  • EXPERIMENTAL STUDY OF CHITOSAN FOR PREVENTION OF BONE INFECTION

    Abstract In order to study the influence of chitosan on bone infection, 27 New-Zealand rabbits were randomly divided into 3 groups. According to the Norden technique, the model of osteomyelitis of the tuberosity of tibia was produced experimentally by injection of staphylococcus aureus.Immediately following injection of the staphylococci, the chitosan gel, acetic acid solution and sterile distilled water were injected into the bones in the 3 groups respectively. The latter two groups were served as control. The severity of the infection was evaluated by clinical symptoms and signs, radiographicdata as well as the bone culture and bacterial counts. Compared the results from chitosan with the other two controls, it was found that the local injection ofchitosan could not reduce the incidence of bone infection, however, it could provide actual improvement when other data were concerned. The effects of chitosangel might be attributed to its bacteriostatic and immunological activity as well as its slow degradation in the body.

  • OSTEOMYOCUTANEOUS LATISSIMUS DORSI SCAPULAR COMBINED FLAP WITH VASCULARIZED RIB TO REPAIR THE LARGE DEFECT OF TIBIA

    Objective To design a combined flap of subscapular axis including vascularized lateral scapular,rib and latissimus dorsi to repair the large defect of tibia. Methods The patient was a 39-year-old man who got a posttraumatic 12 cm defect of tibiaafter primary debridement and external fixation because of open fracture 5 months ago. There was a 12 cm×6 cm scar involved the proximal medial segment of tibia.After resection of scar and fibular tissue over the bone defect floor, alatissimus dorsi myocutaneous flap 14 cm×5 cm pedicled with subscapular artery-thoracodorsal artery,a flap 12.5 cm on the outside of the scapular pedicled with thoracodorsal artery, and 6th rib flap 13 cm by serratus were prepared.The tibialis posterior and saphenous vein were used for astomosis. A proximalanatomic plate was applied to the fixation of tibia. Results Thecompound flap survived the operation. The follow-up period was 2 years. Bone union occurred 6 months after operation. Conclusion This combined flap is successful and can provide alternative to the resolution of large defect of tibia.

  • EXPERIMENTAL STUDY ON THE HEMOSTATIC PROPERTIES OF COLLAGEN SPONGE

    OBJECTIVE: To validate the hemostatic properties of collagen sponge made in China. METHODS: The experimental model of superficial cut of liver was established in 20 Sprague-Dawley adult rats, which were divided into two groups randomly. Collagen sponge or gelatin sponge was used to cover the cut respectively. Hemostatic result was observed. Afterwards, standard liver trauma model by resection left front liver lobe was made, wound was treated with collagen sponge or gelatin sponge respectively. Hemostatic result was observed. Concurrent hemostatic time and bleeding amount were noted. At 7, 14 and 20 days after operation, intra-abdominal adhension, infection and healing state of liver were observed by exploratory laparotomy. The histological changes of regenerate liver tissue were observed by microscopy. RESULTS: Collagen sponge adhered to wound well. Concurrent hemostatic time and bleeding amount in collagen sponge group were superior to those of gelatin sponge (P lt; 0.05). The histological examination showed that collagen sponge was absorbed and degraded rapidly, regenerative hepatocytes could be induced. CONCLUSION: Collagen sponge has fine hemostatic properties and can induce regeneration of hepatocytes effectively. It is worth popularizing for its convenience in clinical application and its properties of rapid degradation and absorption.

  • CL INICAL STUDY ON CHITOSAN FOR PREVENTION OF EL BOW ADHESION

    OBJECTIVE To study the clinical effect of chitosan on prevention of elbow adhesion after elbow arthrolysis. METHODS Twenty six patients with elbow ankylosis were performed elbow arthrolysis, which divided into two groups, in chitosan group, 12 patients were injected 2% chitosan into the elbow joint cavity, and no chitosan used in the other 14 patients as control group. The average range of extension and flexion of elbow joint was detected to evaluate the clinical results. RESULTS All patients were followed up 8 to 51 months, averaged 24 months. In the chitosan group, the average range of extension and flexion of elbow joint was restored to 92.9 degrees +/- 20.9 degrees, with an average increase of 55.0 degrees +/- 15.9 degrees compared with preoperation. In the control group, the average range of extension and flexion of elbow joint was restored to 75.4 degrees +/- 17.5 degrees, with an average increase of 38.2 degrees +/- 11.9 degrees. The outcome showed significant difference between the chitosan group and the control group (P lt; 0.01). CONCLUSION Chitosan can prevent or reduce elbow adhesion after elbow arthrolysis.

  • A PRELIMINARY CLINICAL OBSERVATION OF GIANT CELL TUMOR OF BONE TREATED BY ADRIAMYCINLOADED CHITOSAN DRUG BELIVERY SYSTEM

    In order to observe the curative effect and general reaction of locally used adriamycin (ADM)-loaded chitosan drug delivery system on giant cell tumor of bone after curettage. The cavities of 4 cases of giant cell tumor after curettage were filled with ADM-loaded chitosan drug delivery system with 4 times the dosage usually used for intravenous application. After operation, the concentration of ADM in plasma on the 1st, 2nd and 5th day, and the functions of liver and kidney on the 1st week, 1st month and 6th month were all investigated. The results were that the concentration of ADM in plasma was (143.05 +/- 27.55) ng/ml, (52.17 +/- 11.28) ng/ml and (4.25 +/- 3.07) ng/ml respectively, and the functions of liver and kidney were all normal in 6 months. After a follow-up of 7-19 months, no local or general reactions were observed and X-ray showed no recurrence. Therefore, it was concluded that the locally used ADM-loaded chitosan delivery system was safe and effective in treatment of giant cell tumor of bone after curettage.

  • A PRELIMINARY REPORT ON CLINICAL APPLICATION OF BIOPOLY ESTER LIGAMENT

    Sixty-one cases of fracture or dislocation were treated with France-made biopolyester ligament. They consisted of 18 cases of fracture of patella, 13 cases of fracture of olecranon, 8 cases of fracture of distal clavicle, 10 cases of dislocation of acromino-clavicular joint, 6 cases of separation of lower tibio-fibular joint, 3 cases of rapture of cruciate ligament and 3 cases of fracture of upper third of ulna with dislolcation of radial head. The follow-up period lasted from 3 to 12 months with an average of 7.3 months. The result showed that of the 56 follow-up cases, 49 were excellent and 7 were satisfactory. There was no redisplacement occured in this group. The Biopolyester ligament was believed to be a good and safe material for fractures or dislocations and was of good strength of extension and easy to use.

  • A PRELIMINARY REPORT OF OPEN REDUCTION AND NUMELOCK II POLYAXIAL SYSTEM FIXATION FOR TYPE C FRACTURE OF DISTAL RADIUS

    Objective To study the cl inical outcomes of Numelock II polyaxial system in treatment of type C fractures of the distal radius. Methods From March 2006 to June 2007, 12 cases of type C distal radial fractures (6 males and 6 females)were treated with open reduction and internal fixation. The mean age of the patients was 48.2 years (34-64 years).The locations were left wrist in 5 cases and right wrist in 7 cases, including 1 case of old fracture and 11 cases of fresh fractures. All fractures were closed. The mean course of disease was 9 days and 7 hours (4 hours to 60 days). According to AO/ASIF classification, there were 4 cases of C1 type, 5 cases of C2 type and 3 cases of C3 type. Preoperatively, the palmar tilt angle was —30° to 30° (— 5° on average), and the radial inclination angle was 5° to 13° (7.7° on average), and the radial shortening was 5 mm to 15 mm (9 mm on average). One case combined with dislocation of elbow joint and 2 cases combined with multi ple injuries. Results All incision healed by first intention. Postoperative follow-up ranged from 14 months to 29 months (15.6 months on average). The X-ray films showed that the union of fractures was achieved 6-8 weeks (6.6 weeks on average). No screws breakage occurred. The articular facets weresmooth in 11 of 12 patients. After operation, the palmar tilt angle was 0° to 15° (6.7° on average), and the radial inclination angle was 5° to 15° (10.2° on average), showing significant differences when compared with those before operation (P lt; 0.05). All the radial shortening was corrected. The ROM of the wrist was 55%-100% of the normal side. The grip strength was 55%-90% of the normal side. The results were excellent in 9 cases, good in 2 cases, and poor in 1 cases by X-ray film and wrist function assessment, the excellent and good rate was 91.7%. Conclusion Numelock II polyaxial system fixation is an ideal method to treat type C fractures of the distal radius. Numelock II polyaxial mechanism may provide the free adjustabil ity of screw trajectories and a higher degree of overall stabil ity compared to monoaxial locking plates.

  • EXPERIMENTAL STUDY ON RECONSTRUCTION OF PHYSIOLOGICAL REFLEX ARC AFTER MEDULLARYCONE INJURY IN RATS

    To establ ish the animal model of the artificial physiological reflex arc with the reconstruction of the sensory and the motorial functions of atonic bladder simultaneously in the rats, and to provide the foundation to furtherinvestigate the repairing effectiveness of this technique. Methods There were 20 adult male SD rats (weighing 280-300 g)which were randomly divided into 2 groups (n=10): group A and group B. Group A was anastomosis of the ventral roots(VR) and the dorsal roots (DR) between L6 and S2 simultaneously to establ ish the model of the artificial physiological reflex arc. Group B was anastomosis of the main trunk between L6 and S2 to establ ish the model. The contents of the observation included: ① To measure the external diameter of the VR, DR and the main trunk of L6 and S2 with the sl iding cal iper; and to measure not only the distance between L6 and S2 but also the separable length of L6 with the ruler. ② Fast Blue dyeing of the VR, DR and the main trunk of L6 and S2 was performed to count their nerve fibers assisted by the Leica FW4000 system 2 weeks after opertation. ③ The observation of the urination of the rat and BBB scoring to evaluate the motorial function of the lower l imbs was performed postoperatively. Results ① L6 located in the lateral side of the S1-4 in the vertebral body of L6. The external diameters of the VR, DR and the main trunk of L6 were (0.68 ± 0.13), (0.88 ± 0.10) and (1.54 ± 0.33) mm, respectively, while those of S2 were (0.62 ± 0.08), (0.79 ± 0.14) and (1.39 ± 0.42) mm, respectively. The distance between L6 and S2 was (14.21 ± 1.95) mm, and the separable length of L6 was (10.76 ± 2.11) mm. Furthermore, the microdissection indicated the VR and the DR between L6 and S2 could be anastomosed respectively with no-tension at the level of the vertebral body of L6; and the main trunk of L6 and S2 could be anastomosed with no-tension at the level of the confluens of L5, 6. ② With Fast Blue dyeing, there were 892 ± 32, 354 ± 26 and 532 ± 17 nerve fibers of the VR, DR and the main trunk of L6, respectively. And there were 788 ± 29, 325 ± 19, and 478 ± 22 nerve fibers of the VR, DR and the main trunk of S2, respectively. There were no volar ulcer,trichomadesis and self-eating of the affected l imbs in the both groups postoperatively. The urinations of the rats after operationwere not different from those before operation. The mean BBB scores of pre- and postoperation in group A were 20.20 ± 0.35 and 19.80 ± 0.23, respectively; the mean BBB scores of pre- and postoperation in group B were 20.20 ± 0.35 and 19.20 ± 0.31, respectively. There was no significant difference of the above indexes between group A and group B (P gt; 0.05). Conclusion Anastomosis of the VR and the DR between L6 and S2 simultaneously in rats is an ideal animal model to establ ish the artificial physiological reflex arc owing to its simple and reproducible procedures.
