  • POLYMORPHONUCLEAR AGGREGATION OF LEUKOCYTE IN ACUTE CHOLANGITIS AND ITS ROLE IN HEPATIC DAMAGE

    The mumber of Polymorphonuclear leukocyte (PMN) in hepatic tissue increased in the rats with cholangitis, PMN infiltration was mainly in the hepatic sinus in the early stage; and PMN infiltration presented around the hepatocytes 12 hours after infection. Degeneration and necrosis of the hepatic cells was also observed in the rats with acute cholangitis. Only 40 percent of the rats survived 24 hours after infection. Depletion of circulating PMN decreased the damage and necrosis of hepatocytes and improving the survival rate of the infected rats. The results suggest that PMN infiltration plays an important role in hepatic damage in acute cholangitis.

  • THE EFFECT OF INTERCELLULAR ADHESIVENESS MOLECULE-1, E-SELECTIN ON HEPATIC MICROCIRCULATION IN ACUTE CHOLANGITIS

    To evaluate the effect of intercellular adhesiveness molecule-1 (ICAM-1), E-selectin on hepatic microcirculation in acute cholangitis. The Changes of hepatic tissue, content of blood flow and Evan′s blue (EB) in hepatic tissue in acute cholangitis were determinated. Results: The number of PMN in hepatic tissue and sinusin increased, degenaration and necrosis of the hepatic cells and hepatic sinusoidal endothelial cells and content of blood flow in liver were reduced, and content of EB in hepatic tissue increased remarkbly in the rats with acute cholangitis. Pretreatment of anti ICAM-1 and E-selectin mAb reduced the damage of hepatic microcirculation. Conclusion: ICAM-1 and E-selectin may play an important role in damage to hepatic microcirculation in acute cholangitis.

  • The Change of Electromyophysiology of the Rat with Cathartic Colon

    ObjectiveTo explore etiopathogenesis of slow transit constipation(STC). MethodsThe model of rat with “cathartic colon” was established, and the changes of colonic electromyography of the rat was examined. ResultsThe frequency and amplitude of slow wave in vivo of rats with cathartic colon was decreased markedly. Postmeal spike potential and duration was decreased, showing that gastrocolonic reflex of rats colon was decreased.ConclusionLong term abuse of stimulant laxuatives could damage enteric nervous system and accelerate the pathological changes of STC.

  • RESECTION OF COLORECTAL CARCINOMA WITH METASTASIS TO THE LIVER IN 32 PATIENTS

    We evaluated the surgical results in 32 patients with liver metastasis from colorectal carcinoma. Twenty four patients had 1-3 metastatic hepatic nodules and 20 patients had synchronous hepatic metastasis. Liver resection was carried out simultaneously with radical resection of the primary tumour in 15 patients, 5 patients experienced resection 2 to 4 weeks later. Liver and primary tumour were resected as a whole in 5 patients with infiltrating metastasis from colonic carcinoma.Other operative types included atypical resections, left lateral lobectom and right posterior lobectomy, and right hemihepatactomy, right trilobectomy.Hepatic metastasis were all documented by pathology. The 3year and 5year survival rate were 37.5% and 25.0%, with no operative death. The authors believed that the number of metastasis is the most important factor influencing the surgical result, and liver resection is an effective form of treatment for patients with resectable liver metastasis from colorectal carcinoma, but the type of surgery shall be choosed reasonably.

  • INTESTINAL OBSTRUCTION DUE TO CARCINOMA OF LEFT HEMICOLON(ANALYSIS OF 128 CASES)

    One hundred and twenty eight patients with intestinal obstruction due to cancer of left lemicolon are presented. In this series 71 patients suffered from partial intestinal obstruction and 57 patients from complete obstruction, the latter were in later Dukes stages, with lesser resectability of the tumor and higher mortality. The transition from partial obstruction to complete obstruction takes a slow course. Purgatives and coarse fibered food should not be given to the patients with partial obstruction, or else will induce acute obstruction. Several types of operation for partial and complete obstruction are discussed. Methods and results of intraoperative colonic irrigation are presented. The authors believe that intraoperative colonic irrigation is a good emergency management for cancer obstruction of the left colon. Complication of this disease are also discussed.

  • DIAGNOSIS AND TREATMENT OF INTESTINAL TUBERCULOSIS (REPORT OF 30 CASES)

    In this series of 30 patients of intestinal tuberculosis, diagnosis was made by the typical appearance of caseous change of intestines and mesectery, pathological evidence of endoscopic biopsy or biopsy at operation. In 16 patients preoperative chest X-ray showed pulmonary TB lesion. X-ray gastrointestinal exmaination was performed in 24 patients and 20 patients underwent clonic endoscopy,both of them showed abnormal but nonspecific manifestation.Three patients underwent partial resection of small intestine, 20 patients right hemicolectomy, 3 patients subtotal colectomy, 1 patient abdominal resection of rectum, 2 patients partial resection of small bowel and right hemicolectomy and 1 patient partial resection of ileum and right hemicolon and part of rectum. It is believed that intestinal tuberculosis could manifest itself in segmental changes and bridge of mucosa, and should differented from Crohn’s disease. Resection and anastomosis after exploration and postoperative antituberculosis therapy is necssary.

  • TREATMENT OF OPEN FRACTURE BY VACUUM SEALING TECHNIQUE AND INTERNAL FIXATION

    OBJECTIVE: To investigate the effect of vacuum sealing(VS) technique and emergency internal fixation on the management of limbs open fracture and soft tissue dirty defects. METHODS: Fourteen patients (18 limbs) with open fracture and soft tissue dirty defects were treated by the VS technique and internal fixation after debridement and 14 patients managed by traditional method as control group. Wound surface were covered with polyvinyl alcohol foams with embedded drainage tubes connected with vacuum bottle (negative pressure of 50 to 60 kPa) after wound surface were debrided and fracture were fixed. Wound closure was performed with secondary suturing, or free flap, or loco-regional flap and mesh-grafts after 5 to 7 days. RESULTS: All wound surface healed completely. No complications (systemic and local) were found. After 4-6 months follow-up on average, the fracture healed well. There was significant difference in time of treatment, total cost of treatment and complication rate between 2 groups (P lt; 0.01). CONCLUSION: The VS procedure can drain the wound surface completely, decrease infection rate and stimulate the proliferation of granulation tissue. A combination of VS with emergency internal fixation is a simple and effective method in treatment of limbs open fracture and soft tissue dirty defects.
