  • High Intensity Focused Ultrasound for Myoma of Uterus Fibroid: A Meta-Analysis

    Objective To systematically review the effectiveness and safety of high intensity focused ultrasound (HIFU) for myoma of uterus fibroid. Methods Such databases as PubMed, EMbase, Web of Science, The Cochrane Library (Issue 10, 2012), CBM, CNKI, and WanFang Data were electronically searched from inception to November 2012 for randomized controlled trials on HIFU for myoma of uterus fibroid. Two reviewers independently screened literature according to inclusion and exclusion criteria, extracted data, and assessed the methodological quality of the included studies. Then, meta-analysis was performed using RevMan 5.0 software, and the quality levels of evidence were graded using GRADEpro 3.2 software. Results A total of 6 trials were included, involving 643 patients. The results of meta-analysis showed that, HIFU was similar to surgery in complete and partial fibroid ablation, with no significant difference; HIFU and radiofrequency were different in complete and partial fibroid ablation, with significant differences; and HIFU was superior to mifepristone in complete fibroid ablation, with a significant difference. Conclusion HIFU is an alternative, non-invasive, safe and effective treatment for myoma of uterus fibroid.

  • Clinical Observation of High Intensity Focused Ultrasound in Treatment for Human Mammary Fibroadenoma

    目的对高强度聚焦超声(high intensity focused ultrasound, HIFU)治疗乳腺纤维腺瘤的疗效进行评价。方法回顾性分析我科收治的24例乳腺纤维腺瘤患者的治疗资料。结果HIFU治疗术后恢复快,平均于术后1.5 d出院,无并发症发生; 术后随访3~12个月,有2例患者乳房包块消失,5例包块不同程度缩小,未消失包块针吸活检显示为坏死组织。结论HIFU治疗乳腺纤维腺瘤较为安全、有效,既能保持乳房外形美观,又不损害乳房功能,具有良好的应用前景。

  • Application of High Intensity Focused Ultrasound in the Treatment of Mid-Late Stage Liver Cancer

    Objective To study the mechanism of high intensity focused ultrasound (HIFU) and discuss its clinical significance in the treatment of mid-late stage liver cancer. Methods The patients with mid-late stage liver cancer were treated with HIFU. The changes of photography, pathology and immunology after operation were evaluated. Results It was shown that the results of the photographic, pathologic and immunologic examinations changed during the treatment of the patients with HIFU. Conclusion HIFU is effective and safe for the treatment of mid-late stage liver cancer.

  • Effects of High Intensity Focused Ultrasound on AFP mRNA in Peripheral Blood of Patients with Hepatocellular Carcinoma

    ObjectiveTo evaluate the effects of high intensity focused ultrasound (HIFU) on hepatic cancer cells spreading in blood.Methods AFP mRNA in peripheral blood of 19 patients with hepatocellular carcinoma was detected before and after HIFU therapy by RTPCR.Results①Before HIFU therapy, 11 of 19 cases were AFP mRNA positive (57.9%), while the control group were all negative. AFP mRNA was correlated with some clinical parameters such as serum AFP level, tumor size, portal vein embolism and extrahepatic metastasis (P<0.05). ②In 8 cases with preoperative AFP mRNA negative, only 2 cases became AFP mRNA positive immediately after therapy, and one of the 2 cases became negative again after 72 hours. One week after HIFU therapy, the AFP mRNA positive rate (31.6%) was much lower than the preoperative positive rate (57.9%), but there was no statistical significance (Pgt;0.05). ③After one week of HIFU therapy, the AFP mRNA positive rate of the group with tumor size less than 8 cm was much lower than that of tumor size larger than 8 cm (P<0.05). ConclusionHIFU may reduce the spreading of hepatocellular carcinoma cells in blood. It is effective for patients with tumor size less than 8 cm.

  • The Application of High Intensity Focused Ultrasound in the Tumor Treatment

    bjectiveTo evaluate the efficacy and limits of high intensity focused ultrasound (HIFU) in tumor treatment. MethodsThe references about the application of HIFU in tumor treatment in recent years were reviewed.ResultsHIFU caused localized hyperthermia at predictable depth in a few seconds to make the tumor tissue coagulative necrosis without injuring surrounding tissue. HIFU treatment had the advantages of low morbidity, noninvasiveness, avoidance of systemic side effects, and repeatitiveness. However, the utilization of HIFU sometimes could be limited by some factors such as imaging technique, organ movement, incomplete tissue destruction, etc.ConclusionHIFU is a promising noninvasive therapy for tumor treatment, though there are lots of problems to be further studied.

  • PRIMARY CLINICAL APPLICATION OF HIGH-INTENSITY FOCUSED ULTRASOUND ON INFANT HEMANGIOMAS

    Objective To explore the effectiveness and appropriate energy parameters of high-intensity focused ultrasound (HIFU) in treating infant hemangiomas. Methods Between January 2009 and September 2010, 60 infants with hemangioma were treated. There were 23 boys and 37 girls, aged from 3 to 30 months with an average of 10 months. These hemangiomas were located at head and face (24 cases), trunk (15 cases), l imb (16 cases), buttocks (2 cases), perineum (1 case),and multiple lesions (cervix, abdomen, and upper l imbs, 2 cases). The size of hemangiomas ranged from 0.8 cm × 0.6 cm to 6.0 cm × 5.0 cm. The 60 infants were randomly divided into 3 groups: groups A, B, and C (n=20) based on different ultrasound energies used in treatment. The lesion surface was irradiated with 3-5 mm/second for 5 continuously by ultrasonic therapeutic apparatus at a frequency of 9 MHz, impulse of 1 000, and 10% of scanning overlap; the powers of 3.5, 4.0, and 4.5 W were used in groups A, B, and C, respectively, 3 times as a course of treatment with 1 month interval. The effect and ulcer and scar risk in irradiation region were observed after 6 months of treatment. Results All cases were treated for one course. After 6 months of treatment, no significant difference in the effect was found among 3 groups based on hemangioma treatment judging criterion (P gt; 0.05). Neither ulcer nor scar occurred in group A; ulcer occurred in 4 cases (20%) of group B with superficial scars, and in 7 cases (35%) of group C with obvious scars. The rates of ulcer and scar in groups B and C were significantly higher than that in group A (P lt; 0.05). Conclusion HIFU irradiating is one of effective methods for treating infant hemangioma, but the appropriate energy was below 3.5 W.

  • 高强度聚焦超声致Ⅲ度烧伤的治疗探讨

    目的 探讨高强度聚焦超声治疗并发症的临床治疗。 方法 2001年4月~2005年11月,收治6例高强度聚焦超声Ⅲ度烧伤患者,应用局部皮瓣治疗。男5例,女1例;年龄45~78岁。烧伤均位于右季肋部。皮肤坏死范围3 cm×3 cm~6 cm×4 cm。手术清创后创面范围为9 cm×5 cm~12 cm×7 cm,切取局部皮瓣范围10 cm×6 cm~13 cm×8 cm。 结果 术后1例皮瓣发生红肿,经红外仪局部照射理疗后痊愈,余皮瓣均成活,创面Ⅰ期愈合。供区植皮成活,Ⅰ期愈合。术后随访3个月,皮瓣外观质地佳,无感染、积液。结论 应用局部皮瓣治疗高强度聚焦超声烧伤创面效果满意。

  • Analysis on Therapeutic Effects and Influencing Factors of Focused Ultrasound in 7027 Patients with Cervicitis Related Diseases

    Objective To explore the effectiveness, safety and applicability of focused ultrasound (FU) in rural patients with cervicitis-related diseases. Methods Patients with cervicitis-related diseases (n=8 352) from 80 township health centers of 9 counties in Chongqing were enrolled in this multicenter clinical study, and they were treated with CZF Model of FU for one time and then were visited in the following 3 months. According to the category 4 scoring method and the curative index, the effectiveness, safety and applicability in 7027 cases (1 156 mild cases, 5 312 moderate cases, 559 severe cases) with complete records were analyzed. The influencing factors were also analyzed with logistic regression analysis. Results A total of 6 413 cases (91.26%) were treated within 5 minutes. After 3-month follow-up, the total effective rate was 99.59%, and 5 012 cases (71.32%) were cured. Just little or medium amount of vaginal bleeding occurred in 656 cases (9.34%) after treatment, while only 12 cases (0.17%) had bleeding in the similar amount tomenstruation, which were relieved by hemostasis and anti-inflammatory treatment. Conclusion As a promising new therapy, focused ultrasound therapy for treating cervicitis-related diseases is safe and effective, with little postoperative bleeding and short-term of vaginal effusion, so it is suitable to be popularized in rural areas.

  • Clinical Study on the Chronic Cervicitis with Focused Ultrasound

    目的:探讨聚焦超声治疗慢性宫颈炎的安全性和有效性。方法:回顾性分析2003年1月至2006年12月我院门诊诊断治疗的慢性宫颈炎患者,行聚焦超声治疗后并于3月内随访的574例患者,分析其安全性及有效性。结果:574例中,痊愈 378例(65.9%),显效155例(27.0%),总有效率96.7%。治愈率与糜烂面积及深浅程度有关(Plt;0.05)。超声治疗后部分患者出现阴道少量流液及血性分泌物。结论:聚焦超声应用于慢性宫颈炎的治疗安全有效,疗效确切,不良反应及并发症小,值得推广。

  • Prognostic Analysis of High Intensity Focused Ultrasound Application in Primary Hepatocellular Carcinoma

    Objective To assess the survival of patients receiving high intensity focused ultrasound (HIFU) and investigate the prognostic factors for primary hepatocellular carcinoma (PHCC) victims with HIFU application. Methods One hundred and eighty-seven patients with PHCC undergoing HIFU treatment in our department were enrolled into this study from June 2004 to June 2007. Among them, 101 patients were males and 86 were females (mean age: 47.7 years old, range: 19-79 years old). The average tumor size was 5.7 cm (range: 0.5-18.0 cm). Of these 187 patients, numbers according to Child-Pugh grade of A, B and C were 104, 52 and 31, respectively. According to TNM system, 45, 111 and 31 patients were in stage Ⅱb, Ⅲa and Ⅲb respectively. Kaplan-Meier model and log-rank test were used in univariate analysis and Cox regression model was used in multivariate analysis to identify prognostic factors for survival. Results Survival period was (17.3±2.5) months after HIFU treatment of PHCC. The overall survival rate of 3-month, 6-month, 1-year and 2-year were 79.1%, 60.1%, 35.7%, and 29.3%, respectively. It was significant that tumor number (P=0.02), size (P=0.04), AFP (P=0.04), Child-Pugh grade (P=0.00), TNM stage (P=0.01), tumor metastasis (P=0.03) before HIFU, and tumor recurrence after HIFU (P=0.02) and standard treatment (P=0.02) were prognostic factors by single factor analysis. The following factors were identified as independent prognostic factors for overall survival by multivariate model: standard treatment protocol (P=0.000), and TNM stage (P=0.004) and Child-Pugh grade (P=0.009) before HIFU. Conclusion It is used for improving overall survival rate to found PHCC early, protect liver function, examine comprehensively before HIFU treatment, focus on standard treatment and auxiliary treatment.
