  • Negative Pressure Treatment for Wound of Different Types at Different Parts

    【摘要】 目的 探讨负压引流技术对不同部位、不同类型创面的方法和效果。 方法 回顾性分析2009年5月-2010年12月采用负压引流技术治疗85例创面的临床资料。其中男51例,女34例;年龄4~75岁,平均年龄48岁。 结果 79例创面行1次负压治疗后、6例创面连续行2次负压治疗后情况明显改善,二期行游离皮片移植术或皮瓣转移术修复创面,术后创面均Ⅰ期愈合,皮片或皮瓣存活良好。 结论 负压吸引治疗技术简便有效,可靠安全,能加速清除不同部位、不同类型创面的坏死组织,改善创面条件,缩短创面治疗时间。【Abstract】 Objective To explore the effects of negative pressure treatment on wound of different types at different parts. Methods The clinical data of 85 patients (51 males and 34 females; 4-75 years old) with wound who received negative pressure treatment between May 2009 and December 2010 were retrospectively analyzed. Results After receiving negative pressure treatment for once in 79 patients and twice in 6, the wound improved obviously. All cases received regular methods such as skin grafting or tissue flap transferring to reconstruct the wound. All of the 85 patients healed by first intention and the skin grafts or flaps survived well. Conclusion Negative pressure treatment is a simple, effective, stable and safe method for wound treatment, which can accelerate eliminating necrotic tissue of wound of different types at different parts, improve the wound condition and shorten the period of therapy.

  • The mechanism research of sodium hyaluronate to rabbit knee osteoarthritis based on metabolomics research

    ObjectiveTo detect the metabolites of the serum and joint fluid from rabbits’ osteoarthritis model with 1H nuclear magnetic resonance spectroscopy (NMR) technique, study the metabolic differences and connections of serum, synovial and cartilage of rabbits after the articular cavity injection of sodium hyaluronate, and explore osteoarthritis and metabolic mechanism in the process of treating sodium hyaluronate using sodium hyaluronate, thus provide new ideas and basis of the specific mechanisms in the treatment of osteoarthritis via sodium hyaluronate.MethodsWe selected 30 healthy New Zealand white rabbits, 6 months old, and randomly divided them into three groups as follows: blank control group, model phosphate buffer saline (PBS) liquid injection group and model injection of sodium hyaluronate group, with 10 rabbits in each group. Ten weeks after surgery, all experimental animals were put to death and observed in correlation studies regarding general condition, imaging examination, and histological examination. Metabolites 1H NMR detection and data preprocessing were performed in the serum and joint fluid samples.ResultsThe results considering general condition, general sample observation, imaging examination and histology indicated advantages in sodium hyaluronate group over PBS group. Metabolomics analysis showed statistically significant changes of metabolites in the serum and joint fluid compared with the PBS group and the blank control group (P<0.05). According to the relevant ways of differences metabolites retrieval, analysis found that the effect of sodium hyaluronate on osteoarthritis might be related to protein biosynthesis, amino acid circulation, the metabolic process of pyruvic acid, gluconeogenesis and other metabolic pathways.ConclusionsBased on the research of 1H-NMR metabolomics, the results suggest that the effect of sodium hyaluronate on osteoarthritis is mainly related with the activation of protein metabolism, abnormal lipid and energy metabolic pathways. This study provides new ideas and basis on the concrete mechanism in the treatment of knee osteoarthritis using sodium hyaluronate.

  • Application of modified inferior pedicle technique with inverted T pattern for severe breast hypertrophy

    ObjectiveTo explore the effectiveness of modified inferior pedicle technique with inverted T pattern for severe breast hypertrophy.MethodsBetween January 2016 and May 2017, 15 patients of severe breast hypertrophy had undergone breast reduction using inferior pedicle technique with inverted T pattern combined with dermal suspension sling technique. The patients were 20 to 49 years old, with an average age of 31.6 years. Body mass index ranged from 24.9 to 32.5 kg/m2, with an average of 30.8 kg/m2. Among them, 11 cases had a history of childbearing. The degree of breast ptosis was rated as degree Ⅱ in 6 cases and degree Ⅲ in 9 cases. The unilateral breast reduced 615 g on average (range, 480-1 050 g).ResultsThe skin flap necrosis at the " T” trilateral junction occurred in 3 cases, and healed after dressing changes. The incisions of 12 cases healed and no fat liquefaction, hematoma, or seroma occurred. The sensation of nipple and areola declined at early period after operation in 2 cases, and gradually recovered. All patients were followed up 6-18 months (mean, 13 months). The shapes of bilateral breasts and the height and symmetry of nipple-areolar complex were good, and no obvious scar was found. The effectiveness was evaluated by surgeon and showed that there were 12 cases with satisfactory breast shape and 3 cases with unsatisfactory breast shape; 3 cases with obvious scare and 12 cases with insignificant scar; 13 cases with normal nipple sensation and 2 cases with hypoesthesia; 11 cases with symmetric nipples and 4 cases with asymmetric nipples. The effectiveness was evaluated by patients and showed that the satisfactory breast shape in 10 cases, relatively satisfactory breast shape in 4 cases, and unsatisfactory breast shape in 1 case; highly acceptable scar in 9 cases, moderately acceptable scar in 4 cases, and unacceptable scar in 2 cases; overall satisfactory in 10 cases, relatively satisfactory in 4 cases, and unsatisfactory in 1 case, with the overall satisfaction rate of 93.3% (14/15).ConclusionFor severe breast hypertrophy, the modified inferior pedicle technique with inverted T pattern can obtain satisfactory appearance and avoid the mastoptosis.

  • Effectiveness of internal mammary artery perforator propeller flap repair combined with radiotherapy for chest keloid in female patients

    ObjectiveTo investigate the effectiveness of internal mammary artery perforator (IMAP) propeller flap repair combined with radiotherapy for chest keloid in female patients.MethodsBetween January 2015 and December 2016, 15 female patients with chest keloids were treated, aged 28-75 years (mean, 45.2 years). The keloid disease duration was 1-28 years (median, 6 years). The causes of disease included secondary keloid caused by folliculitis in 7 cases, cardiac surgery in 4 cases, skin abrasion in 2 cases, mosquito bite in 1 case, and unknown etiology in 1 case. The size of keloid ranged from 5 cm×3 cm to 17 cm×6 cm. The IMAP propeller flaps were used to repair the defects after chest keloid excision. The size of flaps ranged from 7 cm×5 cm to 14 cm×8 cm. The donor sits were sutured directly. The routine radiotherapy was performed after operation.ResultsAll IMAP propeller flaps survived well, and the donor sites healed by first intention. All 15 patients were followed up 12-24 months (mean, 16 months). No telangiectasia or incision dehiscence occurred. No radiation-related carcinogenesis occurred during follow-up. The patients were satisfied with the breast shape and symmetry after operation. The symptoms of pain and itching relieved at keloid area in 13 cases (86.7%), with no obvious recurrence of keloid at the donor site and the primary site. Only 2 cases (13.3%) recurred and were treated with continuously conservative treatment.ConclusionIMAP propeller flap is an ideal reconstruction method for repairing the wounds after chest keloid excision in female patients, which can preserve the good breast shape. The IMAP propeller flap repair combined with early postoperative radiotherapy can effectively reduce the recurrence rate, and the effectiveness is satisfactory.

  • Investigation on AIDS knowledge and sexual behavior of middle school students in the key areas of HIV epidemic in Liangshan, Sichuan

    Objective To investigate the AIDS knowledge and sexual behavior of middle school students in the key areas of HIV/AIDS epidemic in Liangshan, Sichuan Province in 2015 and 2020, so as to understand the epidemic trend and acquire possible references for future prevention. Methods A survey on AIDS knowledge and sexual behavior of middle school students was conducted in 2015 and 2020 in the key areas of HIV/AIDS epidemic in Liangshan, Sichuan Province, respectively. The prevalences of HIV/AIDS knowledge and sexual behavior were analyzed. Results The rate of AIDS education was increasing, which was 75.4% in 2015 and 89.3% in 2020, respectively. However, the awareness rate of AIDS remained low, which was 44.4% in 2015 and 53.3% in 2020, respectively. The reported rate of sexual behavior decreased, which was 12.8% in 2015 and 1.6% in 2020, respectively. The condom usage rates were still low, with 47.4% and 55.8%, respectively in 2015 and 2020. Conclusions The AIDS publicity and education work in Liangshan is effective, and the awareness rate of AIDS, the rate of sexual behavior and the usage rate of condom are improving well. However, the slow increase in the awareness rate of AIDS and the usage rate of condom require further in-depth analysis aiming at the specification of this area so that effective scientific measures can be carried out to prevent the HIV/AIDS diffusion.
