  • Different acupuncture methods on pregnancy outcome of in vitro fertilization-embryo transfer: a network meta-analysis

    Objective To analyze the efficacy and safety of different acupuncture methods on outcome of in vitro fertilization-embryo transfer (IVF-ET). Methods The PubMed, EMbase, Cochrane Library, CNKI, VIP, WanFang Data and CBM databases were searched to collect randomized controlled trials (RCTs) related to the objectives of the study from the inception to April 16, 2023. After two investigators independently screened the literature, extracted the data and evaluated the risk of bias of the included studies, a network meta-analysis was performed using Stata 16.0 software. Results There were 62 trials total with 9844 patients, involving 7 interventions. Network meta-findings analysis revealed the following: ① Clinical pregnancy rate (CPR): needle warming > auricular acupressure > transcutaneous electrical acupoint stimulation (TEAS) > electroacupuncture > acupuncture > sham acupuncture > no adjunctive treatment; ② Live birth rate (LBR): electroacupuncture > auricular acupressure > TEAS > acupuncture > sham acupuncture > no adjunctive treatment. Conclusion Needle warming assisted IVF-ET is superior to other acupuncture therapies in improving CPR, especially during the promotion period of excretion, and the selection of Zusanli, Guanyuan and uterine acupoints for 3-month cycles may have the best effect. And for the LBR, the effect of electroacupuncture is better than that of other therapies. Besides, auricular acupressure may have good therapeutic potential. Due to the limited quality and quantity of the included studies, more high quality studies are required to verify the above conclusions.

  • Computer-aided prediction and molecular mechanism investigation of active components in compound Kushen injection inhibiting p21-activated kinase 1

    Targeting p21-activated kinase 1 (PAK1) is a novel strategy for pancreatic cancer treatment. Compound Kushen injection contains many anti-pancreatic cancer components, but the specific targets are unknown. In this study, 14α-hydroxymatrine, an active component of Kushen injection, was found to possess high binding free energy with the allosteric site of PAK1 by molecular docking based virtual screening. Molecular dynamics simulations suggested that 14α-hydroxymatrine caused the α1 and α2 helices of the allosteric site of PAK1 to extend outward to form a deep allosteric regulatory pocket. Meanwhile, 14α-hydroxymatrine induced the β-folding region at the adenosine triphosphate (ATP)-binding pocket of PAK1 to close inward, resulting in the ATP-binding pocket in a “semi-closed” state which caused the inactivation of PAK1. After removal of 14α-hydroxymatrine, PAK1 showed a tendency to change from the inactive conformation to the active conformation. We supposed that 14α-hydroxymatrine of compound Kushen injection might be a reversible allosteric inhibitor of PAK1. This study used modern technologies and methods to study the active components of traditional Chinese medicine, which laid a foundation for the development and utilization of natural products and the search for new treatments for pancreatic cancer.

  • Application of Sysmex XT-4000i hematology analyzer in cell count and cell differential count of body-fluid samples

    Objective To evaluate the value of Sysmex XT-4000i hematology analyzer in its body-fluid mode in cell count and cell differential count of pleural effusion, ascites and cerebrospinal fluid samples. Methods A total of 95 pleural effusion, ascites and cerebrospinal fluid samples were collected from patients hospitalized between May and September 2015. The samples were tested by Sysmex XT-4000i hematology analyzer (instrument method) and modified Neubauer hemocytometer (manual method) for cell count, and the results of them were compared and analyzed. Results The instrument method and the manual method had a good consistency in nuclear cell count and erythrocyte count (kappa=0.965,P< 0.001; kappa=0.988,P<0.001). There was no significant difference in the count of mononuclear cells (P> 0.05). However, there was a significant difference in the count of multiple nuclear cells (P<0.05). Conclusions Hematology analyzer in its body-fluid mode may replace manual method in cell count of pleural effusion, ascites and cerebrospinal fluids for its high precision, high efficiency and easy operation. However, cell differential count of this method needs microscopic examination assistance.

  • The duration of epilepsy in sleep effect of progesterone diet on epilepsy syndrome in sleep and recurrent epilepsy syndrome and EEG changes

    ObjectiveTo analyze the efficacy of ketogenic diet (KD) in the treatment of epilepsy syndrome associated with Electrical status epilepticus during sleep (ESES) and the changes of electroencephalogram (EEG). MethodsThe clinical data of 58 children with ESES-related epilepsy syndrome and cognitive impairment admitted to the outpatient and inpatient department of Xuzhou Children’s Hospital from March 2020 to June 2022 were collected. They were divided into observation group (group A) and control group (group B) by random number table method, with 29 cases in each group. On the basis of not changing the original anti-seizure drugs, group A was treated with ketogenic diet, and group B was treated with hormone. The clinical efficacy, cognitive function and Spike-wave index (SWI) of children with ESES-related epilepsy syndrome were observed before and 3 and 6 months after ketogenic diet and hormone treatment. Results The total clinical effective rate and control rate of group A after 3 months of treatment; The total effective rate and significant efficiency of EEG and the verbal quotient, operation quotient and total intelligence quotient of Wechsler Intelligence Scale for children were significantly lower than those of group B, and the differences were statistically significant (P<0.05). However, the total clinical effective rate and control rate of group A after 6 months of treatment; The total effective rate and significant efficiency of electroencephalogram and the verbal quotient, operation quotient and total intelligence quotient measured by Wechsler Intelligence Scale for children were similar to those of group B, and the difference was not statistically significant (P>0.05). There were no cases of EEG SWI and recurrent clinical symptoms in group A during the follow-up period; In group B, the recurrence rate of EEG SWI was 55.2%, and the recurrence rate of clinical symptoms was 37.9% after 6 months of treatment. There were no serious adverse reactions in the two groups during the treatment period. In group A, about 27.6% (8 cases) of the children had gastrointestinal symptoms, but they were mild and quickly relieved, and no serious adverse reactions occurred in the maintenance period. However, 89.7% (26 cases) of the children in group B showed different degrees of weight gain, and 69% (20 cases) of them showed kexing features such as full moon face and buffalo back. ConclusionsKetogenic diet can improve the curative effect and EEG changes of children with ESES related epilepsy syndrome, and improve their cognition. Compared with hormone pulse therapy, the recurrence rate of ESES related epilepsy syndrome in the treatment of ketogenic diet is lower, and the incidence of adverse reactions is relatively mild and low.

  • The Value of Transvaginal Color Doppler Ultrasonography in the Diagnosis of Endometrial Polyps

    【摘要】 目的 探讨经阴道彩色多普勒超声诊断子宫内膜息肉的价值,进一步提高子宫内膜息肉的诊断准确率。 方法 2009年1月-2010年12月,对48例子宫内膜息肉患者,经阴道彩色多普勒超声发现病灶后观察其位置、大小、内部回声及病灶内部和周边的彩色血流情况,并与手术病理结果对照。 结果 经阴道彩色多普勒超声诊断子宫内膜息肉的准确率为87.5%, 2例误诊为子宫黏膜下肌瘤,3例误诊为子宫内膜增厚,1例漏诊。 结论 经阴道彩色多普勒超声对子宫内膜息肉有较高的临床诊断价值。【Abstract】 Objective To investigate the value of transvaginal color doppler ultrasonography in the diagnosis of endometrial polyps and to further improve the diagnostic accuracy. Methods From January 2009 to December 2010, fourty-eight patients with endometrial polyps participated in this study. Transvaginal color doppler ultrasonography was used to observe the location, size, and internal echo of the lesions, and the color flow within and around the lesions. Then, we compared all the ultrasonic features with pathological findings. Results The diagnostic accuracy of transvaginal color doppler ultrasonography in the diagnosis of endometrial polyps was 87.5%. Two cases were misdiagnosed as submucosal uterine fibroids, 3 as endometrial thickening, and 1 missed. Conclusion Transvaginal color doppler ultrasonography has a high clinical value in the diagnosis of endometrial polyps.

  • Comparison of diagnostic significance between myeloperoxidase staining and cytoplasm myeloperoxidase in acute leukemia cells

    Objective To detect the difference between the peroxidase (POX) by cytochemical staining and cytoplasm myeloperoxidase (cMPO) by flow cytometry in acute leukemia cells, and provide a more accurate basis for the classification of leukemia. Methods The positive rate of POX in acute leukemia cells was detected by cytochemical staining. The positive rate of cMPO in acute leukemia cells was detected by flow cytometry. Then the positive rate of POX and cMPO, and the positive cells score were analyzed. Results The positive rate and the positive cells scores between POX and cMPO in acute lymphoblastic leukemia were significantly different (P<0.05), the positive rate and the positive cells scores of POX were significantly higher than those of cMPO. The positive rate between POX and cMPO in acute non-lymphoblastic leukemia (ANLL) had significant differences (P<0.05), the positive rate of cMPO was higher than that of POX; but no difference was found between POX and cMPO positive cells scores in ANLL (P>0.05). In acute myelocytic leukemia (AML)-M1 subtype, significant difference was found in the positive rate between POX and cMPO (P=0.006); cMPO positive rate was significantly higher than that of POX, but the POX positive cells score was significantly higher than that of cMPO (P=0.001). There were no significances of positive rate and positive cells score in AML-M2, AML-M3, AML-M4, AML-M5 subtypes between POX and cMPO (P>0.05). Conclusions There are not major differences between positive rate of POX and cMPO, as well as the positive cells scores in acute leukemia, especially acute myelocytic leukemia. We can choose the better method according to the actual situation and the sensitivity requirements. The two methods should be replenished by each other and used alternately.

  • Spectral CT imaging in detecting breast cancer: a pilot study

    Objective To analyze the ability of spectral CT imaging in displaying breast cancer lesions and explore the value of spectral CT imaging in detecting breast cancer. Methods The spectral CT images with different parameters of sixty-eight breast cancer lesions confirmed by pathology between July 2013 and February 2016 were retrospectively analyzed. The contrast noise ratio (CNR) and signal to noise ratio (SNR) as well as the ability in detecting breast cancer of the images with different parameters were compared. Results Fat-water material decomposition images showed breast cancer lesions best. Iodine-water material decomposition images had the lowest CNR. 70 keV monochromatic images and the monochromatic of best CNR images had better SNR. Fat-water material decomposition images detected all of the breast cancer lesions. Conventional CT plain scan detected least lesions. Conclusion Spectral CT imaging, especially fat-water material decomposition images, can show breast cancer lesions well, which has the potential application for detection of breast cancer lesions.

  • Efficacy and safety study of perampanel monotherapy in self-limited epilepsy with central temporal spikes

    ObjectiveTo investigate the efficacy and safety of Perampanel (PER) monotherapy in the treatment of self limited epilepsy with central temporal spikes (SeLECTS). Methods The clinical data of the first confirmed SelECTS in the outpatient and inpatient department of Xuzhou Children's Hospital affiliated to Xuzhou Medical University in December 2021 were collected, and the clinical data of PER monotherapy were retrospectively analyzed. The Seizure of 12 months old children were followed up to observe the efficacy and safety of PER monotherapy with spikes in the central temporal region, and the changes of Electroencephalography were observed. Result A total of 45 children with SeLECTS were included, of which 43 had complete medical records, including 13 males and 30 females, aged 4 ~ 14 years old, with a course of disease ranging from 1 month to 1.5 years. All 45 patients had focal seizures or focal secondary generalized tonic clonic seizures. Among them, 43 patients treated with PER alone for 12 months had epilepsy control efficacy rates of 74.41% (32/43) and no seizure rates of 60.46% (26/43), respectively. Seven children (15.56%, 7/45) experienced adverse reactions, characterized by dizziness, unstable gait, and irritability. Conclusion The third generation anti Seizure drug PER has a clear effect in the treatment of SelECTS, 2 ~ 4 mg PER can control Seizure well, and has no significant impact on cognitive development.

  • Arginine-Containing Desensitizing Toothpaste for Dentine Hypersensitivity: A Meta-analysis

    Objective To estimate the effect of desensitizing toothpaste containing arginine on dentine hypersensitivity. Methods Such databases as CNKI, PubMed, Web of Science and Cochrane Trials Register (Issue 4, 2008) were retrieved, and Google was used as a supplementary tool to search the information up to March 2010. Randomized controlled trials (RCTs) of treating dentine hypersensitivity with arginine-containing toothpaste were included, and the relevant information was extracted and the quality evaluation was undertaken. Meta-analyses were performed with RevMan 5.0 software. Results Five RCTs with 397 patients were included. The results of meta-analyses showed that at 8 weeks, arginine-containing toothpaste was significantly different from potassium-containing toothpaste in terms of tactile sensitivity test (SMD=1.32, 95%CI 0.68 to 1.96) and air blast test (SMD= –0.77, 95%CI –1.22 to 0.32) with a better therapeutic effect. Conclusion Current literature evidence shows that the arginine-containing toothpaste is effective for the dentine hypersensitivity. However, this study is based on a small number of RCTs and samples, so further studies with high-quality and large-sample RCTs are needed.

  • Evaluation of sublingual microcirculation on prognosis of elderly patients with severe pneumonia undergoing mechanical ventilation

    Objective To evaluate the relationship between sublingual microcirculation differences and weaning success rate and prognosis in elderly patients with severe pneumonia. Methods A retrospective observation cohort study was conducted. Forty-two elderly patients with severe pneumonia who underwent mechanical ventilation in the intensive care unit of Sir Run Run Hospital, Nanjing Medical University from February 2022 to August 2022 were recruited in the study. They were divided into a high-flow nasal cannula oxygen group (HFNC group, n=33) and a non-invasive positive pressure ventilation group (NIPPV group, n=9) according to the mode of post-weaning ventilation. The differences of N-terminal brain natriuretic peptide precursor (NT-proBNP), cardiac index (CI) and sublingual microcirculation indexes between the two groups were analyzed. The receiver operating characteristic (ROC) curve was used to analyze the predictive value of each parameter on weaning success rate and case fatality rate. Results Compared with the NIPPV group, CI, propotion of perfused vessels (PPV) and perfused vessel density (PVD) were higher, and NT-proBNP and total vessel density (TVD) were lower in the HFNC group (all P<0.05). The prediction value of PPV combined with PVD was the largest, with area under the ROC curve (AUC) of 0.875, sensitivity of 75.8%, specificity of 88.9%. CI, NT-proBNP, CI combined with NT-proBNP, PPV, PVD all had predictive value. Compared with the death group, the survival group had higher CI, central venous-to-arterial carbon dioxide difference [P(v-a)CO2] and PPV. For the prediction value of weaning success, CI combined with NT-proBNP had the largest predictive value, with AUC of 0.919, sensitivity of 81.8%, specificity of 100.0%, followed by CI. NT-proBNP, PPV, PVD, PPV combined with PVD all had predictive value. Compared with the death group, the survival group had higher CI, P(v-a)CO2 and PPV (all P<0.05). For predictive value assessment of 28-day survival rate, CI plus PPV had the largest AUC of 0.875, with sensitivity of 69.4%, and specificity of 100.0%. CI, P(v-a)CO2 and PPV all have predictive value. Conclusions Both CI and PPV can be used as predictors of weaning success rate and survival rate. PPV combined with PVD is an ideal predictor of survival rate.
