  • CLINICAL STUDY ON TREATMENT OF OBSTINATE INGROWN NAIL WITH DISTAL PHALANX OF GREAT TOE AND SOFT TISSUE ORTHOPAEDICS

    Objective To investigate the clinical curative effect of distal phalanx of great toe and soft tissue orthopaedics for treatment of obstinateingrown nail. Methods From October 1997 to May 2006,31 patients(38 nails) suffering from obstinate ingrown nail were treated by the distal phalanx of great toe and soft tissue orthopaedics. There were 23 males(27 nails) and 8 females(11 nails) with an average age of 17.5 years(12-28 years). The disease course was 2years and 1 month to 14 years(average, 31-6 months). At the same time, thirty-eight patients with diseases of feet were selected randomly as controls. The depth of the nail groove was measured.The X-ray films were taken to calculate the rate of upward projection of tuberosity (r). Results Thedepth of the nail and r value of 31 patients were 2.87±0.31 mm and 0.149±0.013,respectively. There were statistically significant differences when compared with control group(1.06±0.10 mm and 0.060±0.019)(Plt;0.01). Thirty patients(37 nails) had a primary healing; 1 patient(1 nail) had a delayed healing. Twenty-nine patients(36 nails) were followed up for 8 to 29 months(average, 21 months). The appearance of the nail was satisfactory. No relapse occurred in all patients. Conclusion The upward projection of tuberosity of distal phalanx of great toe and deepened nail groove are the most important anatomical causes for ingrown nail. The distal phalanx of great toe and soft tissue orthopaedics is aneffective treatment for obstinate ingrown nail.

  • CLINICAL APPLICATION OF LOWER ROTATING POINT SUPER SURAL NEUROCUTANEOUS VASCULAR FLAP

    Objective To explore the clinical effect of the lower rotating point super sural neurocutaneous vascular flap on the repair of the softtissue defects in the ankle and foot. Methods From May 2001 to February 2006, 24 patients with the soft tissue defects in the ankle and foot were treated with the lower rotating point super sural neurocutaneous vascular flaps. Among the patients, 15 had an injury in a traffic accident, 6 were wringedand rolled by a machine, 1 was frostbited in both feet, 2 were burned, 25 had an exposure of the bone and joint. The disease course varied from 3 days to 22 months; 19 patients began their treatment 3-7 days after the injury and 5 patients were treated by an elective operation. The soft tissue defects ranged in area from 22 cm × 12 cm to 28 cm × 12 cm. The flaps ranged in size from 24 cm × 14cm to 30 cm × 14 cm, with a range up to the lower region of the popliteal fossa. The rotating point of the flap could be taken in the region 1-5 cm above thelateral malleolar. The donor site was covered by an intermediate thickness skingraft. Results All the 25 flaps in 24 patients survived with asatisfactory appearance and a good function. The distal skin necrosis occurred in 1 flap, but healing occurred after debridement and intermediate thickness skin grafting. The follow-up for 3 months to 5 years revealed that the patients had a normal gait, the flaps had a good sense and a resistance to wearing, and no ulcer occurred. The two point discrimination of the flap was 5-10 mm. Conclusion The lower rotating point super sural neurocutaneous vascular flap has a good skin quality, a high survival rate, and a large donor skin area. The grafting is easy, without any sacrifice of the major blood vessel; therefore, it is a good donor flap in repairing a large soft tissue defect in the ankle and foot.

  • MCKAY OPERATION IN THE CORRECTION OF RIGID CLUBFOOT

    For the evaluation of the result from McKay operation in the correction of rigid clubfoot, 71 patients (103 feet) were followed up postoperatively for 2 to 9 years. According to the criteria concerning the functional recovery of the foot, the external appearance of the foot after correction and the roentgenographic findings, the result was evaluated as excellent in 55 patients (78 feet), good in 11 patients (16 feet), fair in 5 patients (9 feet) and with no recurrence. It was considered that the favorable time for operation was when the child being 6 to 18 months old. In 7 patients (12 feet), there were morphological changes of the calcaneal and navicular bones in 12 feet, limitation of ankle movement in 9 patients and resultant flatfoot deformity in 5 patients (7 feet). The factors influencing the functional recovery of the foot and the relationship between the corrective result and the age of patient when the operation being given were discussed in details.

  • ECONSTRUCTION OF THUMB AND FINGER USING FREE NEUROVASCULAR BIG TOE NAIL SKIN FLAP WITH FROZEN FINGER COMPOSITE ALLOGRAFT AND SECOND TOE FREE GRAFT

    From April 1984 to March 1994, 31 reconstructive thumbs or fingers were followedup, including 16 cases with free neurovascular big toe nail skin flap and frozen preserved phalanxjointtendon composite allografts as well as 15 cases withfree second toe transfer. The method had the advantage of more fingers could bereconstructed and fewer toes would be lost. The decision of the site of reconstruction of finger, the augmentation of narrow web space between the thumb and the index finger, the prevention and treatment of vascular crisis and the degeneration of allogenic joint were discussed. It had been found that preserving the allogenic finger below -30℃ may lower the immunoreaction of the allogenic tissues. It was emphasized that the viable tissues should be preserved during the emergency debridement, so as to facilitate the following reconstruction procedure.

  • CLINICAL APPLICATION OF THE DISTALLY BASED SURAL ISLAND FLAP AND MYOFASCIOCUTANEOUS FLAP

    Objective To investigate the clinical efficiency of thedistally based sural island flap and myofasciocutaneous flap in reconstruction of defect and osteomyelitic cavity of the ankle and foot. Methods From June 1997 to October 2004, 21 patients with soft tissue defects and osteomyelitis in the ankle and foot were treated with the distally based sural island flap and myofasciocutaneous flap. There were 20 males and 1 female aging from 6 to 78 years. The defect was caused by soft tissue defect trauma(18 cases) and electrical injury ( 3 cases). Among 21 patients, 17 were treated with island flaps, 4 by the myofasciocutaneous flap. The size of flaps ranged from 4 cm×5cm to 16 cm×22 cm. The donorsites were closed directly in 4 cases. Results The flaps completely survived in 21 cases and healing by first intention was achieved. After a follow-up of 36 months, no complication occurred. The color and texture of the flaps were good. The appearance and the function were satisfactory. Conclusion Distally basedsural flap is a reliable flap. This flap has rich blood supply without sacrifice of major arteries. Flap elevation is easy. It is very useful in repairing large soft tissue defects of the lower leg, the ankle and the foot, especially inrepairing deep soft tissue defects and osteomyelitic cavities .

  • Mechanisms of adrenergic β-antagonist for wounds and its application prospect in diabetic foot ulcers

    ObjectiveTo review the research progress of adrenergic β-antagonists on wounds and diabetic chronic cutaneous ulcers healing in recent years, and to investigate its application prospect in diabetic foot ulcer (DFU).MethodsThe latest literature about the role of adrenergic β-antagonists in wounds and diabetic chronic cutaneous ulcers healing was extensively reviewed, and the mechanisms of adrenergic β-antagonists for wounds and its potential benefit for DFU were analyzed thoroughly.ResultsThe adrenergic β-antagonists can accelerate the wound healing. The possible mechanisms include accelerating re-epithelialization, promoting angiogenesis, improving neuropathy, and regulating inflammation and growth factors, etc. At present clinical research data showed that the adrenergic β-antagonists may be an adjuvant treatment for diabetic chronic cutaneous ulcers.ConclusionAdrenergic β-antagonists maybe promote the healing of wounds and diabetic chronic cutaneous ulcers. However, more long-term follow-up and high-quality randomized control studies are needed to further verify their efficacy and safety for DFU.

  • 游离足背皮瓣修复口腔颌面部恶性肿瘤术后软组织缺损二例

    目的 总结应用足背皮瓣游离移植修复口腔颌面部恶性肿瘤术后软组织缺损的可行性及临床经验。 方 法 2005 年2 月- 2005 年10 月,收治鼻背、面部皮肤鳞状细胞癌及口咽、颊、磨牙后区鳞状细胞癌各1 例。年龄分别为70 岁、69 岁,病程分别为4 年、6 个月,肿瘤分期分别为T3N1M0 和T3N0M0。术中将肿瘤扩大切除,制备9.5 cm ×7.5 cm 和8 cm × 6 cm 大小的带腓浅神经游离足背皮瓣,其血管、神经分别与颈部相应血管及颈部、面部神经吻合。足背部供区取对侧股外侧中厚皮片移植修复。 结果 术后皮瓣均成活,生长良好。颜面及口腔内外形及功能(包括感觉功能)恢复良好。足部供区愈合良好,无功能障碍。患者获2 年随访,肿瘤无复发。 结论 游离足背皮瓣修复口腔颌面部软组织缺损获得较高成功率,既扩展了足背皮瓣的应用范围,又拓宽了口腔颌面部缺损修复方式的选择范围。

  • REVERSED SURAL NEUROVASCULAR FASCIO-CUTANEOUS FLAP FOR RECONSTRUCTION OF SOFT-TISSUE DEFECTS IN ANKLE AND FOOT

    Objective To evaluate clinical significance of reversed sural neurovascular fasciocutaneous flap for reconstruction of softtissue defects in ankle and foot. Methods From July 1994 to December 2002, 52 cases of soft-tissuedefects in the ankle and foot were reconstructed by use of reversed sural neurovascular fascio-cutaneous flap, including 47 cases of traumatic defects, 3 cases of chronic ulcer and 2 cases of tumors. The flap area ranged from 4 cm×6 cm to 10 cm×21 cm. Results The flaps survived in 48 cases; the distal part necrosed and secondary free-skin graft were further conducted in 4 cases. All soft-tissue defects were repaired and their accompanied bone and tendon exposurehealed. Forty-six cases were followed-up for 5 months to 48 months, the color and texture of the flaps were excellent and 2point discrimination was 11-17 mm(14 mm on average).The functions of ankle joints were good.Conclusion The reversedsural neurovascular fascio-cutaneous flap is convenient in design and dissection. Its use can retained and replace vascular anostomosed flaps to certain degrees.
