Objective To study the role of the complement receptor 1 and 3 ( CR1 and CR3) on neutrophils in distinguishing bacterial infection in patients with acute exacerbation of chronic obstructive pulmonary disease ( AECOPD) . Methods 40 patients with AECOPD were divided into two groups according to the detection of bacteria in subairway. 20 patients with stable COPD and 20 healthy subjects with no history of smoking were also included. According to Anthonisen criteria, 40 AECOPD patients weredivided into type Ⅰ( 11 cases) , type Ⅱ ( 12 cases) , and type Ⅲ( 17 cases) . The levels of CR1 and CR3 in blood were measured by flow cytometry. Results In AECOPD patients, 25 cases were detected bacteria,and 15 cases were not detected bacteria. The level of CR1 and CR3 were highest in the bacterial infection group than other groups, and highest in type Ⅰ AECOPD patients than other types. ROC analysis showed that CR1 and CR3 had good diagnostic value in bacterial infection in AECOPD, with optimal cutoff values of 11 and 52, respectively. Conclusion CR1 and CR3 may be good index of distinguishing bacterial infection in AECOPD.
Objective To evaluate the clinical efficacy and safety of domestic cefepime in the treatment of acute bacterial lower respiratory tract infection. Methods A randomized, single-blind, controlled clinical trial was performed. The positive control was imported cefepime. The dosages of cefepime were 1g for moderate infection and 2g for severe infection, twice a day intravenously. The duration of the treatment was 7-10 days. Results Thirty-one patients were enrolled in the trial, of whom 30 were evaluable (15 in the triagroup and 15 in the control group). No significant differences were observed between the trial group and the control group with respect to the cure rate (40% vs. 27%), the effective rate (80% vs. 87%), the bacterial clearance rate (92% vs. 100%), and the incidence of adverse drug reactions (12.5% vs. 13%) (Pgt;0.05). Conclusion Domestic cefepime injection is effective and safe in the treatment of acute bacterial lower respiratory tract infection.
【 Abstract 】 Objective To investigate the features and risk factors of bacterial infection after orthotopic liver transplantation (OLT) and summarize the experiences for prevention and treatment of infection. Methods The clinical materials of 79 patients underwent OLT from December 2004 to January 2007 were reviewed. The incidence and sites of postoperative bacterial infections as well as species of bacteria and the situation of drug resistance were retrospectively analyzed. Results Thirty-two (40.5%) out of 79 patients developed bacterial infections. Infection mainly occurred in lung, abdominal cavity and blood, which were mainly caused by Escherichia coli, staphylococcus aureus, klebsiella pneumoniae, staphylococcus epidermidis and enterococcus bacteria. The main risk factors included delayed restoration of gastrointestinal function (gt;5 d), poorly preoperative liver function (Child C grade), hypoproteinemia and persistent postoperative hyperglycemia (gt;11.0 mmol/L). Perioperative decontamination in digestive tract was a protective factor in the prevention of bacterial infection. Conclusion Bacterial infection is one of the most severe complications after OLT. Therefore, it is very important to remove those risk factors, make early diagnosis and take effective treatment .
0bjective To study the effect of bacterial infection on acute exacerbation of chronic obstructive pulmonary disease(AECOPD),and to compare the airway inflammation caused by different isolated bacteria.Methods A total of 159 sputum samples were collected from AECOPD patients diagnosed according to GOLD 2004 standard,in which conventional culture and identification of bacteria was conducted.The patients with purulent sputa were divided into different groups according to bacteria separated.Levels of IL-6.IL-8 and TNF-α in sputum supernatant were assayed and compared in different bacteria groups.the purulent sputum without isolated bacteria group(NG)and normal control group(NC). Results One hundred and twenty-nine strains of bacteria were isolated in 159 qualified sputa,including 26 strains of Klebsiella pneumoniae(KB),21 strains of Hemophilus influenza(Hi),17 strains of Pseudomonas aeruginosa(PA),37 strains of Haemophilus parainflb~enzae(HP)(mixed infection not included)and 28 strains of other bacteria.Among of all samples,20 were double infection of Haernophilus parainfluenzae with another bacterium.Ninety-seven purulent sputa were collected.According to bacteria isolated,these sputa were divided into five groups,named HP(24 samples),Hi(20 samples),PA(16 samples),KB(19 samples)and NG(18 samples).Contrast to NC,concentration of IL-8 and TNF-α rose in sputa from which PA,Hi,KB were isolated(Plt;0.05).The level increased much more in sputa from which PA and Hi were isolated compared with KB(Plt;0.05).Concentration of IL-6 rose in sputa of each group collected from AECOPD patients contrast to sputa collected from NC(Plt;0.05),without significant differences among all other group except for NC.Conclusions Bacterial infection plays an important role in AECOPD characterized with high level of inflammatory factors especially when PA,Hi,KB were infected bacteria.This study provides evidence for antibacterial therapy in AECOPD patients.
With the development of photothermal nanomaterials, photothermal therapy based on near-infrared light excitation shows great potential for the bacterial infected wound treatment. At the same time, in order to improve the photothermal antibacterial effect of wound infection and reduce the damage of high temperature and heat to healthy tissue, the targeted bacteria strategy has been gradually applied in wound photothermal therapy. In this paper, several commonly used photothermal nanomaterials as well as their targeted bacterial strategies were introduced, and then their applications in photothermal antibacterial therapy, especially in bacterial infected wounds were described. Besides, the challenges of targeted photothermal antibacterial therapy in the wound healing application were analyzed, and the development of photothermal materials with targeted antibacterial property has prospected in order to provide a new idea for wound photothermal therapy.
Objective To design and construct a graphene oxide (GO)/silver nitrate (Ag3PO4)/chitosan (CS) composite coating for rapidly killing bacteria and preventing postoperative infection in implant surgery. Methods GO/Ag3PO4 composites were prepared by ion exchange method, and CS and GO/Ag3PO4 composites were deposited on medical titanium (Ti) sheets successively. The morphology, physical image, photothermal and photocatalytic ability, antibacterial ability, and adhesion to the matrix of the materials were characterized. Results The GO/Ag3PO4 composites were successfully prepared by ion exchange method and the heterogeneous structure of GO/Ag3PO4 was proved by morphology phase test. The heterogeneous structure formed by Ag3PO4 and GO reduced the band gap from 1.79 eV to 1.39 eV which could be excited by 808 nm near-infrared light. The photothermal and photocatalytic experiments proved that the GO/Ag3PO4/CS coating had excellent photothermal and photodynamic properties. In vitro antibacterial experiments showed that the antibacterial rate of the GO/Ag3PO4/CS composite coating against Staphylococcus aureus reached 99.81% after 20 minutes irradiation with 808 nm near-infrared light. At the same time, the composite coating had excellent light stability, which could provide stable and sustained antibacterial effect. ConclusionGO/Ag3PO4/CS coating can be excited by 808 nm near infrared light to produce reactive oxygen species, which has excellent antibacterial activity under light.
