ObjectiveTo investigate the change of blood glucose and its clinical significance in patients with acute pancreatitis (AP). MethodsThe regularity of blood glucose change and the relation between the regularity and the prognosis were analyzed in 115 patients with AP and hyperglycemia.ResultsBlood glucose was increased with a median (M) of 8.7 mmol/L,18.45 mmol/L and 27.22 mmol/L, which gradually decreased to normal value within 3-17 days, 7-26 days and 24-46 days after treatment,respectively in patients with mild AP, type Ⅰ of severe acute pancreatitis (SAP) and type Ⅱ of SAP. There was marked statistical difference among the three groups. A smaller dose of regular insulin was used for 36 patients with mild AP; however, a larger dose of regular insulin was used for all 30 patients with SAP.ConclusionThe level of blood glucose, the dose of regular insulin and the duration of hyperglycemia increase with the severity of AP.
Objective To investigate whether the biomechanical effect of mushroom shaped surface prosthesis on femoral neck is in the scope of safety after the replacement. Methods Four donated fresh-adult specimens of upper femur under the age of 55 years old were used. The strains of detecting points A (lateral) and B (medial) on the narrow place of femoral neck in the standing position were simulated for three stages before or after the prosthesis replacement, namely the pre-replacement, the initial stage during which the interspace of the prosthesis was filled with cancellous bone, and laterstage during which the interspace of the prosthesis was filled with bone cement. Then they were compared by using l inear regression analysis in Excel and rel iabil ity analysis. Results The regression analysis showed that the values of correlation coefficient r were all more than 99% at the different stages, indicating the strain of femoral neck’s cortical bone was proportional to the load and there was no occurrence of the plastic deformation of the femoral neck. For point A, the slope of the trend l ine of strain was 0.671 9 at the pre-replacement stage. The value of the initial stage after replacement was 0.619 2 and its change rate was —7.8%; while corresponding value was 0.662 7 and —1.4% at the later stage after replacement. For point B, the slope of the trend l ine of strain was —1.056 1 at the pre-replacement stage. The value of the initial stage after replacement was — 1.129 2 and its change rate was 6.9%; while corresponding value was —1.085 1 and 2.7% at the later stage after replacement. Conclusion The mechanical strength of femoral neck is in the scope of safety after surface replacement of the femoral head. The change rate of strain at the later stage is smaller than the initial stage.
Objective To evaluate the effects of pseudo-ginseng Tongshu capsule on changes of bloodstream, blood plasma lipid and Fibrin of patients with Brain Infarction. Methods The patients with Brain Infarction were randomly selected, among whom 120 received pseudo-ginseng Tongshu capsule for 3 month and 86 received the routine therapy. Before and after the treatment, change of bloodstream, blood plasma lipid and Fibrin were tested. Results A total of 120 patients were included in the treatment group and 86 in the control group. In the 3rd month, the level of total cholesterol (TC) and Fibrinogen was decreased significantly in the patients with Brain Infarction (Plt;0.05). Conclusion The level of blood plasma lipid and Fibrinogen is decreased significantly in the patients with Brain Infarction who accept pseudoginseng Tongshu capsule.
Objective To explore the change of constitution in thyroid diseases of West China Hospital between 2000 and 2012, in order to provide clinical evidence. Methods Clinical data, including gender, age, and pathological diagnosis of patients with thyroid disease who underwent primary thyroid surgery in our hospital from 2000 to 2012 were collected retrospectively and analyzed statistically. Results A total of 9 642 patients were enrolled, including 1 893 male patients and 7 749 female patients. The ratio of male to female patients was 1 to 4.09. In male patients, the proportion of thyroid carcinoma were significantly higher than those of female group (P=0.02);in male patients younger than 45 group, the proportion of thyroid carcinoma were significantly higher than those of female group (P<0.01). There was no statistical difference on the proportion between male and female patients older than 45 group (P=0.90). Proportion of thyroid carcinoma, especially proportion of papillary thyroid carcinoma (PTC) increased in general. Proportion of Hashimoto thyroiditis (HT) increased in general too. HT with thyroid carcinoma accounted for an increasing proportion of all patients with HT. Proportion of nodular goiter (NG) increased at first and then declined. Proportion of thyroid adenoma (TA) decreased on the whole. Conclusions Proportion of thyroid carcinoma, especially proportion of PTC increase in recent years on the whole in patients underwent surgery. All these changes need to be given sufficient attention.
Community-acquired pneumonia (CAP) is still a common disease that seriously affects people’s health. It is of great clinical significance for proper anti-infective therapy to identify the characteristics and changes of the pathogens. Along with the accelerated process of aging population, increased use of immunosuppression agents, and increased morbidity of malignant tumor and underlying diseases, the pathogenic spectrum of patients with CAP varies as well. This article reviews the important pathogenic changes of CAP in recent years.
The quality of clinical trials is key to determine the value of a clinical research and whether it can transform achievements. In this paper, the characteristics and direction of the implementation of quality control in clinical trials in China, and the characteristics of quality control in clinical trials of Chinese medicine are discussed, and the development direction of quality control in clinical trials in China is explored in order to improve the overall level of clinical trials in China.
The concept of clinical trial transparency has been promoted for more than 40 years. The act of clinical trial registration, report guidelines development, and data sharing has has been strongly pushed forward and become a common practice. The clinical trial process being the key procedure of trial operation and quality control, determines the accuracy of the results. However, the process report of clinical trials is insufficient. In this article, we summarize the importance of clinical trial process report and provide corresponding suggestions. We propose that medical journals, reporting guidelines developers and clinical trial registration platforms should work together to strengthen the process report of clinical trials and promote full transparency of clinical trials.
