  • Effect of Nutrition Therapy of Low Glycemic Index Foods on Type 2 Diabetic Patients

    摘要:目的:探讨低血糖指数膳食干预对2 型糖尿病病人营养治疗效果的影响。方法:选择住院2 型糖尿病病人109例,随机分为营养组56例和对照组53例。营养组由营养师根据患者情况提供低血糖指数营养治疗饮食,对照组则自行控制饮食。分别于住院第1天与3个月复查时对两组患者进行膳食调查及相关生化指标测定, 以观察营养治疗的效果。结果:采用干预措施后, 两组空腹血糖、餐后2 h血糖、糖化血红蛋白、血清胆固醇、甘油三酯等生化指标均降低, 但营养组与对照组相比效果更为明显(P<005);营养组的饮食结构更为合理。结论:低血糖指数膳食可有效控制2 型糖尿病病人的血糖、血脂水平,对促进患者康复有积极意义。Abstract: Objective: To observed the effect of nutrition therapy of low glycemic index foods on type 2 diabetic patients. Methods: A total of 109 subjects with the hospitalized diabetes were randomly allocated into two groups: The nutrition group(56 cases) were provided with weighed individual low glycemic index foods and the control group(53 cases) went on diet dominated by themselves. patients in both groups were investigated on meals, diabetic nutrition knowledge and were detected for correlative biochemical indices. Results:After the nutrition treatment, patients biochemical indices of fasting bloodglucose, blood sugar 2 hours after meal, hemoglobin of glycosylation, cholesterol and triglyceride in serum in both groups were significantly lower. Compared with the control group, the effect of the nutrition group was even better. The acknowledgement rate of nutrition knowledge on diabetes of the nutrition group improved significantly, and their meals were more scientifically arranged. Conclusion: The nutrition therapy of low glycemic index foods would be very helpful for type 2 diabetic patients to control their bloodsugar level and improve the nutritional state and outcome.

  • The Nutritional Therapy of Diabetic Children

    目的:探讨儿童糖尿病患者的营养治疗方法以提高治疗效果。 方法:随机选择20例胰岛素依赖型儿童糖尿病患者,结合年龄、体重、病情、胰岛素用量、营养状况、生长发育需要设计个体化营养治疗方案。结果:通过营养治疗,除1例患儿家属在病情控制相对稳定,但血糖未有效控制提前出院外,其余19例患者均坚持营养治疗。治疗前后,随机血糖、餐后2 h血糖差异有统计学意义,P<0.05;糖化血红蛋白差异无统计学意义。结论:个体化的营养治疗有助于儿童糖尿病患者血糖控制,并能促进儿童的生长发育及提高生活质量。

  • 经外周静脉置入中心静脉导管在肠外营养支持治疗胃肠肿瘤患者中的应用现状调查及效果分析

    目的探讨经外周静脉置入中心静脉导管(PICC)在营养支持治疗胃肠肿瘤患者中的应用效果并进行分析。 方法2012年2月-4月选取需行肠外营养支持治疗的胃肠肿瘤患者418例为研究对象,进行静脉穿刺次数、穿刺点出血、穿刺周围皮肤红肿、皮温增高、穿刺肢体肿胀的临床观察及分析。 结果所有患者中,自愿选择PICC置管69例,置管期间穿刺点24 h内出血61例(88.4%)、24 h后出血2例(2.9%),周围皮肤发红8例(11.6%),皮温增高4例(5.8%),穿刺肢体肿胀2例(2.9%);选择静脉留置针349例,置管期间穿刺点24 h内出血4例(1.1%)、24 h后出血18例(5.2%),周围皮肤发红68例(19.5%),皮温增高54例(15.5%),穿刺肢体肿胀62例(17.7%)。 结论早期选择PICC静脉治疗方案,可降低患者静脉治疗中静脉炎、肢体肿胀等并发症及穿刺痛苦,优化管理整体治疗周期静脉治疗费用,是静脉治疗专科护理发展的趋势。

  • Two Nutrition Support Methods in Infants after Ventricular Septal Defect Repair Operation: A Case Control Study

    ObjectiveTo compare the outcome between two nutrition support methods, total enteral nutrition (TEN) and enteral nutrition combined with parenteral nutrition, in infants after ventricular septal defect (VSD) repair operation. MethodsWe retrospectively analyzed the clinical data of 76 infants who underwent VSD repair operation in Xinhua Hospital in 2012 year. There were 46 males and 35 females aged 1.6-11.9 (5.5±2.5) months. Nutrition support was started from the first day after operation. There were 35 patients in the group A with TEN 60 kcal/(kg·d), and 41 patients in the group B with both enteral nutrition at 30 kcal/(kg·d) and parenteral nutrition at 30 kcal/(kg·d). ResultsThere was no statistical difference between two groups in demography data and preoperative clinical indicators. The number of patients suffered abdominal distension and gastric retention was more in the group A (22.9% vs. 4.9%, 68.6% vs. 2.4%, P<0.05). There was no difference in diarrhea. The completion of nutrition support in the group A was worse than that in the group B. In the group A, only 40% of the goal calorie was finished on the first and the second day after operation. It was improved until the third day, and the goal calorie could be finished on the seventh day. In the group B, the nutrition support method could be finished on the first day. The prealbumin level in the group B was significantly higher on the third, fifth and seventh day (P<0.05). The blood urea nitrogen (BUN) level in the two groups on the first day after operation was higher than that before operation (P<0.05), and persisted in the group A, while decreased to the normal level gradually in the group B. Following up to discharge, the weight was higher and the length of stay was shorter in the group B (P<0.05). There was no statistical difference in the cost of hospitalization between the two groups (P>0.05). ConclusionThe nutrition support method, enteral nutrition combined with parenteral nutrition, is better than TEN for infants after VSD repair operation.

  • Study on the treatment effect of early enteral nutrition on severe acute pancreatitis

    Objective To explore the effect of early enteral nutrition in the clinical treatment of severe acute pancreatitis (SAP). Methods Forty-two SAP patients who improved with non-surgical treatments between January 2012 and December 2014 were selected and randomly divided into control group and study group with 21 patients in each. Patients in the control group accepted whole venous nutrition, while those in the study group had early enteral nutrition. Nutrition indexes, blood biological indexes, and Acute Physiology and Chronic Health Evaluation (APACHE) Ⅱ score on day 7 and 14 after treatment, and average hospital stay of the two groups of patients were recorded and compared. Results No differences in body weight reduction, lymphocytes, serum albumin, calcium, glucose, APACHE Ⅱ score between the two groups after 7 days of treatment were observed (P>0.05). But there were significant differences in body weight, lymphocytes, serum albumin, and APACHE Ⅱ score between these two groups after 14 days of treatment (P<0.05). Hospital stay of the study group was obviously shorter (P<0.01). Conclusions Early enteral nutrition support for patients with severe acute pancreatitis can improve nutritional status and shorten hospital stay. It is worthy of wider clinical application.

  • The effects of early enteral nutrition therapy combined with micro-ecological preparation on intestinal function recovery in patients with severe acute pancreatitis

    Objective To explore the effects of early enteral nutrition therapy combined with micro-ecologicalpreparation on early intestinal function recovery in patients with severe acute pancreatitis (SAP). Methods The clinical data of 48 patients with SAP treated from January 2012 to January 2015 were retrospectively analyzed. Out of the 48 cases, 23 were treated with early enteral nutrition therapy (the control group), and 25 were treated with early enteral nutrition therapy combined with micro-ecologicalpreparation (the observation group). The counts of white blood cells (WBC), the levels of blood amylase, serum lipase, lactic dehydrogenase (LDH), and the scores of gastrointestinal function 1 day before treatment and on the 7th and 14th day of treatment were observed. The incidences of complications, case fatality rates, and lengths of hospital stay were recorded. Results One day before treatment, the differences in the counts of WBC, the levels of blood amylase, lipase, and LDH between the two groups were not statistically significant (P>0.05). On the 7th day of treatment, the counts of WBC, the levels of blood amylase, lipase, and LDH in the two groups decreased in varying degrees, and the decreasing in the observation group was more obvious (P<0.05). On the 14th day of treatment, the levels of blood amylase in the two groups were almost normal, and the difference between the two groups was not statistically significant (P>0.05); the levels of lipase, LDH and the counts of WBC in the observation group were lower than those in the control group (P<0.05). One day before treatment, the difference in the scores of gastrointestinal function between the control group (1.34±0.76) and the observation group (1.46±0.62) was not statistically significant (P>0.05); on the 7th and 14th day of treatment, the scores of gastrointestinal function in the observation group (0.37±0.18, 0.29±0.06) were lower than those in the control group (0.63±0.32, 0.47±0.08), and the differences were statistically significant (P<0.05). There were significant differences between the two groups in terms of the incidence of total complications and length of hospital stay (P<0.05), while the difference in the case fatality rate between the two groups was not statistically significant (P>0.05). Conclusion Early enteral nutrition combined with micro-ecological preparation is benefit to the intestinal function recovery in patients with SAP.

  • Effects of enteral nutrition support therapy on immune and nutritional status in patients with severe acute pancreatitis

    ObjectiveTo explore clinical effect of enteral nutrition (EN) through nasojejunal tube in severe acute pancreatitis (SAP).MethodsThe clinical and pathological data of 76 patients with SAP who met the inclusion criteria and treated in the Third People’s Hospital of Henan Province from June 2015 to December 2018 were retrospectively collected. According to the nutritional support therapy adopted in the course of treatment, the patients were divided into an EN group (n=44) and a parenteral nutrition group (PN group, n=32), the patients in the EN group were given the EN through the nasojejunal tube and the patients in the PN group were given the routine PN. The APACHE Ⅱ and SOFA scores, nutritional status, and immune function were observed before the treatment and on week 2 after the treatment in the two groups.ResultsThere were no significant differences in the general data of the gender, age, body mass index, etc. between the two groups (P>0.050). ① The score of APACHEⅡ or SOFA score on week 2 after the treatment in both groups was significantly lower than that before the treatment (P<0.050), which in the EN group was significantly lower than that in the PN group on week 2 after the treatment (P<0.050). ② The levels of albumin and prealbumin were increased significantly and the hemoglobin level was decreased (P<0.050) on week 2 after the treatment as compared with those before the treatment in the two groups (P<0.050), which in the EN group were significantly higher than those in the PN group on week 2 after the treatment (P<0.050). ③ The CD4, CD8, and CD4/CD8 and IgG, IgM, and IgA on week 2 after the treatment were significantly lower than those before the treatment in these two groups (P<0.050), which in the EN group were significantly higher than those in the PN group on week 2 after the treatment (P<0.050). ④ The levels of endotoxin, D-lactic acid, diamine oxidase, and serum high mobility group protein B1 on week 2 after the treatment were significantly lower than those before the treatment in the two groups (P<0.050), and which in the EN group were significantly lower than those of the PN group on week 2 after the treatment (P<0.050). ⑤ The complications rate in the EN group was significantly lower than that in the PN group [20.45% (9/44) versus 53.13% (17/32), χ2=8.786, P=0.003].ConclusionEN through nasojejunal tube has a good effect on patients with SAP, which is helpful to improve their immune and nutritional status.
