  • Establish Evaluation Systems of Medical Service Capability Related to Total Payment Control of Medical Insurance

ObjectiveTo explore effective evaluation tools as well as systems of medical service capability related to total payment control of medical insurance. MethodsCombining references and using the Delphi method, the evaluation indicators of total payment control of medical insurance were screened and identified. Then, based on analytic hierarchy process, a weight questionnaire was designed and weighted coefficients of all-level indicators were also calculated. ResultsWe proposed a mathematical model to evaluate medical service capability related to total payment control of medical insurance using three types of primary evaluation indicators and seventeen types of secondary evaluation indicators with their weighted coefficients. ConclusionThe establishment of the evaluation systems of medical service capability in medical institutions could objectively reflect medical service capability to some extent, and provide references on total payment control of medical insurance for medical insurance agencies.

  • A research for reasonable configuration standard of electrocardiogram monitors in surgical nursing units of a large public hospital based on analytic hierarchy process

ObjectiveTo find out the influencing factors of electrocardiogram (ECG) monitor configuration decision in surgical nursing units and form a scientific configuration standard, so as to provide a basis for the reasonable configuration of ECG monitors.MethodsFrom May to June 2018, the indexes and weights affecting the configuration of ECG monitors in surgical nursing units of a large public hospital were determined by interview survey method and analytic hierarchy process.ResultsThe influencing factors for configuration of ECG monitors in surgical nursing units were the number of operations, number of rescues, number of emergencies, number of deaths, and number of patients transferred to and out of intensive care unit, and the weights were 0.459 7, 0.224 9, 0.155 3, 0.111 2, and 0.049 0, respectively. The classification of nursing units was taken as plan, and the configuration standard of ECG monitors was established.ConclusionThe configuration model of ECG monitors in surgical nursing units based on analytic hierarchy process realizes the combination of qualitative and quantitative analysis, which provides scientific and reasonable reference for the configuration of ECG monitors.

  • Analysis of gastrointestinal endoscopy failure components based on analytic hierarchy process and maintenance data

ObjectiveTo explore the main failure components related to maintenance cost and downtime of gastrointestinal endoscopy.MethodsThe analytic hierarchy process (AHP) was used to process and analyze the maintenance data of gastrointestinal endoscopy in West China Hospital of Sichuan University from January 2010 to April 2019.ResultsThe results involved 110 gastrointestinal endoscopy, 322 major repairs, 364 minor repairs, and a total of 1 651 failed components, in which the top three components were buttons (380 pieces, 23.02%), the insertion tubes (223 pieces, 13.51%), and lenses (179 pieces, 10.84%). The main failure components causing high maintenance cost and long downtime of major repairs were charge coupled devices (F=849.702, P<0.001; F=9.525, P=0.002) and light guides (F=8.190, P=0.005; F=6.384, P=0.012). The failure components affecting the maintenance cost of minor repair were lenses (F=25.464, P<0.001) and angle steel wires (F=5.652, P=0.018). The failure components prolonging the maintenance time of minor repair were lenses (F=10.384, P=0.001) and electrical interfaces (F=4.816, P=0.029).ConclusionQuantitative analysis of maintenance data is helpful to objectively analyze the main failure components of maintenance cost and downtime.

  • Developing the indicator system for evaluating health in all policies in earthquake-stricken areas with Delphi method

ObjectiveTo construct a health in all policies (HiAP) evaluation index system for earthquake-stricken areas based on national health policies.MethodsFrom April to December 2020, this study combined the methods of comprehensive review of relevant literature, on the foundation of the evaluation indicators pool was preliminarily constructed. The Delphi method and analytic hierarchy process were adopted to construct the evaluation indicators system in earthquake-stricken areas and analyzed the weights of indicators.ResultsA total of 5 first-level indicators (investment, action, short-term effect, medium-term effect, long-term effect), 15 second-level indicators, and 44 third-level indicators with satisfactory logical consistency of HiAP evaluation indicators system in earthquake-stricken areas had been constructed.ConclusionsThe HiAP evaluation indicators system constructed in this study can be used to comprehensively evaluate the implementation of HiAP earthquake-stricken areas.

  • Construction of an evaluation index system for the competitiveness of private hospitals

Objective To construct an evaluation index system of the competitiveness of private hospitals, and to provide references for guiding, supervising, and managing the high-quality development of private hospitals. Methods An index pool was constructed by the literature analysis method. Index screening was completed using the modified Delphi method. The analytic hierarchy process, entropy weight method, and combination weight method were used to determine the index weight. Results The competitiveness evaluation index system of private hospitals was constructed, which included 5 primary indexes and 36 secondary indexes. The combination weight methods were resource allocation (0.366 8), service capacity (0.470 8), service efficiency (0.033 7), quality and safety (0.121 3), and financial management (0.007 3). Conclusion The constructed evaluation index system of competitiveness of private hospitals is scientific, targeted, and operable.

  • A study on the construction of the comprehensive evaluation indicator system of Chinese patent medicines for ischemic stroke

ObjectiveTo construct the comprehensive evaluation indicator system of Chinese patent medicines for ischemic stroke, to determine the weight of indicators, and to provide references for the comprehensive evaluation of the efficacy, safety, and economy of Chinese patent medicines.MethodsTwo rounds of expert consultation by Delphi method were applied to establish the comprehensive evaluation indicator system of Chinese patent medicines for ischemic stroke, and the weight of each indicator was determined by the analytic hierarchy process method.ResultsQuestionnaire recovery rates of 2 rounds were 92.59% and 96.00%, the expert authority coefficient was greater than 0.7, and the coordination coefficients of experts in the total index were 0.224 and 0.370 (P<0.001). A three-level comprehensive evaluation indicator system for Chinese patent medicines for ischemic stroke was established and the three first-level indicators included efficacy, safety, and economy. And there were 15 second-level indicators, and 33 third-level indicators. Through the analytic hierarchy process method, the weights of each first-level indicator were 0.626 4, 0.301 2, and 0.072 4, respectively.ConclusionThe comprehensive evaluation indicator system contains efficacy, safety and economy, and provides a basis for a comprehensive evaluation of Chinese patent medicines for ischemic stroke. The indicator system is of great significance for the design of outcomes for clinical trials of ischemic stroke, the conduction of systematic reviews, and the development of clinical practice guidelines for ischemic stroke patients when selecting study outcomes.
