目的 了解成都市围孕期妇女的孕前保健认知度及需求状况。 方法 2011年7月-2012年1月对成都市225名初产孕妇为调查对象,采用访谈及问卷进行调查,回收有效问卷217 份。 结果 217名中仅13名进行了孕前保健,孕前保健的参与率较低(5.99%);孕妇的年龄和受教育程度对孕前保健的认知度、参与率比较差异有统计学意义(P<0.05);孕前小剂量叶酸服用率11.20%,但正确服用率仅为1.50%。48.84%(106/217)听说过孕前保健,其中36.79%(39/106)知道孕前保健的最佳时间,但不完全了解孕前保健内容;55.67%(59/106)的夫妇知道需在孕前改变不良生活方式;21.69%(23/106)知道部分孕前检查内容,44.34%(47/106)知晓孕前补充小剂量叶酸预防神经管缺陷,仅3.77%(4/106)知道孕前遗传咨询。获取知识途径依次为网络22.64%(24/106),社区宣传或讲座24.53%(26/106),朋友同事33.96%(36/106),医院15.09%(16/106),书刊和广告3.77%(4/106),但仅10.10%的妇女是通过培训渠道获取的知识。高达89.40%的妇女迫切希望参加孕前保健及相关预防出生缺陷的知识培训,并希望去社区或妇幼专科医院接受相关孕前咨询及检查。 结论 成都市围孕期妇女孕前保健认知度较低,导致孕前保健的参与率较低、依从性较差,叶酸服用率低而不规范;孕前保健知识培训及服务形式、内容急待增强。围孕妇女孕前保健需求较高,倾向价廉、便捷的社区医院和妇幼专科医院。
ObjectivesTo investigate the awareness and knowledge of Chinese guideline developers on the patient version of guidelines (PVG).MethodsA questionnaire was developed and distributed to participants of the guideline development workshop on the " 2017 Chinese Medical Doctor Association Annual Conference on Evidence-Based Medicine & Gansu Medical Doctor Association/ Gansu Medical Association Annual Conference on Evidence-Based Medicine”, and guideline developers in Shenzhen, Guangzhou, Xi’an, Beijing and other places were investigated through field surveys. The questionnaire included ten questions centred on respondents’ awareness and their views on the development of PVG. After the invalid questionnaire was excluded, Excel 2013 software was used for data entry, and SPSS 19.0 software was used for data analysis.Results150 questionnaires were distributed and 107 (71.3%) were collected, from which 90 (60.0%) questionnaires with complete response were analyzed. For the awareness of PVG: 30.0% of respondents chose " just know it without more knowledge”, 34.4% chose " never heard of”. The awareness was not associated with educational or departmental background, majors and regions (P>0.05). For opinions on PVG and its development process: 86.7% of the respondents thought PVG is necessary, 90% thought the presentation of PVG needs to follow reporting standards, and 45.6% thought the primary barrier of the development of PVG is lack of cognition.ConclusionsResearch work on PVGs is at an initial stage in China. The awareness among Chinese guideline developers and relevant researchers is lacking and the development methodology requires further exploration. Carrying out research relevant to PVG can promote its development and application in China, so as to improve the clinical practice.
Objective To understand the cognition and acceptance of community hemodialysis centers among hemodialysis patients in Yangzhou, and to provide theoretical basis for the development of community hemodialysis centers. Methods A cluster random sampling method was used to select 400 maintenance hemodialysis patients treated in various areas of Yangzhou in April 2021 for a questionnaire survey to analyze the influencing factors of patients’ medical treatment behavior. Results A total of 390 valid questionnaires were recovered, with an effective recovery rate of 97.50%. Among the patients, 40.51% were very concerned about the construction of hemodialysis centers in the community, 56.67% understood the relevant policies, and 56.92% of the patients were willing to choose the community for dialysis treatment. The results of logistic regression analysis showed that the main factors affecting whether patients choose community for hemodialysis treatment include the patients’ residence [Jiangdu vs. Guangling: odds ratio (OR)=7.183, 95% confidence interval (CI) (2.010, 25.674), P=0.002; Gaoyou vs. Guangling: OR=22.512, 95%CI (7.201, 70.373), P<0.001; Yizheng vs. Guangling: OR=25.137, 95%CI (7.636, 82.744), P<0.001; Baoying vs. Guangling: OR=23.784, 95%CI (7.795, 72.569), P<0.001], degree of concern [some concern vs. very concerned: OR=0.267, 95 %CI (0.137, 0.521), P<0.001; not very concerned vs. very concerned: OR=0.062, 95%CI (0.023, 0.168), P<0.001; not concerned vs. very concerned: OR=0.101, 95% CI (0.023, 0.439), P=0.002], awareness [somewhat know vs. know very well: OR=0.025, 95%CI (0.002, 0.318), P=0.004; don’t know very well vs. know very well: OR=0.035, 95%CI (0.003, 0.439), P=0.009; don’t know vs. know very well: OR=0.006, 95%CI (0.000, 0.084), P<0.001]. Conclusions Hemodialysis patients in Yangzhou have a low level of awareness and acceptance of community-based hemodialysis centers. The patients’ residence, degree of attention and awareness of community-based hemodialysis center directly affect whether they choose the community for treatment. The relevant departments and medical institutions can start from the factors that affect patients’ choice of medical treatment, further strengthen the publicity of community dialysis, optimize the allocation of medical resources, and improve the capacity of community health services.
