ObjectiveTo review the disc degeneration models and the current treatment status of calcitonin. MethodsRecent literature concerning the disc degeneration models and the calcitonin treatment of disc degeneration was extensively reviewed and summarized. ResultsDifferent models of disc degeneration have advantages and disadvantages; calcitonin can relieve disc degeneration at different degrees in vitro and in vivo studies. ConclusionDisc degeneration model for the study of disc degeneration mechanism offers a variety of ideas,and the results of calcitonin treatment in the disc degeneration model can provide the basis for future experiments.
ObjectiveTo understand the role of calcitonin testing for the diagnosis and treatment of medullary thyroid cancer (MTC) as well as recent research progress, so as to provide assistance in the early clinical diagnosis and treatment of MTC and improve patients’ prognosis. MethodThe literatures about the role of calcitonin testing in MTC in recent years were reviewed. ResultsIn recent years, both domestic and international scholars had extensively investigated the role of calcitonin in the early detection of MTC and accurate postoperative prognosis assessment. With respect to early diagnosis, advancements had been made in the three main measurement methods: basal calcitonin, stimulated calcitonin, and calcitonin measurement in the thyroid nodule fine-needle aspiration washout fluid. These developments in calcitonin levels had contributed to improved guidance in surgical treatment and prognosis evaluation. While calcitonin monitoring could inform treatment decisions and improve patients’prognosis, numerous challenges remain to be addressed. ConclusionThe study of calcitonin detection can deepen the understanding of MTC, further research on calcitonin related detection in the future will be of great significance for the diagnosis, treatment and prognosis evaluation of MTC.
Objective To investigate the expression and clinical significance of soluble triggering receptor expression on myeloid cells-1(sTREM-1) in sepsis patients.Methods Serum concentrations of sTREM-1,procalcitonin(PCT),tumor necrosis factor alpha(TNF-α) and interleukin-10(IL-10) were measured by enzyme-linked immunosorbent assay,while high sensitivity C-reactive protein (hsCRP) level was detected by immunoturbidimetry in 68 patients with sepsis,40 patients with no-infective SIRS,and 20 normal individuals. The diagnostic and prognostic value of sTREM-1 and its comparison with PCT and hsCRP were analyzed. The sequential organ failure assessment (SOFA) score system was used to evaluate the severity of sepsis. The relationship between sTREM-1, PCT , hsCRP , SOFA score,TNF-α ,and IL-10 of the sepsis patients was analyzed,respectively. Results The differences in the serum concentrations of sTREM-1,PCT,hsCRP,IL-10 and IL-10/TNF-α ratio had statistical significance among three groups(Plt;0.05). The differences in the serum concentration of TNF-α had no statistical significance among three groups (Pgt;0.05). However,the serum levels of sTREM-1,PCT and hsCRP in the sepsis group were significantly higher than those in the SIRS group (Plt;0.05). The receiver operating characteristic curve (ROC) analysis showed the area under the curve (AUC) for sTREM-1,PCT and hsCRP were 0.772 (95%CI 0.674-0.871),0.718 (95%CI 0.601-0.835) and 0.664 (95%CI 0.532-0.797),respectively. The serum levels of sTREM-1 and PCT in the non-survivors were significantly higher than the survivors in the sepsis group (Plt;0.01),but the differences in the serum concentration of hsCRP had no statistical significance between the non-survivors and the survivors in the sepsis group (Pgt;0.05). There were significantly positive correlations between sTREM-1 and SOFA score,IL-10 or IL-10/TNF-α ratio(r value of 0.453,0.301,0.417,Plt;0.05),but no correlation between sTREM-1 and TNF-α(Pgt;0.05). There was significantly positive correlation between PCT and SOFA score (r=0.436,Plt;0.05),while no relationship between hsCRP and SOFA score(Pgt;0.05). Conclusions The serum level of sTREM-1 not only be valuable in the diagnosis of sepsis,but also may be used as a prognostic marker in sepsis,as it can reflect the severity of sepsis in certain degree. Furthermore,sTREM-1 or PCT may be superior to hsCRP in diagnosis,prognostic judgment and severity assessment of sepsis.
目的 探讨盐酸氨基葡萄糖联合降钙素对绝经后膝骨关节炎基质金属蛋白酶3(MMP-3)和骨桥蛋白(OPN)表达的影响。 方法 2012年1月-6月将120例绝经后膝骨关节炎妇女随机分为盐酸氨基葡萄糖组(A组)、盐酸氨基葡萄糖+依降钙素组(B组)、依降钙素组(C组),每组40例,采用酶联免疫吸附试验测定各组血清MMP-3、OPN、雌二醇、Ⅰ型胶原C端肽(CTX)和Ⅰ型胶原N端前肽(PINP)水平。 结果 A组和B组在治疗后2周和6周其膝关节评分和视觉模拟评分明显优于C组(P<0.05),A组在治疗后2周MMP-3的表达改善明显(P<0.05),优于其他两组。治疗后6周,B组OPN表达水平改善明显(P<0.05),优于其他两组。C组和B组CTX和PINP水平明显改善(P<0.05),优于A组。 结论 盐酸氨基葡萄糖联合降钙素能有效改善绝经后膝骨关节炎的症状,可能通过调节MMP-3和OPN的复合体表达,实现改善关节软骨功能的目的。
ObjectiveTo investigate the prognostic value of serum procalcitonin (PCT) level and its change for patients with severe trauma and sepsis. MethodsEighty-two patients with severe trauma and sepsis treated between June 2013 and June 2014 were chosen to be our study subjects. According to the clinical outcome at day 28, they were divided into survival group (n=54) and death group (n=28). Serum procalcitonin levels at hour 0, 24, 48, 72, 96, and 120 after admitted to the emergency intensive care unit were detected in all the patients. Then we calculated the rate of procalcitonin change in each time points, injury severity score (ISS) score, acute physiology and chronic health evaluation (APACHE)Ⅱ score and white blood cells (WBC) count. ResultsProcalciton level at hour 24, 48, 72, 96 and 120 of the survival group was significantly lower than the death group (P<0.05); the rate of procalcitonin change at hour 24, 48, 72, 96 and 120 was significantly lower than the death group (P<0.05), and the APACHE Ⅱ score was significantly lower than the death group (P<0.01). There was no significant difference in the ISS score and WBC count between the two groups (P>0.05). ConclusionEarly procalcitonin level and its dynamic change are good indicators in judging the prognosis of patients with severe trauma and sepsis, especially the early dynamic change in the rate of procalcitonin.
Objective To evaluate the value of serumprocalcitonin( PCT) guided antibiotic strategy in the treatment of acute exacerbation of chronic obstructive pulmonary disease( AECOPD) .Methods From August 2011 to June 2012, a total of 96 patients hospitalized for AECOPD were randomly assigned into a PCT-guided group( n = 48) and an empirical therapy group( n = 48) . The PCT levels of PCT-guided group were measured by immunochemiluminometric assays before and 5,7, 10 days after treatment. The PCT-guided group was treated with antibiotics according to serum PCT levels, ie. antibiotic treatment was applied when PCT level ≥ 0. 25 μg/L and was discouraged when PCT level lt; 0. 25 μg/L. The empirical therapy group received antibiotics according to physician’s decision. The antibiotics usage rate, length of antibiotic exposure, length of hospitalization, clinical efficacy, hospital mortality, rate of invasive mechanical ventilation and costs of hospitalization were recorded. Results The antibiotics usage rate, length of antibiotic exposure, length of hospitalization, and costs of hospitalization in the PCT-guided group were all lower than those of the empirical therapy group( P lt;0.05) while clinical efficacy, hospital mortality and rate of invasive mechanical ventilation were similar in two groups(Pgt;0.05) . Conclusion PCT guided antibiotic strategy can be used in the treatment of AECOPD, which may reduce the dose of antibiotic drugs to avoid bacterial resistance and lower costs of hospitalization.
ObjectiveTo systematically evaluate the efficacy and safety of procalcitonin guided algorithms of antibiotic therapy in acute exacerbation chronic obstructive pulmonary disease (AECOPD). MethodsWe searched PubMed, EMbase, The Cochrane Library (Issue 6, 2016), CBM, CNKI, VIP, and WanFang Data from the date of their establishment to July 2016, to collect randomized controlled trials (RCTs) about procalcitonin guided antibiotics therapy in patients with AECOPD. References of the included literature were also searched manually for additional studies. The literature screening, data extraction and bias risk assessment of the included studies were completed by two reviewers independently. Statistical analysis was conducted using RevMan 5.2 software. ResultsA total of ten RCTs involving 1 071 patients were included. The results of meta-analysis indicated that compared with the standard treatment group, the antibiotic prescription rate (RR=0.70, 95% CI 0.55 to 0.89, P=0.004), the rate of duration of antibiotic >10 days (RR=0.38, 95% CI 0.26 to 0.56, P<0.000 01) and the superinfection rate (RR=0.23, 95% CI 0.09 to 0.58, P=0.002) were significantly lower in the procalcitonin-guided treatment group. There were no statistical differences in clinical effective rate (RR=0.98, 95% CI 0.91 to 1.06, P=0.61), hospital mortality (RR=0.84, 95% CI 0.52 to 1.73, P=0.43), and the rate of need for intensive care (RR=0.77, 95% CI 0.40 to 1.47, P=0.43). ConclusionProcalcitonin guided antibiotics therapy may reduce antibiotic exposure and superinfection rate in patients with AECOPD. In addition, due to the low methodological quality and limited quantity of the included studies, larger sample-size, and high quality RCTs are needed to verify the above conclusion.
Objective To evaluate the predicting effect of quick Sequential Organ Failure Assessment (qSOFA) on septic shock, and investigate the probability of improving the predicting effect. Methods Patients with sepsis diagnosed in Emergency Department from July 2015 to June 2016 were enrolled. They were divided into shock group and non-shock group based on whether or not they had septic shock during 72 hours after admission. The multivariate logistic regression analysis was used to find out the independent risk factors affecting the incidence of septic shock. Receiver operating characteristic (ROC) curve was used to analyze those risk factors. Modified Early Warning Score (MEWS), Mortality in Emergency Department Sepsis Score (MEDS), Sequential Organ Failure Assessment (SOFA), Acute Physiology and Chronic HealthEvaluation (APACHE)Ⅱ and qSOFA were also compared with ROC curve analysis. The possibility of improvement of qSOFA predicting effect was discussed. Results A total of 821 patients were enrolled, with 108 in septic shock group and 713 in non-septic shock. The result of multivariate logistic regression analysis indicated that respiratory rate, systolic blood pressure, pH value, oxygenation index, lactate, albumin, Glasgow Coma Score and procalcitonin were the independent risk factors (P<0.05). The result of ROC analysis showed that the area under curve (AUC) of pH value, lactate and procalcitonin was 0.695, 0.678 and 0.694, respectively. Lactate had the highest value of specificity (0.868), positive predictive value (0.356) and positive likelihood ratio (3.644), while the sensitivity (0.889) and negative predictive value (0.961) of procalcitonin were the highest. MEWS, MEDS, SOFA, APACHEⅡ and qSOFA were compared with ROC. SOFA had the best predicting effect with the statistical results of AUC (0.833), sensitivity (0.835), specificity (0.435), positive predictive value (0.971), negative predictive value (0.971), and positive likelihood ratio (5.048); and MEWS had the highest negative likelihood ratio (0.581). qSOFA did not show a best predicting value. Conclusion qSOFA is not the best choice to predict the possibility of septic shock, but its predicting value might be improved when combined with pH value, lactate and procalcitonin.
Objective To explore the changes of calcitonin gene-related peptide (CGRP) and substance P (SP) levels after end-toend and end-to-side neurorrhaphy. Methods Twenty female Wistar rats were divided into 4 experimental groups and control group. In the experimental groups, common peroneal nerves were transected on both sides. End-to-side coaptation was performed on the left, while end-to-end coaptation on the right. After 1, 2, 4 and 27 weeks, the rats were sacrificed, and immunoreactivities of CGRP and SP in suture sites, lumbar spine and dorsal root ganglia(DRGs) were evaluated respectively. Results The expression ofCGRP and SP decreased in dorsal horn and DRGs within 1 week postoperatively. After 4 -27 weeks, CGRP and SP in dorsal horn could return to almost normal level, but they had little recovery in DRGs. Although the trend of change between end-to-end and end-to-side was coincident, in most experimental groups, thereexisted differences in the dorsal horn between end-to-end and end-to-side. The sciatic nerve stained by acetylcholinesterase, SP, CGRP and PGP 9.5 showed that the fibers could pass through the suture site of either end-to-end or end-to-side. Conclusion Nerve regeneration can be achieved by end-to-side neurorrhaphy, andthe mechanism of sensory nerve recovery of these two methods is similar. But the recovery in end-to-side coaptation is insufficient to some degree.
