  • Topical Fluoride for the Prevention of Dental Caries in Children: A Systematic Review

    Objective To evaluate the effect of different topical fluorides for preventing dental caries in children. Methods Literature was searched according to the methods of systematic review in the following databases: CENTRAL, Medline, EMbase, CNKI, VIP, WanFang Data, CBM and Gray literature databases such as Open Grey and National Technical Information Service (NTIS) online, and the relevant journals and professional materials were also searched manually. The quality of the selected studies was appraised using Cochrane criteria, and then Meta-analysis was conducted suing RevMan 5.0, GRADEprofiler and Stata.11 software. Results 18 RCTs involving 19 359 children were included. A result of eight RCTs showed that the prevalence rate of caries of the fluoride varnish group (50.62%) was lower than that of the control group (70.89%) (OR=0.43, 95%CI 0.34 to 0.44, Plt;0.000 01). A result of four RCTs showed that the prevalence rate of caries of the fluoride ion group (32.04%) was lower than that of the control group (44.94%) (OR=0.50, 95%CI 0.45 to 0.55, P=0.02). A result of six RCTs showed that the prevalence rate of caries of the fluoride foam group (55.90%) was lower than that of the control group (67.20%) (OR=0.53, 95%CI 0.47 to 0.60, Plt;0.000 01). The quality of the above three results was low according to the GRADE analysis. Conclusion This systematic review shows topical fluoride is effective to prevent dental caries in children without side effects. But more high-quality RCTs are needed to prove this conclusion due to the low quality of the included studies.

  • The effect of polymyxin B hemoperfusion on prognosis of patients with sepsis and septic shock: a meta-analysis

    ObjectiveTo investigate the effect of polymyxin B hemoperfusion on the prognosis of patients with sepsis and septic shock by meta-analysis.MethodsSupplemented by manual search and document traceability, the US National Library of Medicine Pubmed, the Dutch Medical Abstracts Embase database, and the Cochrane clinical trial database were searched. Randomized controlled trials (RCTs) were collected from January 1998 to October 2018 for the treatment of sepsis and septic shock with polymyxin B hemoperfusion, only limited to English publications. The collected RCTs were evaluated and the prognosis of patients with sepsis and septic shock was analyzed by the Cochrane Collaboration.ResultsFinally six RCTs were included, and a total of 926 patients were analyzed, with 471 patients in the polymyxin B hemoperfusion group and 455 patients in the control group. The mortality rate was 36.3% (171/471) in the polymyxin B hemoperfusion group and 39.1% (178/455) in the control group. Hemoperfusion with polymyxin B could not reduce the patient mortality (RR=0.80, 95% CI 0.56 to 1.15, P=0.233). A subgroup analysis was taken on the patients with moderate to severe septic shock. Four RCTs were included in total and 418 patients were analyzed, with 207 patients in the polymyxin B hemoperfusion group and 211 in the control group. The mortality rate was 38.65% (80/207) in the polymyxin B hemoperfusion group and 50.71% (107/211) in the control group were. The hemoperfusion of polymyxin B could significantly reduce the mortality of patients with moderate to severe septic shock (RR=0.70, 95% CI 0.52 to 0.96, P=0.025).ConclusionsOlymyxin B hemoperfusion can not improve the prognosis of patients with sepsis and septic shock. However, compared with conventional treatment, polymyxin B hemoperfusion can improve the 28-day mortality rate of patients of severe septic shock. Due to the limit number of randomized controlled trials, more high-quality trials are needed to a further confirmation.

  • Application of modified anterolateral supra-fibular-head approach in treatment of tibial plateau fractures involving posterolateral column

    ObjectiveTo explore effectiveness of reduction and internal fixation via modified anterolateral supra-fibular-head approach in treatment of tibial plateau fractures involving posterolateral column.MethodsBetween January 2016 and September 2018, 19 patients diagnosed as tibial plateau fractures involving posterolateral column were treated with reduction and internal fixation via modified anterolateral supra-fibular-head approach. There were 11 males and 8 females with an average age of 43.2 years (range, 28-65 years). The causes of tibial fracture were traffic accident (12 patients), falling injury (5 patients), and falling from height (2 patients). According to the Schatzker typing, the tibial fractures were rated as type Ⅱ in 9 cases, type Ⅲ in 4 cases, type Ⅴ in 4 cases, and type Ⅵ in 2 cases. The time from injury to operation was 5-13 days (mean, 8.5 days). There were 2 patients with osteoporosis. The operation time, intraoperative blood loss, and postoperative complications were recorded. The knee X-ray film was reviewed regularly to observe the fracture healing. At last follow-up, the fracture reductions were evaluated by Rasmussen radiological score. The knee joint function was evaluated by Hospital for Special Surgery (HSS) score system.ResultsThe average operation time was 95 minutes (range, 65-130 minutes). The average intraoperative blood loss was 220 mL (range, 150-350 mL). All incisions healed by first intention. No complications such as infection or deep venous thrombosis occurred. All patients were followed up 12-20 months (mean, 15.4 months). X-ray films showed that the fractures healed with the healing time of 12-20 weeks (mean, 14.5 weeks). No complications such as loosening or breakage of internal fixation occurred. At last follow-up, according to the Rasmussen radiological score, the fracture reductions were evaluated as excellent in 13 cases, good in 4 cases, fair in 1 case, and poor in 1 case. HSS scores of knee joint function were excellent in 14 cases, good in 3 cases, fair in 1 case, and poor in 1 case. The knee joint range of motion was 90°-135°, with an average of 113.4°.ConclusionApplication of modified anterolateral supra-fibular-head approach in reduction and internal fixation for tibial plateau fractures involving posterolateral column has the advantages of full exposure, less trauma, safety, and reliable reduction and fixation.

  • Lung ultrasound in the treatment of alveolar recruitment maneuver for patients with acute respiratory distress syndrome

    Objective To Assess the efficacy of using lung ultrasound to guide alveolar recruitment maneuver in patients with acute respiratory distress syndrome (ARDS). Methods Sixty patients with ARDS were randomly divided into two groups, ie, maximal oxygenation group (n=30) and lung ultrasound group (n=30). All the patients had artificial airway and needed mechanical ventilation. The patients in the two groups accepted recruitment maneuver guided by maximal oxygenation or lung ultrasound respectively. During the course of recruitment maneuver, the arterial partial pressure of oxygen (PaO2), positive end-expiratory pressure (PEEP), central venous pressure (CVP), mean arterial pressure (MAP), cardiac output (CO), and extravascular lung water index (EVLWI) were recorded and compared between both groups. Results The PaO2 in lung ultrasound group was higher than that in maximal oxygenation group (P=0.04). The PEEP was higher in lung ultrasound group but without significant difference (P=0.910). There was no significant difference of the other outcomes (CVP, MAP, CO, EVLWI) between the two groups (all P>0.05). Conclusion Lung ultrasound is an effective means that has good repeatability and security for guiding recruitment maneuver in patients with ARDS.

  • Predictive value of mini-fluid challenge for volume responsiveness

    ObjectiveTo evaluate the predictive value of mini-fluid challenge for volume responsiveness in patients under shock.MethodsSixty patients diagnosed as shock were included in the study. A 50 mL infusion of physiological saline over 10 seconds and a further 450 mL over 15 minutes were conducted through the central venous catheter. Cardiac output (CO), global end-diastolic volume index (GEDVI), central venous pressure (CVP) and extravascular pulmonary water index (EVLWI) were monitored by the pulse indicator continuous cardiac output monitoring. If the increase of CO after 500 mL volume expansion (ΔCO500) ≥10%, the patient was considered to be with volume responsiveness. The relevance between ΔCO50 and ΔCO500 was analyzed, and the sensitivity and specificity of the ΔCO50 were analyzed by receiver operating characteristic (ROC) curve.ResultsAfter 50 mL volume injection, the heart rate and systolic blood pressure of the two groups did not change obviously. The CVP of non-responders changed slightly higher than that of responders, but neither of them had obviously difference (P>0.05). The CO of responders had increased significantly (P<0.05) which was in accord with that after a further 450 mL volume injection. GEDVI and EVLWI did not change significantly (P>0.05). ΔCO50 and ΔCO500 were strongly correlated (r=0.706, 95%CI 0.677 - 0.891, P>0.05). The area under ROC curve for ΔCO50 was 0.814 (95%CI 0.707 - 0.922).ConclusionThe volume responsiveness of patients under shock can be predicted by mini-fluid challenge study which is related to normal volume expansion and it does not increase the risk of pulmonary edema.

  • Analysis of microbiology and antibiotic susceptibility of perioperative urinary tract infection in patients undergoing hip or knee arthroplasty

    Objective To explore the microbiological etiology and antibiotic susceptibility of periopertive urinary tract infection (UTI) in patients undergoing hip or knee arthroplasty, so as to provide recommendations for antibiotic treatment. Methods A retrospective review was conducted for patients with perioperative UTI who underwent hip or knee arthroplasty between January 1st, 2013 and October 1st, 2015. Microbiological data and antibiotic susceptibility of bacteria were analyzed. Results A total of 117 strains of bacteria were identified, including 11 types of species. Among the organisms cultured, 86.3% (101 strains) were gram-negative bacteria, in which Escherichia coli was the most common causative organism (70.9%, 83 strains), followed by Klebsiella species (7.7%, 9 strains) and Proteus mirabilis (3.4%, 4 strains). And among the gram-positive bacteria detected, the proportion of Enterococcus faecalis and Feces Enterococcus was 6.8% (8 strains) and 3.4% (4 strains), respectively. The bacteria showed highly resistance to cephalosporins, quinolones and sulfonamides, but showed high sensitive to nitrofurantoin, carbopenems, the enzyme inhibitor complex and aminoglycoside antibiotics. Conclusions There is a diversity of bacteria involved in UTI, and the top 3 pathogens are Escherichia coli, Enterococcus faecalis and Klebsiella species. The resistance rate is high, and nitrofurantoin, amilacin, piperacillin-tazobactam, cefoperazone-sulbactam are the recommended antibiotics to treat the UTI, but the antibiotic should be adjusted according to susceptibility results.

  • One-Month after Earthquake Documentary of Anti-Epidemic Work for Counterpart-Supporting Longmen Township as the Epicenter of Lushan Earthquake

    Objective To report the anti-epidemic work for counterpart-supporting Longmen township, the epicenter of Lushan earthquake, by Mianyang health and epidemic prevention team from the first day to one month after the earthquake. Methods a) The following information was collected: work information and report forms of each counterpart-support small group, government work information, work information statistics and historical epidemic materials of health center, and epidemic prevention materials of Beichuan county. b) The epidemic prevention work of Longmen township from the first day to one month after earthquake were documentarily described, the expert group review was adopted to assess the disaster situation, epidemic situation and health needs, and the visual observation and trace method were used to monitor the vectors. Results a) The first team arrived in Ya’an city at the 19th hour after earthquake. The members of the team put forward the “City in-charge-of Township counterpart-support anti-epidemic mode” and they were approved to counterpart-support Longmen township. b) The second team involving 48 members assigned to the first team within 5 days after earthquake. Totally 224 local people were called up and they carried out a comprehensive work based on the “City in-charge-of Township counterpart-support anti-epidemic mode”: water quality and disease surveillance, disinfection and disinsectization at key sites, epidemic prevention in settlements, large-scale health education, and psychological intervention. c) As of 1 month after the earthquake, Mianyang health and epidemic prevention team had dispatched 20 vehicles and 122 people participated in the post-disaster epidemic prevention in Longmen township. The total disinfection area was 1 725 400 square meters, and disinsectization area was 1 162 500 square meters; 184 water samples were collected, and 7 717 family-times’ drinking water disinfection were guided; nearly 28 000 publicity materials were distributed; 8 636 people were visited and received for diagnosis; 33 cases with watery diarrhea and 16 cases with fever were found; 117 people were trained about mental health service, 3 mental health service stations were set up, and 1 152 people were helped with special population mental health services. Conclusion a) Mianyang health and epidemic prevention team combined the special requirements of epidemic prevention in Lushan earthquake with the proficient experiences and work modes of epidemic prevention in Mianyang as the extremely-severe stricken area in Wenchuan earthquake, and put forward the “City in-charge-of Township counterpart-support mode” which is implemented and verified in the anti-epidemic practice in epicenter of Longmen Township. b) Compared with the epidemic prevention in Qushan township of Beichuan county which is the epicenter of both Lushan and Wenchuan earthquakes: the following 8 aspects (including the “City in-charge-of Township counterpart-support” formed, the first anti-epidemic team entered into the village, full coverage of anti-epidemic team entered into the village, direct reporting network recovered, settlement’s regular administration started, water supply guaranteed, drinking water monitoring initiated, emergency vaccination initiated) were performed earlier in Lushan than Wenchuan, with 42 days, 2 days, 32 days, 4 days, 10 days, 6 days, 7 days, and 19 days in advance, respectively. c) The overall performance is faster, more reasonable, and more efficient. It can provide first-hand information for globally similar earthquake’s epidemic prevention, and also the decision-making and reference for both post-disaster reconstruction and construction of regional state disaster emergency response system.

  • Therapeutic effect of hemoperfusion for absorption of inflammatory cytokines on sepsis

    Objective To evaluate the effect of hemoperfusion for absorption of inflammatory cytokines on sepsis . Method A prospective randomized controlled study was carried out to collect 60 sepsis patients admitted to the Department of Critical Care Medicine of this hospital from June 2019 to December 2021. They were randomly divided into a study group (30 cases) and a control group (30 cases) by using the random number table method. Both groups of patients received routine treatment according to the guidelines, including fluid resuscitation, mechanical ventilation, antibiotic and vasoactive agents. For the patients with renal failure, renal replacement therapy (RRT) was used. Routine vital sign monitoring and serum procalcitonin (PCT) and interleukin-6 (IL-6) determination were recorded. The study group received two times of hemoperfusion to absorb inflammatory cytokines at 0 h and 24 h after enrollment. At 24 h and 48 h after treatment, the vital signs and related physical and chemical indexes of patients were recorded again, including norepinephrine dose, oxygenation index, PCT, IL-6 and blood lactic acid. The changes of physical and chemical indexes and the 28-day survival rate of the two groups were compared. Results There was no difference in the general situation of the two groups when they were enrolled (P>0.05). The dosage of norepinephrine [(0.77±0.48)μg·kg–1·min–1 vs. (0.92±0.62) μg·kg–1·min–1, P=0.030] and the level of blood lactic acid [(2.70±1.43)mmol/L vs. (4.05±2.60)mmol/L, P=0.001] in the study group were significantly lower than those in the control group 24 h and 48 h after treatment. The oxygenation index in the study group was higher than that of the control group 24 h after treatment (212±68)mm Hg vs. (197±42)mm Hg, P=0.042). The inflammation related indexes PCT [(17±24)ng/mL vs. (32±36)ng/mL, P=0.013] and IL-6 [299 (102, 853)pg/mL vs. 937 (247, 2230)pg/mL, P=0.026] in the study group were significantly lower than those in the control group 48 h after treatment. The dosage of noradrenaline, oxygenation index, PCT, IL-6 and blood lactate level in the study group after treatment were improved compared with those before treatment (P<0.05), while those in the control group were not significantly improved after treatment (P>0.05), and oxygenation index in the two groups had no significant difference before and after treatment (P>0.05). There was no significant difference in the 28-day survival rate between the two groups (χ2=0.211, P=0.646). Conclusion Although the hemoperfusion for absorption of inflammatory cytokine factors can not reduce the 28-day mortality of sepsis, it can significantly improve the early physical and chemical indicators of patients, and provide opportunities for follow-up treatment.
