  • Experience of Sonography Guided Freka Trelumina Placement in Patients with Severely Impaired Gastric Emptying by Stylet Displacement

ObjectiveTo assess the clinical efficacy of sonography guided Freka Trelumina placement by stylet displacement in patients with severely impaired gastric emptying. MethodsTwenty-two patients with severely impaired gastric emptying monitored in the Intensive Care Unit from January 8 to May 18, 2016 were chosen to be our study subjects. Freka Trelumina was placed under ultrasonic guidance, and the guide wire displacement was used to determine the location of the catheter. We recorded whether the patient had an intra-gastric injection of warm water, the manual pushing times before the catheter passed through the pylorus, whether the operation succeeded, the time spent on guiding the placement, and the catheter depth. The pros and cons of the method in clinical use, and whether fasting state helped reduce the operating time were analyzed. ResultsAmong the 22 patients, 20 had a successful Freka Trelumina placement, and the success rate was 91%. The number of manual pushing before the catheter passed through the pylorus was 1 in 4 cases (20%), 2 in 5 (25%), and equal to or more than 3 in 11 cases (55%). The catheter could be seen in the third part of duodenum only in 9 cases (45%). The mean placement procedure lasted (20.35±12.93) minutes for the successful cases. The time spent in the 11 patients with empty stomach was (15.00±9.87) minutes, less than (26.89±14.45) minutes in those 9 post prandial patients (P<0.05). ConclusionsWith stylet displacement to determine the location of the catheter, sonography guided Freka Trelumina placement has a high success rate. Ultrasonic guidance facilitates the insertion of the tubes in critically ill patients. For patients with empty stomach, it may help reduce the operating time.

  • Index Analysis on Early Prediction of Survival Time in Severe Multiple Trauma Patients

ObjectiveTo study the value of revised trauma scores (RTS), major trauma outcome study (MTOS)-RTS scores and point of care test (POCT) in the early forecast of survival time in severe multiple trauma patients. MethodsMultiple trauma patients treated in the Emergency Department of our hospital between September and December 2015 were included in our study. We collected such data as the basic information on admission, physical signs (breath, blood pressure, state of consciousness) and POCT indexes, including pH value, hemoglobin, base excess, hematocrit value, lactic acid, sodium, anion gap, and blood glucose. We calculated each patient's RTS and MTOS-RTS scores. According to the 30-day prognosis, the patients were divided into survival group and death group. Risk factors for survival time were screened by Cox regression risk model. ResultsSeventy-five multiple trauma patients were included in our study. Among them, there were 51 males and 24 females. Fourteen of them died. The multivariate analysis in the Cox regression risk model showed that the risk factors for the death of multiple trauma patients included MOTS-RTS score [RR=0.726, 95%CI (0.608, 0.867), P < 0.001) and POCT lactic acid level [RR=1.139, 95%CI (1.010, 1.324), P=0.037]. ConclusionMOTS-RTS combined with lactic acid level may be used in the early forecast of survival time in severe multiple trauma patients.

  • Clinical Study on the Value of Quick Sequential Organ Failure Assessment for Evaluating the Prognosis of Patients with Septic Shock

ObjectiveTo investigate the clinical value of quick sequential organ failure assessment (qSOFA) score in predicting the outcome of patients with septic shock. MethodsWe collected the clinical data of 170 patients with septic shock treated in the Emergency Intensive Care Unit between January 2013 and January 2014. According to the 28-day outcomes of the patients, they were recorded as survival group and non-survival group. We calculated the qSOFA score, acute physiology and chronic health evaluation (APACHE)Ⅱ score on patients' admission. Using receiver operating characteristic (ROC) curve, we analyzed the qSOFA score, the effect of APACHE Ⅱ score in predicting the 28-day prognosis for patients with septic shock. The correlation between qSOFA score and APACHEⅡ score was also assessed. ResultsThe qSOFA and APACHEⅡ scores in non-survivors were higher than those in the survivors. According to ROC curve analysis, the area under the curve for qSOFA score and APACHE Ⅱ score was 0.666 and 0.791, respectively. For qSOFA score with 2 cut-off points to evaluate the prognosis of septic shock, the sensitivity was 62.7%, specificity was 61.1%, positive predictive value was 56.0%, negative predictive value was 67.4%, positive likelihood ratio was 1.61, and negative likelihood ratio was 0.61. For the APACHEⅡ score with 24 cut-off points to evaluate the prognosis of septic shock, the sensitivity was 70.7%, specificity was 80%, positive predictive value was 73.6%, negative predictive value was 67.3%, positive likelihood ratio was 3.54, and negative likelihood ratio was 0.37. The correlation coefficient of qSOFA score and APACHE Ⅱ score was 0.499. ConclusionThe qSOFA score is useful to evaluate the prognosis of the patients with septic shock early in Emergency Department.

  • Correlation Analysis between Rapid Emergency Medicine Score and Therapeutic Intervention Scoring System Score in Critically Wounded Victims in Lushan Earthquake

ObjectiveTo investigate the correlation between rapid emergency medicine score (REMS) and therapeutic intervention scoring system (TISS-28) score and analyze the feasibility of assessing the nursing workload by REMS score for critically wounded earthquake victims, in order to provide reference for rapid and effective resource allocation for earthquake victims. MethodsA retrospective analysis was carried out on 39 Lushan earthquake victims with their acute plysiology and chronic health evaluationⅡ scores higher than 25, who were directly transferred from the earthquake site to the Emergency Department of West China Hospital between April 20 and 27, 2013. Among them, there were 24 males and 15 females aged between 5 and 90 years old averaging (57.1±19.8) years. REMS score and TISS-28 score were calculated for each victim. The relationship between REMS score and TISS-28 score was analyzed by correlation analysis and curve estimation including linear model, quadratic model, composite model, growth model, logarithm model, cubic model and exponential model. Then, we tried to find out the most suitable description for the relationship between REMS score and TISS-28 score. ResultsThe Spearman correlation coefficient between the two score systems was 0.710 and the most suitable description for the relationship between REMS score and TISS-28 score was logarithmic curve model. The formula was TISS=-5.946+4.467lnREMS. ConclusionREMS score can be applied as a nursing workload predicting tool for critically wounded victims in Lushan earthquake and it provides a guidance for rational allocation of health resources.

  • Analysis of the Influencing Factors for Tiered Medical Service in Dog-bite Clinic of Dujiangyan City

ObjectiveTo explore the possible factors which can influence the tiered medication situation in dog-bite clinic in comprehensive hospitals and village (community) health service centers, in order to provide references for the standardization of the diagnosis and treatment for dog-bite exposed patients. MethodsWe conducted a retrospective observational study, searched the database of Dujiangyan Center for Disease Control and Prevention, and collected all the information of dog-bite patients administrated by village (community) health service center or hospital from January 2012 to December 2015.Then we investigated the patients' satisfaction and clinical workers' familiarity of dog-bite injury treatment protocols by questionnaires in March 2016.The data were analyzed with statistical description and chi-square test, to explore the possible factors which might influence the tiered medication situation for patients exposed to dog bite in Dujiangyan city. ResultsThere was one comprehensive hospital and 27 township (community) health centers included in this study.During the study period, the total number and proportion of dog-bite injury patients admitted in the comprehensive hospital decreased year by year, especially for the number of grade-two dog-bite exposure patients. One the contrary, the total number and proportion of patients administrated by township (community) health service centers increased year by year, especially for the number (proportion) of grade-two and grade-three dog-bite exposure patients, which increased to 1 944(44.10%) and 1 248(28.31%), respectively. The results also showed that there were significant differences between the comprehensive hospital and township health centers in the satisfaction and familiarity investigation (P < 0.05). ConclusionsTownship and village (community) health service centers are treating many more patients than the comprehensive hospital, but with a lower satisfaction rate. The distribution of dog-bite exposure patients visiting different degrees of hospitals are generally consistent with tiered medication policy. However, township and village (community) health service centers are recommended to strengthen the staff training about how to deal with dog-bite injuries standardly in order to improve patients' satisfaction.

  • The Role of Platelet Activation in the Acute Aortic Dissection-related Systemic Inflammatory Responses

ObjectiveTo explore whether platelet activation is associated with systemic inflammatory responses. MethodsWe conducted a cross-sectional study to enroll all aortic dissection (AD) patients (AD group) from January 1, 2015 to June 30, 2015 in the Emergency Department. According to the characteristics of AD patients, we matched hypertension (hypertension group) and health participants (health group) with AD patients at a proportion of 1:1:1. Blood samples were collected on admission for blood routine test [mean platelet volume (MPV)/platelet (PLT)] and inflammatory cytokines [tumor necrosis factor (TNF)-α and interleukin (IL)-6] analysis. We compared all parameters among the three groups and performed bivariate correlation analyses. ResultsExpressions of TNF-α and IL-6 in the AD group [(20.9±4.5), (168.8±75.1) pg/mL] were significantly higher than those in the hypertension group [(4.3±1.9), (8.4±2.9) pg/mL] and healthy group [(5.4±1.6), (8.7±3.8) pg/mL] (P<0.05). MPV/PLT in the AD group was significantly higher than that in the hypertension group and healthy group (0.106±0.035 vs 0.049±0.010, P<0.05; 0.106±0.035 vs 0.054±0.019, P<0.05). There were positive correlations between MPV/PLT and TNF-α (r=0.516, P=0.002), and between MPV/PLT and IL-6 (r=0.633, P<0.001) in the AD group. ConclusionIn summary, our study shows that platelets of AD patients can be activated, and the degree of activation is associated with systemic inflammatory responses.
