PURPOSE: To investigate the treatment of severe bacterial endophthalmitis. METHODS:The curative effects of vitrectomy after intravitreal antibiotics and steroids (IVAS)for the treatment of 23 patients with bacterial endophthalmitis (group I)and vitrectomy and IVA at the same time for the treatment of 28 patients with bacterial endopbthalmitis (group I)were analyzed retrospectively. RESULTS: The rate of curative effects of two groups were similar,while the marked curative effects in group I (47.8% )was significantly higher than that of the group I (17.9%). The average period of eliminating infiamation of group I was longer than that of group I , and the incidence of postoperative retinal detachment of group Ⅱ was 3 times more than that of group I . CONCLUSION :It was indicated that vitrectomy after IVAS may increase the security of vitrectomy and the curative effects of severe bacterial ndophthalmitis.
Objective To investigate the clinical characteristics of retinal degeneration (RD) with retinal holes and the therapeutic effect of argon laser therapy. Methods The data of argon laser therapy in 210 RD patients (224 eyes) with retinal holes who underwent the treatment in our department were retrospectively analyzed, which was compared with the data of argon laser therapy in 173 RD patients (198 eyes) without retinal holes. Results In RD patients with retinal holes, 89.7% of the patients were less than 60 years old (53.3% males and 46.7% females). Grid-like degeneration was found in 65.6% of the patients in whom 87.5% had the range of degeneration less than 1 quardrant. There were oval-shaped holes in 60.7% of the patients and accompanied with limited rhegmatogenous retinal detachment (LRRD) in 23.7%. Compared with RD patients without retinal holes, the ratio of patients with the age ofge;35 years, cystic degeneration, retinal lengthways small plica, and subjective symptoms was higher in RD patients with retinal holes; while the therapeutic effect of argon laser therapy on patients with LRRD was obviously less than whom without retinal holes (Plt;0.01 ). Conclusions RD with retinal holes often occurs in youth, most of whom have grid-like degeneration with the range of le;1 qua drant. The major types of retinal holes are oval-shaped degeneration without retinal detachment. There was no sex difference in RD patients with retinal holes and most of the patients have no subjective symptoms. The therapeutic effect of prophylactic argon laser therapy on RD patients with retinal holes but no retinal detachment is satisfying. (Chin J Ocul Fundus Dis, 2006, 22: 39-41)
ObjectiveTo investigate the effect of nerve growth factor (NGF) on recuperate of optic nerve after contusion by clamping in adult rabbits. MethodsSixteen adult rabbits were randomly divided into NGF and the control group with 8 rabbits in each group. After the optic nerve of the right eyes was clamped,tissue engineering nerve containing 0.06 ml NGF(concentration: 5×10-4 g/L, NGF group) and 0.06 ml of PBS (control group) was immediately transplanted into the injured eyes respectively, and 0.02 ml NGF(concentration: 5×10-4 g/L, NGF group)and 0.02 ml of PBS (control group) were injected into the vitreous of right eyes respectively. Flash visual evoked potential (FVEP) test was performed on the eyes 1 day, 2 weeks and 8 weeks after the injury. The number of retinal ganglion cells (RGCs) and changes of optic nerves were observed by light microscopy and electron microscopy at the 8th week after contusion,and a computer-image-analysis system was used to count the optic nerve axons.ResultsThe ratio of amplitude of FVEP of the injured and healthy eyes was 0.765±0.150 in NGF group and 0.494±0.108 in the control at the 2th week after injury with a significant difference between the two groups (Plt;0.05); and was 0.581±0.138 and 0.409±0.119 respectively at the 8th week after contusion with statistical difference between the two groups (Plt;0.05). The results of light microscopy and electron microscopy showed that degeneration of RGCs and optic nerves in the NGF group was lighter than that in the control group 8 weeks after injury, while the amount of optic nerve axons was (10 955±608.7) axons/ mm2 in the NGF group and (7 898±608.8) axons/mm2 in the control with statistical difference between the two groups (Plt;0.05). ConclusionNGF may redound to the survival of RGCs and regeneration of the axons in some degree, which can promote the recuperation of optic nerve and visual function. Objective To evaluate the rapid diagnosis of bacterial and (or) fungal endophthalmitis by multiplex polymerase chain reaction (MPCR). Methods MPCR was performed to detect the DNA segment of bacteria and (or) fungi from standard strains and 41 samples of intraocular fluid or vitreous from 38 patients (3 with double eyes and 35 with single), and the results were compared with the cultured bacteria and fungi. Results Five hours after detected by MPCR, bacteria and (or) fungi in 34 out of 41 samples (82.9%) from patients were detected,in cluding bacteria in 26,fungi in 6,and both bacteria and fungi in 2. The positive rate of MPCR was obviously higher than the cultured ones(χ2=9.60, P<0.05). Conclusion With the advantages of rapidity, sensibility, and specificity, MPCR can make for the rapid and definitive diagnosis of bacterial and (or) fungal endophthalmitis. Objective To observe the amount of endothelial progenitor cells (EPCs) at different stages of diabetic retinopathy (DR) in patients with type 2 diabetes mellitus (DM). Methods Sixty patients with type 2 DM were divided into no DR (NDR) group, non-proliferative DR (NPDR)group and proliferative DR (PDR)group according to the examination of fundus and fundus fluorescein angiography, 20 patients in each group. Twenty healthy people were collected as the control group. 6 ml blood samples were taken from all the subjects, and then the EPCs contents in peripheral blood were detected by flow cytometry. Results The EPCs contents in peripheral blood of the control, NDR, NPDR and PDR group were (0.0179plusmn;0.0047)%, (0.0151plusmn;0.0086)%, (0.0123plusmn;0.1137)%, (0.0316plusmn;0.0 294)%. The EPCs contents in peripheral blood of the PDR group was significantly higher than those in others (chi;2=43.780, P<0.05); the EPCs contents in peripheral blood of the NDR and NPDR group were slightly lower than that in the control group (chi;2=5.244, P=0.73); the EPCs contents in peripheral blood of the NPDR group was lower than that in the NDR group (chi;2=6.016, P=0.12). Conclusion The EPCs contents in peripheral blood decreases in NDR, NPDR patients, while significantly increases in PDR patients.
ObjectiveTo study the prevalence of age-related macular degeneration (AMD) in the population aged 50 and over in Qidong County of Jangsu Province. Methods3644 individuals from 4 villages were randomly selected by clustering sampling method, according to the household registration information and door to door visits. Visual acuity was measured by modified Bailey-Lovie E logMAR chart. The examination of eyelids, cornea, lens and fundus were also carried out. The diagnosis of AMD was made according to the clinical hierarchy system by Age-Related Eye Diseases Study. χ2 test was used to analyze the prevalence of AMD and its related factors. ResultsAmong 3644 selected individuals, 2985 individuals received examination with a participating rate of 81.92%. In total 97 patients (136 eyes) had AMD with a prevalence rate of 3.25%. Among them, 71 patients (105 eyes) had early stage of AMD (2.38%); 26 patients (31 eyes) had late stage of AMD (0.87%). In these late stage patients, there were 9 patients (13 eyes) of exudative lesions (0.30%). There were 32 male (3.11%) and 65 female (3.32%) patients. There was no statistically significant difference between male and female prevalence (χ2=0.29, P > 0.05). Correlation analysis results showed that the long-term smoking (χ2=15.19) and heart cerebrovascular disease (χ2=81.50) was associated with AMD (P < 0.05). ConclusionsThe prevalence rate of AMD is 3.25% in the residents aged 50 and above in the rural area of Qidong County, Jangsu Province. Long-term smoking, high blood pressure and cardiovascular disease are the risk factors of AMD.
