  • PLASTIC REPAIR OF RESIDUAL DISORDERS FOLLOWING THE ABLATION OF DUPLICATED THUMB

    The simple ablation of the duplicated thumb might have some residual deformities after operation that often leads to functional disorders of the hand. Since 1988, the postoperative disorders in 9 patients were treated. The plastic surgery was performed for the adduction of the thumb, lateral deviation and insufficient after plastic repair of the hand. The configuration and the function of the thumb were satisfactory.

  • Clinical Analysis of Therapeutic Effects of Ultrasound-Guided Percutaneous Microwave Ablation for Hepatic Metastases

    Objective To evaluate the therapeutic effects of percutaneous microwave ablation (MWA) for metastatic liver cancer. Methods Ultrasound-guided percutaneous MWA technique was used to treat 27 cases of hepatic metastases with 69 nodules 〔0.9-13.2 (3.0±2.0) cm in diameter〕. Local therapeutic effect, local and distant recurrence rate and survival rate were evaluated respectively. Results The complete ablation (CA) rate which was used to evaluate the local therapeutic effects was 92.8% (64/69), with 100% (34/34), 92.3%(24/26) and 66.7% (6/9) in a diameter of nodules lt;3.0 cm, 3.0-5.0 cm and ≥5.0 cm respectively. The CA rate was lower in the group of a diameter of nodules ≥5.0 cm as compared with other two groups (Plt;0.05). The local recurrence rate was 9.4% (6/64), with 2.9% (1/34), 16.7% (4/24) and 16.7% (1/6) in a diameter of nodules lt;3.0 cm, 3.0-5.0 cm and ≥5.0 cm respectively. The intrahepatic distant recurrence rate was 44.4% (12/27). The follow-up time after MWA was 3-34 (17.0±8.7) months. During the follow-up period, 6 months, 1 and 2-year cumulative survival rate was 88.9%, 63.0% and 34.4% respectively, with a mean survival time of 17.8 months, and with a median survival time of 19.0 months.Conclusion Percutaneous MWA treatment offers satisfactory local tumoricidal efficacy to metastatic liver cancer and the patients with recurrence and new metastases can be therapy repeatedly to improve long-term survival.

  • Comprehensive Treatment of Pancreatic Neuroendocrine Neoplasms

    ObjectiveTo summary the treatment of pancreatic neuroendocrine neoplasms (pNENs). MethodsArticles relevant to pNENs at home and abroad were collected and reviewed. ResultsBecause of rare incidence and non-specific clinical syndromes of pNENs, clinician had no enough cognition about it. For pNENs, surgery was still the preferred option, combining other treatments included chemotherapy, somatostatin analogue, α-interferon, molecular targeted therapy, and peptide receptor radionuclide therapy (PRRT). ConclusionSurgery is still considered as the preferred option for controlling the associated biochemical syndromes and curtailing the malignant progression of pNENs.

  • A New and Minimally Invasive Ablation Technology for Tumors——Irreversible Electro-poration

    ObjectiveTo summary the clinical application of a new and minimally invasive ablation technology for tumors—irreversible electroporation. MethodsRelated domestic and foreign literatures were reviewed. ResultsCompared with traditional ablation technology, irreversible electroporation had advantages of complete ablation with well-demarcated margin, preservation of vital structures, avoidance of heat/cold-sink effect, and short ablation time. Irreversible electroporation may play an import role in patients who couldn't tolerate surgery in reason of combining with other disease, who were not suitable for surgery due to late tumors, and who had solid tumors of less than 3 cm in diameter. ConclusionAs a new and minimally invasive ablation technology for tumors, irreversible electroporation has high safety and efficacy.

  • Mei Mini Maze Procedure for 207 Patients with Atrial Fibrillation

    ObjectiveTo explore the effectiveness and safety of Mei mini maze procedure for atrial fibrillation (AF). MethodsWe analyzed the clinical data of 207 patients with 111 males, 96 females at 58.9±14.8 years in our hospital between October 2010 and February 2014. Among them, 98 patients were with paroxysmal AF and 109 patients were with persistent AF. The procedure was performed through three ports on left chest wall. Radiofrequency ablation procedures of AF included pulmonary veins isolation and ablations of the roof and posterior wall of left atrium, which were achieved by bipolar radiofrequency ablation. Ganglionic plexus ablation was made by the ablation pen. Left atrial appendage was excluded. ResultsTime of the procedures was 112.4±32.5 minutes. No conversion to sternotomy or pacemaker implantation occurred and no patients died. The hospital stay was 7.2±3.1days. The mean follow-up time was 24.2±8.9 months. A total of 187 (90.3%) patients were in sinus rhythm. And 20 patients could not maintain sinus rhythm. Stroke, thrombus in the left atrium and stenosis of pulmonary vein were not found after their procedures. ConclusionMei mini maze procedure is safe and presents optimistic outcomes for the atrial fibrillation.

  • Short-term outcome of mitral valve replacement and atrial fibrillation ablation procedure for patients with mitral valve stenosis and atrial fibrillation

    Objective To evaluate the short-term outcome and influence of atrial fibrillation ablation and mitral valve replacement for patients with mitral valve stenosis and atrial fibrillation. Methods Retrospective analysis was conducted for 44 patients with rheumatic mitral valve stenosis and atrial fibrillation who experienced mitral valve replacement with or without surgical atrial fibrillation ablation procedure in our hospital from January 2016 to June 2017. Eighteen patients experienced mitral valve replacement and surgical atrial fibrillation ablation procedure (a group 1), and the other 26 patients experienced mitral valve replacement without surgical atrial fibrillation ablation procedure (a group 2). In th group 1, there were 4 males and 14 females, aged 43-67 (55.67±7.56) years, and in the group 2 there were 6 males and 20 females, aged 40-72 (54.81±8.81) years. The patients’ data, preoperative echocardiography, surgery procedures, perioperative events, echocardiography and electrocardiogram at postoperative three months were collected to evaluate the short-term outcome and influence of surgical atrial fibrillation ablation procedure for those patients. Results There was no statistical difference in the operation duration (P=0.867) and ICU stay (P=0.550) between the two groups. But the group 1 had longer extracorporeal circulation duration (P=0.006) and aorta arrest duration (P=0.001) than the group 2. No patient died perioperatively and one patient from the group 1 experienced reoperation because of too much chest tube drainage. At three months after operation, echocardiography and electrocardiogram examination showed that 16 patients in the group 1 and 2 patients in the group 2 had sinus rhythm. There was no statistical difference between postoperative and preoperative examination about variation in left ventricle ejection fraction, pulmonary arterial systolic pressure, left atrial diameter and left ventricular end diastolic diameter between the two groups (all P>0.05). Conclusion Atrial fibrillation ablation does not increase the risk of mitral valve replacement for patients who have mitral valve stenosis and atrial fibrillation. The rate of converting to sinus rhythm is high, but additional atrial fibrillation ablation procedure does not have positive or negative influence on short-term recovery of cardiac structure and function after operation.

  • New advances in bronchoscopic ablation of malignant airway stenosis

    Malignant airway stenosis generally refers to airway lumen stenosis caused by various primary and metastatic malignant tumors and restricted airflow, which can be manifested as dyspnea to varying degrees or even asphyxia and death. It seriously affects the quality of life of patients with airway stenosis.
