  • The Application of Double Using Nagative Pressure Drainage-tube Used in Hepatic Operation

    目的:比较肝胆手术后两种腹腔引流管的应用效果。方法:将我院2005年5月~2007年3月间行肝脏手术病人106例随机分为实验组(57例)和对照组(49例),实验组采用自制双套管持续负压冲洗引流,对照组采用传统的单腔腹腔引流管引流,分析、比较两组引流的疗效。结果:采用双套管负压冲洗引流的病人发生引流管堵塞3例,隔下感染及积液4例,采用传统的单腔腹腔引流管引流的病人发生引流管堵塞12例,隔下感染及积液15例,差异有统计学意义(P<0.05)。结论:肝脏手术后应用双套管持续负压冲洗引流在防止引流管堵塞、隔下感染疗效优于传统的单腔腹腔引流管引流。

  • Protective Effect of Allogeneic Bone Marrow Derived Mesenchymal Stem Cells Transplantation on Hepatic Warm Ischemia Reperfusion Injury in Rats

    Objective To explore repair role of allogeneic bone marrow mesenchymal stem cells (BM-MSCs) transplantation on treating hepatic ischemia reperfusion injury (HIRI) in rats. Methods Ten rats were executed to get BM-MSCs, then BM-MSCs were cultured in vitro and dyed by 4,6-diamidino-2-phenylindole (DAPI). Models of 70% hepatic ischemia reperfusion injury were eatablished. Thirty two rats were randomly divided into sham operation group (Sham group), ischemia reperfusion group (I/R group), Vitamin C group (VC group), and BM-MSCs group. Serum samples were analyzed for ALT and AST, and hepatic tissue were for superoxide dismutase (SOD) and malondialdehyde (MDA). Liver sections were stain with hematoxylin and eosin (HE) for histological analysis, TUNEL staining was applied to detect hepatic apoptosis. Serum and tissues were both collected at 24 h after reperfusion. Results The isolated BM-MSCs maintained vigorous growth in vitro. Specific markers for MSCs antigens CD29 and CD44 were detected by flow cytometry, but antigens CD34 and CD45 were not be detected. Models of HIRI were stable, and BM-MSCs were detected around the periportal area by DAPI staining. Compared with I/R group, levels of ALT, AST, MDA, and AI in the VC group and BM-MSCs group decreased at 24 h after reperfusion (P<0.05), meanwhile SOD level increased (P<0.05). Compared with VC group, levels of ALT, AST, MDA, and AI in the BM-MSC group decreased at 24 h after reperfusion (P<0.05), meanwhile SOD level increased (P<0.05). Conclusion BM-MSCs could protect HIRI by alleviating oxidative stress and inhibiting cellular apoptosis.

  • Liver Function Alterations and Its Influencing Factors after Laparoscopic-Assisted Radical Gastrectomy for Gastric Cancer

    Objective To evaluate the changes of liver function after laparoscopic-assisted radical gastrectomy for gastric cancer and analyze related impact factors. Methods Patients with gastric cancer or colon cancer, who underwent radical gastrectomy or hemicolectomy between Jun 2008 and Jun 2010 in General Hospital of PLA, were included in this study. These patients were divided into open gastrectomy (OG group, n=43), laparoscopic-assisted gastrectomy (LAG group, n=35), and laparoscopic-assisted hemicolectomy (LAC group, n=23). The serum AST, ALT, TB, and ALP levels of all patients enrolled on the preoperative day and post operative day (POD) 1, POD3, POD5, and POD7, and related impact factors were analyzed. Results Compared with the preoperative results, serum AST and ALT levels of patients increased until POD5 in both LAG and OG groups (P<0.05), and there was no changes in liver function after operation of patients in LAC group (P>0.05). In addition to that serum AST and ALT levels of patients in LAG group in POD1 were significantly higher than those in OG group (P=0.035 and P=0.041), and that serum ALT level of patients in LAG group was significantly lower than that in OG group in POD3 (P=0.048), serum AST and ALT levels of patients in two groups in the remaining time points were not statistically significant difference (P>0.05). The serum AST and ALT levels of patients in LAG group were significantly higher than those in LAC group during 5 d after operation (P<0.05).There was no significant changes of serum ALP and TB levels of patients in LAG and OG groups on the before and after operation (P>0.05). The increased serum ALT level of gastric cancer patients after operation related to body mass index (BMI, P=0.038), operative time (P=0.011), intraoperative hepatic injury (P=0.035), and abnormal ligation of hepatic artery (P=0.048), instead of the type of operation (OG vs.LAG, P>0.05). Conclusions Gastric cancer patients who underwent radical gastrectomy have transient liver dysfunction, which attributes mainly to direct liver manipulation or abnormal ligation of hepatic artery, but not CO2 pneumoperitoneum. Laparoscopic-assisted radical gastrectomy is feasible and safe to the patients without serious liver damage or other vital organs disorders.

  • Methods and Applications for Quantitative Measurement of Iron in Human

    Objective To summarize the methods and applications for quantitative measurement of iron in human.Methods The methods and applications for quantitative measurement of iron in human were analyzed retrospectively via reviewing the literatures domesticly and abroad, and summarized the advantages and disadvantages respectively. Results The methods for quantitative measurement of iron included laboratory tests, pathology examinations, CT, superconducting quantum interference device investigation (SQUID), and MRI. Conclusions Laboratory test is the most simple and economic method for quantitative measurement of iron in human. Percutaneous liver biopsy is the gold standardmethod. Radiologic examinations, especially MRI, may be main methods of measuring liver iron content in future.

  • Significance of Indocyanine Green Measurement and Child-Pugh Classification for Evaluation of Preoperative Hepatic Functional Reservation

    ObjectiveTo investigate the effect of indocyanogen green retention rate (ICGR) measurement and Child-Pugh classification of liver function in the preoperative evaluation of hepatic functional reservation. MethodsThe level of ICGR at 15 min (ICGR15) and the percentage of liver fibrosis in 103 patients with liver cancer were measured before hepatectomy, and the Child-Pugh classification of liver function was evaluated before and after the operations, and their connections were analyzed. ResultsAs the rise of Child-Pugh classification of liver function, the percentage of liver fibrosis increased gradually, there were significant differences between any two Child-Pugh classification (Plt;0.05). There was a linear correlation between the ICGR15 and the liver fibrosis percentage (rs=0.960, Plt;0.05). The value of preoperative ICGR15 in patients with postoperative Child-Pugh classification from grade A to grade B or grade B to grade C was gnificantly higher than that in patients with stabilization of Child-Pugh classification before and after operations (Plt;0.05). ConclusionICGR15 combined with Child-Pugh classification can improve the accuracy for the evaluation of preoperative hepatic functional reservation.

  • Selection of Blood Occlusion in Operation of Hepatic Hemangioma

    目的探讨肝血管瘤切除术中血流阻断方法的选择。方法回顾性分析我院收治的19例肝血管瘤患者的手术方式。结果全组均行手术切除,术中出血50~1 500 ml(平均312 ml)。 术中根据血管瘤所在位置选择不同肝血流阻断方法,其中行半肝血流阻断4例,运用Glisson蒂横断式肝切除术或其分段原理阻断Glisson系统分支6例,间断阻断第一肝门7例,预置肝上、下下腔静脉和第一肝门阻断带并间断阻断第一肝门2例。 术后5例并发右侧胸腔积液,均经保守治疗后好转,手术并发症发生率为26.3%(5/19)。 术后住院7~41 d(平均16.9 d),均治愈出院。12例患者获随访,随访0.3~2年(平均1.1年),术前有症状的8例患者症状均消失,无复发,1例残留肝内血管瘤(直径lt;2 cm)。结论肝血管瘤患者肝切除术中的入肝血流阻断应强调个体化,根据肿瘤位置及大小选择不同的阻断方法,使患者术中出血少,术后恢复快。

  • Value of Gd-BOPTA in Diagnosis of Focal Liver Lesion and Biliary System Disease

    Objective To investigate the diagnostic value of a double action MR contrast agent——gadobenate dimeglumine (Gd-BOPTA) for focal liver lesion and biliary system disease. Methods Articles about Gd-BOPTA in CNKI and PubMed for the past few years were searched and the value of Gd-BOPTA in the diagnosis of focal liver lesion and biliary system disease was summarized. Results For focal liver lesion, Gd-BOPTA not only can reveal blood supply of the lesion, but also reveal the hepatocellular functional status in the lesion. For biliary system, biliary excretion of Gd-BOPTA can be used to evaluate the anatomic structure of bile duct, function of gallbladder and biliary system disease. Conclusions Gd-BOPTA has an important value in the diagnosis of focal liver lesion and biliary system disease. Gd-BOPTA may have wider applications in the future.

  • Clinical Application of Hepatic Operative Simulation on Liver Resection for Right Lobe Tumor

    Objective To develop hepatic surgical planning software for hepatic operation on deciding the rational operational scheme and simulating procedures before the operation to accomplish the precise liver resection and decrease the operational risk. Methods3D-econstruction of liver was restored from spiral computed tomography (CT) data by using LiVirtue software. The liver and its anatomic structures were reconstructed to illuminate the location of the tumor and its related vessels to design a rational operational scheme. The virtul results, such as liver volume, hepatic sections, anatomy of portal vein and hepatic veins or possible operation plans, were compared with the actual situations during the operations. Results3D models of liver, tumor and their relative vessels were reconstructed successfully. Preoperative planning and intra-perative navigation based on the models ensured the safety of liver resection in our 32 cases of right lobe tumors. This preoperative simulation allowed surgeons to dissect the liver with reduced complications. These models could be also viewed and manipulated on personal computers.ConclusionThe LiVirtue is very helpful in the hepatic surgery, for clearly disclosing hepatic structures, rationally deciding operation schemes, virtually simulating the operations. This preoperative estimation from 3D model of liver benefits a lot to complicated liver resection.

  • Detection and Significance of β-Glucuronidase mRNA in Human Liver and Kidney Tissues

    Objective To establish a new method for detection of β-glucuronidase (β-G) mRNA in human liver and kidney tissues. Methods β-G mRNA expression was detected by reverse transcription polymerase chain reaction(RT-PCR) in 10 cases of normal liver tissues, 10 cases of normal kidney tissues and 8 cases of hepatocellular carcinoma tissues. Results The expand products of β-G mRNA were expressed in liver and kidney tissues with similar size of 422 bp. The expression contents of β-G mRNA in liver and kidney normal tissues were different (1.71±0.32 vs 1.83±0.22) but without statistical significance (Pgt;0.05). However, the expression content of β-G mRNA in hepatocellular carcinoma tissues was 3.88±0.86, which was significantly higher (P<0.01) than that in normal liver tissues. Conclusion β-G mRNA determination is feasible by checking β-G gene alignment in gene bank and designing draw matter in the tissue of liver and kidney. It may be very significant to explore the change of β-G mRNA in various tissues in studying of molecular mechanism.
