  • Clinical analysis on Phrenitis after Spine Surgery

    【摘要】 目的 探讨脊柱外科手术患者术后发生谵妄的危险因素和有效防治措施。 方法 2007年1月-2009年10月应用ICU谵妄诊断的意识状态评估法观察1 835例脊柱外科术后患者。对于发生术后谵妄的患者随机分为治疗组和未治疗组,治疗组于谵妄诊断明确时即静脉注射氟哌利多5 mg。 结果 术后3 d,136例发生谵妄,谵妄发生率为7.41%。筛选出术后谵妄的可能高危因素包括高龄、术前合并高血压、术前合并糖尿病、术中出血量gt;600 mL、手术时间gt;4 h、术中应用激素、术后电解质紊乱和低氧血症、术后疼痛。发生谵妄的患者中,治疗组(68例)住院时间短于未治疗组(68例),差异有统计学意义(Plt;0.05)。 结论 高龄,术前合并高血压、糖尿病,术中出血量gt;600 mL,手术时间gt;4 h,术中应用激素,术后电解质紊乱、低氧血症及疼痛是脊柱外科手术患者术后发生谵妄的主要高危因素。氟哌利多治疗术后谵妄有效。【Abstract】 Objective To analyze the related factors influencing postoperative phrenitis in patients who have undergone spine surgery. Methods Postoperative phrenitis was evaluated with the confusion assessment method for the intensive care unit in 1 835 patients underwent spine surgery between January 2007 and October 2009. All the patients with postoperative phrenitis were randomly divided into two groups: treatment group and control group. The patients in treatment group underwent intravenous injection with droperidol (5 mg). Results Three days after the operation, 136 patients were diagnosed with postoperative phrenitis. The pre-operative complications of hypertension and diabetes, hemorrhage amount (gt;600 mL) during the operation, operative time (gt;4 hours), hormone usage during the operation, postoperative electrolyte disturbances, hyoxemia and pain were the factors influencing the morbidity of postoperative phrenitis. The length of hospital stay was shorter in the treatment group than that in the control group (Plt;0.05). Conclusions Senility, pre-operative complications of hypertension and diabetes, hemorrhage amount (gt;600 mL) during the operation, operative time (gt;4 hours), hormone usage during the operation, postoperative electrolyte disturbances, hyoxemia and pain were the factors influencing the morbidity of postoperative phrenitis. Droperidol is effective on postoperative phrenitis.

  • MICROENDOSCOPIC DECOMPRESSION VIA UNILATERAL APPROACH FOR LUMBAR SPINAL STENOSIS

    To investigate the effectiveness and safety of microendoscopic decompression via unilateral approach for lumbar spinal stenosis. Methods Between May 2006 and June 2009, 79 patients with lumbar stenosis were treated and divided into 2 groups: posterior lamina fenestration decompression (group A, n=37), endoscopic decompression via unilateral approach (group B, n=42). There was no significant difference in age, sex, segment level, and disease duration between 2 groups (P gt; 0.05). The cl inical outcomes were assessed by using the visual analogue scale (VAS) score and Oswestry Disabil ity Index (ODI). The operation time, blood loss, compl ications were compared between 2 groups. Results Operations were successfully performed in all cases. The operation time, blood loss, and drainage volume were (75.0 ± 25.7) minutes, (140.3 ± 54.8) mL, and (46.5 ± 19.7) mL in group A, were (50.4 ± 18.2) minutes, (80.2 ± 35.7) mL, and (12.7 ± 5.3) mL in group B; there were significant differences between 2 groups (P lt; 0.05). All the wounds healed by first intention. All patients were followed up 12-39 months (mean, 16 months). In group A, 1 patient suffered from intervertebral space infection after operation and recovered after conservative treatment; 4 patients had lumbar instabil ity after operation and recovered after lumbar interbody fusion combined with spine system internal fixation. In group B, 2 patients suffered from spinal dural rupture during operation and recovered after corresponding treatment, and no lumbar instabil ity was found. There was no significant difference in VAS score and ODI between 2 groups at preoperation (P gt; 0.05). Both VAS score and ODI were significantly improved at early stage after operation and last follow-up when compared with preoperation in each group (P lt; 0.05). Comparing with group A, there was significant improvement in VAS score at 24 hours postoperatively and in ODI at 1 month postoperatively in group B (P lt; 0.05), but no significant difference was observed at last follow-up (P gt; 0.05). According to cl inical evaluation of ODI mprovement rate, the excellent and good rate was 89.2% in group A and 92.9% in group B, showing no significant difference (χ2=0.896, P=0.827). Conclusion Comparing with posterior decompression surgery, microendoscopic decompression via unilateral approach is one of effective method to treat lumbar stenosis, with less trauma of fenestration yield and good early outcomes.

  • APPLICATION OF ACUTE EXTREME HYPERVOLEMIC HEMODILUTION IN SPINE SURGERY

    Objective To evaluate the security and validity of the acute extreme hypervolemic hemodilution (AEHH) in spine surgery. Methods Thirteen patients(8 males, 5 females; age, 16-65 years; weight, 50-75 kg) who had undergone major spine operations were enrolled in this study. Eleven of them had undergone anterior decompression, who were given the grafting and the internal fixation for their thoracolumber spinal burst fractures; the other 2 patients were given the correction operation for their scoliosis. The baselines of the haematocrit (Hct)were 0.363-0.481 before operation. The patients had no cardiac, pulmonary, hepatic or renal dysfunction or coagulation abnormality. The hemodynamic status and the haematocrit were observed during operation. The parameters of thromboelastography (TEG),arterial blood gas, and electrolytes were measured and observed at the following time points: before AEHH, after AEHH, 60 minutes after AEHH, 120 minutes after AEHH, and the end of the operation. The total fluid volume was recorded. Results The autologous blood volume was1 050-1 575 ml (average,1 419±198 ml), plasma substitute 2 100-3 150 ml (average,2 838±397 ml), blood loss1 000-3 130 ml (average, 1 747±743 ml), urine 450-1 270 ml (average, 871±374 ml), and the net blood transfusion 1 206-2 661 ml(1 863±598 ml). The homogenous blood of 400 ml was transfused in 1 patient for making upthe blood loss of 3 130 ml. There were no statistically significant differencesin the hemodynamic measurements, arterial blood gas, and electrolyte variables when compared with the baseline values before the hemodilution (Pgt;0.05). The reaction time of TEG was longer 60 minutes after AEHH than before AEHH (Plt;0.05); the other parameters of TEG had no differences when compared with the baseline values (Pgt;0.05). Conclusion The AEHH is safe and efficient in reduction of the perioperative homogenous blood transfusion in spine surgery.

  • Level Management Model in the Application of Nursing for Pressure Ulcers after Spinal Surgery

    ObjectiveTo explore the effect of level management model in the application of nursing for pressure ulcers after spinal surgery. MethodA total of 3 558 patients underwent spinal surgeries between January 2014 and September 2015 were selected. We established a level management model, confirmed the personnel responsibilities, strengthened the pressure ulcers risk assessment of new inpatients, and standardized the processes of reporting pressure sores. We carried out the level management model between January and June 2014, summarized experience and formed a system file between July and December 2014, applied to the clinical work after continuous improvement between January and September 2015. High risk of pressure ulcers reporting rate, incidence of high-risk pressure ulcers, and healing rate of pressure ulcers were compared at the various stages. ResultsThe reporting rate of pressure ulcers raised, the incidence of pressure ulcers increased and the curing rate decreased after continuous improvement (P<0.05). ConclusionsLevel management model may effectively improve the quality of the nursing for spinal pressure ulcers.

  • A retrospective study of the clinical value of whole spine magnetic resonance imaging in neurosurgical spinal surgery

    Objective To discuss the clinical value of whole spine magnetic resonance imaging (WSMRI) in practice of neurosurgical spinal surgery. Method A total of 70 cases examined using WSMRI between January 2015 and December 2016 were collected and analyzed retrospectively. Results All patients got clear images of WSMRI. Eighteen cases got important information, including spinal variation (1 case), multiple lesions (3 cases), combined lesions (6 cases) and large range multi-segmental lesions (8 cases), which were missed by single-segment MRI . Conclusions WSMRI can show all the spine, spinal cord and surrounding tissue in one image at one time. It has high clinical value because of its accurate positioning, comprehensiveness, time saving, and low rate of misdiagnosis and missed diagnosis.

  • Status survey on emotional disorder of inpatients in Department of Spinal Surgery

    Objective To identify the prevalence and related factors of emotional disorder of inpatients in Department of Spinal Surgery . Methods A cross-sectional study was conducted from October 2015 to April 2016 to screen 300 patients undergoing spinal surgery. Huaxi Emotional-distress Index was used to assess the emotional status of the patients, and a self-designed general condition questionnaire was used to evaluate the demographic data. Results The prevalence of emotional disorder of patients in Department of Spinal Surgery was 14.3%. Anxiety was the main type of emotional disorder. Logistic regression analysis showed that the education level and pathogeny were the main factors of emotional disorder. Conclusions In Department of Spinal Surgery, the inpatients’ psychological status is poor, and anxiety is the main emotional disorder. Emotional disorder is related to education level and pathogeny. Timely psychological treatment should be used in order to comprehensively improve the level of recovery of the inpatients.

  • Compliance improvement of rehabilitation exercise for patients after spine surgery

    Objective To explore the compliance improvement of rehabilitation exercise for patients after spine surgery. Methods Forty-nine in-patients who underwent spinal surgery between June 1st and July 14th 2016 were selected as the control group and 50 in-patients who underwent spinal surgery between July 15th and August 30th 2016 were selected as the trial group. The control group received conventional nursing for rehabilitation exercise, and the tiral group adopted a series of quality improvement measures for rehabilitation exercise. The compliance of functional exercise, the accuracy of patients’ exercise and the satisfaction with health education of the patients were compared between the two groups. Results The functional exercise compliance in the trial group (complete compliance in 45 cases, partial compliance in 4 cases, non-compliance in 1 case) was higher than that in the control group (complete compliance in 9 cases, partial compliance in 34 cases, non-compliance in 6 cases) with a statistical difference (Z=–6.910, P<0.001). The functional exercise accuracy rate of patients was higher in the trial group [84.00%(168/200)] than that in the control group [53.06% (104/196)] with a statistical difference (χ2=44.060, P<0.001). The patients’ satisfaction with health education in the trial group (4.64±0.49) was higher than that in the control group (4.20±0.89) with a statistical difference (t=3.084, P=0.003). Conclusion The implementation of nursing quality improvement intervention can improve the compliance and accuracy rate of rehabilitation functional exercise of patients undergoing spinal surgery, and improve the satisfaction of patients, which is worth promoting.

  • Percutaneous full-endoscopic bilateral decompression via unilateral posterior approach for lumbar spinal stenosis

    Objective To design the surgical strategy of percutaneous full-endoscopic bilateral decompression via unilateral posterior approach for bilateral lumbar spinal stenosis (LSS) and to evaluate the effectiveness. Methods The percutaneous full-endoscopic bilateral decompression via unilateral posterior approach for bilateral LSS was designed according to the pathological features of LSS. The technique was used to treat 42 patients with LSS between January 2016 and January 2018. There were 18 males and 24 females with an average age of 61.7 years (range, 46-81 years). The duration of symptoms was 1-20 years, with an average of 9.7 years. The surgical segment at L4, 5 were 27 cases, at L5, S1 were 15 cases. The operation time and perioperative complications were recorded. Lumbar X-ray, CT, and MRI examinations were performed at 1 week, 3 months, and 1 year after operation. Visual analogue scale (VAS) score was used to evaluate the low back pain and leg pain, Oswestry disability index (ODI) was used to evaluate the lumbar function, and single continuous walking distance (SCWD) was used to evaluate lower extremity nerve function. The clinical efficacy was evaluated by MacNab criteria at 1 year after operation. Results All patients underwent surgery successfully. The operation time was 68-141 minutes with an average of 98.2 minutes. All 42 patients were followed up 12-24 months with an average of 18.8 months. There were 2 cases of dural tears during operation, and 1 case of transient dysfunction of the lower limbs of the decompression channel after operation. All of them were cured after corresponding treatment. No serious complications such as death, major bleeding, or irreversible nerve injury occurred during follow-up. No segmental instability was found according to postoperative lumbar hyperextension and flexion X-ray films, and postoperative CT and MRI imaging showed that the stenotic lumbar spinal canal was significantly enlarged, and the compression of the nerve root was sufficient. The VAS score of low back pain and leg pain, ODI score, and SCWD at each time point after operation were significantly improved when compared with those before operation (P<0.05); the indexes were significantly improved over time after operation, and the differences were significantly (P<0.05). The clinical efficacy was evaluated by MacNab standard at 1 year after operation, and the results were excellent in 18 cases, good in 20 cases, fair in 3 cases, and poor in 1 case. The excellent and good rate was 90.5%. Conclusion The percutaneous full-endoscopic bilateral decompression via unilateral posterior approach for LSS is a safe and effective procedure. A well-designed surgical strategy and mastery of its technical points are important guarantees for successful operation and satisfactory results.

  • History and trends of robot-assisted spine surgery

    Spanning two decades since the 1st generation spinal robotics inception, the robot-assisted spine surgery (RSS) technology has evolved through generations, culminating in the 4th generation characterized by real-time visual navigation and wire-free screw placement. The fundamental principles of RSS technology include surgical planning, tracking, image registration, and robotic arm control technologies. Currently, RSS technology is maturely employed in thoracolumbar procedures and is progressively being applied in cervical surgeries, spinal tumor resections, and percutaneous operations, offering advantages in reducing tissue trauma and exposure to radiation, thereby improving patient outcomes. Emerging research also focuses on the cost-effectiveness of clinical applications and robot-specific complications. With the integration of artificial intelligence into surgical planning, RSS technology is poised to further incorporate emerging technologies and expand its application across a broader clinical spectrum.
