  • Application of High Intensity Focused Ultrasound in the Treatment of Mid-Late Stage Liver Cancer

    Objective To study the mechanism of high intensity focused ultrasound (HIFU) and discuss its clinical significance in the treatment of mid-late stage liver cancer. Methods The patients with mid-late stage liver cancer were treated with HIFU. The changes of photography, pathology and immunology after operation were evaluated. Results It was shown that the results of the photographic, pathologic and immunologic examinations changed during the treatment of the patients with HIFU. Conclusion HIFU is effective and safe for the treatment of mid-late stage liver cancer.

  • Clinical Observation of High Intensity Focused Ultrasound in Treatment for Human Mammary Fibroadenoma

    目的对高强度聚焦超声(high intensity focused ultrasound, HIFU)治疗乳腺纤维腺瘤的疗效进行评价。方法回顾性分析我科收治的24例乳腺纤维腺瘤患者的治疗资料。结果HIFU治疗术后恢复快,平均于术后1.5 d出院,无并发症发生; 术后随访3~12个月,有2例患者乳房包块消失,5例包块不同程度缩小,未消失包块针吸活检显示为坏死组织。结论HIFU治疗乳腺纤维腺瘤较为安全、有效,既能保持乳房外形美观,又不损害乳房功能,具有良好的应用前景。

  • Broadband subharmonic active cavitation imaging with high cavitation to tissue ratio

    To improve the cavitation-to-tissue ratio (CTR) of cavitation imaging during the treatment with high-intensity focused ultrasound (HIFU), we proposed a pulse inversion based broadband subharmonic cavitation imaging method (PIBSHI). Due to the fact that the subharmonic signal is a unique nonlinear vibration characteristic of cavitation bubbles, we extracted the broadband subharmonic signal to get a high-CTR cavitation imaging. The simulation showed that the subharmonic signal produced by cavitating bubbles with different sizes varied, and the signal was stronger than other subharmonics when the bubbles’ resonant frequency was close to 1/2 subharmonic frequency. Further experiment results demonstrated that compared with the conventional B-mode images, broadband subharmonic cavitation imaging (BSHI) has improved the CTR by 5.7 dB, and the CTR was further improved by 3.4 dB when combined with pulse inversion (PI) technology. Moreover, when the bandwidth was set to 100%~140% of the 1/2 subharmonic frequency in PIBSHI, the CTR was the highest and the imaging showed the optimal quality. The study may have reference value for the development of precise cavitation imaging during HIFU treatment, and contribute to improve the safety of HIFU treatment.

  • Clinical status of advanced pancreatic cancer treated with high intensity focused ultrasound

    ObjectiveTo explore the application value of high intensity focused ultrasound (HIFU) in the treatment of advanced pancreatic cancer.MethodThe domestic and foreign literatures about studies of HIFU treating advanced pancreatic cancer in recent years were retrieved and summarized.ResultsHIFU could prolong the survival time, control pain, and enhance the body’s immune function in patients with advanced pancreatic cancer. There were no obvious serious complications during the treatment process. The combined treatment with radiotherapy, chemotherapy, and traditional Chinese medicine could obviously prolong the survival time and improve the quality of life for the patients with advanced pancreatic cancer.ConclusionsHIFU is an important component in the comprehensive treatment of advanced pancreatic cancer. However, because there is no uniform standard for the dosage of HIFU treatment, the sample size of many related studies is small, so the research results have certain limitations, so more studies are needed to improve their understanding of advanced pancreatic cancer in order to better serve clinical workin future.

  • The Application of High Intensity Focused Ultrasound in the Tumor Treatment

    bjectiveTo evaluate the efficacy and limits of high intensity focused ultrasound (HIFU) in tumor treatment. MethodsThe references about the application of HIFU in tumor treatment in recent years were reviewed.ResultsHIFU caused localized hyperthermia at predictable depth in a few seconds to make the tumor tissue coagulative necrosis without injuring surrounding tissue. HIFU treatment had the advantages of low morbidity, noninvasiveness, avoidance of systemic side effects, and repeatitiveness. However, the utilization of HIFU sometimes could be limited by some factors such as imaging technique, organ movement, incomplete tissue destruction, etc.ConclusionHIFU is a promising noninvasive therapy for tumor treatment, though there are lots of problems to be further studied.

  • 超声造影与 MRI 增强扫描在高强度聚焦超声治疗子宫肌瘤中的疗效监测

    目的 比较超声造影(contrast-enhanced ultrasound,CEUS)与 MRI 增强扫描在高强度聚焦超声(high intensive focused ultrasound,HIFU)治疗子宫肌瘤中的疗效监测作用。 方法 选取 2013 年 1 月—2014 年 12 月采用 HIFU 治疗的 179 例子宫肌瘤患者(共 234 个肌瘤),对治疗前肌瘤体积及治疗后非灌注区肌瘤消融体积分别采用 CEUS 和 MRI 增强扫描检查。 结果 CEUS 和 MRI 增强扫描在治疗前肌瘤体积对比中差异有统计学意义(P<0.05),在治疗后非灌注区肌瘤消融体积对比中差异无统计学意义(P>0.05)。对治疗后残留两者均可有效检出。 结论 CEUS 和 MRI 增强扫描在 HIFU 治疗前都是治疗必须的检查,两者不可替代;两者均是治疗后疗效评价的有效方法,但 CEUS 比 MRI 增强扫描在治疗中更具灵活性、可重复性,在一定程度上可替代 MRI 增强扫描。

  • Numerical simulation of the focal region modulation to realize uniform temperature distribution during high-intensity focused ultrasound brain tumor therapy

    The temperature during the brain tumor therapy using high-intensity focused ultrasound (HIFU) should be controlled strictly. This research aimed at realizing uniform temperature distribution in the focal region by adjusting driving signals of phased array transducer. The three-dimensional simulation model imitating craniotomy HIFU brain tumor treatment was established based on an 82-element transducer and the computed tomography (CT) data of a volunteer's head was used to calculate and modulate the temperature distributions using the finite difference in time domain (FDTD) method. Two signals which focus at two preset targets with a certain distance were superimposed to emit each transducer element. Then the temperature distribution was modulated by changing the triggering time delay and amplitudes of the two signals. The results showed that when the distance between the two targets was within a certain range, a focal region with uniform temperature distribution could be created. And also the volume of focal region formed by one irradiation could be adjusted. The simulation results would provide theoretical method and reference for HIFU applying in clinical brain tumor treatment safely and effectively.

  • Efficacy and Safety of Focused Ultrasound for Cervical Ectopy: A Meta-analysis

    ObjectiveTo assess the efficacy and safety of focused ultrasound (FU) and microwave therapy (MW) for cervical ectopy (CE). MethodsWe searched the following databases:PubMed, EMbase, The Cochrane Library, CBM, VIP, CNKI and WanFang Data from inception to 30th August 2014. Two reviewers (Tang XL and Gao Z) independently screened literatures according to the inclusion and exclusion criteria, extracted data and assessed the methodological quality of included studies. Then, meta-analysis was performed by using RevMan 5.2.0 software. ResultsA total of 33 randomized controlled trials (RCTs) involving 1 759 patients were included. All the included studies were considered to be at high risk of bias. The results of meta-analysis showed that:compared with MW, FU could significantly reduce the risk of vaginal bleeding (RR=0.09, 95%CI 0.05 to 0.17, P<0.000 01) and vaginal discharge (RR=0.10, 95%CI 0.04 to 0.24, P<0.000 01), increase cure rate (RR=1.10, 95%CI 1.05 to 1.15, P<0.000 1) and total effective rate (RR=1.04, 95%CI 1.02 to 1.06, P=0.000 5). However, there was no difference in decreasing recurrence rate (RR=0.13, 95%CI 0.02 to 1.00, P=0.05). ConclusionCurrent available evidence suggest that FU is safer and more effective than MW for treating CE. Due to the limitation of quality of included studies, more high quality RCTs are needed to verify the above conclusion.

  • Analysis on Therapeutic Effects and Influencing Factors of Focused Ultrasound in 7027 Patients with Cervicitis Related Diseases

    Objective To explore the effectiveness, safety and applicability of focused ultrasound (FU) in rural patients with cervicitis-related diseases. Methods Patients with cervicitis-related diseases (n=8 352) from 80 township health centers of 9 counties in Chongqing were enrolled in this multicenter clinical study, and they were treated with CZF Model of FU for one time and then were visited in the following 3 months. According to the category 4 scoring method and the curative index, the effectiveness, safety and applicability in 7027 cases (1 156 mild cases, 5 312 moderate cases, 559 severe cases) with complete records were analyzed. The influencing factors were also analyzed with logistic regression analysis. Results A total of 6 413 cases (91.26%) were treated within 5 minutes. After 3-month follow-up, the total effective rate was 99.59%, and 5 012 cases (71.32%) were cured. Just little or medium amount of vaginal bleeding occurred in 656 cases (9.34%) after treatment, while only 12 cases (0.17%) had bleeding in the similar amount tomenstruation, which were relieved by hemostasis and anti-inflammatory treatment. Conclusion As a promising new therapy, focused ultrasound therapy for treating cervicitis-related diseases is safe and effective, with little postoperative bleeding and short-term of vaginal effusion, so it is suitable to be popularized in rural areas.
