  • 海德堡视网膜断层扫描仪在青光眼诊断中的应用

    视盘形态学改变是青光眼早期征象,可出现在视野缺损之前。青光眼视盘改变主要表现有视网膜神经纤维层受损、盘沿面积的缩小和杯形的改变等。海德堡视网膜断层扫描仪检查可及时、无创伤地获得视盘三维图像,并可定量分析和随访青光眼视盘改变。现介绍其原理及在青光眼诊断和随访中的应用。 (中华眼底病杂志, 2002, 18: 250-252)

  • The influence of cold provocation on retinal light sensitivity of patients with primary open-angle glaucoma

    Objective To evaluate the effects of cold provoca tion on optic dise blood flow and retinal light sensitivity of primary open-angle glaucoma (POAG) patients,and explore the relationship between the changes of optic disc blood flow and retin al light sensitivity. Methods A total 33 POAG patients (33 eyes)and 13 normal controls (13 eyes) were tested by usin g th e Heidelberg retinal flowmetry (HRF) and Topcon automatic perimeter,and the optic disc blood flow and retinal light sensitivity of POAG patients and normal cont rols were measured at normal conditions and after cold provocation. Results The mean optic disc blood flow,volume and the mean retinal light sensitivity of POAG patients decreased significantly (Plt;0.05) after cold provocation.There was a linear and significant relationsh ip between the decrease of mean optic disc blood flow and mean retinal light sen sitivity of POAG patients (r=0.615,P<0.001). The optic disc blood flow of POAG patients with a history of migraine were more likely to r educe in response to cold provocation and reduced much more than those without such history (Plt;0.05). Conclusion Cold provocation can significantly reduce both the optic disc blood flow and retinal light sensitivity in POAG patients.A close correlation was fo und between the amount of mean optic disc blood flow decrease and the volume of mean retinal light sensitivity decline. (Chin J Ocul Fundus Dis, 2001,17:37-40)

  • Effect of Decorin on Proliferation of Tenon’s CapsuleFibroblasts Stimulated by TGF-β1

    目的:研究转化生长因子β1(TGF-β1)促进Tenon囊成纤维细胞的增殖作用及核心蛋白多糖(decorin)对TGF-β1促Tenon囊成纤维细胞增殖的影响。方法:(1)取青光眼患者的Tenon囊组织,采用组织块贴壁培养法进行原代培养细胞培养,细胞经过冷冻与复苏后,观察细胞形态结构与生物学特性。(2)用不同浓度(0.01、0.1、1、5、10ng/mL)TGF-β1作用于Tenon囊成纤维细胞48 h。(3)终浓度为5 ng/mL的TGF-β1 和不同浓度(0.01、0.1、1、2、5μg/mL)的decorin共同作用于Tenon囊成纤维细胞48h。MTT比色法检测Tenon囊成纤维细胞增殖。结果:(1)体外成功培养Tenon囊成纤维细胞,细胞生长活跃,形状稳定,细胞经过冷冻与复苏后,仍能保持其原来的细胞形态结构与稳定的生物学特性。(2)1~10ng/mL TGF-β1能促进Tenon囊成纤维细胞的增殖,并与浓度成正比。(3)2~5μg/mL decorin 能抑制TGF-β1促Tenon囊成纤维细胞增殖作用,其抑制作用与浓度成正比。结论: TGF-β1可刺激Tenon囊成纤维细胞增殖,Decorin能抑制TGF-β1促进细胞增殖的作用,其机理可能通过与TGF-β1结合抑制青光眼滤过手术后瘢痕形成。

  • Protective effect of estrogen on rat′s retina injured by transient ischemia-reperfusion

    ObjectiveTo evaluate the protective effect of estrogen on survival of retinal ganglion cells (RGCs) after transient retinal ischemia-reperfusion (RIR) in rats.MethodsRIR was induced in 60 ovariectomized adult rats (OVX) by increasing intraocular pressure via an intracameral catheter. All of the rats were divided into two groups randomly: in experimental group, the rats underwent a subcutaneous injection with 17β-estrodiol(100 μg/kg) 2 hours before retinal ischemia; and in the control group, saline water was injected correspondingly. The number of RGCs and the thickness of the inner retinal layers were mesured by HE staining method before and 12, 24, 48, and 72 hours after reperfusion. TdT-mediated biotin-dUTP nick end labelling (TUNEL) staining technique was used to examine the apoptosis of RGCs.ResultsTwenty-four and 48 hours after reperfusion, the number of apoptotic cells in experimental group was obvious lower than that in the control group(Plt;0.05), and the number of RGCs in experimental group was higher than that in the control group(Plt;0.05).ConclusionEstrogen can protect retinal neurons from transient RIR in ovariectomized rats.(Chin J Ocul Fundus Dis, 2005,21:177-179)

  • The effect of electrode positions on multifocal visual evoked potential

    Objective To compare the effect of electrode positions on multifocal visual evoked potential(mf-VEP). Methods Ten healthy individuals were tested with RETIscan multifocal VEP system 3.20, each individual was tested with four electrode positions: the active and reference electrode were placed 2.0 cm above and below the inion in Ch1;3.0 cm above and 4.5 cm below the inion in Ch2;2.0 cm on both right and left side of the inion in Ch3 and 4.0 cm on both right and left side of the inion in Ch4. The summed amplitudes of hemifield mf-VEP were analyzed and compared according to different electrode positions. Results The difference among summed amplitudes of the upper or lower hemifield mf-VEP recorded with Ch1、Ch2、Ch3 and Ch4 was statistically significant;and the difference between Ch2 and the other channels was statistically significant too. The difference among summed amplitudes of the horizontal sites whose amplitudes were small when recorded with Ch2 and summed amplitudes of the corresponding sites recorded with Ch3 and Ch4 were statistically significant;and the difference between Ch4 and the other channels was statistically significant too. Conclusions compared to Ch1、Ch3 and Ch4, Ch2 can make a better recording of mf-VEP. As for some sites, especially those along horizontal line, horizontal electrodes could improve the amplitudes of mf-VEP recorded with vertical electrodes,and Ch4 could do better to improve the mf-VEP recorded with Ch2 than Ch3.   • The influence of cold provocation and calcium channel blocker to the optic disc blood flow of patients with primary open-angle glaucoma

    Purpose To examine the change of optic disc blood flow in primary open angle glaucoma(POAG) patients after cold provocation test and nifedipine administration. Methods Using Heidelberg retinal flowmetry (HRF),the blood flow of optic disc of glaucoma patients and normal control subjects were measured under basal condition, after cold provocation test,and after nifedipine administration. Results The mean optic disc blood volume and flow of POAG patients reduced from 27.1 and 545.4 to 22.3 and 452.4 after cold provocation test (Plt;0.05),and increased to 29.0 and 579.5 after nifedipine adminstration(Plt;0.05).The changes of mean optic disc blood flow of patients with a history of cold extremities show statistic significance compared with whom without such history (Plt;0.05)). Conclusion The changes of blood flow of optic disc in POAG patients may be influenced by cold stimuli and administration of nifedipine,and the history of cold extremities might be connected with the change of optic disc blood flow in POAG patients.   • Analysis on The Safety of Three Ducts and One Pouch in Laparoscopic Cholecystectomy

    目的 探讨三管一壶腹在腹腔镜胆囊切除术中的应用价值。方法 回顾性分析2011年9月至2012年6月期间九寨沟县人民医院外一科行腹腔镜胆囊切除术(术中均遵循三管一壶腹的解剖关系)的362例患者的临床资料。结果 362例患者中,慢性结石性胆囊炎317例,胆囊息肉45例。术中有3例(0.83%)患者中转开腹,2例(0.55%)患者发现副肝管,均未发生胆管损伤,无手术死亡。术后所有患者均随访半年,随访期间均无其他并发症发生,术后恢复均良好。结论 胆囊切除术中遵循三管一壶腹的解剖关系,可有效地预防胆管损伤等并发症的发生,其安全、可靠,值得推广应用。

  • Methods to Combine Standard Deviations of Different Subgroups in Meta-analysis

    The averages and standard deviations of several subgroups in a study are sometimes need to be combined in meta-analysis, the calculating method to do this was given in this paper. Firstly, a commonly used incorrect calculating method to do the combination was pointed out; it origins from metrology and should be applied in the combination of standard deviations of several groups of measurements of the same object, and it's not fit for the combination of several subgroups' data in meta-analysis. Meanwhile the calculating error of this method was tested with real and constructed data and its impact on the conclusion of meta-analysis was analyzed. The incorrect method may result in a big calculating error with the real research data and even lead to an incorrect conclusion with the constructed data. The correct formula was deduced and given, the calculating result of the correct formula equals the true value when neglecting the round error. The error analysis of the incorrect method was then done with the correct formula and an accurate combination result can only be got when meeting the condition given by this paper. The correct method in this paper should be used to combine standard deviations in the future researches.
