  • 难治性哮喘中Th1/Th2失衡机制

    【摘要】 支气管哮喘是一种由多种细胞包括气道的炎性细胞、结构细胞和细胞组分参与的气道慢性炎症性疾病。临床上少数患者通过充分的哮喘治疗包括使用全身性激素治疗后仍不能有效控制, 通常称为“难治性哮喘”。免疫反应在难治性哮喘发病机制中起重要作用,而Th1/Th2失衡贯穿于难治性哮喘发病的整个过程。现就难治性哮喘中Th1/Th2失衡机制,以及影响因子(干扰素-γ、白介素-12、白介素-4、白介素-17,白介素-33和转化生长因子-β等)的研究作一综述,以深入探讨难治性哮喘的发病机制。

  • The effects of ceritinib and alectinib on ALK-rearrangement non-small-cell lung cancer (NSCLC): a single-arm meta-analysis

    ObjectivesTo systematically evaluate the effects of second-generation ALK-inhibitors: Ceritinib and Alectinib on ALK+ NSCLC patients.MethodsPubMed, EMbase, The Cochrane Library, WanFang Data, ClinicalTrials.gov and VIP databases were systematically searched for clinical trials containing treatment of two principal second-generation ALK-inhibitors for ALK (+) NSCLC patients from inception to December 31st, 2017. Two reviewers independently screened the literature, extracted data, and assessed the risk of bias included in the studies. Stata 12.0 software was used for meta-analysis.ResultsEleven studies were included. Meta-analysis showed that the ORR of all was 57% (95%CI 0.48 to 0.66, P<0.001). The ORR of patients with Crizotinib-resistance was 51% (95%CI 0.44 to 0.57,P<0.001). The IDCR of patients who had brain metastases was 78% (95%CI 0.71 to 0.86,P<0.001).ConclusionsThe second-generation ALK-inhibitors has effect on ALK (+) NSCLC. Due to limitation of the included studies, more larger sample studies are required to verify above conclusions.

  • An INTERACT3-based survey of neurosurgeons’ preferences for investigator-initiated trails

    Objective To understand the current status of the preferences and opinions on the investigator-initiated trails (IIT) of the neurosurgeons participating in INTERACT3 in China, as well as the design preference for IIT projects, and to provide a basis for the design and organization of multi-center clinical studies in the future. Methods Neurosurgeons with different seniority and professional titles from 89 domestic research institutions participating in the INTERACT3 project were collected from September to October 2023. The questionnaires were collected by questionnaire star. Results A total of 56 valid questionnaires were collected from 29 units. Among the 56 respondents, 52 neurosurgeons (92.86%) were from teaching hospitals and 45 (80.36%) were from grade A tertiary hospitals. 30 neurosurgeons (53.57%) had experience in conducting various clinical studies, and 55 neurosurgeons (98.21%) had experience in participating in various clinical studies. The main purposes of presiding over or participating in clinical research focused on “accumulating relevant experience and preparing for future projects” and “standardizing clinical diagnosis and treatment”, which were 89.29% and 83.93%. Respectively, regarding the way the case report form completing, respondents preferred to use electronic data collection systems (83.93%). Conclusions The purpose of the neurosurgeons interviewed to host or participate in clinical research is mainly to assist clinical and scientific research. Economic reasons have little impact on whether to participate in clinical research. The rationality and ease of operation of the trail design are the keys to attracting respondents to participate in clinical researches, and the level of remuneration has little impact on the decision-making of the respondents. The safety of clinical studies and the difficulty of enrolling subjects are the key factors that hinder respondents’ participation in clinical studies.

  • Kinematics Modeling and Analysis of Central-driven Robot for Upper Limb Rehabilitation after Stroke

    The present paper proposed a central-driven structure of upper limb rehabilitation robot in order to reduce the volume of the robotic arm in the structure, and also to reduce the influence of motor noise, radiation and other adverse factors on upper limb dysfunction patient. The forward and inverse kinematics equations have been obtained with using the Denavit-Hartenberg (D-H) parameter method. The motion simulation has been done to obtain the angle-time curve of each joint and the position-time curve of handle under setting rehabilitation path by using SolidWorks software. Experimental results showed that the rationality with the central-driven structure design had been verified by the fact that the handle could move under setting rehabilitation path. The effectiveness of kinematics equations had been proved, and the error was less than 3°by comparing the angle-time curves obtained from calculation with those from motion simulation.

  • Association between Interleukin-1α Polymorphisms and Risk of Intracranial Aaneurysm

    目的 探讨白细胞介素1α(IL-1α)基因多态性与颅内动脉瘤发生的关系。 方法 以2010年6月-2012年3月145例颅内动脉瘤患者和181例正常对照者为研究对象,采用聚合酶链反应-限制性片段长度多态性检测IL-1α-889C/T和+4845G/T多态性,统计分析基因多态性与颅内动脉瘤的相关性。 结果 IL-1α-889C/T位点:CT/TT基因型在颅内动脉瘤组中的频率为32.4%,显著高于其在对照组中的频率(21.5%),两组相比差异有统计学意义(χ2=4.90,P<0.05);T等位基因在颅内动脉瘤组中的频率为16.9%,显著高于其在对照组中的频率(10.8%),两组相比差异有统计学意义(χ2=5.17,P<0.05)。IL-1α+4845G/T多态性在两组人群中的分布差异无统计学意义(P>0.05)。 结论 IL-1α -889C/T多态性与颅内动脉瘤的发病有关,-889T等位基因可能是颅内动脉瘤的遗传易感基因。

  • Study on an Exoskeleton Hand Function Training Device

    Based on the structure and motion bionic principle of the normal adult fingers, biological characteristics of human hands were analyzed, and a wearable exoskeleton hand function training device for the rehabilitation of stroke patients or patients with hand trauma was designed. This device includes the exoskeleton mechanical structure and the electromyography (EMG) control system. With adjustable mechanism, the device was capable to fit different finger lengths, and by capturing the EMG of the users’ contralateral limb, the motion state of the exoskeleton hand was controlled. Then driven by the device, the user’s fingers conducting adduction/abduction rehabilitation training was carried out. Finally, the mechanical properties and training effect of the exoskeleton hand were verified through mechanism simulation and the experiments on the experimental prototype of the wearable exoskeleton hand function training device.

  • Research on Control System of an Exoskeleton Upper-limb Rehabilitation Robot

    In order to help the patients with upper-limb disfunction go on rehabilitation training, this paper proposed an upper-limb exoskeleton rehabilitation robot with four degrees of freedom (DOF), and realized two control schemes, i.e., voice control and electromyography control. The hardware and software design of the voice control system was completed based on RSC-4128 chips, which realized the speech recognition technology of a specific person. Besides, this study adapted self-made surface eletromyogram (sEMG) signal extraction electrodes to collect sEMG signals and realized pattern recognition by conducting sEMG signals processing, extracting time domain features and fixed threshold algorithm. In addition, the pulse-width modulation(PWM)algorithm was used to realize the speed adjustment of the system. Voice control and electromyography control experiments were then carried out, and the results showed that the mean recognition rate of the voice control and electromyography control reached 93.1% and 90.9%, respectively. The results proved the feasibility of the control system. This study is expected to lay a theoretical foundation for the further improvement of the control system of the upper-limb rehabilitation robot.

  • Double internal fixations of clavicle and scapula and intraoperative reduction of glenopolar angle for treatment of floating shoulder injuries

    ObjectiveTo investigate the effectiveness of double internal fixations of clavicle and scapula and intraoperative reduction of glenopolar angle in the treatment of floating shoulder injuries.MethodsBetween January 2010 and June 2019, 13 patients with floating shoulder injury were treated with double internal fixation of clavicle and scapula and intraoperative reduction of glenopolar angle. There were 11 males and 2 females with an average age of 48 years (range, 25-65 years). The causes of injury included falling from height in 2 cases, traffic accident injury in 3 cases, heavy object injury in 2 cases, and other injuries in 6 cases. There were 2 cases of Ⅰ-B-3 type, 1 case of Ⅰ-C-2 type, 1 case of Ⅰ-C-3 type, 3 cases of Ⅱ-B-2 type, 1 case of Ⅱ-B-3 type, 1 case of Ⅱ-B-4 type, 2 cases of Ⅱ-C-2 type, 1 case of Ⅱ-C-4 type, and 1 case of Ⅱ-D-3 type according to the classification of floating shoulder injury. All patients had unilateral clavicle fracture with scapular neck fracture, 1-4 superior shoulder suspensory complex (SSSC) injuries. The time from injury to operation was 7-17 days, with an average of 12 days. The glenopolar angle, subacromail space, anteroposterior inclination angle of scapular glenoid, scapular glenoid up and down angle were measured before and after operation; Constant-Murly score and Herscovici score were used to evaluate the recovery of shoulder joint function.ResultsAll incisions healed by first intention, and there was no early postoperative complications such as infection. All 13 cases were followed up 12-48 months, with an average of 25.2 months. Both the clavicle and the scapula had bone union, and the average healing time was 6 months and 4 months respectively. There were no complications such as nonunion, shoulder deformity, plate fracture or failure of internal fixation, acromion impingement syndrome, and frozen shoulder. At last follow-up, the glenopolar angle, subacromail space, anteroposterior inclination angle of scapular glenoid, and scapular glenoid up and down angle were all corrected significantly (P<0.05). The pain, function, activity, muscle strength scores, and total score in Constant-Murly score were significantly improved when compared with preoperative scores (P<0.05). According to the Herscovici scoring standard, the shoulder joint function was evaluated as excellent in 8 cases, good in 3 cases, and fair in 2 cases. The excellent and good rate was 84.6%.ConclusionDouble internal fixation of clavicle and scapula to stabilize SSSC and reduct glenopolar angle during operation is an effective method for treating the floating shoulder injury.
