  • Endobronchial Electrocautery Treatment of Tracheobronchial Obstructive Lesions in Inoperable Tracheobronchial Squamous Cell Carcinoma

    Obstractive To observe the clinical effects and safety of endobronchial electrocautery treatment for tracheobronchial obstructive lesions in inoperable tracheobronchial squamous cell carcinoma.Methods Ninety-five patients with advanced and inoperable tracheobronchial squamous cell carcinoma were included. Thirty-four patients with central airway obstruction were treated with endobronchial electrocautery plus chemotherapy ( group A) and 61 patients without central airway obstruction were treated with chemotherapy alone ( group B) . The chemotherapy consisted of cisplatin or carboplatin, plus another thirdgeneration chemotherapy agent. Results In groug A, there were mean improvements in FEV1 of 41. 1% and in peak expiratory flow( PEF) of 65. 6% . There was no significant difference in the survival rates of the patients with and without central airway obstruction. Median survival time of group A was 11. 3 months and those of group B was 11. 6 months. 3, 6, and 12-month survival rates in group A were 87% , 68% and 39% respectively, and those in group B were 93% , 76% , and 45% respectively. Conclusion Endobronchial electrocautery is an effective and safe approach for inoperable tracheobronchial obstructive malignancies with few complications.

  • Forced expiratory volume in six seconds in the spirometric diagnosis of airway obstruction

    0bjective To evaluate the efficacy of FEV6 and FEV1/FEV6 as surrogates for FVC and FEV1/FVC in the spirometric diagnosis of airway obstruction,and to determine the fixed cut-off point of FEV1/FEV6 which can be used as an alternative for FEV1/FVC lt; 70%.Methods Spirometry measurements were perform ed in 128 participants.FEV1,FEV6,FVC,FEV1%pred,FVC%pred,FEV1/FVC and FEV1/FEV6 were measured and analyzed.FEV1/FVClt;70% was used as the“gold standard”。Severity of obstruction was based on FEV % pred.From ROC curve analysis,the FEV1/FEV6 ratio,which corresponded to optimal combination of sensitivity and specificity,was determined.Correlation between FEV1/FVC and FEV1/FEV6 was studied.Results Of 128 participants,there were 65(51%)with FEV1/FVC ≥70% .Of the 63 participants with FEV1/FVC lt;70% ,there were 5 with FEV1/FEV6 between 70.09% to 71%。There was no significant difference between the mean value of FVC and that of FEV .Lifear correlation was revealed between FEV1/FVC and FEV1/FEV6 with the value close to 1(r=0.9979,Plt;0.0001).From ROC curve analysis,the FEV1/FEV6lt;71.14% was the best cut-off point coresponding to FEV1/FVC lt;70% .Conclusion These results suggest that FEV1/FEV6 is a valid alternative to FEV1/FVC for spirometric diagnosis of airw ay obstruction.There is a b corelation between FEV1/FEV6 and FEV1/FVC.

  • Application of “dual-channel teaching” of the rain classroom platform in airway obstruction teaching

    Objective To explore the application effect of " dual-channel teaching” of the rain classroom platform in airway obstruction teaching. Methods A total of 228 nursing undergraduate students in Grade 2015 were selected as the research subjects by the method of cluster random sampling. Class B (n=115) was randomly selected as the control group and Class A (n=113) was selected as the experimental group. Class B adopted the traditional practical teaching mode, and Class A was integrated with the rain classroom platform on this basis. After the end of the course, the learning situation of the experimental group students were understood through the rain classroom background data, the teaching effects of the two groups were evaluated by the students’ theoretical scores and applied case test scores, and the experimental group student’s evaluation of the rain class was understood by questionnaire survey. Results There were 105 nursing students (92.92%) completing pre-school preparation tasks, 103 (91.15%) participating in the class answering, and 113 (100.00%) completing the after-school exercises. The theoretical scores and applied case assessment scores of the experimental group students were 79.44±6.25 and 83.24±3.64, respectively, and those of the control group students were 68.50±7.96 and 70.59±5.51, respectively, the differences between the two groups were statistically significant (P<0.05). In the experimental group, 92 nursing students (81.42%) liked the rain classroom platform teaching. Conclusion The rain classroom platform teaching can bring about the ecological transformation of the open speech in classroom, and comprehensively improve the overall quality and comprehensive ability of students, which is worthy of promotion and application in teaching.

  • Influencing factors of short-term curative effect and long-term survival time of patients with malignant central airway obstruction after airway stent implantation

    Objective To analyze the influencing factors of short-term curative effect and long-term survival time of patients with tumor-induced malignant central airway obstruction (MCAO) after airway stent implantation. Methods A total of 120 patients with tumor-induced MCAO who underwent airway stent implantation in the hospital from January 2017 to June 2019 were enrolled. According to the cause of stenosis, the patients were divided into two groups: external pressure stenosis group (n=72) and non-external pressure stenosis group (n=48). The general data such as types and staging of tumor, differentiation degree, sites of airway obstruction, obstruction degree and preoperative level of lactate dehydrogenase (LDH). Before and at 7d after stent implantation, partial pressure of oxygen (PaO2), partial pressure of arterial carbon dioxide (PaCO2) and saturation of arterial blood oxygen (SaO2) were detected. Before and at 1 month after stent implantation, diameter at airway obstruction segment, degree of airway obstruction and forced expiratory volume in the first second (FEV1) were detected. Dyspnea index (DI) and scores of Karnofsky performance status (KPS) were evaluated. The survival status at 1 year after surgery was followed up. The survival at 1 year after surgery was analyzed by Kaplan-Meier. The influencing factors of survival after stent implantation were analyzed by COX proportional hazard regression analysis. Results After stent implantation, PaO2, SaO2, diameter at airway obstruction segment, FEV1 and KPS score were significantly increased (P<0.05), while PaCO2, degree of airway obstruction and DI were significantly decreased in external pressure stenosis group and non-external pressure stenosis group (P<0.05). After stent implantation, the KPS score was significantly higher in external pressure stenosis group than that in non-external pressure stenosis group, and the shortness of breath index was significantly lower than that in non-external pressure stenosis group (P<0.05). The survival rate of patients with external pressure stenosis group was 29.17%, and the median survival time was 7.35 months, the survival rate and median survival time in non-external pressure stenosis group was 22.92%, and the median survival time was 6.10 months, and there was no significant difference between the two groups (log-rank χ2=1.542, P=0.214). COX proportional hazard regression analysis showed that tumor staging at stage IV (OR=2.056, P=0.020), preoperative KPS score lower than 50 points (OR=2.002, P=0.027) and no postoperative chemoradiotherapy (OR=4.292, P=0.039) were independent influencing factors of 1-year survival time after stent implantation in MCAO patients. Conclusions The clinical curative effect of airway stent implantation is good on patients with tumor-induced MCAO. Tumor staging at stage IV, preoperative KPS score lower than 50 points and no postoperative chemoradiotherapy are risk factors that affect survival time.
