Objective To compare the immediate therapeutic effects between endovenous laser treatment and conventional surgery for lower extremity varicosity. Methods Data of 350 limbs of 275 patients treated by endovenous laser treatment (endovenous laser treatment group) and 310 limbs of 210 patients by traditional surgery (traditional surgery group) were analyzed and compared in terms of operation time, number of the incision, postoperative pain, complications, postoperative hospitalized duration and 1-year recurrence rate. Results Endovenous laser treatment group had shorter operation time, fewer incisions, less postoperative pain and shorter postoperative hospitalized duration than traditional surgery group (Plt;0.05). The complications and 1-year recurrence rate had no significant differences between two groups (Pgt;0.05). Conclusion As a safe and effective new treatment for lower extremity varicosity with minimal invasiveness and no left scars, endovenous laser treatment has the potential to replace conventional surgery and extend the surgical indications for lower extremity varicosity treatment.
目的观察腔内激光联合聚桂醇泡沫硬化剂治疗下肢静脉曲张的临床效果。 方法回顾性分析2009年2月至2013年7月期间接受腔内激光联合聚桂醇泡沫硬化剂治疗的40例下肢静脉曲张患者(联合组)和单纯接受腔内激光治疗的40例下肢静脉曲张患者(对照组)的临床资料,比较2种治疗方法的效果。 结果联合组和对照组患者的手术时间〔(43.95±6.77)min比(45.20±6.77)min〕和术中出血量〔(16.07±4.30)mL比(16.65±4.50)mL〕比较差异均无统计学意义(P>0.05),但联合组的手术费用低于对照组〔(7 056.83±940.45)元比(7 473.81±1 476.09)元〕,P=0.044。术后对照组的并发症发生率为20.0%(8/40),高于联合组的5.0%(1/40),差异有统计学意义(P=0.029)。术后所有患者获访4~24个月、(15.5±6.7)个月。随访期间,80例患者的下肢曲张畸形静脉均消失,对照组小范围复发3例(7.5%),联合组小范围复发2例(5.0%),2组复发率的差异无统计学意义(P=1.000)。 结论对下肢静脉曲张,腔内激光联合泡沫硬化剂治疗是一种安全和有效的新微创手术方法,具有并发症发生率低的优点,值得临床推广。
ObjectiveTo observe the efficiency of endovenous laser therapy combined with planning sucking operation in the treatment of great saphenous varicose veins. MethodsTotally 48 patients (60 limbs) were treated from May 2011 to May 2014 in general surgery department of our hospital. The main trunk of great saphenous vein was ablated by endovenous laser treatment; and the varicose veins in calf were resected by planning sucking operation. ResultsAll 48 patients (60 limbs) were cured without recurrent during 6-36 months followed-up. The operative time of each side was 18-43 min, the average operative time was 22.6 min; with 1-3 skin incisions. Hospital stay was 5-8 d, the average hospitalization time was 6.7 d. After operation, the varicose veins and the felling of swelling were disappeared, the pigmentation was reduced or disappeared. Local skin numbness showed in 6 cases and recovered in 3-7 months after operation. Felling of burns appeared in 2 cases, and was healed after treatment. Ankle swelling presented in 5 cases, and released in 6-13 d with related treatment. Different degree of subcutaneous bruising appeared without any hematoma, and recovered in 2-4 weeks. Two cases were lost during the followed-up. ConclusionsEndovenous laser therapy combined with planning sucking operation is safe and effective in the treatment of great saphenous varicose. It is worthy of promotion with minimum damage, less pain, fast recovery, no scars, shorter operative time, and shorter hospital stay.
