Objective To explore the clinical and aesthetic results of single axillary approach reverse sequence endoscopic nipple-sparing mastectomy of breast cancer combined with subpectoral implant-based breast reconstruction (IBBR) for breast cancer patients. MethodsThe clinical data of consecutive female patients who underwent single axillary approach reverse sequence endoscopic nipple-sparing mastectomy of breast cancer combined with subpectoral IBBR from May 2020 to December 2022 were retrospectively analyzed. The patient demographics, perioperative parameters, complications and aesthetic results were collected. Results A total of 62 patients (74 breasts) were included. The mean age of patients was 39.0±7.8 years. The total mean operation time was 185.2±45.5 minutes. Five (8.1%) patients suffered from surgical complications. Two (3.2%) patients experienced major complications, and 3 (4.8%) suffered from minor complications. No patient experienced partial nipple-areola complex necrosis. Seventeen (27.4%) patients experienced implant related complications, of which 11 (17.7%) experienced capsular contracture. During a median follow-up time of 30 months, no patients experienced locoregional recurrence, distant metastasis and death. Thirty-seven (88.1%) patients evaluated their breasts as good or excellent. In the surgeon-reported cosmetic results, 40 (95.2%) patients achieved excellent or good results. ConclusionThe surgery is characterized by minimal trauma, short operating time, reliable safety and satisfactory aesthetic effect for small and no or mild ptosis breast patients, deserving further promotion.
Objective To analyze the advantages and disadvantages of various neoadjuvant therapy , provide reference for clinical diagnosis and treatment, and provide direction for further research and exploration. Method The recent domestic and international medical databases (PubMed, EMBASE, Cochrane Library, VIP database, CNKI, WanFang database, etc.) were searched and the relevant literature on neoadjuvant therapy for locally advanced rectal cancer (LARC) were reviewed. Results Neoadjuvant therapy could decrease tumor staging, increase anal reserving rate, and reduce local recurrence rate, but it does not significantly reduce the rates of distant metastasis and lateral lymph node metastasis, nor does it improve long-term survival. More and more optimization neoadjuvant therapy had emerged. Molecular targeted drugs and immunotherapy were being attempted for clinical using, combined with research on emerging biomarkers, to improve the therapeutic efficacy of LARC patients, reduce treatment related side effects, and improve patient survival benefits. Conclusions Neoadjuvant therapy is the standard treatment strategy for LARC, and the exploration of neoadjuvant treatment models is expected to further improve treatment effectiveness, reduce toxic side effects, and improve survival prognosis. By combining tumor molecular biology indicators to identify and screen beneficiaries, it is expected to become an important direction for future research.
Endoscopic and robotic surgeries feature small incision and reducing surgical trauma, and minimized incision scars. However, the oncological safety of their application in breast-conserving surgery and breast reconstruction for breast cancer has always been a focal clinical concern. The breast-conserving surgery and breast reconstruction using the suspension, insufflation, and lipolysis methods could achieve precise tumor resection in the selected patients and under the specific surgical conditions, with the support of appropriate instruments. Meanwhile, the innovative application of the reverse-sequence method and auxiliary port technique has further enhanced surgical efficiency and the precision of tumor resection. Current studies suggest that endoscopic and robotic-assited breast-conserving surgery and breast reconstruction yield in terms of oncological outcomes comparable to those of conventional open breast-conserving surgery, including positive margins, local recurrence, regional recurrence, distant metastasis, and overall survival. These approaches offered advantages in minimally invasive techniques and aesthetic outcomes. However, existing research was limited by short follow-up period and small sample sizes. Future large-scale, long-term prospective randomized controlled trials are needed to further validate their oncological safety and long-term efficacy. These studies could help establish novel techniques as standard surgical approaches for breast cancer, particularly the efficient and streamlined reverse-sequence endoscopic and auxiliary port-assisted techniques.
In conventional open breast surgery, the surgical trauma is significant and the postoperative scar is often noticeable. Endoscopic and robot-assisted breast surgery is increasingly attracting attention due to the advantages such as smaller incisions, lower complication rate, and improved aesthetic outcomes. However, the lack of natural cavities in the breast has become a primary challenge in establishing and maintaining the necessary surgical space for endoscopic breast surgery. We reviewed the research progress of endoscopic and robot-assisted breast surgery, summarized the the innovations and challenges of existing techniques, and focused on introducing the application value of physical and biological properties of gas and liquid in endoscopic breast surgery.
