  • Clinical Study on Catheter-Directed Thrombolysis via Great Saphenous Vein for Mixed Deep Venous Thrombosis of Lower Extremity

    目的 探讨经大隐静脉-穿通支静脉入路行深静脉置管溶栓术(catheter-directed thrombolysis,CDT)治疗混合型下肢深静脉血栓形成(deep venous thrombosis,DVT)的临床疗效。 方法 对42例急性混合型下肢DVT患者,经踝大隐静脉-穿通支静脉入路,将溶栓导管置入深静脉行CDT治疗。回顾性分析该42例患者的临床资料,评价其溶栓疗效。 结果 42例患者均成功经大隐静脉-穿通支静脉入路行CDT治疗。溶栓时间为5~7 d、(4.22±1.43) d;术后所有患者的肢体肿胀均明显好转;大腿周径差由术前的(7.76±1.72)cm缩减为术后的(2.21±0.91) cm(t=14.18,P<0.01),小腿周径差由术前的(4.45±1.33)cm缩减为术后的(1.43±0.69)cm(t=11.92,P<0.01),静脉通畅度评分由术前的12分降为术后的3分(Z=-3.03,P<0.01)。术后发生穿刺处渗血2例,血尿2例,少量咯血1例。38例获访3~26个月,中位数为15个月。随访期间,1例发生支架远端重度狭窄,1例对侧下肢发生DVT。 结论 经大隐静脉-穿通支静脉入路行CDT治疗混合型下肢DVT,其操作简便,溶栓效率高,是临床值得推广的溶栓途径之一。

  • Site-Specific Conjugation of Bifunctional Chelator BAT to Mouse IgG1 Fab’ Fragment

    【Abstract】ObjectiveTo introduce a new approach for site-specific conjugation of Fab’ fragments of mouse monoclonal antibody (MoAb) to a bifunctional chelator 6-〔p-(bromoacetamido)benzyl〕-1,4,8,11 -tetraazacyclo-tetradecane-N,N′,N″,N-tetraacetic acid (BAT).MethodsFab’ fragments of B43 MoAb were obtained by a simple twostep method: 1, stable F(ab’)2 was produced by pepsin treatment of B43 MoAb. 2, Fab’ was obtained by reduction of the F(ab’)2 with cysteine. Fab’ fragments were directly conjugated with BAT. ResultsFab’ contained about 1.8 free thiol per molecule. The conjugation rate was 74% and each Fab’ fragment carried 1.28 BAT. The F(ab’)2, Fab’ and Fab’-BAT all maintained good biologic activity. ConclusionThis study may provide a simple and efficient method conjugating of MoAb Fab’ fragments to BAT.

  • Effect of designed iliac venous stent implantation on local flow field by particle image velocimetry

    Objective To evaluate changes of local flow field following implantation of a designed iliac venous stent. Methods Anin vitro physical model with iliocava junction was established. A modified iliac venous stent was implanted into one side and a conventional stent into the other side. The bilateral local flow fileds following implantation of stents were measured by using particle image velocimetry. Results The results of particle image velocimetry testing revealed that the main flow orientation was shifted to the right side and the region of slowly local flow was observed after implantation of conventional iliac venous stent. However, no evident changes were noted in the local flow field and no whirling flow was detected following implantation of the designed iliac venous stent. Conclusion Conventional stent is implanted into iliac vein by using a traditional technique and stent is proximal to heart and covers opening of contralateral iliac vein, which exerts certain influence upon local flow field. Local flow field has no obvious change following implantation of a designed iliac venous stent.
