  • The Surgical Treatment of Lung Cancer with Cerebral Metastasis

    Objective To investigate the feasibility and clinical value of combined treatment for lung cancer with cerebral metastasis. Methods From May 1999 to May 2005, twentyone patients diagnosed lung cancer with cerebral metastasis received treatment in our hospital. The management consists of cerebral lesion resection, chemotherapy, lung cancer resection and chemotherapy or radiotherapy. Then evaluate the short-term and long-term outcome. Results No severe complications occurred in hospital. The 1-, 2-, and 3-year survival rates were 75.0%(12/16), 37.5%(6/16), and 12.5%(2/16) respectively. Life quality was promoted significantly. Conclusion Although lung cancer with cerebral metastasis is classified as stage Ⅳ,the effect of combined treatment is favorable because of the special metastatic position. But the choice of the patient is important.

  • 卵巢上皮性癌脑转移

    卵巢上皮性癌(EOC)是妇科三大恶性肿瘤之一,其死亡率高。卵巢上皮性癌脑转移的发生率较低,但是其预后差,近年EOC的脑转移发生率有增高趋势。脑转移可通过患者的症状、体征、头颅CT、MRI等方法较易诊断。目前缺乏对卵巢上皮性癌脑转移的标准治疗方案。其治疗方法可选择手术、全脑放射治疗(WBRT)、立体定位放射治疗(SRS)、化学疗法,综合多种治疗方法效果优于单一治疗方式。

  • The Efficacy of Surgical Treatment for Non-small Cell Lung Cancer with Single Brain Metastasis

    目的 探讨非小细胞肺癌合并孤立性脑转移灶的手术治疗效果,分析影响患者生存期的因素。 方法 回顾性分析2005年1月-2011年5月46例接受手术治疗的非小细胞肺癌合并孤立性脑转移灶患者的临床资料,其中男35例,女11例;年龄35~67岁,平均53.2岁;所有患者均行肺部原发肿瘤及脑部转移肿瘤切除术,其中肺叶切除术42例,全肺切除术4例,术后全部患者行全脑放射治疗,部分患者行系统化学疗法3~6周期。对随访患者的生存时间采用对数秩检验,分析影响生存率的因素。 结果 术后病理检查提示腺癌27例,鳞癌15例,大细胞癌2例,其他类型2例。患者1年生存率80%,2年生存率41%,3年生存率14%,中位生存期23个月,平均生存期(27.8 ± 4.5)个月(乘积极限法)。对数秩检验结果提示N0与N1患者比N2患者生存率高(P=0.024),腺癌患者生存期比非腺癌患者生存期长(P=0.002)。 结论 外科手术治疗非小细胞肺癌合并孤立性脑转移灶的患者可以取得良好的治疗效果,腺癌患者及无纵隔淋巴结转移的患者生存期长。

  • Curative Effects Observation of Capecitabine and Hessaitin Combined with Whole Brain Radiotherapy for Treatment The HER2 Positive Breast Cancer with Brain Metastasis

    ObjectiveTo investigate the curative effects of capecitabine and hessaitin combined with whole brain radiotherapy for treatment the HER2 positive breast cancer with brain metastasis. MethodsThe clinical data of 60 cases HER2-positive breast cancer patients with brain metastasis in our hospital in January 2004 to January 2012 were retrospectively analyzed.The 32 cases of observe group were treated by using capecitabine and herceptin combined with whole brain radiotherapy, and the 28 cases of control group were treated by using capecitabine and cisplatin combined with whole brain radiotherapy.The disease control rate, toxicity rate, and survival rate were evaluated. ResultsCompared with the control group, the disease control rate, toxicity rate, and survival rate were all better in observe group (P < 0.05). ConclusionThe capecitabine and hessaitin combined with whole brain radiotherapy for treating patients with HER 2 positive brest cancer transferred to brain are effect and safe.

  • Progress of stereotactic radiotherapy in the treatment of brain metastatic tumor

    Patients with brain metastases are more prone to developing life-threatening neurological symptoms. Initial therapies include surgery, whole brain radiotherapy (WBRT), and stereotactic radiotherapy. With the progress of stereotactic radiotherapy, the indication of stereotactic radiosurgery (SRS) is gradually expanding, and the indications for surgery and WBRT gradually narrowed. The existing studies have shown that SRS can significantly benefit patients who are <50 years old with single brain metastasis, but the specific scope of the application with SRS is still controversial, and a large number of the phase Ⅲ randomized multicenter trials designed around the controversies are also developing. This review summarizes the results of clinical research and came to the conclusion. Firstly, postoperative adjuvant SRS in the treatment of brain metastases is superior to postoperative adjuvant WBRT. Secondly, using SRS in the elderly patients with multiple brain metastases are safe and effective. Thirdly, the use of targeted therapy in patients with brain metastases thereby delaying SRS may lead to poor prognosis. The focus of future research include selection of optimal timing for adjuvant targeted therapy after SRS and the appropriate patient population, as well as prevention of recurrence and metastasis after lacal treatment.

  • Application of stereotactic radiosurgery combined with immune checkpoint inhibitors in brain metastasis

    Brain metastases are the most common intracranial malignant tumors in adults. Radiotherapy isa common treatment for brain metastases. In particular, stereotactic radiosurgery can control tumors well, and can significantly reduce the impact on cognitive function compared with whole brain radiation therapy. Immune checkpoint inhibitors have less toxic side effects in the treatment of patients with advanced tumors, and show good survival advantages. This article introduces radiotherapy, immunotherapy, stereotactic radiosurgery combined with immune checkpoint inhibitors for brain metastases, discusses the mechanism of stereotactic radiosurgery combined with immune checkpoint inhibitors, and its therapeutic value and research progress in brain metastases, aiming to provide a theoretical basis for the better application of stereotactic radiosurgery combined with immune checkpoint inhibitors to brain metastases.

  • Progress in diagnosis and treatment of brain metastasis from colorectal cancer

    ObjectiveTo elucidate the clinical and pathological features and review the progress of diagnosis and treatment in patients with brain metastasis (BM) from colorectal cancer (CRC), so as to provide a reference for the whole process management for patients with BM from CRC in China.MethodThe latest research results and previous literatures about patients with BM from CRC were reviewed.ResultsThe prognosis of BM from CRC was poor, its molecular pathological mechanism was complex and diverse, and some risk factors associated with the occurrence of BM had been identified. Typical imaging features of BM from CRC were helpful to the diagnosis of patients. At present, radiotherapy was still the main treatment. Bevacizumab treatment or immunotherapy combined with radiotherapy was expected to improve the survival of BM from CRC.ConclusionScientific and standardized prevention, diagnosis, and treatment are beneficial to reduce incidence of BM from CRC and improve survival.
