose, blood lipid and blood pressure with intensive policies were significantly higher than those with conventional policy (P<0.05). The intensive group’s role limitations due to physical problems (RP), general health (GH), vitality (VT), role limitation due to emotional problems (RE) and total scores after 6 months intervention were significantly higher than those of baseline (P<0.05). The vitality scores and health transition (HT) of the intensive group were better than those of the conventional group after 6 months intervention. But the QOL scores of the conventional group were not improved after intervention. The difference of QOL’s total scores after intervention was related to that of HbA1c. The total cost-effectiveness rate of blood glucose, blood lipid, blood pressure control and the total cost-effectiveness rate of QOL with intensive policy were higher than those with the conventional policy. Conclusions Quality of life and the targeted rates of blood glucose, blood lipid and blood pressure in newly diagnosed type 2 diabetic patients with multifactorial intensive intervention policy are better and more economic than those with conventional policy.
Objective To summarize the advances in the clinical application of the quality of life(QOL) assessment after surgery of rectal cancer.MethodsThe recent literatures on the advances in the studies on the QOL assessment following the treatment of rectal cancer were reviewed. Results After surgery of rectal cancer, the patients might suffer from short and/or long term of various complications affecting the physical, social and psychological aspects of their well-being. The QOL assessment included many aspects of the studies. Global QOL scores as measured by the specific questionnaires improved significantly after surgery. It had provided clinical evidences of therapeutic methods, operative treatment of choices and bowel reconstructed procedures for the patients with rectal cancer.Conclusion The QOL studies after surgery for rectal cancer are of increasing importance. Apart from the improvements of the survival rates and the recurrencefree period, it is essential to provide an acceptable QOL postoperatively. The QOL assessment for the guidance of clinical treatment should be discussed routinely for a patient with potentially curable rectal carcinoma so that the excellent outcome could be achieved.
目的 探讨糖耐量异常患者的焦虑抑郁状况及其与生活质量的相关性,为糖尿病相关心理问题的早期识别与干预提供参考。 方法 以2010年1月-2012年6月糖耐量异常患者145例为试验组,健康人群147例作为对照组,两组分别填写焦虑自评量表(SAS)和抑郁自评量表(SDS),试验组还需填写世界卫生组织生活质量测定量表简表(WHOQOL-BREF)并对其焦虑、抑郁得分与WHOQOL-BREF的各因子的相关性进行分析。 结果 145例患者中有51例(35.2%)存在抑郁情绪,47例(32.4%)存在焦虑情绪,焦虑合并抑郁情绪者29例(20%)。糖耐量异常患者焦虑、抑郁评分明显高于对照组(P<0.01),其生活质量多个领域评分低于对照组(P<0.01),且生活质量与焦虑、抑郁情绪存在负相关(P<0.05)。 结论 糖耐量异常患者焦虑、抑郁情绪明显高于正常人群,其生活质量偏低,提示了对在该人群进行早期心理干预的必要性。
